A 19-year-old Korean woman presented with left mandibular dental pain and swelling. Periapical radiolucencies were associated with the mandibular left first molar, second molar and the mandibular right second molar. The mandibular right second molar root developed incompletely and has the open apex. Clinical examination revealed worn accessory occlusal cusps of premolars. A diagnosis of dens evaginatus with associated periapical lesion secondary to pulpal necrosis was made. The root canal of the lower right second premolar was sealed with Calcium hydroxide paste for apexification. About two months later Calcium hydroxide paste was removed and the canal was resealed with new Calcium hydroxide paste. After four months the canal was sealed permanently with guttapercha and zinc oxide-eugenol sealer. The root canals of the lower left premolars were irrigated every week with 3.5% NaOCl solution for and half month. And the canals were sealed with gutta-percha and ZOE sealer. Preventive endodontic treatment for the lower right first premolar was undertaken.

Objectives: This study evaluated the biocompatibility and bioactive potential of NeoMTA Plus mixed as a root canal sealer in comparison with MTA Fillapex. Materials and Methods: Polyethylene tubes filled with NeoMTA Plus (n = 20), MTA Fillapex (n = 20), or nothing (control group, CG; n = 20) were inserted into the connective tissue in the dorsal subcutaneous layer of rats. After 7, 15, 30 and 60 days, the specimens were processed for paraffin embedding. The capsule thickness, collagen content, and number of inflammatory cells (ICs) and interleukin-6 (IL-6) immunolabeled cells were measured. von Kossa-positive structures were evaluated and unstained sections were analyzed under polarized light. Two-way analysis of variance was performed, followed by the post hoc Tukey test (p ≤ 0.05). Results: At 7 days, the capsules around NeoMTA Plus and MTA Fillapex had more ICs and IL-6-immunostained cells than the CG. However, at 60 days, there was no significant difference in the IC number between NeoMTA Plus and the CG (p = 0.1137) or the MTA Fillapex group (p = 0.4062), although a greater number of IL-6-immunostained cells was observed in the MTA Fillapex group (p = 0.0353). From 7 to 60 days, the capsule thickness of the NeoMTA Plus and MTA Fillapex specimens significantly decreased, concomitantly with an increase in the collagen content. The capsules around root canal sealers showed positivity to the von Kossa stain and birefringent structures. Conclusions: The NeoMTA Plus root canal sealer is biocompatible and exhibits bioactive potential.
Advancements in bio-ceramic technology has revolutionised endodontic material science by enhancing the treatment outcome for patients. This class of dental materials conciliates excellent biocompatibility with high osseoconductivity that render them ideal for endodontic care. Few recently introduced bio-ceramic materials have shown considerable clinical success over their early generations in terms of good handling characteristics. Calcium enriched mixture (CEM) cement, Endosequence sealer, and root repair materials, Biodentine and BioAggregate are the new classes of bio-ceramic materials. The aim of this literature review is to present investigations regarding properties and applications of CEM cement in endodontics. A review of the existing literature was performed by using electronic and hand searching methods for CEM cement from January 2006 to December 2013. CEM cement has a different chemical composition from that of mineral trioxide aggregate (MTA) but has similar clinical applications. It combines the biocompatibility of MTA with more efficient characteristics, such as significantly shorter setting time, good handling characteristics, no staining of tooth and effective seal against bacterial leakage.

The present report describes the endodontic treatment of an Oehlers type II dens invaginatus in a maxillary lateral incisor with 5 root canals, an extremely rare condition. Apical periodontitis and related symptoms were noted. Cone-beam computed tomography was used to aid the diagnosis, reveal tooth morphology, and assist in canal location. The pulp chamber was carefully accessed, and the root canals were explored under magnification. All root canals were prepared with an R25 Reciproc Blue system and sodium hypochlorite (NaOCl) irrigation. After initial preparation, a self-adjusting file (SAF) with NaOCl and ethylenediaminetetraacetic acid was used to complement the disinfection. Additionally, calcium hydroxide medication was applied. Vertical compaction was used to fill the canals with a calcium silicate-based endodontic sealer and gutta-percha. After 12 months, the patient exhibited healing of the periapical region, absence of symptoms, and normal dental function. In conclusion, this nonsurgical treatment protocol was successful in promoting the cure of apical periodontitis. Both complementary disinfection with an SAF and use of calcium hydroxide medication should be considered when choosing the best treatment approach for dens invaginatus with very complex anatomy.

Biomechanical preparation of root canals with accentuated curvature is challenging. New rotatory systems, such as Reciproc, require a shorter period of time to prepare curved canals, and became a viable alternative for endodontic treatment of teeth with root dilaceration. Thus, this study aimed to report a clinical case of endodontic therapy of root with accentuated dilaceration using Reciproc single-file system. Mandibular right second molar was diagnosed as asymptomatic irreversible pulpitis. Pulp chamber access was performed, and glide path was created with #10 K-file (Dentsply Maillefer) and PathFile #13, #16 and #19 (Dentsply Maillefer) up to the temporary working length. The working length measured corresponded to 20 mm in the mesio-buccal and mesio-lingual canals, and 22 mm in the distal canal. The R25 file (VDW GmbH) was used in all the canals for instrumentation and final preparation, followed by filling with Reciproc gutta-percha cones (VDW GmbH) and AH Plus sealer (Dentsply Maillefer), using thermal compaction technique. The case has been receiving follow-up for 6 mon and no painful symptomatology or periapical lesions have been found. Despite the difficulties, the treatment could be performed in a shorter period of time than the conventional methods.
연구목적: 본 연구의 목적은 수종의 sealer를 이용하여 각기 다른 양의 잔여수분이 존재하는 근관을 충전 시, 근관충전의 sealing ability를 glucose leakage model로 평가하는 것이다. 연구 재료 및 방법: 발치된 90개의 단근치를 치관부를 잘라내고 #45 / 0.06 taper까지 확대한 후, 12개의 실험군 (n = 7)과 2개의 대조군 (n = 3)으로 나누었다. 실험군을 3가지 습윤 상태에서 (a, DRY; b, PAPER POINT DRY; c, WET) 4종류의 sealer로 (Group 1-3: Sealapex; Group 4-6: AH plus; Group 7-9: Tubuli-seal; Group 10-12: EndoRez), warm vertical compaction method를 이용해 충전하였다. $37^{\circ}C$, 습도 100%에서 7일 동안 보관하고 glucose leakage model을 이용하여 치관부로부터 치근부 방향의 미세누출을 정량화하였다. 1, 3, 7, 14, 21, 30일 째 누출된 glucose의 농도를 spectrophotometer로 측정하였다. 결과: 관찰기간 동안 AH plus/DRY군 (Group 4)이 12개 그룹 중 가장 적은 양의 leakage를 보였다 (20.78 mmol/L). 반면에 Tubuli-seal/WET군 (Group 9)이 12개 그룹 중 가장 높은 양의 leakage를 보였다 (178.75 mmol/L). 결론: 본 연구의 결과는 충전 시 근관 내의 습윤 상태와 sealer 종류가 microleakage에 유의한 영향을 미친다는 것을 입증하였다. 그러므로 sealer 종류에 따라 각기 다른 근관 건조 방법을 사용하는 것이 근관치료에서 더 좋은 결과를 얻을 수 있는 방법일 것이다.
근관치료 중 수동 또는 엔진구동파일을 사용하다 과도한 기구 조작을 하게 되면, 근관충전제, 드레싱제 그리고 근관세척제가 근관에서 나와 하악관으로 확산될 수 있다. 이 때 환자는 통증, 지각과민, 지각감퇴, 무감각, 이상감각 등을 호소하게 될 것이다. 이런 문제들은 근관충전제에 포함되어 있으면서 생체적합성이 떨어지는 물질들에 의한 비가역적인 손상을 피하기 위해 가능한 빨리 해결되어야 한다. 비록 근관치료와 관련하여 발생한 하치조신경의 손상을 치료하는 진료지침이 비교연구 되어 있는 것이 없으나, 이 합병증에 대한 통상의 치료는 통증과 염증을 조절하는 것이고, 가능하다면 수술적인 치료로 근원을 처치하는 것이다. 그러나, 비수술적인 치료로 통증을 완전히 개선하거나 감소시키고 또는 감각이상을 치료하는 것이 보고되어왔다. 가바펜틴(gabapentin) 또는 프리가발린(pregabalin) 같은 항간질제는 신경병증 통증의 치료에 이용되어 왔다. 이번 논문에서는 하악 우측 제 2대구치의 근관치료 후의 하치조신경의 손상과 이에 대해 프레드니솔론과 가바펜틴으로 비외과적 치료를 시행한 것과 임상적으로 신경감각검사와 신경전류인지역치검사(Neurometer)를 통해 경과관찰을 시행하였다.
본 연구는 포졸란 시멘트를 기반으로 하는 근관전색제인 Endoseal MTA (Maruchi, Wonju, Korea)를 근관전색제와 근관충전제로 사용하였을 때 나타나는 치근단부 미세누출을 기존의 근관전색제인 AH $Plus^{(R)}$ (Dentsply DeTrey, Konstanz, Germany)와 비교하여 평가하고자 하였다. 만곡되지 않은 영구 전치를 3개의 실험군(A군, E1군, E2군)으로 나누고 A군은 AH $Plus^{(R)}$와 gutta-percha (GP), E1군 은 Endoseal MTA와 GP, E2군은 Endoseal MTA 단독으로 충전하였다. 충전이 완료된 치아는 완전한 경화를 위해 24시간동안 멸균 증류수에 보관하였고 염색액 누출을 위해 치근단 부위를 0.2% 로다민 B 염색 용액에 24시간 동안 노출시켰다. 입체 현미경을 이용하여 미세누출 정도를 평가하였으며 3개의 실험군 사이에 유의한 차이가 관찰되지 않았다. 결론적으로 Endoseal MTA는 GP의 사용에 관계 없이 AH $Plus^{(R)}$와 유사한 밀폐력을 보였으며 이러한 결과는 Endoseal MTA의 근관전색제 및 근관 충전제로써 유용한 사용에 대한 가능성을 제시한다.
목적: 이 연구는 새로운 레진계 근관실러인 Any-Seal의 여러 물성을 AH Plus Jet와 비교하여 평가하고자 했다. 연구 재료 및 방법: 흐름성, 방사선 불투과성, 압축 강도를 ISO 6876/2001기준에 의해 측정하였다. 흐름성은 실러 0.05 mL를 두 개의 유리판 사이에 놓고 100 g의 무게로 눌러 측정했다. 실러 혼합 10분 뒤 무게를 제거하고 눌려진 실러의 지름을 측정했다. 방사선 불투과성은 각 근관실러를 지름 10 mm, 높이 1 mm의 디스크형 시편으로 만들어 aluminum step wedge와 함께 방사선 촬영을 하여 분석했다. 압축 강도는 각 실러를 4 mm 지름과 6 mm 높이의 실린더 형태로 만든 다음 24시간 후와 1주일 후에 측정했다. 결과: 두 근관실러의 흐름성과 방사선 불투과성 값은 ISO 6876/2001조건에 맞았다. 흐름성은 AH Plus Jet와 Any-Seal 사이에 통계적으로 유의한 차이가 없었다(P > 0.05). AH Plus Jet가 방사선 불투과성 값을 나타냈다(P < 0.05). 24시간 후와 1주일 후 모두에서 AH Plus Jet가 더 높은 압축 강도를 보였다(P < 0.05). 결론: Any-Seal이 낮은 압축 강도를 보였으므로 다른 물리적, 생물적 특성에 대해 더 많은 연구가 임상적 활용 전에 필요할 것으로 생각한다.
본 연구는 항생제를 복합 투여하여 유치의 근관 및 치근단 주위 감염에 대한 효과를 임상적으로 관찰하고자 하였다. 근관 및 치근단 주위 병소의 멸균을 목적으로 metronidazole, ciprofloxacin 및 minocyclin을 증류수 및 근관시멘트와 혼합하여 적용하였다. 유치의 근관 입구로부터 하방에 $Fissurotomy^{(R)}$ bur(SS White, USA)를 이용하여 약제 공간을 형성하였고 약제는 치수실 바닥까지 채웠다. 본 증례들에서 단회 적용으로 임상 증상이 개선되었다. 치은종창, 동통, 누공 등 모든 증상이 술 후 1주일 내에 소실되었다. 임상적으로 활용하기 위해서는 보다 많은 수의 증례와 장기간의 임상적 평가가 필요하며 아직까지 근관내 항생제 사용에 따른 부작용에 대한 보고가 없지만 항생제에 민감한 환아에 대해서는 사용에 주의가 필요할 것으로 사료된다. 저자는 행동조절이 곤란한 소아에서 생리적 치근흡수가 진행 중인 감염된 유치를 가진 환아들을 대상으로 항생제를 복합 사용한 바 증상의 개선을 관찰하였기에 보고하는 바이다.
