THE BIPOLAR SMOULDER PROSTHESIS LONGER TERM RESULTS (5-10 YEARS) IN THE MANAGEMENT OF END-STAGE ROTATOR CUFF ARTHROPATHY

  • This prospective clinical study evaluates the 5-10 year results of the BiPolar shoulder Arthroplasty in patients with end-stage RotatorCuff Arthropathy. The study group consisted of 48 patients (59 shoulders). Average age was 72 years and average FU time was 73 months. Results showed that the average UCLA score went from 7.9 Pre-op to 23.3 Post-op. Final Constant score averaged $52\% (unadjusted). Pain relief using the VAS was 1.2(were 0=no pain. 15 = excruciating pain). ROM improved by an average of $20^\circ$. There were 2 reoperations because of periprosthetic fractures. Despite rather poor functional results, these patients were satisfied with their pain relief and the functional gains accompanying pain relief would be an added benefit.

  • Diabetes, a chronic hyperglycemic condition, is caused by insufficient insulin secretion or functional impairment. Long-term inadequate regulation of blood glucose levels or hyperglycemia can lead to various complications, such as retinopathy, nephropathy, and cardiovascular disease. Recent studies have explored the molecular mechanisms linking diabetes to bone loss and an increased susceptibility to fractures. This study reviews the characteristics and molecular mechanisms of diabetes-induced bone disease. Depending on the type of diabetes, changes in bone tissue vary. The molecular mechanisms responsible for bone loss in diabetes include the accumulation of advanced glycation end products (AGEs), upregulation of inflammatory cytokines, induction of oxidative stress, and deficiencies in insulin/IGF-1. In diabetes, alveolar bone loss results from complex interactions involving oral bacterial infections, host responses, and hyperglycemic stress in periodontal tissues. Therapeutic strategies for diabetes-induced bone loss may include blocking the AGEs signaling pathway, decreasing inflammatory cytokine activity, inhibiting reactive oxygen species generation and activity, and controlling glucose levels; however, further research is warranted.

  • The paper reviews the movement in the United States to supplement environmental regulations with volunteer programs that encourage institutions and industry to go beyond compliance to achieve greater reductions in their waste and emissions than might otherwise be accomplished through the exclusive reliance on increasingly rigorous "end of the pipe" regulations. These volunteer programs have as a common element the encouragement of "pollution prevention" as a preferable strategy. Pollution prevention is a term used in the US to describe strategies, technologies, policies, etc. that focus on eliminating waste and emissions at the source rather than just treating and controlling them. In some countries the term "Clean Technologies" is used rather than pollution prevention. In the paper the author reviews selected voluntary programs and reports on accomplishments to date for those programs.

  • The methods solving the 2nd order linear ordinary differential equations of the form y"+H(x)y'+G(x)y=P(x) using Fourier series are presented in this paper. These methods are applied to the differential equations of which the exact solutions are known, and the solutions by Fourier series are compared with the exact solutions. The main results obtained in these studies are summarized as follows; 1) The product and the quotient of two functions expressed in Fourier series can be expressed also in Fourier series and the relations between the Fourier coefficients of the series are obtained by multiplying term by term. 2) If the solution of the 2nd order lindar ordinary differential equation exists in a certain interval, the solution can be obtained using Fourier series and can be expressed in Fourier series. 3) The absolute errors of Fourier series solutions are generally less in the center of the interval than in the end of the interval. 4) The more terms are considered in Fourier series solutions, the less the absolute errors.rors.

  • This study investigated the propriety and the extension of the term of validity about the using watthour meters at present. The subject of study is 1Cent 2W 220V 30(10)A, 1Cent 3W 110V 30(10)A of model III watthour meters. They are the used metes(products in 1984) and the repaired meters(products in 1977) in the end of the period. The investigation is as follows. o Investigation of external appearance o Driving current test o Creeping test o Noise test o Error test o Durability test. Results of the study have been the number of 13.3% exceeded permitting error and the number of 31.3% failed in endurable test of 500 hours. So, the using meters are not suitable to extend the term of validity without the improvement of several characteristics.

  • There-have been many control strategies for the enact joint position tracking of flexible manipulators, but direct cartesian space tracking control methods an not developed well. In this paper, we propose a PD control method based on the cartesian error in the end point trajectory tracking. the proposed controller is composed of PD control combined with nonlinear saturation term hut has a very simple form. the effect of this term is continuous suppression of vibration which is induced by the coupling of rigid motion. This control works both on the regulation and on the tracking cases. The performance and validity of this control method is shown by simulation examples.

  • Operation of LNG (Liquefied Natural Gas) plants requires an effective maintenance strategy. To this end, the long-term and short-term trend of faults, such as mechanical and electrical troubles, should be identified so as to take proactive approach for ensuring the smooth and productive operation. However, it is not an easy task to predict the fault trend in LNG plants. Many variables and unexpected conditions make it quite difficult for the facility manager to be well prepared for future faulty conditions. This paper presents a model to predict the fault trend in a LNG plant. ARIMA (Auto-Regressive Integrated Moving Average) model is combined with Wavelet Transform to enhance the prediction capability of the proposed model. Test results show the potential of the proposed model for the preventive maintenance strategy.

  • Enhanced radio access technologies (RAT) are deployed in Next Generation Convergence Networks by the service providers so as to satisfy the basic requirements of end-users for e.g. QoS. Whenever the available resources are being shared simultaneously and dynamically by multiple users or distribution of allocated channels randomly, the deficiency of spectral resources and dynamic behavior of Network traffic in real time Networking, we may have problem. In order to evaluate the performance of our proposed algorithm, computer simulation has been performed on NS-2 simulator and a comparison with the existing algorithms has been made.

  • Mechanical circulatory support (MCS) in the pediatric heart failure population has a limited history especially for infants, and neonates. It has been increasingly recognized that there is a rapidly expanding population of children diagnosed and living with heart failure. This expanding population has resulted in increasing numbers of children with medically resistant end-stage heart failure. The traditional therapy for these children has been heart transplantation. However, children with heart failure unlike adults do not have symptoms until they present with end-stage heart failure and therefore, cannot safely wait for transplantation. Many of these children were bridged to heart transplantation utilizing extracorporeal membranous oxygenation as a bridge to transplant which has yielded poor results. As such, industry, clinicians, and the government have refocused interest in developing increasing numbers of MCS options for children living with heart failure as a bridge to transplantation and as a chronic therapy. In this review, we discuss MCS options for short and long-term support that are currently available for infants and children with end-stage heart failure.

Long-Term Results of Arteriovenous Fistula for Hemodialysis in Chronic Renal Failure (혈액투석을 위한 동정맥루의 장기관찰 성적)

  • From november, 1985 to May, 1993, 222 arteriovenous fistulae were made in 201 patients with chronic renal failure. Among them, a total of 183 arteriovenous fistulae in 173 patients were reviewed to evaluate the factors influencing patency rate of the vascular access. The results were revealed as follows: There were 102 men and 71 women,aged 10 to 76 years [mean = 45.7 years]. Sixteen patients of them had previous shunts. The procedures included establishment of 214 radiocephalic or brachioocephalic fistulae, 203 side to end, 9 side to side, 2 end to end, 2 autologous saphenous vein grafts, 6 Gore-Tex grafts. There were 28 early shunt failures[12%] due to use of 23 inadequate veins and 5 thrombosis.There were 32 late complications[14%]; 19 thrombosis, 4 aneurysm, 4 venous hypertension, 3 steal syndrome, 2 infections. There were 32 diabetic patients [17 %]. No significant differences in graft patency were noted between diabetic and nondiabetic individuals. There were no significant difference in graft patency between male and female. Overall shunt patency in 183 cases with chronic renal failure was 96% at I month, 95% at 3 months, 93% at I year, 91% at 2 years, 84% at 3 years, 56% at 5years.This Study showed that early postoperative thrombosis and diabetic vasculopathy were most causes of the vascular access failure and suggested that prevention of thrombi and well control of diabetes mellitus were most important to enhance patency rates of the vascular access.
