신고 시간대에 따른 출동거리와 현장도착 시간 간의 상관 관계 (Correlation between En route distance and Role time on call received hours)

  • Purpose: This study was to clarify correlation between en route distance and role time on call received hours. Methods: Data on en route distance(dependent variable), role time(independent variable) from 387 prehospital care reports documented by EMS in Kyonggi Provincial Fire and Disaster Headquarters and Seoul Metropolitan Fire and Disaster Department between 21 and 10 June 2010 were randomly chosen for simple regression analysis using Windows SPSS 12. OK. This analysis was conducted nine times on unit hour divided to eight call received and overall. Results: Statistically significant regression equations( Y=2.414+1.206X for 09:00~11:59, Y=3.753+.662X for 12:00~14:59, Y=2.215+1.458X for 15:00~17:59, Y=2.600+.822X for 21:00~23:59, Y=5. 445+.263X for overall) were derieved from the data. Conclusion: These equations having linear relationship may be utilized as a method for system status management to effectively response to emergency call.

의사 탑승 헬기를 이용한 산악 응급 환자 이송: 시범 연구 (Physician-staffed Helicopter Transport for Mountain-rescued Emergency Patients: a Pilot Trial)

  • Purpose: We aimed to compare the transport time, the proportion of direct hospital visit and the emergency procedures between the current mountain rescue helicopter emergency medical service (HEMS) and physician-staffed mountain-rescue HEMS. Methods: During weekends from October 2, to November 21, 2010, 9 emergency physicians participated as HEMS staff in the mountain-rescue HEMS program of the Seoul fire department. Patient demographic data, transport time, proportion of direct hospital visits, and emergency procedures were recorded. We also collected data on HEMS mountain-rescued patients from June 1, to September 1, 2010, and we compared them to those for the study patients. After an eight-week trial of the HEMS, we performed a delphi survey to determine the attitude of the physician staff, as well as the feasibility of using a physician staff. Results: Twenty-four(24) patients were rescued from mountains by physician-staffed HEMS during the study period, and 35 patients were rescued during the pre-study period. Patient demographic findings were not statistically different between the two groups, but the transport time and the emergency procedures were. During the study period, the time from call to take-off was $6.1{\pm}4.1min$ (vs. $12.1{\pm}8.9min$ during the pre-study period, p-value=0.001), and the time from call to arrival at the scene was $15.0{\pm}4.8min$ (vs. $22.3{\pm}8.1min$ during the pre-study period, p-value=0.0001). The proportions of direct hospital visit were not different between the two groups, but more aggressive emergency procedures were implemented in the study group. The delphi survey showed positive agreement on indications for HEMS, rapidity of transport and overall satisfaction. Conclusion: A pilot trial of physician-staffed HEMS for mountain rescue showed rapid response and more aggressive performance of emergency procedures with high satisfaction among the attending physicians.

자율차량 안전을 위한 긴급상황 알림 및 운전자 반응 확인 시스템 설계 (A Design of the Emergency-notification and Driver-response Confirmation System(EDCS) for an autonomous vehicle safety)

  • 현재 자율주행차량 시장은 3레벨 자율주행차량을 상용화하고 있으나, 여전히 운전자의 주의를 필요로 한다. 3레벨 자율주행 이후 4레벨 자율주행차량에서 가장 주목되는 부분은 차량의 안정성이다. 3레벨과 다르게 4레벨 이후의 자율주행차량은 운전자의 부주의까지 포함하여 자율주행을 실시해야 하기 때문이다. 따라서 본 논문에서는 운전자가 부주의한 상황에서 긴급상황을 알리고 운전자의 반응을 인식하는 자율차량 안전을 위한 긴급상황 알림 및 운전자 반응 확인 시스템을 제안한다. 긴급상황 알림 및 운전자 반응 확인 시스템은 긴급상황 전달 모듈을 사용하여 긴급상황을 텍스트화하여 운전자에게 음성으로 전달하며 운전자 반응 확인 모듈을 사용하여 긴급상황에 대한 운전자의 반응을 인식하고 운전 권한을 운전자에게 넘길지 결정한다. 실험 결과, 긴급상황 전달 모듈의 HMM은 RNN보다 25%, LSTM보다 42.86% 빠른 속도로 음성을 학습했다. 운전자 반응 확인 모듈의 Tacotron2는 deep voice보다 약 20ms, deep mind 보다 약 50ms 더 빨리 텍스트를 음성으로 변환했다. 따라서 긴급상황 알림 및 운전자 반응 확인 시스템은 효율적으로 신경망 모델을 학습시키고, 실시간으로 운전자의 반응을 확인할 수 있다.

건설사업장 산업재해 피해 저감을 위한 비상사태 대응방안 (Emergency Response Strategy for Mitigating Industrial Disasters on Construction Site)

  • Construction sites have various risks, and safety managements are enforced based on the law such as Industrial Safety and Health Act, etc. Safety management by the law saves lives and preserves health of workers by removing and understanding risks in advance. When the emergency situation has come, poor response causes loss of lives and physical damages. This study suggests basic strategy that can mitigate the industrial disaster in all construction sites. A survey was conducted to identify the current safety consciousness and to evaluate first aid ability of coworkers. Finally, four measures were suggested: (1) designating muster points; (2) hiring occupational health managers; (3) increasing the first aid ability of coworkers; and (4) carrying safety brochure. The proposed suggestions can be helpful in mitigating the construction disaster, and in responding the major accident efficiently within a short time.

Fresh Agricultural Products Online Retailer's Emergency Logistics Capability Framework During the Pandemic

  • Purpose: During the pandemic, a large number of orders forced online retailers of fresh produce to quickly change their emergency logistics capabilities to meet the needs of ordinary consumers during the pandemic. Our research aims to help online retailers establish an emergency physical distribution framework for fresh produce during disasters to meet consumer needs. Research design and methodology: 160 effective responses were collected from the online response team in Wuhan, China, and exploratory factor analysis was used to determine the emergency logistics capability framework. Twelve experts were invited online to collect their scoring opinions and use the entropy method to determine the importance of emergency logistics capabilities. Results: Our results have identified a total of 17 emergency logistics factors for online retailers, and we found that Transportation route planning and reconstruction capabilities, Emergency plan planning ability, and Supply chain real-time information sharing capability are the most important in the overall framework. Conclusions: This research completely established the physical distribution framework of fresh agricultural products online retailer in emergency situations. It enriches academic resources in the field of emergency distribution and provides a scientific basis for corporate managers to improve their physical distribution capabilities in emergency situations.

원자력발전소 비상직무에 대한 인적수행도 분석 (Human Performance Analysis of Emergency Tasks in Nuclear Power Plant)

  • Reduction and prevention of human error is one of the major interests for the enhancement of system safety and availability in Nuclear Power Plants (NPPs). As human beings have become the weak point in the system safety, a systematic evaluation on human performance during emergency situation should be performed in advance to identify the potential vulnerability of human tasks. Though the data gathering and analysis from real field is an important precondition, there were no available data in nuclear field of korea. This paper presents the result of human performance analysis on emergency tasks in NNPs. Firstly, a task analysis was performed to identify the characteristics of operator tasks during emergency condition and to classify them into a set of generic emergency tasks. Secondly, simulation data were collected and analyzed for the emergency tasks using the full scope simulator of Younggwang NPPs. The analyzed human performance information cover the event diagnosis time, the execution time of each procedural step, observation parameters, typer of irrelevant response, pattern of communication among staffs, and so on. These performance data would be used for human reliability analysis and the research of human error as technical bases.

119 구급대를 이용하여 야간에 응급실로 내원한 환자 현황 분석 - 전남대학교병원 응급의료센터를 중심으로 - (The present condition analysis of patients who transferred to the emergency room by 119 Rescue service at night - Focused on the Emergency Center of Chonnam National University Hospital -)

  • In general, the patients who transferred to the hospital by 119 rescue service at night go to the emergency room(ER) of general hospital rather than that of their nearby the 1st(clinic) and 2nd(local hospital) hospital. And the hospital is mainly selected not by 119 EMST but by patients or his/her conservators. Therefore we had studied retrospectively with emergency situation diary and medical chart for 697 patients of being transferred to one emergency medical center for 6 months since January 2004, and results are as follows. 1. The 280 patients(42.5%) of being transferred hospital at night by 119 rescue service were not emergency case and their average staying time in hospital was about $7.15{\pm}10.06$ minutes. 2. Transfer time was distributed in each time intervals of 1819, 2021, 2223, and 2401 and patients ratio in each intervals were 15.1%, 17.8%, 16.4%, and 15.2%. 3. In response of ambulance, the average time from the spot to the hospital was $14.53{\pm}9.27min$. and average distance of that was $7.95{\pm}9.21km$. 4. Diseases rather than traffic accidents or traumatic injury were main causes of ambulance calling and its value was 533(76.5%), and accidents were mainly occurred in patients' house and its value was 479(68.7%). 5. In time of transfer by 119 rescue service, hospital was mainly selected by patient/conservator and its value was 648(93.0%). In result. the hospital was selected not by EMST but by patient/conservator. 6. The case that the 1st grade EMT was rode in ambulance was 161(23.1%), and the case that 2nd EMT and emergency team member who educated for emergency were rode in ambulance were 504(72.3%). So the number of the 1st grade EMT was short in fire station of Kwangju metropolitan city than other city. 7. The first aids for patients before reaching hospital were limited to oxygen inhalation, airway control, and BLS for maintaining limbs and spine. So it seems to be a simple patients transfer. Consequently, to establish an efficient emergency medical system, it has been thought that it should be advanced a moderate education and public information about the appropriate use of emergency medical system toward citizen, and also need the hospital selection by the patients categorizing standards for 119 rescue service member, securing the 1st grade EMT, appropriate first-aids education, and securing professional human power in emergency room of the Ist(clinic) and 2nd(local hospital) hospital at night.

응급실 종사자의 폭력 경험에 따른 폭력 반응 소진 및 직무 만족 (Violence Response, Burnout and Job Satisfaction according to Violence Episode of Emergency Room Worker)

  • 본 연구는 대학병원 응급실에 근무하는 응급실 종사자를 대상으로 폭력 경험에 따른 폭력 반응, 소진 및 직무 만족도를 조사하였다. IRB 심의 면제 후 설문지를 이용하여 대학병원 응급실 종사자 167명을 대상으로 2015년 7월부터 8월까지 자기기입식으로 수집하였다. 수집된 자료는 SPSS 21.0 프로그램을 이용하여 빈도와 백분율, 평균과 표준편차, t-test, Pearson's correlation coefficiency, Multiple regression로 분석하였다. 이에 따른 연구 결과는 다음과 같다. 대상자의 응급실 폭력 경험에서 대상자가 폭력에 노출 경험이 있는 경우가 84.4%로 다수를 차지하며 언어폭력이 가장 높게 나타났다. 폭력 가해자는 환자와 보호자에 의한 발생이 많았으며, 보호자의 경우 음주상태인 경우가 많았고 폭력 발생은 야간 근무 때 높게 나타났고, 폭력 경험은 의료진이 가장 많았다. 폭력 경험에 따른 정서적 반응이 유의하게 높게 (t=2.528, p=0.12)로 나타났다. 응급실 폭력 경험 대상자의 폭력 반응과 소진은 유의한 양의 상관관계로 나타났다(r=.616, p=<.001). 응급실 폭력 경험 대상자의 폭력 반응과 직무 만족은 유의한 음의 상관관계를 나타내었으며 (r=-.512, p=<.001) 응급실 폭력 경험 대상자의 소진과 직무 만족은 유의한 음의 상관관계로 나타났다(r=-.568, p=<.001). 직무 만족에 가장 큰 영향 요인은 폭력에 대한 정서적 반응과 소진으로 파악되었다. 이 번 연구는 폭력 예방을 위한 프로그램과 함께 폭력에의 노출 후 정서적 지지 및 스트레스나 소진 관리를 위한 전략이 필요함을 알 수 있었다.

긴급차량 우선교통신호시스템 사례에 관한 연구: 충청북도 내 일개도시 시범운영 사례를 중심으로 (Study on Cases of Priority Traffic Signal System for Emergency Vehicles: Based on a City's Pilot Operation Cases in Chungcheongbukdo Province)

  • 이 연구는 화재진압 및 구조·구급현장에서 응급환자 대응을 위한 골든타임 확보를 위한 해결방안을 제시하는데 목적이 있다. 골든타임은 법제적, 사회적 공감 뿐 아니라 시스템 구축을 위한 정책적 방안이 필요하며, 선행연구들을 통해 긴급차량 우선교통신호시스템 도입의 필요성과 적용가능성은 끊임없이 언급되어왔다. 이에 본 연구는 2017년 4월 3일부터 2017년 4월 28일까지 26일간 일개 도시에서 시행된 긴급차량 우선교통신호시스템 시범 운영사례를 분석하였다. 119신고 접수는 총 58건으로 그 중 우선교통신호시스템이 반영된 화재출동이 16건, 구급출동이 11건이었다. 출동시간은 화재출동이 평균 3 min 50 s, 구급출동은 평균 3 min 50 s로 단축되었다. 차량정체에 따른 불편신고 접수는 총 4건이었으며, 전년도 동기간 대비 6건의 교통사고가 총 1건으로 줄었다. 본 연구결과 시범 운영기간동안 도출된 문제점과 개선사항을 보완하면 골든타임 확보를 위한 긴급차량 우선교통신호시스템이 효과적으로 운영될 것으로 판단된다.

스마트 시티에서의 이머전시 사운드 감지방법 (A Emergency Sound Detecting Method for Smarter City)

  • Because the noise is the main cause for decreasing the performance at speech recognition, the place or environment is very important in speech recognition. To improve the speech recognition performance in the real situations where various extraneous noises are abundant, a novel combination of FIR and Wiener filters is proposed and experimented. The combination resulted in improved accuracy and reduced processing time, enabling fast analysis and response in emergency situations. Usually, there are many dangerous situations in our city life, so for the smarter city it is necessary to detect many types of sound in various environment. Therefore this paper is about how to detect many types of sound in real city, especially on CCTV. This paper is for implementing the smarter city by detecting many types of sounds and filtering one of the emergency sound in this sound stream. And then it can be possible to handle with the emergency or dangerous situation.
