Flexible Intelligent Exit Sign Management of Cloud-Connected Buildings

  • Emergencies and disasters can happen any time without any warning, and things can change and escalate very quickly, and often it is swift and decisive actions that make all the difference. It is a responsibility of the building facility management to ensure that a proven evacuation plan in place to cover various worst scenario to handled automatically inside the facility. To mapping out optimal safe escape routes is a straightforward undertaking, but does not necessarily guarantee residents the highest level of protection. The emergency evacuation navigation approach is a state-of-the-art that designed to evacuate human livings during an emergencies based on real-time decisions using live sensory data with pre-defined optimum path finding algorithm. The poor decision on causalities and guidance may apparently end the evacuation process and cannot then be remedied. This paper propose a cloud connected emergency evacuation system model to react dynamically to changes in the environment in emergency for safest emergency evacuation using IoT based emergency exit sign system. In the previous researches shows that the performance of optimal routing algorithms for evacuation purposes are more sensitive to the initial distribution of evacuees, the occupancy levels, and the type and level of emergency situations. The heuristic-based evacuees routing algorithms have a problem with the choice of certain parameters which causes evacuation process in real-time. Therefore, this paper proposes an evacuee routing algorithm that optimizes evacuation by making using high computational power of cloud servers. The proposed algorithm is evaluated via a cloud-based simulator with different "simulated casualties" are then re-routed using a Dijkstra's algorithm to obtain new safe emergency evacuation paths against guiding evacuees with a predetermined routing algorithm for them to emergency exits. The performance of proposed approach can be iterated as long as corrective action is still possible and give safe evacuation paths and dynamically configure the emergency exit signs to react for real-time instantaneous safe evacuation guidance.

Emergency response team activation in the outpatient clinic of a single dental teaching hospital in Korea: a retrospective study of 10 years' records

  • Background: To prepare for possible emergency situations during dental treatment, it is helpful to know how often and what kinds of emergencies may arise. This study set out to evaluate the incidences, causes, treatments, and outcomes of emergency situations in the outpatient clinic of a dental teaching hospital in Korea. Methods: We retrospectively reviewed the records of patients who had experienced an emergency situation and emergency response team activated in a selected outpatient clinic between November 2004 and November 2013. Specific information about the emergency cases was collected, including the patient characteristics and the frequency, types, treatments, and outcomes of the emergency situations. Results: We identified 35 instances of emergency situations in 2,890,424 patients (incidence = 0.012 per 10,000 outpatients). The number of cases was as follows: 10 (28.6%) in the Department of Periodontics, 10 (28.6%) in the Department of Oral and Maxillofacial Surgery, 6 (17.1%) in the Department of Oral and Maxillofacial Radiology, 4 (11.4%) in the Department of Prosthodontics, 2 (5.7%) in the Department of Conservative Dentistry, 2 (5.7%) in the Department of Pediatric Dentistry, and 1 (2.9%) in the Department of Orthodontics. Three (8.6%) of the emergency situations arose before treatment, 22 (62.9%) during treatment, 7 (20.0%) after treatment, and 2 (5.7%) in a patient's guardian. Conclusions: In accordance with the growing elderly population and more aggressive dental procedures, the number of emergency situations may increase in the future. We recommend that clinicians keep in mind airway management and the active control of emergency situations.

국내 민간항공사 조종사들의 비기술적 훈련 도입을 위한 사례연구: Startle 및 Surprise 비상상황 훈련 사례를 중심으로 (The Case Study of Startle and Surprise Emergency Flight Training for Introduction of Non-Technical Flight Training to Commercial Airline Pilots in Korea)

  • 자동화된 운항 시스템 도입으로 항공안전은 크게 개선되었지만, 운항을 직접 담당하는 항공기 조종사에게는 새로운 도전이 생겼다. 항공운항 관점에서의 조종사의 스트레스를 Startle과 Surprise구분할 수 있다. Startle을 총소리 같이 갑작스럽거나 위협적인 자극에 대한 짧고 강한 생리적인 반응이며, Surprise는 자신의 예상을 빗나가는 예기치 못한 사건에 대한 인지-감정적 반응이다. Martin 등이 실시한 Startle 효과 실험(2012년)에서 조종사는 Startle 상태에서 반응 지연과 심박 수 증가 등 생리적인 반응을 확인하였고, Rahim의 Startle/Surprise 실험(2020년)에서 사전 계획된 비상조치훈련은 조종사의 호흡수와 맥박수의 변화가 없으나, 비계획된 상황과 항공기가 복잡할수록 호흡 수와 심박 수가 증가되었다는 사실을 확인하였다. 이 실험 결과를 바탕으로 국내 조종사 훈련에서도 기술적 능력을 전달에만 집중하는 것이 아닌 비기술적 능력에 대한 고려가 필요하며, 법령에 의한 기계획된 단순 반복적인 훈련 항목보다는 항공사의 상황을 고려한 비정형화된 다양한 비상조치 훈련의 도입이 필요하다고 하겠다.

Effectiveness and patient satisfaction of dental emergencies in Pitié Salpêtrière Hospital, Paris, during the COVID 19 pandemic

  • Background: A previous study reported the effectiveness and patient satisfaction in the dental emergency unit (DEU) of the Pitie Salpetrière Hospital in Paris before coronavirus disease 2019 (COVID-19). The same methodology was used during the COVID-19 pandemic to compare pain, anxiety, and patient satisfaction during the two periods. Methods: This prospective study was conducted in 2020 (NCT04354272) on adult patients. Data were collected on day zero (D0) on site and then by phone during the daytime on day one (D1), day three (D3), and day seven (D7). The primary objective was to assess the pain intensity at D1. Secondary objectives were to assess pain intensity at D3 and D7, anxiety intensity at D1, D3, and D7, and patient satisfaction. Patients were evaluated on a 0-10 numeric scale on D1, D3, and D7; mean scores were compared with non-parametric statistics (ANOVA, Dunn's). Results: A total of 445 patients were given the opportunity to participate in the study, and 370 patients consented. Seventy-one were lost during follow-up. Ultimately, 299 patients completed all the questionnaires and were included in the analysis. In the final sample (60% men, 40% women, aged 39 ± 14 years), 94% had health insurance. The mean pain scores were: D0, 6.1 ± 0.14; D1, 3.29 ± 0.16; D3, 2.08 ± 0.16; and D7, 1.07 ± 0.35. This indicates a significant decrease of 46%, 67%, and 82% at D1, D3, and D7, respectively, when compared to D0 (P < 0.0001). The mean anxiety scores were D0, 4.7 ± 0.19; D1, 2.6 ± 0.16; D3, 1.9 ± 0.61; and D7, 1.4 ± 0.15. This decrease was significant between D0 and D7 (ANOVA, P < 0.001). Perception of general health improved between D1 and D7. The overall satisfaction was 9.3 ± 0.06. Conclusion: DEU enabled a significant reduction in pain and anxiety with high overall satisfaction during COVID-19, which was very similar to levels observed pre-COVID-19 pandemic.

초등학생을 위한 응급상황에 대한 대처교육 교사용 지침서 개발 (Development of Guidebook for Teachers about Coping Behavior Education to Emergency Situations for Elementary School Students)

  • Purpose: The purpose of this study was to develop a guidebook for teachers about coping behavior education in emergency situations for elementary school students. Method: Dick and Carey's teaching model (1996) and Keller's ARCS theory (1985) was applied to develop this guidebook. Results: The contents consist of subjects, reference materials for teachers, a case study, and worksheets for students. The contents of the 5 units are as follows: 1)Identifying a dangerous situation around the school area and in school, 2)Coping tips in dangerous situations that can occur at school, 3)Prevention and coping method of sexual violence, 4)Coping method for natural disasters, and 5)First aid in emergency situations. The presented case study was a real case scenario and the worksheets contained various activities initiated by students. Conclusions: This guidebook can be an effective tool in coping education classes for emergency situations at elementary schools. The goal of the guideline book is reach as many elementary schools as possible.

심폐소생술 시 가족 입회에 대한 응급의료센터 의료인의 태도와 인식 (Perception and Attitude of Emergency Medical Staff to Family Presence During Cardiopulmonary Resuscitation)

  • Purpose: This study was performed to determine the perception and attitude of emergency medical staff by the presence of family members while performing Cardiopulmonary Resuscitation (CPR). Methods: Data were collected from June and August in 2010 using questionnaire. The participants were 187 doctors and nurses who were working at emergency medical centers located at eight hospitals. Results: Approximately half of the medical emergency staff had previous experience of having requests from a patient's family members to remain present at the time of performing CPR. Most of the subjects did not know that the 2005 American Heart Association (AHA) guideline recommended including willing family members' during CPR. Doctors were more likely than nurses to recognize that the positive effects of family members being present during CPR. Conclusion: This study indicates that health professionals are not aware of the AHA guidelines for including family members during CPR of a patient and further that more doctors than nurses recognized the value of including willing family members during CPR.

응급실 간호사가 경험한 폭력과 폭력반응에 관한 조사연구 (Violence Episodes and Responses of Emergency Room Nurses)

  • Purpose: The purpose of study was to identify violence episodes by patients and/or their guardians and responses from emergency room nurses. Method: A convenient sample of 172 emergency nurses was selected from general hospitals with more than 200 beds. A survey design was used and data were collected from October 2 to October 11, 2006. using self-report questionnaires which were developed by the authors. Descriptive statistics, with the SPSS PC program were used to analyze the data. Results: The majority of nurses(97.1%) had experienced violence by patients or their guardians in the previous month. Violence offenders who were drunk comprised 68.6% of total offenders. Causes of violence were delays in examination and treatment(59.9%). The major coping behavior in dealing with violence episodes was ignoring(40.1%). Responses to violence experienced by emergency nurses were rated as 2.39. Response to violence according to general characteristics of the nurses showed no statistically significant differences. Conclusion: Considering the high prevalence of violence experienced by emergency nurses, hospitals must develop and manage preventive education programs and policy.

의식하 진정에 사용된 약물에 관한 임상적 연구 (CONSCIOUS SEDATIVE DRUGS: A CLINICAL STUDY)

  • The purpose of this retrospective study is to investigate the effects of conscious sedation in an outpatient setting. Data from 292 patients (154 males, 138 females), sedated for minor oral surgery, were studied using various parameters including age and sex distribution, as well as according to procedures and each drug group. Results show that this intravenous conscious sedation technique is a safe and effective method of controlling behaviour, but one has to be prepared to deal with any emergencies or complications. The specific results are as follows: 1. Most procedures (265/292) were used to Valium mixed with Demerol and Midazolam mixed with Demerol. 2. Valium revealed a high incidence of pain upon injection and symptoms limited to daily activity and thrombophlebitis. 3. Nausea and vomiting manifested in the procedure using Demerol. 4. The patient manifested comfort similar in response to the same drugs used in another treatment.

  • 급속한 도시화와 더불어 도시화의 기반이 되는 막대한 사회기반시설물이 필요하게 되었다. 현재 시설물 관리기관별로 관리되고 있는 전화, 전기, 가스, 상하수도 등과 같은 사회기반시설물이 전체적인 입장에서 조정될 수 있다면 응급상황발생시 효율적으로 대처할 수 있을 뿐만 아니라 이들 시설물의 유지관리시 발생되는 공사비 중복 및 주민의 불편을 상당부분 해소할 수 있게 될 것이다. 따라서 본 논문에서는 우리나라의 사회기반시설물을 관리하고 있는 기관의 시설물 관리업무를 조사하여 GIS구축시 기초자료가 되는 기관별 업무수행시 조사항목을 체계적으로 정리하였으며, 기관별로 활용하고 있는 도면 및 장부를 함께 조사분석하여 시설물 관리에 필요한 수치코드 표준화방안, 기본도에 포함되어야할 사항, 기본도의 축척, 기본도 관리기관 및 기본도 운영방법 등을 제시하였다.

Blind nasal intubation as an alternative to difficult intubation approaches

  • Airway difficulties are a major concern for anesthesiologists. Even though fiberoptic intubation is the generally accepted method for management of difficult airways, it is not without disadvantages-requires patient cooperation, and cannot be performed on soiled airway or upper airways with pre-existing narrowing pathology. Additionally, fiberoptic bronchoscopy is not available at every medical institution. In this case, we encountered difficult airway management in a 71-year-old man with a high Mallampati grade and a thick neck who had undergone urologic surgery. Several attempts, including a bronchoscope-guided intubation, were unsuccessful. Finally, blind nasal intubation was successful while the patient's neck was flexed and the tracheal cartilage was gently pressed down. We suggest that blind nasal intubation is a helpful alternative in difficult airway management and it can be a lifesaving technique in emergencies. Additionally, its simplicity makes it a less expensive option when advanced airway technology (fiberoptic bronchoscopy) is unavailable.
