방광 자극증상을 호소하는 복압성 요실금 환자에서 마이크로칩을 이용한 전기자극치료의 효과 (The Effects of Electrical Stimulation Therapy with Microchip for the Treatment of Bladder Irritability Symptoms in Stress Urinary Incontinent Women)

  • 복압성 요실금에 동반되는 다양한 방광 자극증상은 빈뇨, 잔뇨감, 야간뇨, 절박뇨, 절박성 요실금, 배뇨통의 순이었으며, 이 증상들의 복합 정도가 많으면 삶의 질에 영향을 주고 있음을 알 수 있었다. 그리고 마이크로칩을 이용한 전기자극기는 특별한 부작용 없이 환자의 만족도가 높은 치료기로 생각되었으며, 복압성 요실금과 동반되는 야간뇨, 배뇨통 등의 방광 자극증상을 경감시킴으로 인해 현재의 배뇨 상태가 일상생활에 미치는 영향을 감소시키는 것을 관찰할 수 있었다.

체내 통신을 이용한 신경 보철용 원격 통신 시스템 (A Telemetry System using Intra-body Communication for Neural Prosthesis)

  • 체내 통신은 인체를 통신 매체로 하여 신호를 전송하는 무선 통신 방식이다. 체내 통신 방식은 체내와 송수신 시스템 그리고 외부 접지를 통한 하나의 전류 패스를 형성함으로써 이루어지는데, 인공 와우와 같은 신경 보철 장치의 경우 피하에 이식되어 있기 때문에 외부 접지를 사용하기 어렵다. 따라서 본 논문에서는 이와 같은 접지의 영향을 받지 않는 체내 통신을 제안하여 신경 보철 장치를 위한 시스템을 개발하였다. 개발된 시스템은 이식된 보철 장치의 체내에 위치한 전극으로의 신호 전송이 가능하도록 설계되었다. 효과적인 통신을 위하여 실험동물의 피부 위 실험 및 피하 실험을 통해 신호 전송 특성을 조사하였으며, 피부 위 실험의 경우 약 10MHz, 피하 실험의 경우 약 3MHz 이상의 주파수 대역에서 최대 전송 이득을 가지는 것을 확인하였다. 본 시스템은 데이터 전송률 480kbps를 갖는 pulse width modulation (PWM) 방식을 사용한 인공 와우용 내부 전류 자극기에 적용하여 그 성능을 입증하였다.

뇌졸중 후 연하곤란 환자에서 기능적 전기자극 치료의 중복 적용 효과 (The Effect of Double Application of Functional Electrical Stimulation in Patients with Dysphgia after Stroke)

  • Objectives : The objective of this study was to investigate the outcomes of functional electrical stimulation (FES) which was applied twice a day in patients with dysphagia after stroke. Methods : Eleven patients with dysphagia after stroke were participated. The electrical stimulator with two channels was employed for forty minutes daily or forty minutes twice a day for fifteen days. Participants were divided into two groups by random method; The FES was performed twice a day for Twice-FES group (n=6), and once a day for Standard-FES group (n=5). For evaluation of dysphagia, the functional dysphagia scale by videofluoroscopic swallowing study, and swallow function scoring system by six clinical swallowing stage were assessed at pre- and post-treatment. Results : In both groups, there was a significant decrease of total functional dysphagia scales after FES treatment (p<0.05) and the results mainly affected the pharyngeal phase of deglutition. There was no significant difference between the two groups in total functional dysphagia scales, but the Twice-FES group had a decreased residue in oral cavity compared to the standard FES group. In both groups, there were significant improvements in swallow function scoring system (p<0.05). The twice-FES group had more high clinical swallowing stages. Conclusions : The results demonstrated that FES is a clinically effective intervention in treatment of stroke patients with dysphagia. Moreover, the treatment applied twice a day had relatively positive effects on the reduction of oral cavity residue and the improvement of clinical swallowing stage.

경근추나요법과 경피전기자극치료가 급성 요통 환자의 통증 및 척추기립근 경근전도에 미치는 영향 (The Impacts of Muscle Energy Technique (MET) and Transcutaneous Electrical Nerve Stimulation (TENS) Performed on Elector Spinae Muscle of Acute Low Back Pain Patients on Pain and Meridian Electromyography)

  • Objectives To evaluate the clinical usefulness between muscle energy techniques (MET) and transcutaneous electrical nerve stimulator (TENS), we performed both on elector spinae muscle of acute low back pain patients. Methods After performing MET and TENS, we compared both in terms of electrical activity. We performed MET or TENS on elector spinae muscle of acute low back pain patients in each group (n=15,15). After performing MET or TENS, we analyzed root mean square (RMS), median edge frequency (MEF) and asymmetry index (AI). Results 1. After performing MET on elector spinae muscle of acute low back pain patients, RMS was significantly decreased compared with before (p<0.005). 2. After performing TENS on elector spinae muscle of acute low back pain patients, RMS was significantly decreased compared with before (p<0.005). 3. After performing MET on elector spinae muscle of acute low back pain patients, asymmetric index was significantly decreased compared with before (p<0.05). Conclusions According to above results, performing MET on elector spinae muscle of acute low back pain patients has effect in terms of RMS and asymmetric index. And performing TENS on elector spinae muscle of acute low back pain patients also has similar effect in terms of RMS but has not in terms of asymmetric index.

저빈도 전기자극이 개의 치아 및 치은에 대한 동통억제효과 (THE INHIBITORY EFFECT OF LOW FREQUENCY ELECTRICAL STIMULATION ON THE DENTAL AND GINGIVAL PAIN OF DOG)

  • The purpose of this study was to investigate the effect of electrical anesthesia induced by non-acupuncture point stimulation on inhibition of amplitude of digastric EMG evoked by noxious electrical stimuli in teeth and gingiva. Experiments were performed with dogs anesthetized with intraperitoneal pentobarbital sodium in an initial dose of 30mg/kg. Maintenance doses of 4.0ml/hour were given through a cannula in the femoral vein using a constant infusion pump. Anterior belly of digastric muscle was exposed and a pair of 0.1mm wire electrodes were inserted for E.M.G. recording. Bipolar electrodes were inserted into the labial and lingual surface of upper canine and the labial area of upper gingiva. Noxious stimuli were delivered to the tooth and gingiva through those electrodes by electric stimulator. Non-acupuncture point stimulation of 2Hz was delivered bilaterally to the femoral area. Amplitudes of digastric E.M.G. were measured from the oscilloscope and the monitor connected to amplifier at different intensities of electronic anesthesia of 1 volt, 4 volt and 10 volt. The inhibited rate of the amplitudes of digastric E.M.G. were analysed statistically with paired t-test. The following results were obtained : 1. Non-acupuncture point stimulation with intensities of 1 volt, 4 volt and 10 volt showed the inhibitory effect on pain of 15%, 25% and 16% in teeth and 15%, 18% and 12% in gingiva respectively 2. In tooth, statistical significance was observed between control and each group. In gingiva, there was statistical significance between control and group 1, 2 except group 3 From these results, low frequency electrical stimulation of non-acupuncture point resulted in reducing of dental and gingival pain, it could be used as adjunct to other pain control methods.

국내 연하장애 기능적 전기 자극치료기 사용 현황과 장비관리 실태 분석 (Bio-signal Based Rehabilitation Program and Data Management)

  • 본 연구는 현재 국내 연하장애 환자를 대상으로 하는 연하장애 기능적 전기 자극 치료 장비의 사전 사후 점검과 유지 보수와 관련한 설문조사를 실시하고 장비사용 현황과 관리 실태를 분석함으로서 체계적인 치료 장비 관리 가이드라인 개발의 필요성을 제시하는데 그 목적이 있다. 연구대상은 상급종합 병원, 종합병원, 재활병원, 요양병원을 포함하는 총62개 기관 중 최종적으로 설문에 응답한 50개 기관이었다. 연하장애 기능적 전기 자극 치료 장비의 사전 사후 점검과 유지 보수 현황과 실태를 파악하기 위한 설문지를 개발한 후, 이를 기초로 2013년 8월 21일부터 2013년 11월 10일간의 연구 자료를 분석에 사용하였다. 연구 결과, 장비는 활발히 사용하고 있었으나 장비의 사용 전 작동 검사, 사용 후 작동 검사, 유지 및 보수, 소독 보관 등의 항목에서 체계적인 관리를 하지 않고 있었다. 혼수상태이거나 인지가 현저히 저하된 환자에게 적용하는 방법과 관련한 항목에서도 표준화된 방법이 없거나 가이드 라인이 제시되었더라 하더라고 실제 사용하지 않고 있는 것으로 밝혀졌다. 이는 장비의 사전 사후 점검과 유지 보수에 대한 표준화된 가이드라인 및 질 관리 지표 개발에 대한 연구의 필요성을 제시한다.

  • This paper describes and discusses the employment of EMG pattern analysis to provide upper-motor-neuron paraplegics with patient-responsive control of FES (functional electrical stimulation) for the purpose of walker-supported walking. The use of above - lesion EMG signals as a solution to the control problem is considered. The AR (autoregressive) parameters are identified by Kalman filter algorithm using DSP chip and classified by fuzzy theory. The control and stimuli part of the below-lesion are based on microprocessor(8031). The designed stimulator is a 4-channel version. The experiments described above have only attempted to discriminate between standing function and sit-down function. A further advantage of the this system is applied for motor rehabilitation of social readaption of paralyzed humans.

  • In this study, we developed transdermal direct drug delivery device using micro-needle painlessly. We has fabricated micro-needle that is 130 ${\mu}m$ thickness and 250 ${\mu}m$length with 10 ${\mu}m$ spiral groove for rolling down drug. Head part of micro-needle device is composed of 20ea micro-needles, an on-off valve and a protective cap. Glass bottle for containing drug is connected to head part of micro-needle device. We examined the puncture characteristic testing using porcine skin and drug delivery testing using porcine, rat skin with Indian Ink.

EGS가 악관절동통 환자에 미치는 영향 (Effect of EGS on the Patients with TMJ Pain)

  • Author could get the result as below after EGS(an electrical nerve stimulator) treatment, one of the pain-reliaving methods, to 30 patients in whom author could confirm the objective improvement of symptoms among the patients who visited SNUH between July the first and December the 31th. 1984. because of TMJ pain. 1. The average number of treatment were 5.5 times/pt., and mean treatment duration was 20 minutes. 2. The severities of pain before the treatment were distressing (46.7%), discomforting (26.7%), mild (20.0%) and horrible (6.7%), but the percentages of them became changed to mild (50%), discomforting (33.3%), none (10%) and distressing (6.7%) after treatment. 3. The activities of mouth opening before the treatment were moderately restricted (53.3 %), serverely restricted (33.3%) and slightly restricted (13.3%), but the percentages of them became changed to slightly restricted (66.7%), normal (16.7%), modorately restricted (13.3%) and severely restricted (3.3%) after treatment. 4. The effects of the pain upon one's personality before treatment were severely upset (43.3%), moderately upset (36.7%), slightly upset(16.7%) and normal (3.3%), but the percentages of them became changed to slightly upset (50%), normal (33.3%), morderately upset (10%) and severely upset (6.7%) after treatment.

Effect electric pulse application on the fruit body production of Tricholoma matsutake-In situ condition

  • Effect of electric pulse stimulation was tested on the fruit body formation of valuable mushroom Tricholoma matsutake in the field of natural habitat of this mushroom. After applying the electric stimulator to the specific area of pine forest we found that the treatment especially stimulated the fructification of T. matsutake. And the most valuable findings of our study was that only our treatment plots showed fruit body formation whereas the control plots and the whole natural habitats of our study area showed zero production of this mushroom during the same time. From the point of view of mushroom production, our experiment shows that fruit body production can be upgraded by using pulsed power as an electrical stimulation in the field of the natural habitat of this mushroom. These findings from our experiment confirm the effectiveness of the significance of pulsed power technology for the improvement of T. matsutake fruit body production in the natural habit of this mushroom.
