Research of policy direction of aging society's elderly residential centralized towards medical service (고령화 사회에 대응하는 노인의료주거단지의 유니버설디자인 계획특성 연구)

  • Purpose : An aging phenomenon was recognized as only negative aspect, which impediment a development with the decrease of a developing population and increase of social burden. Nevertheless, the problem of aging cannot be treated as a problem of individuals or social burden. Because Every mankind sometime will be the person directly involved the aging. Methods : Korea, predict to reach the aging society with over 20 percent of elderly in the whole population in 2020, should find an active political plan for responding. Because a previous discussion about the aging phenomenon focused on social welfare, the housing plan for the elderly with a consideration of the physical environment for sustaining elderly's life with happiness will be primary task. Results : This research considers housing policy, focusing on the medical service. It is because statistics, which 85 percent of elderly people suffer from chronic diseases because of physical aging signify the importance of medical service to the elderly in every-day-life. Implications : We prescribe the elderly as a population over 65. In this duration, the elderly retirees from the workplace e and spend the majority of time in their home. Thus, for the elderly, the residential space is a field of action and passageway connecting with the surroundings For the solution of physical space preparing the aging phenomenon, the combination of residential and medical function creates a new type of a lifestyle.

  • This study is aiming to suggest baseline date for the establishment of policy alternative to make healthy consumption life of the elderly through investigating and analyzing actual condition of consumption related with the awareness of health functional food such as purchase behavior and consumer's problems about health functional food. Under the assumption that the vitalization of health functional food market will become an important market in the present and in the future, the fundamental marketing information about elder consumers is more important than any other information that is essential for successful marketing to domestic corporations and senior policy experts. In addition, there was a fundamental significance to provide necessary basic data for health promotion of the elderly by offering information about rights and interests of elder consumers who are members of vulnerable social group or right choice of purchasing or intake. The limitations of this study are as follows. First, the subjects were selected who live in Gangwon-do with the age of 60 and over due to the limitation of sampling, and that might be shown local characteristics. Therefore, the study result could not be generalized on behalf of all elderly in Korea and it is difficult to apply the result to more segmented market. To solve this problem, studies containing sampling by regional groups might be needed.

The Effects of Medical Expenditure on Income Inequality in Elderly and Non-Elderly Households by Income Class (소득계층별 노인과 비 노인가구의 의료비 지출이 소득불평등에 미치는 영향)

  • This study aims to identify the inequalities and characteristics of health care expenditure of the elderly and non-elderly households by income level. As a result, health care expenditure of elderly households was statistically significantly higher than that of non-elderly households. As a result of calculating the concentration index of health care expenditure by income level, inequality was higher in order of non-elderly households, elderly households, and total households. In order to confirm the effect of health expenditure on household income inequality, we calculated the concentration index of income excluding total health care expenditure from total income. As a result, inequality was higher in order of elderly households, whole households, and non-elderly households. There was not much difference in inequality of health care expenditure among elderly households and non-elderly households. And, the health care expenditure of elderly households was much higher than that of non-elderly households. Also, inequality of health care expenditure by income group was serious. There should be no cases where the medical care support policy for elderly households can not use necessary medical services.

Relationship between Physical Function Factors and Discrimination Experiences of the Elderly in Korea: The 2014 and 2017 National Survey on Living Conditions and Welfare Needs of Korean Older Persons (우리나라 노인의 신체기능 요인과 차별경험의 관련성 연구: 노인실태조사(2014, 2017) 자료 분석)

  • Background: Supporting the elderly population is presented as a social issue, and it affects age discrimination, which forms a negative perception and avoids the elderly. Since age discrimination lowers the quality of life of the elderly and hinders social unity, it may be important to research related factors. This study examined the physical function as a factor that influences discrimination experiences and aimed to identify the relationship between physical function factors and discrimination experiences. Methods: In this study, we analyzed 20,225 elderly from the 2014 and 2017 survey of living conditions and welfare needs of Korean older persons, conducted nationwide among older than 65 years. Physical function factors are activities of daily living, instrumental activities of daily living, sight discomfort, hearing discomfort, chewing discomfort, and leg strength as factors of interest. We performed multivariable logistic regression that reflected survey characteristics, adjusting for socio-demographic factors, health related factors, and social-support factors. Results: The number of elderly who faced discrimination experiences was 1,175 (5.8%). The results showed that the odds ratio of facing a discrimination experience in the dependent group was significantly higher compared to the independent group when being transferring out of the room (4.05; 95% confidence interval [CI], 1.50-10.88) and difficulty in hearing (1.25; 95% CI, 1.05-1.49). Even with respect to chewing ability, which was significant in models 1 and 2, they face more discrimination experiences (1.30; 95% CI, 1.11-1.53). Conclusion: These research results indicate that an important strategy for preventing age discrimination is to understand the physical function conditions of the elderly and promote the physical functions of the elderly related to transferring, hearing, and chewing; it is necessary to develop a practical intervention plan that considers these aspects.

The Effects of the Expansion of Formal Home Care Services for the Elderly on Time Allocation of Family in Korea (노인 재가서비스의 확대가 가족의 생활시간에 미친 영향: 삼중차이 방법의 적용)

  • This study examines whether formal home care has led to a reduction in family member's allocation of time to care of elderly and changing the allocation of other time: informal care, market work, leisure, etc. Long-term care for the elderly is not the entire burden of elderly families anymore due to the increasing of the formal home care service for elderly since mid-2000's. This study uses triple difference(DDD) methods for evaluating policy reforms when reforms are equally and nation-wide accessible and analyzes data from the 2004 and 2009 Time Use Survey Data of the National Statistical Office in Korea. The treatment group is composed of the families of the elderly who need care and low- and middle- income family. Controlling for demographic characters, the three-way interaction term has statically significant and negative impact on care time of family members in treatment group statistically significant. The results reveal that the policy changes in elderly home care service affect time allocation of family members with old adults, which decreases caregiving time in a daily life. This result becomes apparent when we limit the case of women. But the level of reduction is not enough to change other kinds of time-informal care, market work, leisure, etc. The results indicate that formal home care for the elderly can reduce the burden of caregiving in families, but only minimally.

Attitudes and Preferences about Elderly Communal Housing-focused on developing Elderly Assisted Living in Korea- (노인공동생활주택에 대한 태도의 선호- 한국에서의 노인생활지원주택 개발 방향을 중심으로-)

  • The Purpose of this study was 1) to analyse the attitudes about Elderly Assisted Living (EAL) among urban Korean 2) to consider the policy implications for elderly communal housing. The research methods used were 1) social survey research with questionnaire to find attitudes and preferences about EAL among potential elderly client in Seoul, Korea 2) interpretative suggestion for appropriate policy implications to facilitate developing EAL in urban Korea. A questionnaire was used to find attitudes/preferences about EAL was adapted from the results of the review about communal housing in the UK. The survey was undertaken from September to October, 2000 in Seoul, Korea. Data was collected from 413 subjects and analysed using various statistical techniques. From the results, location should be differentiated into urban type and suburban type. The urban type would prefer around 5 stories multi-family house style with elevator and suburban type would prefer single detached house style with shared garde. The most preferred characteristics found by factor analysis were factor I named as communal facilities and environment(the items belong to factor I was neighbour environment, communal spaces and facilities, heating system, and elevator). The most important advantages of EAL were social activities and convenience. The most salient disadvantages of EAL were difficulty of family contact. The higher the occupational status and income, the lower the preference for the characteristics of EAL with a preference for finding appropriate living arrangements in present housing market. But the higher the educational level, the higher the degree of preference for the characteristics of EAL.

The Spatial Distribution of Elderly Welfare Service in South Korea

  • Purpose: This study aims to not only measure the elderly welfare supply index but investigate spatial patterns and determinants of local elderly welfare services in South Korea. Research design, data, and methodology: The index for local elderly welfare services is measured by employing standardized scores for critical variables related to elderly welfare. The present study utilized the GIS technique and Moran's I index to examine spatial patterns of elderly welfare services. The determinants of local elderly welfare service are analyzed by a series of regression models using R. Results: Spatial imbalance and asymmetric distribution were serious in the supply of elderly welfare service. It was also confirmed that the factors affecting the level of welfare services for the elderly vary depending on the type of service. In particular, the higher the proportion of the elderly population and the social welfare budget, the lower the level of welfare services for the elderly. Conclusions: Given the circumstance of spatial mismatch between supply and demand for elderly welfare services, it is necessary to consider and provide policy tactics about how the economic benefits and welfare budgets generated in the region can contribute to strengthening the welfare service system for the elderly.

Employment Support for the Low-income Elderly in the OECD Countries: Implications for Senior Employment Policy (OECD 국가의 저소득 고령자 고용지원정책 : 노인일자리사업에 주는 함의)

  • The Korean government has implemented the senior employment policy as a direct job creation policy since 2004. A realistic discussion of policy alternatives and orientation for this has been given little attention even though senior employment policy has been carried out for the last 10 years and it will be expanded next year. This study tries to examine active labor market policy especially focusing on direct job creation programs and policies for the disadvantaged low-income elderly in OECD countries, and then it suggests some developmental alternatives for senior employment policy based on the study's results. The main results from this analysis are summarized in two points. Firstly, except pension policies, employment policy for older workers in the OECD countries is highly proportional to the tackling of objective factors reducing the demand for older workers (wage subsidies, reduced social security contribution rate etc). And the strategies of improving employability have not been relatively important and direct job creation policy has been marginal. Secondly, employment support policies for the low-income elderly can be divided into three types: support for the low-income elderly, alleviating early retirement and support for full employment according to the criteria which are determined by policy objectives and the social economic index. Korea's employment support policies belong to the type of direct job creation among them. This seems to be due to the fact that the rate of elderly poverty is extremely high and an income security system has not been developed in Korea. However, the policy objective is still uncertain. Therefore, this policy needs to set up clear objectives and establish a proper system for the achievement of its goals. If we focus on the strength of its employment characteristics, we need to modify the policy's plan in the perspective of labor market policy. But if we intend to keep both of the current objectives, it is better for this policy to be divided into two parts: social participation and income supplements. Or it also may be a solution to transform the system into an employment service, a training system which supports participants to move into unsubsidized jobs such as SCSEP in the U. S.

Categorization of Regional Delivery System for the Elderly Chronic Health Care and Long-Term Care (지역별 노인 만성기 의료 및 요양·돌봄 공급체계 유형화)

  • Background: By applying the suggested criteria for needs-based chronic medical care and long-term care delivery system for the elderly, the current status of delivery system was identified and regional delivery systems were categorized according to quantity and quality of delivery system. Methods: National claims data were used for this study. All claims data of medical and long-term care uses by the elderly and all claims data from long-term care hospitals and nursing homes in 2016 were analyzed to categorize the regional medical and long-term care delivery system. The current status of the delivery system with a high possibility of transition to a needs-based appropriate delivery system was identified. The necessary and actual amount of regional supply was calculated based on their needs, and the structure of delivery systems was evaluated in terms of the needs-based quality of the system. Finally, all regions were categorized into 15 types of medical and care delivery systems for the elderly. Results: Of the total 55 regions, 89.1% of regions had an oversupply of elderly medical and care services compared to the necessary supply based on their needs. However, 69.1% of regions met the criteria for less than two types of needs groups, and 21.8% of regions were identified as regions where the numbers of institutions or regions with a high possibility of transition to an appropriate delivery system were below the average levels for all four needs groups. Conclusion: In order to establish an appropriate community-based integrated elderly care system, it is necessary to analyze the characteristics of the regional delivery system categories and to plan a needs-based delivery system regionally.

A Study on Research Trends and Development Process of Health Policy for the Elderly (노인보건정책의 발전과정 및 연구동향에 대한 일고찰)

  • This paper is to suggest policy issues to be done in the future health policy for the elderly, analyzing contents of results suggested in academic papers published in Journal of the Korean Gerontological Society. Number of Papers published in the Journal is 61, occupying most of papers related with chronic disease, medical expenditures and health delivery system except dental and psychiatric health. Method used to be analyzed is mostly random sampling among community inhabitants or hospital patients. Policy issues are as follows in case of being based in the above analysis results. Firstly, it is necessary that the development of health promotion specialized for the elderly be developed and activated. Secondly, it is necessary that the primary health care system specialized for the elderly be constructed. Thirdly, it is necessary that the subacute care system specialized for the elderly be constructed after acute care services. Finally, it is necessary that the health care system specialized for the elderly in need of long-term care be constructed also.
