Objectives : We examined the association between alcohol consumption and incidence of colorectal cancer in elderly Koreans. Methods : The cohort members (n=14,304) consisted of 4,834 males and 9,470 females derived from the Korea Elderly Pharmacoepidemiologic Cohort (KEPEC), a population-based dynamic cohort. They were aged 65 years old or older and lived in Busan between 1993-1998; they were beneficiaries of the Korean Medical Insurance Corporation (KMIC). Baseline information was surveyed by a self-administered, mailed questionnaire. This study population was restricted to 14,304 participants who reported alcohol drinking habits on the questionnaire and had not been diagnosed with colorectal cancer at baseline. The adjusted hazard ratios (aHR) of status, type, frequency and daily average amount of alcohol consumption were computed with Cox's proportional hazard model, with the never-drinkers as a reference group and controlling for age and gender. Results : After 4.82 person-years of mean follow-up 112 cases of colorectal cancer occurred. The incidence densities of colorectal cancer were 161 (95% CI=123-200) for never-drinkers, 219 (95% CI=125-339) for ex-drinkers, and 137 (95% CI=84-189) for current-drinkers per 100,000 person-year. The status, type, frequency, and daily average amount of alcohol consumption were not significantly related to the incidence of colorectal cancer after controlling for age and gender. Conclusions : There was no significant association between alcohol consumption and colorectal cancer among elderly people after controlling for age and gender.
본 연구는 가구유형에 따른 장애노인 특성과 우울 영향 요인을 연구하였다. 분석자료는 한국장애인고용공단 2차 웨이브 3차 장애인고용패널조사 자료로 대상자는 608명이다. 통계분석은 기술통계, 카이스퀘어, 로지스틱 회귀분석 하였다. 연구결과 첫째, 단독가구 장애노인은 여성, 비수도권, 중증장애, 건강상태가 나쁜 경우, 경제수준이 낮은 경우, 일상생활도움이 필요한 경우, 차별경험이 높은 경우, 관계만족도가 낮은 경우, 사회생활참여를 하지 못하는 경우 분포가 높게 나타났다. 둘째, 장애노인 우울 영향 요인은 차별경험(p<.05), 사회활동참여(p<.05)이고, 다인가구 장애노인은 성별(p<.05), 거주지(p<.01), 건강상태(p<.001), 경제수준(p<.05)으로 단독가구 장애노인은 일상생활도움(p<.05)으로 나타났다. 장애노인의 우울 감소를 위해 차별경험 감소 및 장애인식개선, 보건의료 및 경제적 지원 등이 필요하다. 본 연구는 가구유형을 구분하여 연구를 진행한 점에서 의의가 있으며, 향후 종단적 연구를 제언한다.
Purpose: The purpose of this study was to investigate the factors influencing health promoting lifestyle in the elderly. Method: The subject of this study was 305 elderly person over the age of 60, living in rural and urban, Korea. For the analysis of collected data, descriptive statistics, t-test, analysis of variance and stepwise multiple regression were used for statistical analysis with SPSS statistical program. Results: The average item score for the health promoting lifestyle was 2.46, The higher score on the subscale was nutrition(2.65). The lowest score on the subscale were physical activity(2,36) and stress management(2,36). General characteristics showing statistically significant difference in health promoting lifestyle were age, residential district, live together spouse, education, religion and pocket money in the elderly. Stepwise multiple regression analysis revealed that the most powerful predictor of health promoting lifestyle in the elderly was prior related behavior(R2=.554). A combination of prior related behavior, perceived benefits of action, perceived self-efficacy, commitment to a plan of action, and interpersonal influences accounted for $64.3\%$ of the variance in health promoting lifestyle in the elderly, Conclusion: The factors influencing on health promoting lifestyle for elderly were prior related behavior, perceived benefits of action, perceived self-efficacy, commitment to a plan of action, and interpersonal influences.
The author performed on epidemiological study of the TMD in 189 elderly people and 195 young people with Helkimo index. The clinical dysfunction index was based on data from clinical examination and the anamestic dysfunction index was based on data from the interview with the investigated person. The results were as follows : 1. In the elderly population, 27.5% reported that they had subjectively symptom of TMD but 43.4% had sign and symptoms of TMD in clinical examination. 2. The mean values for maximal opening differed significantly between elderly and young populations, 47.22mm and 51.44mm (p<0.001) and less than a 40mm opening was observed 7.9% in elderly population, 0.5% in young population (p<0.001). 3. The mean values for lateral movement to the right and left did not differ with age, which were 8.56mm and 8.47mm in elderly population, 8.90mm and 8.81mm in young population, but the mean value for protrusion differed significantly between elderly and young populations, 6.89mm and 7.64mm (p<0.01). 4. A higher incidence of TMJ noise was recorded in the elderly and young populations than young population, especially crepitus, but a higher incidence of clicking was recorded in young population (p<0.05).
This study was undertaken to compare the gait characteristics between the Korean elderly and young adults, we measured the plantar pressure and contact time of gait with barefoot along a walkway at their preferred walking speed. The results indicate that older people exhibited significantly less plantar pressure than young adult in all 3 regions (FF, MF and RF) and significantly less time % on the initial contact phase (ICP), forefoot push-off phase (FFPOP) and significantly more % forefoot contact phase (FFCP) and foot flat phase (FFP). The converted plantar pressure value to percentage, it showed more pressure in forefoot (FF) in the elderly person than the young adults. It could be explained that the forward shifting in plantar pressure are associated with a more flexed posture of elderly such as actual stabilizing fearrelated adaptations. Longer total foot contact time in the elderly means that the old people show the decreased gait velocity. In other words, lower velocity was found to be associated with pre-existing fear of falling. With longer contact time and slower stepping movement, the elderly become more unstable. With these findings, it could be confirmed that there were significant changes in foot characteristics which contribute to alter the plantar pressure and contact time during gait with advancing age. Further research is required to establish possible links to risk of falling and development of footwear in the elderly adults.
This study has been performed to analyze living arrangement in later life. In this study, 276 Koreans aged 60 or more were interviewed for their current household structure. The results are drawn as follows: 1. Of all the households with at least one person aged 60 or more, those living in three generations account for the largest proportion, 39.9 percent. For the regional difference, the rate was slightly higher in urban areas than in rural areas. 2. 48.7% of the subjects living in owned homes were households with elderly only, but only 23% of those living in rented homes are elderly only. Overall, the rate of elderly-only households becomes the highest for those living in owned single structure homes. This result indicates that the elderly with higher economic status are more likely to live without children than others. 3. As for birth order, those who grew up as the first child are most likely to live in elderly-only households, while others are in three generation homes. 4. The elderly with few children are more likely to live independently than those with many children, not because they are without dependable children, but because spend less resource for childbearing and therefore are more independent. 5. Overall in this, it is supported that socioeconomic status and independability are more influential factors on living arrangement of the elderly than regional or emotional factors.
Purpose: This study aimed to evaluate economic viability of public health center visiting nurse services for the low-income elderly with long-term care needs. Method: The sample consisted of 252 community dwelling elderly who enrolled in public health center visiting nurse services for three months or more. Data was collected on physical (ADL and IADL) and cognitive impairments of the elderly, contents and frequency of visiting nurse services, cost per visit, and costs of alternative services for long-term care. Result: The mean score of ADL and IADL levels of the elderly was 2.80.4904, which indicated these patients were mostly independent. Eighty four percent of the elderly subjects were cognitively intact. Among visiting nurse services supplied, providing assessment was 34%, followed by education and counseling 26%, medication 22%, and referral. The mean cost per visit was 17,824.1 won, which transformed into a total cost per person per year of 161,130.2 won. Comparing the cost of a visiting nurse service with those of other long-term care alternatives, the visiting nurse service was the least costly alternative, followed by an outpatient clinic, hospital based home care, and nursing home. Conclusion: Overall, the results of the study provide evidence of the economic viability of visiting nurse services for the low-income elderly among long-term care alternatives.
본 논문은 주거공간에서 고령자 청력손실을 고려한 소음 및 잔향에 따른 음성 전송 성능을 청취실험을 통해 평가하였다. 주거환경 소음으로 바닥충격음, 교통소음, 공기전달음과 배수소음을 대상으로 하였으며, 공동주택의 잔향환경을 모사하기 위해 실내음향 컴퓨터시뮬레이션을 실시하여 충격응답를 추출하였다. 청취실험 음원은 고령자 청력손실(65세 남성)을 반영하기 위해 소음 및 단어 음원의 고주파대역의 음압레벨을 저감시킨 음원(고령자 음원)과 정상청력을 반영한 원음(청년 음원)을 대상으로 하였다. 청취실험은 각각 3개의 소음레벨($L_{Aeq}$ 30, 40, 50 dB)과 잔향시간(0.5, 1.0, 1.5 s)을 갖는 음환경 조건에서 제시된 단어($L_{Aeq}$ 55 dB)의 음성요해도(speech intelligibility)와 듣기 어려운 정도(listening difficulty)를 평가하는 것으로 하였다. 청취실험 결과, 음성레벨이 55 dB($L_{Aeq}$)일 때 잔향시간 1.0 s 이하 조건에서 충격소음(점핑음) 50 dB($L_{i,Fmax,AW}$)와 정상소음(도로, 음악, 배수 소음) 40 dB($L_{Aeq}$) 이하의 소음레벨에서는 고령자 및 청년 음원 모두 90 % 이상의 음성요해도와 30 % 이하의 듣기 어려운 정도를 확보할 수 있을 것으로 판단된다. 고령자 청력손실을 반영한 고령자 음원의 경우 청년 음원 보다 음성요해도는 0 % ~ 5 % 낮았고 듣기 어려운 정도는 2 % ~ 10 % 높은 것으로 나타났다.
The main objective of this study was to investigate how people prepare for old age by looking at financial statements from single-person households in Gyeonggi province; the policy implications regarding low fertility and population aging are considered. The subjects were members of 600 households who were aged 26 to 44 years old and not married. The data were analyzed by t-test, ANOVA, and dichotomous logistic regression using SPSS WIN 12.0. Respondents pointed out that financial problems are the greatest challenges for those facing old age in single-person households; they said that a stable job is the most important factor for secure living. About 57% of household residents responded that they have prepared for retirement, and a majority of them said they hoped to spend their elderly years with a marriage partner. The financial statements from single-person households were inferior to those from other types of households. The assets of single-person household members were less than those of other households. Only 10.5% of respondents have owned homes. The study discusses policy implications for those who want marriage and for those who don't want it. For those who want marriage, job-seeking support was proposed, and for the others, tax deductions and family programs for single-person households were proposed.
The present study aimed to examine the influence of mental health factors on oral health factors using data from the Korean National Health and Nutrition Examination Survey (KNHANES). A total of 1,548 elderly individuals aged over 65 years who completed the screening and health questionnaire were selected as the final study participants. The psychological health factors related to perceived oral health status were depression and stress when the gender was controlled, and in terms of gender, both elderly male and female individuals were stressed and depressed. These results suggested that the psychological health of elderly individuals negatively affects not only perceived oral health but also the quality of life. In addition, the psychological health of the elderly individuals should be systematically managed along with oral health care and overall health care and lifestyle. If a comprehensive health management system is available for low-income groups or an elderly person living alone, it should positively affect and improve the quality of life of elderly individuals.
