The convergent relationship between experience of elderly oral health care education by specialist and attitudes toward the elderly (치과위생사의 노인구강관리 전문가 교육경험에 따른 노인을 대하는 융합적 태도 관계)

  • Dental hygienists have received specialist training and experience to identify relationships that affect the attitude towards the elderly dental hospitals, general hospitals, working in a university hospital were survey of 264 people. The relationship between the elderly oral health specialist education experience and the attitude toward the elderly was influenced by the attitude toward the elderly (r=0.160, p<0.01), the experience of the elderly oral tissue regeneration education was related to the attitude toward the elderly (r=0.178, p<0.01), and feeding and swallowing function training education was positively correlated with the attitude toward the elderly (r=0.173, p<0.01). Therefore, it is necessary to develop curriculum in the maintenance training course for the dental hygienists who are experts in oral health care for the elderly, and to develop the curriculum for elderly dental hygiene course in the school education.

Factors related to chewing discomfort in the elderly in Korea: using the 8th (2020) National Health and Nutrition Examination survey (우리나라 노인의 저작 불편감 관련 요인: 제8기(2020년) 국민건강영양조사 활용)

  • The purpose of this study was to identify chronic diseases and oral health care behaviors in 65-year-old individuals and to determine the factors that influence masticatory mobility. The raw data from the 8th National Health and Nutrition Examination Survey in 2020 were utilized to analyze factors associated with chewing discomfort in 1,278 elderly Koreans. Using IBM SPSS Statistics 27.0, we conducted complex sample cross-analysis to examine the difference in chewing discomfort based on general characteristics, chronic diseases, and oral health care behaviors. Additionally, we performed complex sample multiple logistic regression analysis to identify the factors associated with chewing discomfort. As a factor influencing chewing discomfort, unmet dental care was 3.33 times higher (p<0.001), and subjective oral health was 0.18 times more likely to be moderate (p<0.001) and 0.48 times more likely to be good (p<0.001) lower. In the case of arthritis, the level of chewing discomfort was high (OR=1.08, p=0.694), but there was no significant difference. As a result of the aforementioned factors, it is believed that there is a need for policies and systems to provide dental care to the elderly before their oral health deteriorates. Additionally, tailored oral health promotion programs should be implemented.

A Study on the Converged Oral Environment of the Elderly with Dementia in the Nursing Home (요양시설 치매노인들의 융합적 구강환경 연구)

  • This study was conducted to identify the relationship among the cognitive function of the elderly with dementia in the nursing home and the oral health condition and pathogenic microorganisms in the mouth. Data was collected for a month in February 2019 for 30 people through a non-probable sampling method due to the special nature of dementia elderly people aged 70 years or older, living in nursing homes in Daegu. Technical statistics, ANOVA, and Pearson's correlation coefficient were analyzed using the SPSS window ver. 21.0 program. According to the study, the lower the rating of long - term care classification in nursing homes, the more the number of high-risk oral pathogens, and the more the number of existing permanent teeth, the more the number of microbes in the mouth. However, there was no significant difference in cognitive function. Therefore, it is suggested that oral health care, an elderly dementia people in nursing homes, should have different management methods depending on the oral environment, and also professional managers should be able to manage oral health condition for actual care.

A study on knowledge and attitude toward the elderly in dental personnels - in Daejeon & Chungnam area - (치과종사자들의 노인에 대한 지식과 태도 연구 - 대전·충남지역을 중심으로 -)

  • This study was surveyed the knowledge and attitude toward the elderly in dental personnels, who play an important role in oral health of the elderly in the aging society, In order to arrange the basic data that is conducive to development in the mouth care of the elderly, the research was conducted from January 7 to February 14, 2008, targeting 270 dental personnels in Daejeon & Chungnam area, who are in charge of oral duties in the current clinic. As a result of analyzing so that questionnaire can be prepared with the self-administered questionnaire, the following results were obtained 1. The knowledge level on the elderly in dental hygienist was 13.47 marks out of 25-mark perfection. There was no difference in knowledge depending on job category and volunteer-work activity experience, And, there was no difference even depending on physical & physiological sphere, psychological sphere, and family & social sphere. 2. The attitude level toward the elderly in dental hygienist was 91.63 marks out of 150-mark perfection. Dental hygienist showed positive attitude in personality characteristic, emotional characteristic, and self-management ability by sphere, and showed statistically significant difference (p=0.011). 3. In case of having volunteer-work activity experience, the attitude level toward the elderly was 92.57 marks out of 150-mark perfection, A case of having volunteer-work activity experience showed positive tendency in emotional characteristic, self-management ability, and judgement-ability characteristic by sphere, And, the attitude toward family relation was indicated to be negative tendency, thus there was statistical significance(p=0.022). 4. As for the correlation between knowledge and attitude toward the elderly, dental hygienist was indicated to have high interest in the volunteer-work experience, the elderly education experience, and the elderly problem(r=0. 444). The knowledge and attitude toward the elderly had slightly positive correlation(r=0.155). Dental hygienists are being required gradually as the primary staff for the elderly people's dental care in the aging society. A continuous education is needed so that dental hygienists can have positive sight in understanding, knowledge, and attitude. And, the development in a mouth care program for the elderly in line with it is considered to be necessary.

The relationship between dental care needs and dental service use in Korean elderly (한국 노인의 치과 의료 필요와 치과 서비스 이용의 관련성)

  • Objectives: This study aimed to assess the relationship between dental care needs and dental service use in Korean elderly. Methods: Using the sixth Korea National Health and Nutrition Examination Survey, from 22,948 individuals, 1,572 (male 701, female 871) elderly individuals aged above 65 years were included in the study. All analyses were stratified by sex. Results: In males, the group with subjective needs was 3.74 (95% confidence interval [CI]: 2.58-5.41) times more likely to use dental services than the group without subjective needs. For females, the group with subjective needs was 2.17 (95% CI: 1.57-2.98 ) times more likely to use dental services than the group without subjective needs. Conclusions: To conclude, the elderly with symptoms, used the dental services for pain relief and functional recovery. Fundamental efforts to improve oral health are needed, such as providing treatment, prevention and educational services, besides efforts to provide treatment-oriented dental services. Further research is needed for the middle and elderly individuals who are in the blind spot of the existing policy.

A study on the state of customized visiting oral health programs (맞춤형 방문구강보건사업 현황조사)

  • Objectives : This study was to examine the state of customized visiting oral health programs in a bid to help facilitate the unified operation of the programs and the development of required guidelines. Methods : The subjects in this study were 49 dental hygienists who were professionals responsible for customized visiting health care programs across the nation. Results : 1. Regarding the form of employment of the dental hygienists were investigated many contract and daily workers. 2. As to the possession of equipment necessary for visiting oral health programs, denture cleaners(12.2%) were most widely possessed in some regions, followed by mobile scalers(10.2%) and mobile suctions(8.2%). In terms of expendable devices and materials, dental mirrors, pincettes and explorers were the most widely possessed dental checkup devices, and the most widely possessed oral hygiene supplies were toothbrushes, interdental brushes and denture cleaners. Those devices and materials were in more possession than the other types of devices and materials. The most widely possessed equipment for educational purpose was laptop computers, followed by beam projectors and screens. The most widely possessed teaching materials were dentiform, followed by CD-ROMs. 3. Those whom they visited the most for oral health care service were elderly people, followed by the disabled and patients with chronic diseases. The dental hygienists who went out to visit those people outnumbered the others who stayed at public health centers. Concerning the types of visiting oral health care service, the most prevalent service provided to the elderly included denture cleaning/management, oral massage and preventive treatment against dental caries. The most dominant service provided to the disabled involved education of the oral health care act, preventive treatment against dental caries and toothbrushing by professionals. The most common service offered to patients with chronic diseases was education of the oral health care act and oral health education. The dental hygienists paid a visit to a mean of 5.8 households a day. The average weekly number of households cared by the dental hygienists was 27.3. It took a mean of 37.1 minutes for them to take care of each household. 4. As for satisfaction level with the implementation of the visiting oral health programs, they expressed the greatest satisfaction at teamwork with professionals($3.56{\pm}0.94$), followed by the professionalism of their work($3.21{\pm}0.94$) and workload($3.08{\pm}0.94$). Their satisfaction level with the work conditions required for creative job performance($2.75{\pm}0.98$) and partnership with other institutions($2.64{\pm}1.03$) was below 3.0. In regard to the impact of their characteristics, marital status made a statistically significant difference to satisfaction level with workload. The unmarried dental hygienists were more pleased with their workload than the married ones(p<0.05). 5. As to needs for education for professionalism improvement, they asked for education about visiting oral health care skills the most, followed by education about oral health care for patients with chronic diseases, education of planning/evaluation and education of oral health care for the disabled. Conclusions : The top priority for the vitalization of the programs was the procurement of budget, followed by the procurement of equipment and educational media and the procurement of human resources.

Factors influencing oral health according to life cycle characteristics of patients with diabetes (생애주기별 당뇨질환자의 구강건강영향요인)

  • Objectives: This study aimed to identify the factors influencing the relationship between diabetes and oral health in Korean adults. Methods: We analyzed 5,319 adults who were included in the 9th Korea National Health and Nutrition Examination Survey. The data were analyzed for demographic characteristics, daily health care, and oral health care according to diabetic conditions using a complex sample analysis. Multiple logistic regression analysis was performed to analyze the factors influencing the oral health of patients with diabetes. Results: The factors influencing the oral health of patients with diabetes varied according to life cycle. In late middle-aged adults, statistically significant differences were observed in sex (p<0.001), educational level (p=0.030), economic activity (p=0.018), aerobic exercise (p=0.034), smoking (p=0.004), periodontal therapy (p=0.011), and prosthesis production/repair (p=0.025). In younger elderly individuals, statistically significant differences were found in terms of whether they lived together (p=0.027) and educational level (p=0.032). Conversely, no statistically significant differences were observed in the older elderly group. Conclusions: The results of this study showed that the level of oral health of patients with diabetes is already determined in middle and old age; therefore, a system should be prepared to ensure that health care can be systematically performed in late middle-aged adults.

Necessity of Oral Health Care for Patients with Hypertension and Diabetes Related to Gwangju Community Care Project

  • In this study, 293 middle-aged and elderly people in Gwangju·Jeonnam were analyzed for prevalence of hypertension and diabetes and health behavior index using composite logistic regression analysis using SPSS ver 21.0. The prevalence of hypertension was significant in age, education level, and household type, and the prevalence of diabetes was significant in education level. The prevalence of hypertension according to health behavior related indicators was significant in subjective health awareness, prosthesis repair, and discomfort chewing, and the prevalence of diabetes was significant in subjective health awareness, oral examination, discomfort chewing. The variables affecting the prevalence of hypertension were age and prosthetic repair, and the variables affecting the prevalence of diabetes were age, prosthetic repair, discomfort chewing, and annual drinking frequency. The purpose of this study was to investigate the relationship between hypertension, diabetes mellitus prevalence and health behavior indicators of middle-aged and elderly people, and to help the importance of Community Care and oral health care improvement program.

A study on the performance and necessity of dental hygienists for oral health promotion activities for the elderly according to their learning experiences and performance experiences (노인 구강건강증진 활동에 대한 치과위생사의 학습경험과 수행경험에 따른 수행가능성 및 필요성 조사)

  • Objectives: The purpose of this study was to investigate the awareness of the performance and necessity of oral health promotion activities and seek ways to revitalize the professional role of dental hygienists in oral healthcare for the efficient oral care of the elderly. Methods: Eighty-five dental hygienists in charge of oral health promotion projects at public health centers and 38 dental hygienists in network dental clinical trials were investigated for their learning experience, performance experience, feasibility, and necessity of dental hygienists for general and oral health service items for the elderly. The collected data were analyzed using frequency analysis, chi-square test, and Mann-Whitney U test. Results: The degree of performance possibility according to the learning experience and performance experience of the dental hygienist for the whole body and oral health promotion activity items for the elderly showed that the degree of performance possibility was higher among those with experience compared to the non-experienced person, and it was statistically significantly higher (p<0.05). Conclusions: The dental hygienist's professional oral health service is a necessary system to improve practical knowledge and skills and to provide a wide range of professional oral health services for the elderly.

The subjective recognition of halitosis in the elderly in some areas (일부지역 노인들의 주관적인 구취에 대한 인식 및 실태에 관한 연구)

  • Objectives : The purpose of the study is to investigate the appropriate prevention and management of halitosis and management for the elderly. Methods : A self-reported questionnaire was filled out by 320 elderly over 65 years old in Seoul and Gyeonggi from June to October, 2013. Except 28 incomplete answers, 292 copies were used for the analysis. Results : The correlation coefficient was 0.702 which was the degree of present halitosis and everyday halitosis. Elderly within 65-69 years old felt the halitosis very seriously. the level of halitosis felt everyday was also highest in those within 65-69 years old with mean $2.00{\pm}0.862$ points. Conclusions : The dissemination of correct knowledge of halitosis management can improve the subjective perception of the elderly oral health care.
