구조방정식 모형을 이용한 고령자의 기능 수준, 환경적 요인 및 라이프스타일이 건강과 삶의 질에 미치는 융합적인 영향 분석 (Analysis of Convergent Influence of Functional level, Environmental Factors and Lifestyle on Health and Quality of Life among Elderly using Structural Equation Model)

  • 본 연구의 목적은 고령자의 기능 수준, 환경과 라이프스타일이 건강과 삶의 질에 미치는 영향을 분석하고자 하였다. 자료수집은 2019년도 4월부터 2019년도 5월까지 자기기입식 설문지를 사용하였으며, 조사대상은 전국의 65세 이상, 지역사회에 거주하는 고령자 200명으로 하였다. 자료 분석은 SPSS 23.0과 AMOS 23.0 통계프로그램을 사용하였다. 연구결과, 고령자의 기능 수준과 신체적 활동은 직접적으로 건강에 영향을 미치며, 기능 수준, 활동참여, 식이와 건강은 삶의 질에 직접적인 영향을 주는 것으로 나타났다. 또한, 건강과 삶의 질에 직접적인 영향을 미치지는 않지만, 환경적 요인은 식이와 활동참여에 영향을 미치는 것으로 나타났다. 따라서 고령자의 건강과 삶의 질 향상을 위해서는 이에 영향을 미치는 기능 수준, 환경과 건강한 라이프스타일 관리가 필요할 것으로 사료된다. 본 연구의 결과는 향후 노인보건복지 관련 정책 및 서비스를 수립하는 데 기초자료로 활용될 것으로 기대된다.

Impact of socio-demographic factors, lifestyle and health status on nutritional status among the elderly in Taiwan

  • BACKGROUND/OBJECTIVES: Aging is an imperative problem for many countries in this century, and presents several challenges for the maintenance of good nutritional status. This study aims to assess the impact of socio-demographic factors, lifestyle and health status on the nutritional status among the elderly in Taiwan. SUBJECTS/METHODS: A cross-sectional study was carried out in Taiwan. Data were obtained from the Mei Jau Health Management Institution, which is a private health evaluation provider with multiple health screening centers in Taiwan and Asia. This study included 7947 adults aged 65 years or above. The data were extracted between 2001 to 2010. Nutritional status was assessed using anthropometric data, biochemical data and dietary intake information. RESULTS: Among the 7947 participants with mean age of 70.1 (SD = 4.5) years, 20.2%, 6.6%, 10.5% and 52.5% experienced underweight, protein malnutrition, anemia and inadequate dietary intake in the past month, respectively. Age was negatively correlated with body weight (r = -0.19, P = 0.02), body mass index (r = -0.41, P < 0.001), albumin level (r = -0.93, P < 0.001) and hemoglobin level (r = -0.30, P = 0.008). Age above 70 years, gender, unmarried status, retirement, lack of education, low family income, smoking, alcohol drinking, sleep duration of 6-8 hours, vegetarian diet, multiple medications, comorbidity and dysphagia were positively associated with malnutrition in older adults. CONCLUSIONS: Underweight and inadequate dietary intake are prevalent among the elderly in Taiwan. Vegetarian diet, multiple medications, comorbidity, dysphagia and lifestyle factors such as smoking, alcohol drinking and sleep duration of 6-8 hours are risk factors for undernutrition in older adults.

대구.경북지역 노인의 의치장착실태 및 관련 요인 (Relate Factors on the Wearing Dentures Status of the Elderly in Daegu.Kyungpook Areas)

  • Purpose: This study was conducted to investigate the wearing dentures status of the elderly and understand the relationship between demographic sociological properties, oral health care habits, lifestyle, and whether they wears dentures or not. Methods: In a surveying of 349 elderly who lives in Daegu and Kyung-pook, following results are obtained. Results: Wearing dentures rates of the aged are 58.5%. The more the aged is female(p=0.025), elderly(p=0.000), a low education level(p=0.000), no job(p=0.006), and a solitary person(p=0.016) the higher rate they wears dentures. The more the aged brushes their teeth after each meal(p=0.029) and uses the oral health devices(p=0.000) the lower rate they wears dentures. The more the aged have dinner alone(p=0.009), do not kept up with the exercises (p=0.000), and would like to dine off a soft food(p=0.000) the higher rate they wears dentures. Conclusion: This study showed that an old people's condition of oral health care who live in Daegu Kyungpook is very serious. Therefor not only an elderly but other aged people is need of constant and organized education for taking their oral health care habits and changing their lifestyle. Because wearing dentures rates of the aged by demographic sociological properties, oral health care habits, and lifestyle is lower.

뉴실버 여성소비자 지각연령과 라이프스타일에 따른 의복쇼핑성향 연구 (제2보) (A Study on the Shopping Orientation, Related to the Cognitive Age and the Types of Lifestyle of the Female Consumers in the Elderly Generation (Part2))

  • This study researches age of discretion and lifestyles for aged women consumers and examines their social and psychologically recognized age of discretion and the difference in shopping orientation on fashion. Also it is purposed to support implications for marketing strategy on fashion market for the aged. A survey was carried out regarding the consumers of the new elderly generation aged 55 and above. 561 respondents were surveyed, residing in Seoul and Metropolitan areas. Factor analysis, Cronbach's ${\alpha}$, K-average community and multivariate analysis, multi-recurrence and crossing analysis($X^2$ verified) were used for statistical analysis. In conclusion, first, people in their 70s to 80s perceive their age 20 to 30 years younger than their real age. Second, appearance-oriented and high technology-oriented were the most highlighted lifestyles as to the study of the vital statistical, related to the lifestyle in correspondence to the real-age and the cognitive age. Third, people who feel older than they actually are in terms of shopping orientation were less likely to shop depending on their mood. Throughout the results, there is no doubt that people in the elderly generation is a main target within marketing trends in the elderly generation industry. A research based on comparison between the male and the female consumers in the elderly generation is considered to be very meaningful.

장노년층 건강증진을 위한 모바일 헬스 기반 라이프스타일 케어 앱의 사용자 요구도 분석 (Analysis the of User's Needs for Developing a Mobile Health Based Lifestyle Care Application for Health Promotion among the Elderly)

  • 목적 : 최근 건강한 삶의 질 증진을 위해 모바일 기반의 헬스 서비스 제공을 원하는 개인 늘어나고 있다. 본 연구는 고령자의 건강한 라이프스타일 확립을 위하여 라이프스타일 평가와 케어를 할 수 있는 앱 개발의 기초자료로써 활용하고자, 사용자의 요구도와 선호도를 조사, 분석하였다. 연구방법 : 지역사회에 거주하는 55세 이상의 스마트폰을 이용하는 장노년층 총 84명을 대상으로 설문 조사가 실시되었다. 자료 분석은 SPSS version 25.0을 이용하여 기술통계와 일원 배치 분산분석을 하였다. 결과 : 라이프스타일 평가 및 케어 앱의 콘텐츠와 관련하여 고령자가 중요하게 여기는 분야는 하루 걸음수 측정, 전반적 신체활동, 혈압측정, 수면 수준 측정, 균형잡힌 식습관 측정, 일일 수행 활동의 종류 및 시간 측정에서 전반적으로 높은 요구도를 보였다. 이러한 라이프스타일이 관리 될 수 있도록 앱 안에서 보여지는 기록에 대한 선호도로는 '하루 걸음 수를 그래프로 기록 및 관리기능', '하루음식 섭취 열량을 제공', '일일 운동의 시행 횟수와 시간을 제공', '시간별 활동의 종류와 수행시간을 제공' 등이 라이프스타일의 관리를 위한 기록 부분에 필요한 콘텐츠로 조사되었다. 또한 그룹 간 차이로는 장년층에서는 활동참여를 주요한 콘텐츠로 보는 반면, 고령층은 이에 대한 중요도를 적게 보였다. 결론 : 본 연구는 고령자의 건강과 삶의 질 증진을 위한 모바일 헬스 기반의 라이프스타일의 프로파일링 및 케어를 앱 개발을 위해 사용자가 필요로 하는 컨텐츠를 조사하고, 컨텐츠의 디스플레이방식에 대한 선호도를 조사, 분석하였다, 이는 향후 모바일 헬스 기반의 라이프스타일 평가 및 케어 앱 개발에 있어서 중요한 기초자료가 될 것으로 사료된다.

라이프스타일을 고려한 유비쿼터스 주거 공간 계획 연구 (A Study on the planning of Ubiquitous housing for the Lifestyle)

  • According to the ROK 2010 census, representatives living arrangement are changing from 4 people per household to 2 people. Increasing number of elderly household and single household, the variety of lifestyles there has been a type of family structure. In this study, 1) through a precedent study review and case analysis to deduct representative group of lifestyle of Korean household. 2) Proposing spatial configuration according to the preferred lifestyle. 3) Deducting residential U-services from which specified from "Ubiquitous City Construction Project Operation Guide (2009)" of the korean Ministry of Land, Transport and Maritime Affairs. 4) Deduct highly expected U-service by each of lifestyle. As a result, spatial configuration and U-Services are proposed by changes of lifestyle which is based on ROK 2010 census.

Wheel of Wellness Counseling in Community Dwelling, Korean Elders: A Randomized, Controlled Trial

  • Purpose: The purpose of this study was to investigate the effects of Wheel of Wellness counseling on wellness lifestyle, depression, and health-related quality of life in community dwelling elderly people. Methods: A parallel, randomized controlled, open label, trial was conducted. Ninety-three elderly people in a senior welfare center were randomly assigned to two groups: 1) A Wheel of Wellness counseling intervention group (n=49) and 2) a no-treatment control group (n=44). Wheel of Wellness counseling consisted of structured, individual counseling based on the Wheel of Wellness model and provided once a week for four weeks. Wellness lifestyle, depression, and health-related quality of life were assessed pre-and post-test in both groups. Results: Data from 89 participants were analyzed. For participants in the experimental group, there was a significant improvement on all of the wellness-lifestyle subtasks except realistic beliefs. Perceived wellness and depression significantly improved after the in the experimental group (n=43) compared to the control group (n=46) from pre- to post-test in the areas of sense of control (p =.033), nutrition (p =.017), exercise (p =.039), self-care (p <.001), stress management (p =.017), work (p =.011), perceived wellness (p =.019), and depression (p =.031). One participant in the intervention group discontinued the intervention due to hospitalization and three in the control group discontinued the sessions. Conclusions: Wheel of Wellness counseling was beneficial in enhancing wellness for the community-dwelling elderly people. Research into long-term effects of the intervention and health outcomes is recommended.

Impact of Lifestyle Diseases on Postoperative Complications and Survival in Elderly Patients with Stage I Non-Small Cell Lung Cancer

  • Background: The influence of lifestyle diseases on postoperative complications and long-term survival in patients with non-small cell lung cancer (NSCLC) is unclear. The aim of this study was to determine whether lifestyle diseases were significant risk factors of perioperative and long-term surgical outcomes in elderly patients with stage I NSCLC. Methods: Between December 1995 and November 2013, 110 patients aged 65 years or older who underwent surgical resection of stage I NSCLC at Dong-A University Hospital were retrospectively studied. We assessed the presence of the following lifestyle diseases as risk factors for postoperative complications and long-term mortality: diabetes, hypertension, chronic obstructive pulmonary disease, stroke, and ischemic heart disease. Results: The mean age of the patients was 71 years (range, 65 to 82 years). Forty-six patients (41.8%) had hypertension, making it the most common lifestyle disease, followed by diabetes (n=23, 20.9%). The in-hospital mortality rate was 0.9% (n=1). The 3-year and 5-year survival rates were 78% and 64%, respectively. Postoperative complications developed in 32 patients (29.1%), including 7 (6.4%) with prolonged air leakage, 6 (5.5%) with atrial fibrillation, 5 (4.5%) with delirium and atelectasis, and 3 (2.7%) with acute kidney injury and pneumonia. Univariate and multivariate analyses showed that the presence of a lifestyle disease was the only independent risk factor for postoperative complications. In survival analysis, univariate analysis showed that age, smoking, body mass index, extent of resection, and pathologic stage were associated with impaired survival. Multivariate analysis revealed that resection type (hazard ratio [HR], 2.20; 95% confidence interval [CI], 1.08 to 4.49; p=0.030) and pathologic stage (HR, 1.89; 95% CI, 1.02 to 3.49; p=0.043) had independent adverse impacts on survival. Conclusion: This study demonstrated that the presence of a lifestyle disease was a significant prognostic factor for postoperative complications, but not of survival, in elderly patients with stage I NSCLC. Therefore, postoperative complications may be influenced by the presence of a lifestyle disease.

노인공동주거시설 단위주호의 설계지침 중요도 분석 (Analysis on the Importance Degree of Design Checklist for the Elderly Housing Unit)

  • A rapid increase of the elderly population and changes in the social structure, family type and lifestyle bring us to the moment for considering the various aspects of supporting the elderly including the elderly housing facilities. Traditionally it is considered as a merit for a son to support parents in his own house. Hut recently the elderly increasingly want to live independently without support from their children. To satisfy these needs, new housing facilities for the elderly were developed by private non-profit foundations. A number of outstanding housing facilities for the elderly have been built in the last five years. These facilities show significant progress in housing for the elderly. Unfortunately, there u e no standard design checklists or guidelines for housing for the elderly available in Korea. The housing facilities were built based on foreign design guidelines or architect's experience. This research aims to develop an affordable design checklist that can meet the needs of the Korean elderly lifestyle. This study evaluated the senior residential facilities using Woo's checklist which consolidated the guidelines available here and abroad. As the result of this study, we presented the checklist subject in upper group $25\%, middle group, lower group $25\% distribution and 6, 5, 4, categories in order to importance. When designing the future senior residential facilities, a constraint can be made based on the scope and financial factors of the facility first and the determine how extensive the application of the checklist should be.

일 복지관 이용 노인의 생활습관 및 우울이 영양상태에 미치는 영향 (Effects of Lifestyle and Depression on the Nutritional Status of Elderly People)

  • Purpose: This study was conducted to evaluate nutritional status, and the effects of lifestyles and depression on nutritional status of elderly people (NSE). Method: The participants were 280 elders who visited the Y city Senior Welfare Center between August 2006 and October 2006. Data were collected using a structured questionnaire that included general characteristics, lifestyle checklist, depression scale, and nutritional risk index. Also, Body Mass Index (BMI) was calculated from height and weight. T-test, ANOVA, Duncan test, Pearson coefficients correlation and stepwise multiple regression were used to analyze the data using the SPSS Win 12.0 program. Results: The NSE mean score was mid level at 5.03 BMI was 23.42 which indicates overweight. Depression was the factor that influenced the nutritional risk index most strongly, accounting for 17.3% of the total variance in the NSE. A combination of significance of health, concerns about health, housing, condition of teeth, activity, and smoking pattern accounted for 29.8% of the variance in the NSE. Condition of teeth accounted for only 3.8% of the variance in BMI. Conclusion: It is necessary to be aware that variance in the NSE can be affected by several factors including depression and lifestyle and that depression is strongly related to poorer NSE.
