According to the increase of the proportion of aged people, the medical demand for a senile chronic disease has been increased; therefore, aged people call for a geriatric hospital for special geriatric medical service. The main purpose of this study was to analyze the general characteristics and financial status of geriatric hospitals. For the study, a questionnaire was designed and sent to the geriatric hospitals to fill out the patient statistics, number of headcount by department, etc. to find out the stability, profitability, activity and so on financial statements of the hospitals were analyzed. The major findings of this study were as belows. 1. The ratio of the medical expenses to the revenue of the geriatric hospitals is much lower than acute care hospitals. But the probability of bankruptcy is higher due to the high ratio of the liabilities therefore it is required to stabilize the financial position by donating more money. 2. Government budget for the elderly people is not enough. To support the geriatric hospitals by going subsides, government should increase the budget. 3. Portion's of the patient of the geriatric hospitals are government support patient. Since the government doesn't pay the medical charges quickly, geriatric hospitals have a serious cash flow problem. Therefore, it is required that government is to prepay the bill. 4. Since geriatric hospitals treat elderly patient and most patients are government support patients, geriatric hospitals can be said to operate under the strict. 5. When we introduce the daily medical charge, the self-liability will be reduced on approximately 50% of current. This affection will bring a huge progressing financial structure to the medical profit of the geriatric hospital, and also patient family will feel less economical burden.
Co-residence is a type of intergenerational private transfers of resources: money, time and space. Adult daughters and their elderly parents decide to co-reside, depending on their utility levels before and after co-residence that mainly depend on the health status of the elderly. Therefore, co-residence implies positive net benefits to both parties in the sense that, when they co-reside, elderly parents share childcare and adult daughter provide elderly care. In other words, formal (paid) care can be substituted with informal (unpaid) one. Both marriage and giving births are considered as the major obstacles to labor market attachment of women who bear burdens of home production and childcare. Co-residence can be a solution for married women to avoid career interruption by sharing burdens with their elderly parents. However, most previous studies using the U.S. data on intergenerational private transfers focused on elderly care and have concluded that they reduce government expenditures associated with public subsidies to the elderly. This study focuses on adult daughters and it examines effects of co-residence on labor supply of married women in Korea, who face limited formal childcare programs in terms of both quantity and quality. It applies the Tobit model of married women's labor supply to the data from the Second Wave of the Korean Labor and Income Panel Survey( 1999), in order to investigate effects of co-residence and the work and health status of the co-residing elderly as well as their own health status. Four specifications of the empirical model are tested that each includes co-residence with elderly parents, their gender, or their work and health status. Estimation results show that co-residence, co-residence with female elderly, and co-residence with not-working female elderly have significant positive effects on labor supply of married women while poor health status of co-residing female elderly does not bring about any negative effects. However, co-residence with male elderly, regardless of their work and health status, has no significant effect The results indicate that co-residence is closely related to sharing of home production among female elderly and adult daughters who are married and, through intergenerational private transfers of resources in terms of time, it helps women avoid career interruption.
Purpose: This study tries to explore experiences with a special rating dementia service among family caregivers of elderly people with dementia. Methods: The participants were 11 family caregivers of elderly people with dementia and had used a day-care service from the special rating dementia service for more than 3 months. Data were collected through individual in-depth interviews and analyzed using content analysis. Results: Three categories emerged: "reminiscence of an entangled life", "feeling relieved from a stifled life", and "hope for a better life through help and support" with 8 sub-categories and 23 codes. By using the special rating dementia service, the participants experienced relief from the burden of caring for their elderly family member with dementia and restored their social relations or family relationships. They experienced a positive transition in which their caring for elderly family members with dementia was integrated into their daily lives, and they expressed their desires for better policies. Conclusion: These findings imply the importance and necessity of the special rating dementia service. Practical and systematic family support programs are necessary and proactive publicity activities by the government and public agencies are suggested to ensure that family caregivers can easily use the special rating dementia system.
Objectives: The purpose of this study was to investigate the oral health status and oral health behavior and identify the factors related to the oral health status by age-specific groups in vulnerable elderly individuals. Methods: We used data from the 7th Korea National Health and Nutrition Examination Survey. The differences in the oral health status and oral health behavior by age-specific groups were analyzed using complex sample chi-square tests and a generalized linear model. The relationship between the oral health status and oral health behavior by age-specific groups was analyzed using a complex samples general linear model. Results: The DMFT index of the young-old elderly was 10.65±0.60, and that of the old-old elderly was 12.78±0.72, which was higher. The condition of the mandibular prosthesis was more common in the old-old elderly, and there was a statistically significant difference (p<0.05). The DMFT index in the old-old elderly was found to be higher as the average number of brushings per day decreased, and no oral examination was performed. Conclusions: The vulnerable elderly themselves are interested in maintaining their own health, and the government needs to undertake efforts to reduce the medical blind spots by supporting institutional and environmental conditions so that health care services can be provided to the vulnerable elderly.

Government came to enforce long-term care insurance system in preparation for the rapid aging society. Whether this system is successful or not depends on the professionalism of long-term care-givers who are professional population in charge of care service. Currently in the early stage of enforcement, such problems as a race cutting fee resulted from numerical increase of educational facilities, insolvent operation, degradation of education level resulted from unprofessional instructor, are pointed out. As a mean of manpower-training on long-term care insurance system, this study is to research public-private-university network model of the Academy of Continuing Education attached to Ko Sin University which is the case of Yong Do Gu Busan city. Networking between the vision and development strategy of Yong Do Gu on continuing education city, education system on community manpower-training supported by Ko Sin University, and service field of welfare for the elderly can not only contribute to the professionalism of long-term care-givers but also play an ideal role in manpower-utilization and arrangement of community. Through this networking, high quality of education level and circumstance, using the existing infra, manpower-training and utilization for continuing education of Yong Do Gu can be accomplished. Additionally, the connection with facilities related with welfare for the elderly can contribute to professionalism and accountability of manpower-networking.
With a continuous and steep increase in life expectancies, Korean society is expected to enter the aged society by year 2020. And as the number of elderly increases, the burden of medical and health care expenses for them becomes greater in every developed society. Hence, the preventive approach for chronic degenerative diseases remains to be the best solution for the above-mentioned problem and warranting optimal nutrition would be one of the most important approaches. We performed a nutrition survey on 585 older adults of 50 years of age and older, residing in 3 metropolitan areas including Daejun, Daeku and Kwangju. Anthropometry, including body composition analysis based on the bioelectrical impedance analysis using InBody 3.0 and dietary intake survey by semi-quantitative flood frequency questionnaires, were used in collecting data. As one of the most important factors affecting the health and nutritional status of the elderly, we focused on living arrangements. Analyses were performed on the data from 550 subjects only, after excluding statistical outliers. Three hundred and sixty-eight of them(66.9%) were female and the number of elderly(65 years of age and older) was 485. According to the statistical analyses, the female elderly were more vulnerable to malnutrition than the male elderly. And the older they befame, the less adequate they were in nutrient intake. In addition to this, the elderly living alone showed the poorest patterns of nutrient intake and anthropometry. Although the exact effect of living alone could differ among different sex-age groups, the fact that the elderly living alone is vulnerable to malnutrition would remain concrete. This raises the utmost necessity of nutrition intervention to be devised and directed to the targeted population, namely the living-alone elderly from the government level. The intervention may include nutrition education, nutrition counseling and support In forms of meal service by networking the efforts of central as well as local governments to ensure the good health of the Korean elderly.
Purpose: This study conducted a survey on the elderly of 65 years and over with physical debilities and ones capable of living at home residing in Jeju Special Self-Government Province to compare and analyze demo-sociological characteristics and factors influencing on oral health related living quality. And also this study intended to provide basic data for developing effective public medical policies and health promotion programs to increase oral health related living quality of the elder. Methods: The elderly of 65 years and over living in Jeju Special Self-Government Province were interviewed individually from 7 February 2011 to 18 April and interview results of a total of 220 subjects were analysed for this study. Results: Current status of the elderly including ones with & without physical debilities influenced functional limitation and behavioral aspects of GOHAI criteria used in this study. The elderly with physical debilities experienced less limitation in food chewing and swallowing, and pronunciation than ones capable of living at home. On the analogy of the previous study(by Park, N. G., 2010) in which oral health related quality and satisfaction of life of the elderly with physical debilities were different due to their physical, circumstantial and psychological limitations, the former's oral health conditions are worse than the ones capable of living at home and because of medical care accessability limitation they suffer from deteriorated oral condition. By the comparison of factors influencing on the living quality relating to the oral health of the elderly with physical debilities and ones capable of living at home, 2 factors, age and living area, were meaningful factors commonly influencing on the oral health related living qualities of both. The elderly of 75 years and over were more affected by psychological and behavioral aspects of oral health related living quality than the ones of 65-74 years, and the ones living in country suffered from functional limitations, pains and discomfort more than ones in city. Additionally, being different from the elderly capable of living at home, the ones with physical debilities were influenced by the factors of average monthly income and medical security type. Conclusion: Improvement of programs and systems to increase oral health related quality of life needs to be carried out preferentially for the elderly of 75 years and over, and dwelling in country. Also this study suggests that the policy of paying the denture insurance allowance in 2012 need effective planning considering the elderly's current status, age, living area, medical security type.

This study examined the relationship between the sociodemographic characteristics, health related factors, physical functions (ADL, IADL), mental functions (CES-D, MMSE-K), oral health impact profile (OHIP-14), and the quality of life (WHOQOL-BREF) for the elderly in long-term care facilities. The research was conducted in 602 facility allowance beneficiaries authorized to be eligible for long-term care in long-term care facilities through personal interviews using a structured questionnaire from May 1 to June 30, 2016. As a result, the quality of life was lower among females than males in the group receiving government subsidies than the group whose livelihood was maintained by themselves or their children, in the group with a longer period of care, in the drinking group than the non-drinking group, in the group with irregular exercise than the regular exercise, in the group with irregular meals than regular meals, in the group with poor subjective health conditions than good subjective health condition, in the group with a smaller number of daily toothbrushing, in the group with xerostomia than no xerostomia, in the group with a lower OHIP-14, in the group with a lower ADL and IADL, and in the group with a lower CES-D and MMSE-K. In particular, the quality of life was affected more by health-related factors and CES-D and MMSE-K than by other factors. Therefore, it is necessary to make efforts to prevent depression and cognitive impairment, including health-related behavior, with the objective of improving the quality of life for the elderly in long-term care facilities.
This study was conducted to identify and compare the needs of the integrated by local government, and to provide basic data for the provision of integrated care services. This is a secondary data analysis study using data from 10,299 elderly people living in 17 local governments who participated in the '2017 National Survey on the Aged People in Korea'. The difference in medical needs, daily living support needs, and social activity support needs were 2.4, 6.0, and 2.0 times higher in the highest regions than in the lowest regions. In addition, the size of the medical needs group, welfare needs group, and integrated care needs group varied by region. Through this study, it was confirmed that the level of integrated care needs varied by region. Therefore, in order to efficiently provide the services at the national level, it is necessary to accurately assess the needs of the aged in each local government, and accordingly, prioritize and identify available resources within the care service and appropriate allocation.
The South Korean government has introduced a new Customized Care Service for Older Adults by eliminating the six existing care services for them since 2020. It is a significant change of care service system for older adults and the service would play a key role in providing preventive services for them. The aim of the study is to examine the meaning and tasks of the introduction of the Customized Care Services for Older Adults in South Korea by using the framework of Gilbert and Terrell. The study found that the new service was designed under the government's policy contexts of the establishments of public-based social service providers and the emphasis of community care, and it has brought about a number of significant institutional changes of care system in terms of allocation, benefits, delivery, and finance. The institutional coverage of the service was increased to lower 70% of older adults and a large number of kinds of health and social care services would be provided for them. The sphere of living was set in each local area and the new service providers are entirely in charge of providing the services in each sphere. The financing system was changed to support service providers by providing government subsidies. Although a number of positive results would be expected to occur for the elderly, it is likely that some challenging tasks would happen in the fields as follows: the inadequate coverage, the vagueness of the application of self-care principle and the decrease in the amounts of direct care services, the inappropriate area of service provision, and the supplier-oriented provision of services rather than user-oriented.
