Factors Influencing Care Burdens of Caregivers of Elders with Dementia who Request Dementia Domiciliary Welfare Services (재가복지서비스 신청 치매노인 주부양자의 부양부담감 영향 요인)

  • Purpose. The purpose of the study was to identify the factors, which predict care burden perceived by caregivers of elders with dementia. Methods: The participants in this descriptive survey were 92 caregivers who used one of six daycare centers located in Incheon. The data were collected by questionnaires composed of items on general characteristics of the elders and caregivers, care burden, caregiver fatigue, and functional status of the elders. Results: General characteristics of the elders associated with care burden were age, gender, religion, and the presence of a spouse. Features of caregivers related to care burden were education, relationship with elderly, amount of rest, intention to care, monthly family income, and perceived family economic status. There were significant correlations among care burden, fatigue of caregiver and functional status of the elders. In stepwised multiple regression analysis, significant influencing factors were identified as caregiver fatigue, functional status of the elders, intention to care, relationship with the elders, amount of rest and elder's gender. Those variables explained 46% of variance of care burden. Conclusion: Functional status of elders and amount of rest were significant predictors and are supported by other previous studies. Future interventions for caregivers need to be focused on the slowing down of functional status of elders and increasing of number and length of caregiver's rest periods.

Estimate over the Number of Chronic Disease Patients and Medical Care Expenditure at the Time of Transition of Baby Boomer into 65 Years Old Aging Population (베이비붐세대가 65세 노인인구로 전환 시의 만성질환 환자수와 진료비 예측)

  • Purpose: The purpose of study is to estimate the number of chronic disease patients and medical care expenditure at the time baby-boomers belong to 65 years old aging population, and compare with current 65 year-old aging population. Methods: Analysis method used an estimating formula devised by the researcher and estimated the number of chronic disease patients and medical care expenditure of each generation. Results: When comparing the estimated number of chronic diseases patients of each generation, 40.6% of the first generation, 76.4% of the second generation, 95.2% of third generation are expected to get chronic disease. When comparing each generation's total medical care expenditure, based on the estimated number of chronic diseases patients of each generation, the second generation( 1,206,251,224 thousand won) showed higher than other generation. This study compared the number of chronic disease patients and medical care expenditure between the second generation of the elderly and current elder generation. As a result, the second generation patients was higher than the fourth generation in high blood pressure, diabetes, psychological and behavioral disorder, and neurological diseases whereas the fourth generation is only high the number of patients in heart disease. As for total medical care expenditure, the second generation paid more in high blood pressure, psychological and behavioral disorder while the fourth generation in neurological disease and heart disease. Conclusion: It is desired that considering the number of chronic disease patients and medical care expenditure of baby boomers accounting for 14.6% of total population, in-depth follow-up study is carried out that inquires into what are issues with a current chronic disease management project, what business is needed in order to manage these issues, and how to fund to cover increasing medical care expenditure.

Korean Long-Term Care Insurance System and Caring Justice (노인장기요양보험제도와 돌봄 정의)

  • The study aims to analyse Korean Long-Term Care Insurance system in terms of caring justice on the premise that elder care should be included in discussions and policies of care. Caring justice means an ideal of equal sharing duties and rights of care by all citizens. Four dimensions of caring justice(decommodification, defamilialization, degenderization and elderly participation and power) were established for the analysis. The results of the analysis were presented that Korean Long-Term Care Insurance system was maintained by commodificated and gendered care services attempting defamilialization with the exclusion of elderly beneficiaries, which represented typical caring injustice. Policy suggestions were made to realize caring justice: improving the status of caring labour by achieving proper service price and public employment, reorganization of life cycle based caring system integrating children, disabled adults and elders, and developing user-centered long-term care system to guarantee participation and choice of people in caring relationships.

  • In this paper, Zigbee/PLC gateway system is designed for Ubiquitous health care. A protocol conversion algorithm for smooth information exchange between Zigbee and PLC communication has been implemented on the gatway. If a moving object is detected by wireless sensor, the data is transmitted to gateway. Zigbee/PLC gateway analyzes received data and transmits to Power Line Communication for real-time monitoring. Implemented system is can support elder who lives alone activity analysis, crime prevention system, Home network service.

A Study of the Elderly's Needs for the Development of Elder Care Programs in Rural Communities (마을공동체 중심의 노-노(老-老) 돌봄 개발과 활용을 위한 농촌노인의 욕구 조사)

  • The purpose of this study was diagnosed to grasp the needs for the elderly-elderly care activity in rural areas. The research was conducted in questions and case studies in 7 farming villages. The major findings of this study as follows: First, the senior citizens in rural areas were not in good health, and the illnesses were revealed to be arthritis, high blood pressure, and diabetes. At the occurrence of an illness, they complained about the discomfort in doing daily life routines and expected eir spouses or offsprings to te care of them. Elders in the rural community responded that they had difficulty and financial problems in family maintenance. Leisure activities included watching TV, playing card/board games, or spending time without any specific activities. Second, recipients of elderly-elderly care expected to receive services from a woman in her 50s $1{\sim}2$ hours, once to twice a week. Third, the service providers wanted to be elderly-elderly care takers, hoping to work once a week for $2{\sim}3$ hours. They wanted the education for the elderly-elderly care to be once a week for 3 hours, with understanding the elderly, consultation, preventing senile dementia, and health as the contents.

  • The substance of this study can be summarized as follows. In introduction, this paper traces the factors of the advent of social care insurance in Germany and Japan and Premises that it is necessary to study social care insurance. In Chapter II, this paper examines the new idea of the recent social security system in japan, centering on the limit of social security policy today, the increase in the number of the poor and the needy and securing financial resources for elder welfare. In Chapter III, this paper studies the process of materialization for the introduction of social care insurance by considering the process of making "Outline of the Bill of Social Care Insurance System" and the general frame of the system in this "Outline". In Chapter IV, this paper explains the facilities in charge and the system of them, the contents and methods of social care insurance, all the arrangements needed in the process of practice, and the like, in connection with all the arrangements for and practical problem of social care insurance which will take effect in earnest in April, 2000 in Japan.

Factors Affecting Adaptation for Elders in Long-term Care Facilities (노인 장기 요양시설 적응 영향요인)

  • The purpose of this study was to identify the facility adaptation characteristics, and the relationship between facility adaptation and the potential affective factors; ADL, self-efficacy, and quality of care facility, and to identify the affective factors on facility adaptation for elders in long-term care facility. Data were collected from 177 none dementia elders over 65 in 9 long-term care facilities from Jan.2, 2011 to March 15, 2011. The data were analysed through t/F test, correlation coefficient, and multiple regression (stepwise). The major findings of this study were as follow; Facility adaptation shows significant difference from admission decision maker(p=.004), religion(p=.027), and motive of admission(p=.009). And facility adaptation has weakly correlated with ADL and quality of care facility. The affective adaptation factors were decision maker, motive of admission, personal interview, religion, and perceived health status, but showed 22.8% of explanation. Thus the findings from this study could be used as bases for nursing intervention supporting elders' care facility adaptation.

A Study on Research Trends and Development Process of Health Policy for the Elderly (노인보건정책의 발전과정 및 연구동향에 대한 일고찰)

  • This paper is to suggest policy issues to be done in the future health policy for the elderly, analyzing contents of results suggested in academic papers published in Journal of the Korean Gerontological Society. Number of Papers published in the Journal is 61, occupying most of papers related with chronic disease, medical expenditures and health delivery system except dental and psychiatric health. Method used to be analyzed is mostly random sampling among community inhabitants or hospital patients. Policy issues are as follows in case of being based in the above analysis results. Firstly, it is necessary that the development of health promotion specialized for the elderly be developed and activated. Secondly, it is necessary that the primary health care system specialized for the elderly be constructed. Thirdly, it is necessary that the subacute care system specialized for the elderly be constructed after acute care services. Finally, it is necessary that the health care system specialized for the elderly in need of long-term care be constructed also.

The Level of Importance on Education of long Term Care Nursing Assistants Perceived by Caregivers of the Elderly (가족, 간병인, 간호사가 인지하는 노인요양보호사 교육의 중요도)

  • Purpose: This study aimed to explore the level of importance perceived by caregivers of elderly on the education of long term care nursing assistants (LTCNAs) taking care of elders with dementia or stroke. Method: Data was collected from 296 participants (112 families, 98 NAs, and 86 RNs) from October 2006 to February 2007. A structured questionnaire was used for data collection. Result: The item of 'attitude while caring for the aged' was identified as most important. The family group reported that 'position change' and 'understanding of geriatric diseases' were the most important for education, while LTCNAs, highly identified, 'bathing', and 'bed sheet change'. The RNs group regarded 'position change', and 'bathing' as most important. Institutions demanding LTCNAs' activities were special facilities for elderly care and special hospitals for the aged, and the need was also high in families with an elder incapacitated by illness. Conclusion: Nurses need to take an interest in education for enhancing the quality of LTCNAs and thus advance the quality of nursing care as well as the quality of life for the aged.

The Relationship between Sense of Loss and Suicidal Ideation among Rural Elders use Primary Health Care Post's Services : Mediating Effect of Depression (보건진료소를 이용하는 농촌노인의 상실감과 자살생각과의 관계: 우울의 매개효과)

  • Purpose: The purpose of this study was to examine the mediating effects of depression on relationship between sense of loss and suicidal ideation among Rural Elders use Primary Health Care Post's Services. Methods: This study was conducted using a descriptive survey design. Participants were 345 rural elders use Primary Health Care Post's Services in Jeollanam-do. Data were collected from August 1 to 31, 2017 using self-report questionnaires. Data were analyzed using descriptive statistics, Pearson's correlation coefficient, hierarchical regression and Sobel test with SPSS 21.0. Results: There were significant positive correlations between sense of loss, depression and suicidal ideation. It was found that physical loss is very high and significantly different only subjective health status on suicidal ideation. Also, Depression partially mediated the relationship between sense of loss and suicidal ideation of Rural Elders. Addtionally, The results show that depression plays a key role in suicidal ideation of Rural Elders. Conclusions: Based on the present findings, nursing interventions should be developed to decrease depression, with additional consideration towards sense of loss, specially physical loss, in order to prevent suicidal ideation among Rural Elders use Primary Health Care Post's Services.
