Impacts of Emergency Medical Technicians' Personal Traits on Job Related Outcome Variables (응급구조사의 성격유형이 직무관련 산출변수에 미치는 영향)

  • The purpose of this study is to identify the effects of emergency medical technicians' personal traits and job characteristics on job/social stress, job satisfaction, role conflicts, organizational commitment, and self-efficacy. The study population was emergency medical technicians who is working at the hospitals in Yeungnam province. The 200 questionnaires were administered by using ground mail, e-mail, and personal visits and 156 questionnaires were returned(82.5% response rate). Social stress, job satisfaction and role conflicts were significantly determined by personal traits and job characteristic variables. However, job stress, organizational commitment and self-efficacy was only significantly determined by job characteristic variables. In social stress and role conflicts, the subjects with challenge, sociability, acceptance and prudence traits were tend to be significantly higher scores compared to the stability traits. Additionally, among social stress regression models, adding job characteristics to the personal traits model, $R^2$ was increased up to 19% and adding personal traits to the job characteristics model, $R^2$ was increased up to 14%. In conclusion, the study found that personal traits and job characteristics are important variables in explaining social stress, job satisfaction and role conflicts, that would have important managerial implications for recruiting, hiring and managing either new or current emergency medical technicians efficiently.

The Study on Skin Safety and Efficacy of Codonopsis Lanceolata Root Fermentation Extract (더덕 발효추출물의 피부 안전성 및 화장품제형에서의 피부개선효과에 관한 연구)

  • Codonopsis lanceolata belongs to the Campanulaceae family, which is a perennial herb. It is widely used as an antitussive agent and an expectorant in oriental medicine. Codonopsis lanceolata contains large amounts of polysaccharide and phenolic compounds and is known to have anti-oxidative effect and improve immunity. We evaluated the efficacy and the skin safety and stability of cosmetic that is applied Codonopsis lanceolata extract fermented by lactic acid bacteria. As result, formulation of Codonopsis lanceolata extract improved the human skin moisturizing effect, increased TEWL 196%, skin hydration 15%, and the sebum rate 131%.

Pemetrexed is Mildly Active with Good Tolerability for Treatment of Patients with Colorectal Cancer

  • Purpose: This systematic analysis was conducted to evaluate the efficacy and safety of pemetrexed based salvage chemotherapy for treatment of patients with metastatic colorectal cancer. Methods: Clinical studies evaluating the efficacy and safety of pemetrexed based regimens on response and safety for patients with colorectal cancer were identified using a predefined search strategy. Pooled response rates (RRs) were calculated. Results: For pemetrexed based regimens, 4 clinical studies including 201 patients with advanced colorectal cancer were considered eligible for inclusion. The analysis suggested that, in all patients, pooled RR was 20.4% (41/201). Major adverse effects were neutropenia, anorexia, fatigue, and anemia. No treatment related death occurred with pemetrexed based treatment. Conclusion: This systematic analysis suggests that pemetrexed based regimens are associated with mild activity with good tolerability in treating patients with metastatic colorectal cancer.

Efficacy of 10-Injection Induced Modified Rush Immunotherapy in Dogs with Atopic Dermatitis

  • Modified rush immunotherapy (IT), by combination of rush IT and conventional IT, provides a faster method to reach maintenance dose, leading to higher patient adherence when compared with conventional IT, decreasing systemic adverse reactions when compared with a standard rush IT. Ten atopic dogs of this study include fulfillment of Favrot's criteria. Offending allergens were identified by the use of IDST. During the induction period, the dogs were received a total of 10 injections. Five injections were administrated every 30 minutes in a day with gradually increasing amounts and concentrations of allergens, and the last 5 injections were administered every 3 days. The efficacy of 10-injection induced mRIT was assessed using the canine atopic dermatitis extent and severity index (CADESI). During maintenance period, reduction rate from baseline scores varied between 3.2% and 60.9% and the after 6 months of therapy for CADESI-03 score in 6 of the 10 dogs. Adverse reactions were not observed in these dogs during induction period by mRIT with 10 injections. Based on these results, our modified rush IT protocol is considered to be a useful protocol to treat canine atopic dermatitis.

Irinotecan as a Second-line Chemotherapy for Small Cell Lung Cancer: a Systemic Analysis

  • Purpose: This analysis was conducted to evaluate the efficacy and safety of irinotecan based regimens as second-line chemotherapy in treating patients with small cell lung cancer. Methods: Clinical studies evaluating the efficacy and safety of irinotecan based regimens as second-line chemotherapy for patients with small cell lung cancer were identified using a predefined search strategy. Pooled response rates (RRs) of treatment were calculated. Results: In irinotecan based regimens as second-line chemotherapy, 4 clinical studies which including 155 patients with small cell lung cancer were considered eligible for inclusion. In all chemotherapy consisted of irinotecan with or without nedaplatin. Pooled analysis suggested that, in all patients, the pooled RR was 27.1% (42/155) in irinotecan based regimens. Nausea, vomiting, diarrhea and myelosuppression were the main side effects. No grade III or IV renal or liver toxicity was observed. No treatment related death occurred with the irinotecan based treatments. Conclusion: This systemic analysis suggests that irinotecan based regimens as second-line chemotherapy are associated with mild response rate and acceptable toxicity for patients with small cell lung cancer.

Biological Control of Soybean Anthracnose by Pseudomonas sp. (Pseudomonas sp.를 이용한 콩 탄저병의 생물학적 방제)

  • Pseudomonas sp. antagonistic to Colletotrichum truncatum and C. gloesporioies was selected as a biological control agent for soybean anthracnose. Pseudomonas sp. inhibited the mycelial growth of pathogens effectively as the funhicides such as benomyl and fluazinam in vitro tests without any adverse effects on soybean. Seed treatment with Pseudomonas sp. increased emergence rate of soybean seeds significantly after inoculaton with C. truncatum. When the suspension of Pseudomonas sp. was sprayed on soybean plants, the control efficacy was not different from that of fungicides, benomyl and fluazinam two weeks after treatment, however the efficacy did not last long enough after three weeks.

A Comparison of Hydromorphone-Bupivacaine and Fentanyl-Bupivacaine in Patient Controlled Epidural Analgesia after Thoracotomy (개흉술 후 경막외 통증자가조절을 설시한 환자에서 Hydromorphone-Bupivacaine과 Fentanyl-Bupivacaine 투여의 비교)

  • Background: Hydromorphone has an intermediate lipid solubility range that falls between morphine and fentanyl. Lipophilic activity during opioid epidural administration is important in relation to both the side effects and analgesic efficacy. The purpose of this study was to compare epidural hydromorphone and fentanyl when concomitantly infused with bupivacaine in patients undergoing a thoracotomy. Methods: Seventy-seven thoracotomy patients, with patient-controlled epidural analgesia (PCEA), were blindly allocated into two groups [group F (n = 34); 0.1% bupivacaine and fentanyl $5{\mu}g/ml$, group H (n = 34); 0.1% bupivacaine and hydromorphone $16{\mu}g/ml$)]. The basal PCEA rate and demand dose were 4 ml/hr and 3 ml, respectively. The visual analogue scale (VAS) for pain, and pruritus, sedation and nausea were measured at 6, 12 and 24 hours after the operation. Results: There were no significant differences in the VAS pain scores and the incidences of pruritus, nausea and sedation between the two groups. The total infused volume after 24 hours was lower in H compared to that of F group (P < 0.05). Conclusions: We conclude that epidural hydromorphone or fentanyl administration has a similar analgesic efficacy and shows similar incidences of side effects, when concomitantly infused with bupivacaine, in the management of acute pain following a thoracotomy.

Removal of Serratia marcescens Aerosols Using an Electrostatic Precipitator Air-Cleaner

  • We characterized the efficacy of an electrostatic precipitator (ESP) air-cleaner in reducing the concentration of Serratia marcescens in an enclosed space. We used an experimental room ($4.5{\times}3{\times}2.9\;m$) in which electrostatic air-cleaners were located. Two air-cleaners enhanced the equivalent ventilation rates in the chamber by about 3.3 air changes per hour (ACH) over the 2 ACH provided by the mechanical ventilation system. Natural die-off of the organisms provided an additional equivalent of 3 ACH, so that the total ventilation rate with the ESP air-ccleaners was 8.3 ACH. We also examined whether the ESP air-cleaners altered the deposition of Serratia marcescens aerosols on the experimental room surfaces. We did not find any significant differences in the number of colony forming units recovered from surfaces with and without the air-cleaners. We installed UV lights inside the ESPs and determined if UV light, in addition to electrical fields, increased the efficacy of the ESPs. The presence of UV light inside the ESP reduced S. marcescens aerosols by approximately 2 ACH. Finally, a box model indicates that the efficiency of the air-cleaner increases for both biological and nonbiological particles at ventilation rates of 0.2-1, which are typical for residential settings.

Clinical Efficacy of Endoscopic Pancreatic Drainage for Pain Relief with Malignant Pancreatic Duct Obstruction

  • Objective: This study was conducted to investigate the efficacy of pancreatic drainage for pain relief in advanced pancreatic cancer. Method: Seventy-one patients with pancreatic carcinoma were divided into two groups: dilated and non-dilated pancreatic ducts. All patients underwent endoscopic retrograde cholangiopancreatography (ERCP), endoscopic biliary stenting and pancreatic stenting. Visual Analog Scale (VAS) scores, pain remission rates and survival time were evaluated during follow-up. Results: The post-ERCP VAS score of the dilated group was lower than that of the non-dilated group at 1 and 3 months post-ERCP. There was no difference at 6 months. The pain remission rate in the dilated duct group was significantly higher than that in non-dilated duct group in 1 and 3 months post-ERCP. The median survival times were 8.17 and 8.22 months respectively. Conclusion: Endoscopic pancreatic drainage can relieve pain of advanced pancreatic cancer accompanied by safe dilation of the pancreatic duct.

  • The discharge characteristics of a flat plasma backlight with long electrode gap are investigated. The effect of operating voltage and repetition rate on brightness and luminance efficiency is investigated. A new high efficacy mode is found at low frequencies around 15-40 KHz; a lumen efficacy of 15.3 lm/W is achieved at a luminance of $2400\;cd/m^2$. In the high brightness mode, present at high voltage, we find a maximum luminance of $5900\;cd/m^2$ at 30KHz.
