Enhancement of bacterial disease resistance in rockish(Sebastes schlegeli) by $\beta$-glucan administration ($\beta$-Glucan 투여에 의한 조피볼락(Sebastes schlegeli)의 세균성 질병에 대한 저항성 향상)

  • The effect of $\beta$-glucan as an immunostimulant to increase resistance to bacterial diseases by enhancing non-specific defense mechanism in rockfish (Sebastes schlegeli) was examined by oral and bath administration. After oral or bath administration with $\beta$-glucan, the injection challenges with Vibro ordalii, Staphylococcus epidermidis and Edwardsiella tarda were performed to assess $\beta$-glucan efficacy. After injection of V. ordalii, oral administration for 30 days with 1% $\beta$-glucan showed 25% of survival rate. But all control fish died within 3 days after the injection. After injection of S, epidermidis, oral administration group for 20 and 30 days showed a remarkably increased survival rate of 95%. But oral administration of $\beta$-glucan to rockfish did not induce protection against experimental E. tarda infection. $\beta$-Glucan bath administration with or without formalin-killed V. ordalii showed that no protection was observed at 10 days after challenge. The results show that $\beta$-glucan to rockfish was effective to increase survival rate of bacterial infections of S. epidermidis and V. ordalii but not against E. tarda.

Results of a Ivor-Lewis Operation for Esophageal Squamous Cell Carcinoma (식도 편평세포암에시 Ivor-Lewis수술 성적)

  • Background: The Ivor-Lewis operation has been widely applied for treating thoracic esophageal cancer, but more acceptable results from three-field lymph node dissection have recently been reported. In this study the efficacy of the Ivor-Lewis operation was evaluated. Material and Method: Among the 273 patients, who underwent operation for esophageal cancer between September 1994 and August 2004, we retrospectively studied 172 patients with esophageal squamous cell carcinoma and who had no other primary cancer and who underwent complete resection with an Ivor-Lewis operation. The postoperative complications, the short and long-term survival and the recurrence patterns were analyzed. Result: The postoperative staging was as follows: stage I in 40 cases, IIA in 48 cases, IIB in 18 cases, III in 55 cases, IVA in 5 cases and IVB in 6 cases. The operative mortality rate was 4% (7 of 172 pts). Postoperative complication occurred in 32 patients (18%) and tumor recurred in 55 patients (32%). The overall 5-year survival rate was 48%; it was 85.6% in stage I patients, 47.6% in IIA patients, 65% in IIB patients, 22.8% in III patients and 0% for those in IV (p<0.05). The 5-year survival rate according to the location of esophageal cancer was 26.5% for patients with tumor in the upper 1/3 of the esophagus and 52.4% for patients with tumor in the mid and lower 1/3 (p>0.05). Conclusion: The Ivor-Lewis operation is an acceptable surgical procedure for thoracic esophageal squamous cell carcinoma. Yet it is necessary to consider other surgical procedures, and especially three-field lymph node dissection for treating upper 1/3 esophageal cancer.

Possibility of simultaneous control of pine wilt disease and Thecodiplosis japonensis and or Matsucoccus thunbergianae on black pine (Pinus thunbergii) by abamectin and emamectin benzoate (곰솔(Pinus thunbergii)에서 Abamectin과 Emamectin benzoate를 이용한 소나무재선충과 솔잎흑파리 밑 솔껍질깍지벌레의 동시방제 가능성)

  • Abamectin 1.8% EC and emamectin benzoate 2.15% were evaluated the simultaneous control effect of both insect pests on black pine (Pinus thunbergii) against Thecodiplosis japonensis and Matsucoccus thunbergianae. Abamectin 1.8% EC was highly effective against T. japonensis and M thunbergianae, however, emamectin benzoate was highly effective against only M. thunbergianae when abamectin and emamectin benzoate were applied through trunk injection against T. japonensis and M thunbergianae at the rate of 1 ml per cm in diameter of breast height. The rate of gall formation of T. japonensis was 0% at the applied year, but 85% at the following year. However, the rate of gall formation was over 90% at the treatment of emamectin benzoate showing no difference compared with control. Abamectin 1.8% EC and emamectin benzoate 2.15% showed high control efficacy representing 89.1% and 91.4% against M. thunbergiana, respectively.

Development of a Guidelines of the Herbal Medicine Treatment for Gastric Cancer on the Use of Systemic Review and Delphi Technique (체계적 문헌 고찰과 델파이 기법을 활용한 위암의 한약 치료에 관한 한의표준임상진료 지침 개발)

  • Objectives: This study was conducted towards developing guidelines of herbal medicine treatment for gastric cancer. Methods: We performed a systematic review and meta-analysis designed to investigate the efficacy of herbal medicine treatment for gastric cancer on four cancer questions; survival rate, metastasis, immune function, and quality of life. Based on the findings, we utilized a two-round delphi process with panel of 22 experts for their level of agreement. Results: Combined therapy group, herbal medicine treated with chemotherapy, was significantly higher in the 1-year survival rate (RR=1.27, 95% CI: 1.14 to 1.40, P=0.005, $I^2=71%$) and 3-years survival rate (RR=1.41, 95% CI: 1.16 to 1.71, P=0.91, $I^2=0%$) than chemotherapy group. The suppression of metastasis was higher in the combined therapy group (RR=0.62, 95% CI: 0.45 to 0.84, P=0.09, $I^2=54%$). The immunology function was higher in the combined therapy group compared with the chemotherapy group (MD=16.43, 95% CI: 13.25 to 29.61, P<0.001, $I^2=99%$). The quality of life score was higher in the combined therapy group compared with the chemotherapy group (RR=1.55, 95% CI: 1.21 to 2.00, P<0.66, $I^2=0%$). Conclusions: Among the Randomized controlled trials (RCT) included, the levels of survival rates, suppression of metastasis, immune function, and quality of life of the group treated with chemotherapy were lower compared to those treated with herbal medicine in addition to chemotherapy.

Usefulness of Small Caliber Catheter Insertion for a Spontaneous Pneumothorax (자연 공기가슴증 치료에서 소구경 도관 흉강삽입술의 유용성)

  • Background: The large caliber catheter used in the treatment of pneumothorax causes great damage to the chest wall and organs. The purpose of this study was to prove that the use of a smaller caliber catheter is effective in treating pneumothorax with decreasing admission period and that the recurrence rate of spontaneous pneumothorax is low. Methods: Patients who had been admitted for treatment of first time occurrence of pneumothorax between May, 2004 and December, 2008 were included in the study. The caliber of catheter used this study is 18 Guage (1.2mm). The efficacy of treatment, admission period and recurrence rate of treating pneumothorax with small caliber catheter were compared to the control group using a tube thoracostomy for treatment. Results: The admission period for primary spontaneous pneumothorax was 10.8$\pm$3.6 days for the group (n=68) using tube thoracostomy compared to 4.5$\pm$1.3 days for the group (n=31) using the small caliber catheter (p<0.05). There was no statistically significant difference in recurrence rate between the two groups. Conclusion: The use of a smaller caliber catheter for the treatment of pneumothorax reduces the admission period without a significant increase in recurrence rates.

The Clinical Study on Effects of Bee Venom Pharmacopuncture Therapy in Patients with FBSS(Failed Back Surgery Syndrome) (척추수술 후 증후군(Failed Back Surgery Syndrome) 환자 30례에 대한 봉약침 병행치료 효과의 임상적 연구)

  • Objective : This study was designed to evaluate the effect of bee venom pharmacopuncture therapy in patients with FBSS(failed back surgery syndrome). Methods : We investigated 30 cases of patients with FBSS, who had been treated from October 2010 to July 2011. We divided patients into two groups : group I was treated by acupuncture therapy only, and group II was treated by bee venom pharmacopuncture therapy and general acupuncture. We measured the efficacy of treatments using the numerical rating scale(NRS) and grade and straight leg raising(SLR) test. Results : 1. The treatment method for group II was more effective than that of group I in reducing the NRS score on the fifteenth day after admission but there was no statistically significant difference between the results of two groups on the fifth day after admission and the tenth day after admission. 2. The treatment method for group II was more effective than that of group I in NRS improvement rate from its admission day to the fifth day after admission and from the tenth day after admission to the fifteenth day after admission but there was no statistically significant difference between the performance of two groups from the fifth day after admission to the tenth day after admission. 3. Group II had a higher grade improvement rate from the seventh day after admission to the fifteenth day after admission than group I but no statistically significant difference was observed between the results of two groups from its admission day to the seventh day after admission. 4. In SLR test improvement rate the treatments applied to group II was more effective than those applied to group I. Conclusion : The results of this study suggest that bee venom pharmacopuncture therapy is effective in reducing pain for patients with FBSS. Further clinical research is needed to verify these results and findings.

Control Indian meal moth Plodia interpunctella by gas treatment

  • The Indian meal moth, Plodia interpunctella, is one of the most important pests of stored food in the food processing industry worldwide. To control the Indian meal moth, methyl bromide, phosphine, high carbon dioxide, sulfuryl fluoride and plant essential oil fumigation have been considered. However, these treatments have disadvantages. For example, depleting the ozone layer, showing resistance in insect, low control efficacy or need high cost for treatment. Chlorine dioxide ($ClO_2$) is strong disinfectant and insecticide. The gas caused a malfunction in enzymes. The oxidative stress induced by $ClO_2$ gas treatment damaged to a physiological system and all life stages of P. interpunctella. The gaseous $ClO_2$ is a convincing alternative to methyl bromide for controlling P. interpunctella. The gaseous $ClO_2$ was generated by a chlorine dioxide generator (PurgoFarm Co., Ltd., Hwasung, Korea). It generated highly pure $ClO_2$ gas and the gas blown out through a vent into a test chamber. Gas entry to the chamber was automatically controlled and monitored by a PortaSene II gas leak detector (Analytical Technology, Collegeville, PA, USA). The properly prepared eggs, larvae, pupae, and adults of P. interpunctella were used in this experiment. Data were analyzed using SAS 9.4. Percentage data were statistically analyzed after arcsine-root transformation. Analysis of variance was performed using general linear model, and means were separated by the least significant difference test at P < 0.05. Fumigation is an effective management technique for controlling all stages of P. interpunctella. We found that $ClO_2$ gas treatment directly effects on egg, larvae, pupae and adults of P. interpunctella. The gas treatment with proper concentration for over a day achieved 100 % mortality in all stages of P. interpunctella and short time treatment or low concentration gas treatment results showed that the egg hatchability, pupation rate, and adult emergency rate were lowered compare with untreated control. Also, abnormal pupae or adult rate were increased. Gaseous $ClO_2$ treatment induced insecticidal reactive oxygen species (ROS), and it resulted in fatal oxidative stress in P. interpunctella. Taken together, these results showed that exposure proper concentration and time of the gas control all stages of P. interpunctella by inducing fatal oxidative stress. Further studies will be required to apply the gas treatment under real-world condition and to understanding physiological reaction in P. interpunctella caused by oxidative stress.

Effect of Latex Coated Urea on Nitrogen Use Efficiency and Yield in Drill Seeded Rice (벼 무논골뿌림재배시(栽培時) Latex 입힌 요소의 시용(施用)이 질소(窒素) 이용(利用)과 수량(收量)에 미치는 영향(影響))

  • Latex Coated Urea(LCU) was compared with ordinary urea under different methods of application in terms of N use efficiency and yield of rice, 1995 and 1996. The study was carried out on Jeonbug silty clay loam, in Honam Agricultural Experiment Station. The fertilizer treatments involved (1) conventional application of urea (44kg N/ha at transplanting, 33kg N/ha at five leaves stage, 33kg N/ha. (4) 55kg N/ha at transplanting and 33kg N/ha as urea at panicle initiation stage, and (5) without N. It was found that by combining LCU(as basal application) and urea(as topdressing at panicle initiation stage), at the rate of 80% of conventional rate with ordinary urea is most effective for the saving of N and increasing the use efficiency of N by rice. It was, however, observed that the efficacy of LCU was affected by the temperature during the growth of rice.

Efficacy and Safety of Neurokinin-1 Receptor Antagonists for Prevention of Chemotherapy-Induced Nausea and Vomiting: Systematic Review and Meta-analysis of Randomized Controlled Trials

  • Objectives: Can addition of neurokinin-1 receptor antagonists (NK1-RAs) be considered as an ideal strategy for the prevention of chemotherapy-induced nausea and vomiting (CINV)? Researchers differ on this question. Materials and Methods: Electronic databases were searched for randomized control trials (RCTs) that evaluated the effectiveness and safety of NK1-RAs in preventing CINV. The primary end point was complete response (CR) in the acute, delayed, and overall phases after chemotherapy. Subgroup analyses evaluated the types of NK1-RAs, routines of administration, types of malignancies, regimens used in combination with NK1-RAs, and age of patients included in the studies. The incidences of different types of adverse events were also extracted to estimate the safety of NK1-RAs. Results: A total of 38 RCTs involving 13,923 patients were identified. The CR rate of patients receiving NK-RAs was significantly higher than patients in the control groups during overall phase (70.8% vs 56.0%, P<0.001), acute phase (85.1% vs 79.6%, P<0.001), and delayed phase (71.4% vs 58.2%, P<0.001). There were three studies including patients of children or adolescents, the CR rate was also significantly higher in the treatment group (overall phase: OR=2.807, P<0.001; acute phase: OR=2.863, P =0.012; delayed phase: OR=2.417, P<0.001). For all the other outcomes, patients in the NK1-RAs groups showed improvements compared to the control groups (incidence of nausea: 45.2% vs 45.9%, P<0.001; occurrence of vomiting: 22.6% vs 38.9%, P<0.001; usage of rescue drugs: 23.5% vs 34.1%, P<0.001). The pooled side effects from NK1-RAs did not significantly differ from previous reports and the toxicity rates in patients less than eighteen years old also did not diff between the two groups (P=0.497). However, we found that constipation and insomnia were more common in the patients of control groups, whereas diarrhea and hiccups were more frequently detected in patients receiving NK1-RAs. Conclusions: NK1-RAs improved the CR rate of CINV. They are effective for both adults and children. The use of NK1-RAs might be associated with the appearance of diarrhea and hiccups, while decreasing the possibility of constipation and insomnia.

Gefitinib Alone or with Concomitant Whole Brain Radiotherapy for Patients with Brain Metastasis from Non-small-cell Lung Cancer: A Retrospective Study

  • Background: Gefitinib, a tyrosine kinase inhibitor (TKI) of epidermal growth factor receptor (EGFR), is used both as a single drug and concurrently with whole brain radiotherapy (WBRT) the standard treatment for brain metastases (BM), and is reported to be effective in a few small studies of patients with BM from non-small-cell lung cancer (NSCLC). However, no study has compared the two treatment modalities. This retrospective analysis was conducted to compare the efficacy of gefitinib alone with gefitinib plus concomitant WBRT in treatment of BM from NSCLC. Methods: We retrospectively reviewed 90 patients with BM from NSCLC who received gefitinib alone (250mg/day, gefitinib group) or with concomitant WBRT (40Gy/20f/4w, gefitinib-WBRT group) between September 2005 and September 2009 at Sun Yat-Sen University Cancer Center. Forty-five patients were in each group. Results: The objective response rate of BM was significantly higher in gefitinib-WBRT group (64.4%) compared with gefitinib group (26.7%, P<0.001). The disease control rate of BM was 71.1% in gefitinib-WBRT group and 42.2% in gefitinib group (P=0.006). The median time to progression of BM was 10.6 months in gefitinib-WBRT group and 6.57 months in gefitinib group (P<0.001). The median overall survival(OS) of gefitinib-WBRT and gefitinib alone group was 23.40 months and 14.83 months, respectively (HR, 0.432, P=0.002). Conclusion: Gefitinib plus concomitant WBRT had higher response rate of BM and significant improvement in OS compared with gefitinib alone in treatment of BM from NSCLC.
