협응적 이동 훈련 프로그램 적용이 편마비 환자의 균형과 보행능력에 미치는 효과 (Effects of Coordinative Locomotor Training Program on Balance and Gait of Stroke Patients)

  • Purpose: This study sought to examine the effect of coordinative locomotor training (CLT) program on the balance and gait of stroke patients and to develop effective programs and training methods to improve the functions of such patients. Methods: Subjects included 29 patients with hemiplegia caused by stroke. The subjects were randomly divided into an experimental group (n=14) that participated in CLT program and a control group (n=15) that participated in general exercise therapy. The experimental group underwent CLT program, while the control group underwent general exercise therapy, for 30 minutes, 3 days per week for a 6-week period. timed up and go test (TUG), four square step test (FSST), figure-of-8-walk test (F8WT), and 10m walking test (10MWT) were conducted to evaluate changes in balance and gait. Results: After the intervention, significant differences (p<0.05) were seen in the TUG, FSST, F8WT, and 10MWT in both groups. The experimental group showed more significant improvement than the control group(p<0.05). Conclusion: The results from this study indicate that a CLT program is extremely effective for improving the balance and gait in stroke patients.

중풍환자의 불면에 대한 산조인 단미(안심산)의 유효성 및 적응증 평가 (Effectiveness of Zizyphus seed (Ansim-san) for Insomnia in Stroke Patients)

  • Objectives : Zizyphus seed has long been used in hypnotics and sedatives in oriental medicine, and it is reported that the Zizyphus seed elicited a variety of pharmacological actions besides CNS depression. We did a clinical study on the effectiveness of Zizyphus seed (Ansim-san) for insomnia in stroke patients. Methods : To 41 stroke patients suffering from insomnia, we administered the extract of Zizyphus seed (Ansim-san). Outcome measures were subjective in nature and included a morning questionnaire, visual analog scales, and observation forms by study personnel. We also, to find the applicable symptom-complex of Zizyphus seed (Ansim-san), examined accompanying symptoms. Results : 1. Zizyphus seed extract use meant that sleep latency was shorter, total sleep time was longer, number of wakenings was lower, wake time during sleep was shorter, sleep quality was improved, condition upon awakening was improved, ability to concentrate was improved, and the patients fell asleep more easily (P<0.001). There were no differences in morning sleepiness. 2. Among accompanying symptoms, an oppressed feeling in the chest was significant (P=0.035) in the patients who improved sleep quality. Conclusions : The results of this clinical study suggest that in stroke patients, Zizyphus seed (Ansim-san) can be effective for insomnia, and significantly effective in patients who have an oppressed feeling in the chest.

개인맞춤형 주거환경개선이 작업수행만족도 및 일상생활활동에 미치는 효과 : 뇌졸중 환자를 대상으로 한 사례연구 (The Effects of Personalized Residential Environment Improvement on Occupational Performance Satisfaction and Activities of Daily Living : Case Studies in Stroke Patients)

  • Purpose: The purpose of this study was to investigate the effects of personalized residential environment improvement on occupational performance satisfaction and activities of daily living(ADL) in stroke patients, and desire to use as the basis for presenting an effective method for improving the residential environment of the disabled patients. Method: This study has been carried out with 3 stroke patients undergoing therapy for rehabilitation at the S hospital from August 2014 to January 2015. Residential environment improvement was conducted based on the desired space. Occupational performance, satisfaction and ADL assessed by modified COPM, K-MBI. Intervention has provided grab bar and aids fit to the environment of each person. Result: After residential environment improvement, ADL score was improved, but improved scores for specific items only. In occupational performance and satisfaction, there was a significant difference. Conclusion: The results of this study were to find out that there is a positive effect of personalized residential environment improvement on occupational performance satisfaction and activities of daily living in stroke patients, could be used as a basis for presenting an effective way to residential environment improvement of the disabled patients.

뒤로 걷기 운동에 따른 뇌졸중 편마비 환자의 균형능력 및 보행능력의 변화 (Changes in Balance and Gait Following Backward Walking Exercise in Hemiplegic Stroke Patients)

  • PURPOSE: The purpose of this study was to identify whether backward walking exercise was more effective than conventional physical therapy for balance and gait in hemiplegic stroke patients. METHODS: Eighteen patients with chronic stroke were randomly assigned to the experimental (n=9) or control (n=9) group. The experimental and control group performed backward walking exercise and conventional physical therapy, respectively, for 8 weeks. Stability Index (SI) and Weight Distribution Index (WDI) during standing were assessed using the Tetrax Balance System. The Timed Up and Go (TUG) test and Korean version of the Berg Balance Scale (K-BBS) were used to evaluate balance and fall risk. Walking speed, stride length, and step length on the affected side were measured using the 10-Meter Walk and ink foot printing tests. Wilcoxon signed-rank and Mann-Whitney U tests were used for within- and between- group comparisons, respectively. RESULTS: The experimental group showed significantly higher changes in SI (p<.01), WDI (p<.01), TUG (p<.001), and BBS score (p<.001) following intervention compared with the control group. The experimental group also showed significantly greater improvements in walking speed (p<.01), stride length (p<.001), and step length on the affected side (p<.001) after intervention compared with the control group. CONCLUSION: Backward walking exercise is an effective intervention to improve balance and gait in hemiplegic stroke patients.

The Effects of Water-Based Exercise on Physiological Cost Index and Balance in Stroke Patients

  • Purpose: This study attempts to find the effects of water-based exercise performed on stroke patients in their physiological cost index and static and dynamic balance. Methods: The subjects were 30 stroke patients, who were randomly divided a water-based exercise group and a land-based exercise group, each with 15 patients. Both exercises ware performed 3 day per week, for 40 minutes a day, for a period of eight weeks. Results: The Water-based exercise group showed a decreased physiological cost index compared to the land-based exercise group, with the physiological cost index of the water-based exercise group decreasing, while the land-based exercise group, showing a significant decrease. Water-based exercise increased static and dynamic balance capacity better than land-based group, showing a significant difference and better efficiency of water-based exercise when compared to land-based exercise. Conclusion: From the result of the study, we found that water-based exercise is more effective in improving PCI and static and dynamic balance capacity. Through this study, training in water-based stroke patients could use cardiovascular endurance and buoyancy and the vortex, as an effective treatment that can enhance the growth and voluntary participation in the range of the patient's own movement the thought is, in consideration of the changes in the physiological cost index due to the difference in the ground and water, and should establish a training program that matches the purpose.

발반사 마사지가 뇌졸중환자의 중추성 통증, 피로감 및 수면에 미치는 효과 (The Effects of Foot Reflexology Massage on the Central Pain, Fatigue and Sleep in Stroke Patients)

  • Purpose: This study was to evaluate the effects of foot reflexology massage on the central pain, fatigue and sleep in stroke patients. Methods: The study utilized a non-equivalent control group pretest and posttest design. Subjects were contacted at the oriental medicine ward of D hospital in B city for strokes. The subjects were 25 in the experimental group and 23 in the control group. Collected data were analyzed central pain, fatigue and the change of sleep between pre and post foot reflexology massage using a t-test, $x^2-test$, Fisher's exact test by SPSS 12.0 Win program. Results: There were significant decrease in the central pain, fatigue and change of sleep between pre and post foot reflexology massage. Conclusion: The results proved that foot reflexology massage was an effective stroke patients to decrease in the central pain, fatigue and change of sleep. Therefore, this foot reflexology massage should be effective as a stroke patients.

척유(脊愈) 약침을 이용한 뇌졸중 후 통증 치험 1례 (Case of Central Post-Stroke pain Patients Treated with Chukyu pharmacopuncture)

  • Objectives : This study report case of central post-stroke pain patient mainly treated by Chukyu pharmacopuncture. Methods : patient with central post-stroke pain were treated by Chukyu pharmacopuncture at the oriental medicine hospital of Daejeon University. Chukyu pharmacopuncture was treated once a day. Then, we observed the patients' appearances of pain. Results & Conclusions : After treatment, pain were reduced. In conclusion, Chukyu pharmacopuncture are effective to treat central post-stroke pain, and future studies will be required to ascertain this method on central post-stroke pain.

  • Objectives : The purpose of this study is to search for more effective methods of diagnosis and treatment of Communication Disorder with Post-Stroke. Methods : Literature review on Communication Disorder with Post-Stroke in view of oriental and western medicine. Conclusions : Communication Disorder with Post-Stroke is relative with aphasia in western medicine, Sul-um(舌瘖) oriental medicine Aphasia, apraxia of speech, and dysarthria come after strock Each of them has its own mechanism. Yomchon(CV23), Amun(GV15), Chuldol(CV22), Tongni(HT5), Pungnyung(ST40), Pungbu(GV16), Chigu(TE6), Yongchon(KI1), Hapkok(L14), Peakoe(GV20), Kokchi(LI11), Puryu(KI7), Shinsu(BL23), Kumjin-okaek, and Chohea(KI6) was used on Communication Disorder with Post-Stroke treatment.

뇌졸중에 대한 목단피사백산의 치료 효과 연구 : 후향적 의무기록 분석 (The Effects of Mokdanpisaback-san on Stroke: A Retrospective Medical Chart Review)

  • Objective: The aim of this study was to report the improvement of stroke symptoms in patients using Mokdanpisaback-san. Methods: This study was carried out on patients hospitalized at the Department of Korean Internal Medicine of Jecheon Korean Medicine Hospital of Semyung University from October 2014 to September 2019. A retrospective study was performed on 43 patients who were diagnosed with stroke and Soyangin and treated with Mokdanpisaback-san. Results & Conclusion: 1. Mokdanpisaback-san is effective in improving various symptoms of stroke, including dysarthria, dysphagia, movement disorder, paresthesia, quadriplegia, and dizziness. 2. Mokdanpisaback-san significantly improves the motor disorders of stroke, including lower extremity muscle strength, standing, and walking disorders. 3. Mokdanpisaback-san does not affect the liver or kidney function.

Effect of PNF Approach to Scapular Adductor Muscles on Scapular Movements and Walking Ability in Patients with Stroke

  • This study investigated the effects of indirectly applying proprioceptive neuromuscular facilitation (PNF) to the scapular adductor muscles of stroke patients on their scapular movements and walking ability. Five patients who were diagnosed with stroke participated in this study as a single group. PNF patterns were applied to the scapulae anterior elevation and posterior depression patterns and upper limbs patterns of the patients in side lying and sitting positions together. The data were analyzed with a paired t-test in order to identify within-group differences in the measurements before and after the intervention. The scapular movements of the upper and lower parts, weight bearing and walking speed were significantly improved in the stroke patients after the application of PNF (p<0.05). These results suggest that PNF training effective in improving the scapular movements and walking ability in patients with stroke.
