Effects of Chogam-Tang on Rat Skin Induced the Allergic Contact Dermatitis (백서(白鼠)에 유도된 알러지성 피부염에 대한 초감탕(椒甘湯)의 효과)

  • Background and Objectives: Allergic contact dermatitis is a common environmental health issue and seriously affect the patient's quality of life. The more our environment industrialized, the number of material that could cause the allergic contact dermatitis has been increased, consequently the prevalence of allergic contact dermatitis has been increased. In oriental medicine, clinically Chogam-Tang has been used fur the treatment of allergic dermatitis, eczema, atopic dermatitis etc. The objective of this study is to investigate the effects of Chogam-Tang on allergic contact dermatitis Meterial and Methods: Fifteen Sprague-Dawley rats were divided into three groups: normal, control, experimental group. Control and experimental group were induced allergic contact dermatitis, by DNCB. Experimental group was orally administered the Chogam-Tang. Each group was observed after 24, 48 and 72 hours. Contact hypersensitivity assay, melanin-erythema measurement, pH measurement skin moisture measurement and biopsy were performed. Results: 1. In contact hypersensitivity assay, experimental group showed decreased ear swelling compared with control group at 48hours. 2. ln melanin measurement there was no difference in three groups. 3. In erythema measurement experimental group showed reduction at 48. 72 hours. 4. In pH measurement, experimental group and control group showed increase in pH but there was no statistical significance. 5. In skin moisture measurement, experimental group showed higher skin moisture level than control group at 24 hours and showed lower skin moisture level at 72 hours, but there was no statistical significance. 6. In biopsy, experimental group showed decrement of Iymphocyte as time goes by, and regeneration of keratin layer was increased compared with normal group. Conclusions: Chogam-Tang shows anti-inflammatory effect in biopsy, improves hydration levels of skin, decreases erythema level on allergic contact dermatitis.

Macrophage Activation Syndrome Triggered by Herpes Viral Infection as the Presenting Manifestation of Juvenile Systemic Lupus Erythematosus (헤르페스 바이러스 감염으로 발생한 대식 세포 활성 증후군을 첫 증상으로 한 소아 전신 홍반 루푸스)

  • Macrophage activation syndrome (MAS) is a rare complication in systemic lupus erythematosus (SLE) that can be triggered by infections. Due to the fact that MAS may mimic clinical features of underlying rheumatic disease, or be confused with an infectious complication, its detection can prove challenging. This is particularly true when there is an unknown/undiagnosed disease; and could turn into an even greater challenge if MAS and SLE are combined with a viral infection. A-14-year-old female came to the hospital with an ongoing fever for 2 weeks and a painful facial skin rash. Hepatomegaly, pancytopenia, increased aspartate aminotransferase, elevated serum ferritin and lactate dehydrogenase were reported. No hemophagocytic infiltration of bone marrow was reported. The patient was suspected for hemophagocytic lymphohistiocytosis. Her skin rashes were eczema herpeticum, which is usually associated with immune compromised conditions. With the history of oral ulcers and malar rash, positive ANA and low C3, C4 and the evidence of hemolytic anemia, she was diagnosed as SLE. According to the diagnostic guideline for MAS in SLE, she was diagnosed MAS as well, activated by acute HSV infection. After administering steroids and antiviral agent, the fever and skin rash disappeared, and the abnormal laboratory findings normalized. Therefore, we are reporting a rare case of MAS triggered by acute HSV infection as the first manifestation of SLE.

Analysis of Case Studies of Treating Atopic Dermatitis - focusing on Korean Herbal Medicine Used in Cases - (아토피 피부염 증례 연구 분석 - 한약 처방을 중심으로 -)

  • Objectives : The purpose of this study is to investigate the use of herbal medicine for the treatment of atopic dermatitis in cases and to find the general tendency of herbal medicine treatment and to establish the primary treatment direction of Atopic dermatitis. Methods : In the domestic databases, Oriental medicine Advanced Searching Integrated System(OASIS), Korean Traditional Knowledge Portal(KTKP), National Discovery for Science Leader(NDSL), Research Information Sharing Service(RISS), we selected among the papers published in the last 20 years using search terms related to "Atopic dermatitis & Cases". Reports based on Sasang constitutional medicine and Six meridian pattern identification were excluded and a total of 21 papers were finally selected. Results : 113 herbal medicines were retrieved from 21 papers. The most commonly used herbs are Scutellariae Radix(黃芩), Rehmanniae Radix(地黃), and Glycyrrhizae Radix et Rhizoma(甘草). Atractylodis Rhizoma(蒼朮), Akebiae Caulis(木通), Smilacis Rhizoma(土茯?), Paeoniae Radix(芍藥) used mostly when Oozing exists, Trichosanthis Radix (瓜蔞), Asparagi Radix(天門冬), Persicae Semen(桃仁), Carthami Flos(紅花) used mostly when Dryness, Lichenification, and Pigmentation exist. The average score of SCORAD index was improved after Herbal medicine treatment. Conclusion : Through this study, we could find out the tendency of herbal medicine to treat Atopic dermatitis along with Symptoms.

Analysis of Potential Active Ingredients and Treatment Mechanism of Ponciri Fructus Immaturus for Dermatitis Accompanied by Pruritus Using Network Pharmacology (네트워크 약리학을 이용한 소양증을 동반한 피부 염증에 대한 지실(枳實)의 잠재적 치료기전 탐색)

  • Objectives : To identify the active ingredient of Poncirus Trifoliata Immaturus and to explore the mechanism expected to potentially act on dermatitis accompanied by pruritus. Methods : We conducted the network pharmacological analysis. We selected effective ingredients among the active compounds of Poinciri Fructus Immaturus. We found the target protein of the selected active ingredient, disease(dermatitis accompanied by pruritus) and fexofenadine. Then we established the network between the proteins which Poinciri Fructus Immaturus and fexofenadine intersected with disease respectively, and the coregene was also extracted. After that, the active pathways in the human body involving the groups and coregenes were searched. Results : Total of 7 active ingredients were selected, and 202 target proteins were collected. There were 756 proteins related to inflammatory skin disease accompanied by pruritus, and 75 proteins were related to fexofenadine. 42 proteins crossed by Poinciri Fructus Immaturus with a disease, and 31 proteins crossed by fexofenadine with a disease. 12 proteins were found as a coregene from the proteins that cross Poinciri Fructus Immaturus and disease. Coregenes are involved in 'Nitric-oxide synthase regulator activity', 'Epidermal growth factor receptor signaling pathway'. 2 groups that extracted are invloved in 'Fc receptor signaling pathway', 'Central carbon metabolism in cancer', 'Phosphatidylinositol 3-kinase complex, class IB', and 'omega-hydroxylase P450 pathway'. Conclusion : It is expected that Poinciri Fructus Immaturus will be able to show direct or indirect anti-pruritus and anti-inflammatory effects on skin inflammation accompanied by pruritus in the future. And it is also expected to have a synergy effect with fexofenadine on skin disease.

A Statistical Analysis of Outpatients in Dermatology of Korean Medicine at Kyung Hee University Korean Medicine Hospital in 2018-2022 (2018-2022년 경희대학교 한방병원 한방 피부과 외래환자에 대한 통계적 분석)

  • Objectives : The objective of this study is to identify the characteristics of outpatients and the frequently diagnosed diseases in dermatology of Korean medicine through periodic research and comparison with previous studies. Methods : We analyzed medical records of new outpatients who visited department of dermatology at Kyung Hee university Korean medicine hospital between January 1, 2018 and December 31, 2022. We analyzed the patient demographic characteristics and the frequency of diagnosed diseases. Then we compared these results with previous studies in 2000, 2007 and 2016. Results : 1. Average number of patients and proportion of male patients have increased from 1996 to 2022. 2. From 1996 to 2022, there was a decrease in the proportion of patients aged 0-39 and an increase in the proportion of patients aged over 40. 3. The distribution proportions of eczema, atopic dermatitis, and rosacea during 2018-2022 have increased compared to those during 2011-2015. 4. The distribution of diseases according to age appears to vary based on the epidemiological characteristics of the diseases. 5. The distribution of diseases according to gender also appears to vary based on the epidemiological characteristics of the diseases, as well as cosmetic issues related to the affected areas. Conclusions : Considering the sustained trend of a high proportion of non-infectious inflammatory diseases since 1996, it is important to enhance treatment capacity through continuous research.

Inhibitory effects of ethanol extract from Vicia amoena on LPS(Lipopolysaccharide) induced nitric oxide and prostaglandin E2 production in RAW264.7 macrophage cell (갈퀴나물 에탄올 추출물의 RAW264.7 대식세포에서 LPS(Lipopolysaccharide)로 유도된 nitric oxide 및 prostaglandin E2 생성 저해효과)

  • Vicia amoena has traditionally been used to treat disease of rheumatism, arthralgia, muscular paralysis, abscess and eczema, and it has anti-inflammatory properties. However, validity of the anti-inflammatory activity has not been scientifically in vestige acted so far. Therefore, the aim of this study was to investigate the anti-inflammatory potential of V. amoena using the ethanolic extract. To evaluate the anti-inflammatory effects, we examined the inflammatory mediators such as nitric oxide (NO) and prostaglandin E2 (PGE2) on RAW264.7 cells. Our results indicated that ethanolic extract of V. amoena significantly inhibited the LPS-induced NO and PGE2 production in RAW264.7 cells. The ethanolic extract of V. amoena has inhibited the PGE2 production by 88.0±0.8 % at the concentration of 40㎍/ml. This results showed that ethanol extract of V. amoena is expected to be a good candidate for development into source of inflammation inhibitor

Analysis of the Component and Immunological Efficacy of Chamaecyparis obtusa Leaf Extract (편백나무 잎 추출물의 성분분석과 면역효능에 관한 연구)

  • Chamaecyparis obtusa (CO) has recently been attracting attention because of its beneficial effects on skin allergies, atopic dermatitis, and skin diseases, such as acne and eczema. In the present study, the extract from CO leaf grown in Jangseong gun, Jeollanam-do, Korea was evaluated for its anti-oxidant, anti-inflammatory, and anti-allergic effects in vitro. The total polyphenol content of the CO leaf extract was $25.89{\pm}0.31mg$ gallic acid equivalents (GAE)/g. Gas-chromatography mass-spectrometry (GC-MS) analysis revealed the presence of six compounds in the CO leaf extract: ${\alpha}-terpinene$ (3.03 mg/g), ${\alpha}-terpineol$ (9.48 mg/g), limonene (5.96 mg/g), borneol (59.78 mg/g), myrcene (4.85 mg/g), and sabinene (11.31 mg/g). The $RC_{50}$ values of the CO leaf extract for $H_2O_2$ and ABTS radical were $5.47{\pm}0.13mg/mL$ and $4.00{\pm}0.01mg/mL$, respectively. In addition, the CO leaf extract showed significant inhibitory effects on lipopolysaccharide-induced nitric oxide production in RAW 264.7 cells and IgE-induced release of ${\beta}-hexosaminidase$ (degranulation) in mast-cell like RBL-2H3 cells. The cell viability assay showed that the CO leaf extract ($100{\sim}800{\mu}g/mL$) did not affect the viability of human normal skin fibroblast CCD-986sk cells significantly. Overall, these results suggest that the CO leaf extract is a potential functional cosmetic ingredient that can exert anti-oxidant, anti-inflammatory, and anti-allergic effects.

Effect of cytokines in breast milk on infant during the first two-years of life (모유 내 사이토카인 함량과 2 년간 추적관찰 후 알레르기 발생)

  • Purpose : Breast milk contains several components that provide specific immunity and affect the maturation of the infant's immune system. Allergic disease (AD), including atopic eczema, asthma, allergic rhinitis, and food allergy is characterized by an imbalance between cytokines produced by distinct T-helper cell subtypes. The aim of the study was to investigate the concentrations of cytokines and chemokines that were involved in allergic reactions in breast milk from allergic and nonallergic mothers and then analyse the effect of breastfeeding duration on the prevalence of allergic disease in the age of two. Methods : The breast milk samples were collected from mothers with AD (n=88) and without AD (n=47). Breast milk was collected at the second day (colostrum) and four weeks later (mature milk).The level of Interlukine (IL)-4, IL-5, IL-6, IL-8, IL-10, IL-13, $TGF-{\beta}1$, $TGF-{\beta}2$, RANTES in breast milk were determined by commercially available enzyme-linked immunosorbent assay (ELISA) kits according to the manufacturer's instructions. Results : Mothers with AD had a higher concentration of IL-8 in colostrum compared with those without AD (P=0.021). But, $TGF-{\beta}1$ and $TGF-{\beta}2$ were higher concentrated in colostrum of mother without AD (P=0.013, P=0.001). Whereas concentrations of other cytokines were not significantly different between the two groups. There was no association between levels of cytokines and chemokines in the breast milk and allergic development during the first 2 years of life in the infants. Conclusion : The higher concentration of $TGF-{\beta}1$ and $TGF-{\beta}2$ in colostrum from non-allergic mothers may explain the protective effect. But, the higher concentrations of IL-8 in colostrum from allergic mothers may in part explain the controversial results on the protective effect of breastfeeding against allergic diseases. We conclude that there is no convincing evidence form a relation between cytokines in breast milk and allergic diseases in infants. Longer follow-up are necessary to evaluate the effects of breast milk components on AD.

  • Atopic dermatitis, also called congenital fever, is a allergic eczema of chronic itching disease. It is a recurrent and familial disease and appears on a wide age group from infant to adult. It is very common, and the ratio of occurrence is about $9{\~}l2\%$ of a child. However. it is showing trend of continuous increase by social and natural environment, food culture, and life style, recently. The human skin plays a barrier role against a physical and chemical stimulus from external environment. According to the latest study, the decreased amount of ceramide in horny layer impairs the bier function and moisture-maintaining function of skin in atopic dematitis patient. Ceramide is a kind of the sphingolipid in which a fatty acid is connected to sphingosin. Ceramide constitutes about $40\%$ of total lipid between keratinocytes and has the function of defense wall and building regular structure to suppress moisture vaporization in horny layer. In horny layer of skin a comified cell is composed of multi-layer structure of a brick shape, and, as for this cornified cell, it is strongly connected by ceramide, cholesterol, and free fatty acid. Here, we described the effects of a cream containing ceramide on the recovery of skin harrier function of atopic dermatitis patient. The safety and efficacy of latex and liquid formula were evaluated as cosmetics for atopic dermatitis. The latex products was composed of intercellular lipid components-ceramide, cholesterol, and free fatty acid-to restore skin barrier function in atopic dermatitis patients. The liquid one contained beta-glucan, magnolia extracts, and licolice extracts, which have skin immunomodulatory and anti-inflammatory effects. It is also confirmed that their possibility on new cosmetic market of atopic dermatitis.

  • In 1991, Obesity rate of South Korea has reached to 18.7%. Because of economical development, the pattern of diet is exchanged from carbohydrate to rich protein and fat. The more problem is not only obesity of adult but also one of little child. Obesity is induced to diabetes mellitus, hypertension, artherosclerosis, hyperlipoidemia. heart and C.V.A disease, etc. In Woman, special important ploblem is the complex of beauty about Woman's figure. In Oriental Medicine, the factor of obesity is mainly regarded as dampness. And there are many treatments and methods to body weight loss, but obesity patients dislike to use them because of their side effects and inconvenience, intolerance. Now ear acupuncture is applied on so many disease because of its easy handly, non-side effect and high efficiency in clinics. Here obesity acupuncture is used to ear and whole body acupuncture. Because they react eachother for lack point. Therefore, in order to investigate the effect of obesity acupuncture and develop non-drug, non-starvation etc, we analyzed 453 the cases of body weight loss patients treated with ear and whole body acupuncture in Oriental Medicine Hospital of Jeon-Ju Woo-Sug University from April.1.1992. to March.17. 1993. The results were summarized as follows. 1. Distribution of sex ; male (4.4%), Female(95.6%) 2. Distribution of age in descending order ; 30s, 20s, 40s, 10s, 50s, below 10s, abowe 60s. The 20s-30s are group made up 60.7% of the group. 3. Distribution of occupation in descending order; housewife, student, service, salaried, merchant, teacher, farmer, inoccupation. 4. Distribution of human coporal constitution in descending order : Tae-Eum-In, So-Eum-In, So-Yang-In. 5. Distribution of body height and weight, 155-164cm ; 71.1%, 60-70kg, 74.6% are majority. 6. Distribution of weight variation, 2-6kg(71.0%) is majority, also 13-14kg(0.4%). 7. Distribution of duration in descending order ; 1-3 years, 3-6 years, 1-12months, above 10 years but in success, 1-12 months, 1-3 years, 3-6 years, above 10 years. Therefore, we know that the shorter duration of obesity is, the more loss of body weight. 8. Past experiences to body weight loss; Yes(69.5%), No(30.5%). The success rate accordant with the past temporary experiences shows that the cases without experience is higher than the ones with experience. 9. In distribution of times(treatments), 10 times is top. The rate of body weight loss is the highest in 14 times. Therefore, I think that one would need at least 10 times. in order lose body weight 10. Distribution of body weight variation in treatments times is at 2 times(3-4kg loss), and surprisingly is 14kg loss at above 15 times. 11. Distribution of symptoms improvement, in descending order ; heavy sense in body, dec. of appetite, inc.of exercise, lumbago, edema, knee pain, inc.of urine, inc. of fullness sense, thirsty, disease of gynecology, white tung, chest burning, heart burning, dec.of tobacco, drink taste. motion sickness, allergy, water eczema, arthma, belching. 12. Distribution of snack; Yes(87.4%), No(78.6%) 13. Distribution of exercise; Yes(21.4%), No(78.6%) 14. Distribution of sleeping times, above 7 hours(79.0%) 15. Distribution of the reason to body loss, the complex of beauty(68.7%) is top. 16. Distribution of side effect in obesity acupuncture, constipation (17.4%) is top. 17. Distribution of method in body weight loss ; dietary treatment (31.1%), sauna(26.7%), exercise(19.7%), the center of body weight loss (15.0%) herb-med and starvation treatments (5.1%), hand-finger acupuncture (hand-foot acupuncture) is 1.6%, diet pill(0.3%), etc(0.6%).
