Spontaneous closure of perimembranous ventricular septal defect in a cat

  • We discuss the case of a 5-month-old male British Shorthair cat referred to our hospital following the detection of a heart murmur during a routine vaccination appointment. Two-dimensional echocardiography revealed a 1.18 mm ventricular septal defect (VSD) located immediately below the aortic valve, without signs of secondary cardiac remodeling. Given the absence of cardiac dysfunction, no treatment was administered, and the cat was periodically monitored over the next 2 years. Echocardiography at 29 months of age revealed no signs of the VSD. Future studies are needed to increase the evidence base for spontaneous VSD closure in small animals.

  • A 6-month-old male Ragdoll cat presented with exercise intolerance. On physical examination, there was a grade 2/6 systolic murmur at the right apex. Diagnostic tests, including SpO2 measurement, blood tests, radiography, echocardiography, contrast echocardiography, and electrocardiography, were performed. Severe right atrial dilation, tricuspid valve leaflets and orifice displacement, right ventricular atrialization, septal leaflet adherence, anterior leaflet tethering, and right atrioventricular junction dilation were noted on echocardiography, alongside a right-to-left atrial septal defect. Cor triatriatum dexter and left ventricular aneurysm were observed. We diagnosed this case as having Ebstein anomaly with rare congenital heart deformities; which is rare in cats.

Subclinical left ventricular dysfunction in children after hematopoietic stem cell transplantation for severe aplastic anemia: a case control study using speckle tracking echocardiography

  • Purpose: Severe aplastic anemia (SAA), a fatal disease, requires multiple transfusion, immunosuppressive therapy, and finally, hematopoietic stem cell transplantation (HSCT) as the definitive treatment. We hypothesized that iron overloading associated with multiple transfusions and HSCT-related complications may adversely affect cardiac function. Left ventricular (LV) function was assessed in children after HSCT for SAA. Methods: Forty-six consecutive patients with a median age of 9.8 years (range, 1.5-18 years), who received HSCT for SAA and who underwent comprehensive echocardiography before and after HSCT, were included in this study. The data of LV functional parameters obtained using conventional echocardiography, tissue Doppler imaging (TDI), and speckle-tracking echocardiography (STE) were collected from pre- and post-HSCT echocardiography. These data were compared to those of 40 age-matched normal controls. Results: In patients, the LV ejection fraction, shortening fraction, end-diastolic dimension, mitral early diastolic E velocity, TDI mitral septal E' velocity, and STE LV longitudinal systolic strain rate (SSR) decreased significantly after HSCT. Compared to normal controls, patients had significantly lower post-HSCT early diastolic E velocity and E/A ratio. On STE, patients had significantly decreased LV deformational parameters including LV longitudinal systolic strain (SS), SSR, and diastolic SR (DSR), and circumferential SS and DSR. Serum ferritin levels showed weak but significant correlations (P<0.05) with LV longitudinal SS and SSR and circumferential SS and DSR. Conclusion: Subclinical LV dysfunction is evident in patients after HSCT for SAA, and was associated with increased iron load. Serial monitoring of cardiac function is mandatory in this population.

  • Dilated cardiomyopathy in two dogs showing dyspnea was diagnosed at veterinary teaching hospital. In radiographs, cardiomegaly, pleural effusion, pulmonary edema, ascites, and hepatomegaly were seen at case 1. ‘Reversed D’ shape due to right heart enlargement was shown at case 2. In echocardiography, dilatation of chambers was predominant in case 1 and 2. Arrythmia was complicated in case 2.

  • Tricuspid valve dysplasia was diagnosed in 2 month-old german shepherd dog. Radiographic and echocardiographic examination was performed and they revealed severe dilatation of the right atrium, dilatation of the right ventricle, and septal leaflet tethered to the right side of the septum. On Doppler echocardiography, tricuspid valve incompetence was detected, flow velocity ranging from 3-4 m/sec. Color Doppler examination showed large turbulent jet in the right atrium.

심내막상 결손증 술후 추적 관찰에 관한 연구 (Postoperative Follow up Study in ECD)

  • Endocardial cushion defect is a rare congenital heart disease. From September 1985, we experienced 20 cases of endocardial cushion defects and postoperative follow up was taken.We reviewed preoperative clinical data, echocardiography, cardiac cath data, operative method & time. After operation, we performed echocardiography and examined mitral valve function & integrity of patch closure sites. Postoperative follow up was taken regularly & follow up period was from 2 month to 91 months. Several postoperative complications were overcome with adequate treatment. There were 3 cases of early death and mortality rate was 15% [3/20].

원발성 심장 종양의 수술적 치료 - 27례 보고- (Surgical Treatment of Primary Cardiac Tumor -Report of 27 cases -)

  • 대상 및 방법: 가톨릭대학교 의과대학 흉부외과학 교실에서는 1979년 부터 1996년까지 27명의 원발성 심장 종양 환자를 수술하였으며, 환자의 연령은 21세부터 67세까지로 평균 45.1세이었다. 술후 병리학적 진단결과 24례는 점액종, 2례는 연골 육종, 1례는 혈관 육종이며, 가장 흔한 술전 증상은 심부전이었다. 진단은 심장초음파로 확진되었고 호발 부위는 난원와였다. 종양 주위를 세심히 관찰후 심방 중격을 포함하여 완전 절제를 시행하였으며 술후 대부분의 환자 에서 증상 호전이 있었다. 결과: 1례에서 수술 직후 저심박출증으로 사망하였고, 2례에서 국소 재발로 인해 만기 사망하였다. 결론: 결론적으로, 수술시 완전 절제가 중요하며 술전 심장 초음파 검사상 양성으로 보일지라도 악성의 가능성을 배제해서는 안되며 CT나 MRI가 진단에 도움이 되며 수술시에 악성이 의심되면 광범위한 절제와 더불어 술후 항암제및 방사선 요법이 도움이 되리라 생각된다.

  • 저자들은 재태 $31^{+5}$주 산전 심초음파검사에서 좌심형성부전 및 대동맥축삭으로 진단되어 추적 관찰한 태아가 출생 후 전부하의 증가에 의해 단기간 내에 좌심실 용적의 증가를 보임으로 생후 최소 며칠은 심근세포 증식의 가능성이 있음을 경험하였기에 문헌고찰과 함께 보고하는 바이다.

  • 4년령의 중성화된 수컷 Dachshund가 만성적인 호흡곤란 및 청색증을 주증으로 내원하였다. 혈액검사에서 헤마토크리트 79.7%의 심각한 적혈구증가증이 확인되었다. 흉부 방사선 영상에서는 경도의 우심 종대가 관찰되었다. 심초음파 검사에서 좌심실유출로 대동맥판막 근위부에서 큰 심실 중격 결손이 확인되었으며, 도플러 검사에서 이 결손을 통한 우-좌 션트가 확인되었으며 조영 심초음파를 통하여 최종적으로 확진되었다. 이러한 진단적 결과 및 환자의 임상증상을 바탕으로 환자는 우-좌 단락 심실 중격 결손증으로 진단되었다. 사혈 및 산소 공급을 실시하였으나 환자는 내원 다음날 집에서 사망하였다. 본 증례보고에서는 심초음파를 통한 우-좌 단락 심실 중격 결손증의 진단을 서술하였다.
