The Relationship Between Bone mineral Density and The Environmental Factors in Korean Pubescent Girls (한국 사춘기 소녀들의 골밀도와 환경요인들과의 관계(II))

  • Bone mass accretion during puberty appears to be critical in the development of peak bone mass. Although bone density of females in Korea has been studied, only a few studies have related bone mass with anthropometric patterns or puberty in the pubescent girls. This study was conducted as part of a study of major determinants of bone development during puberty. Subjects were aged 14∼16 yr(mean 14.97), and had no history of disorders or dedication use likely to influence bone or calcium metabolism. Bone mineral density and content were measured by dual energy X-ray absorptiometry using a Lunar DPX+Scanner (Lunar Madison, WI). Also, total body fat, and total lean body mass were assessed using a Lunar DPX dual-energy X-ray absorptiometer, Pubertal status was assessed according to the Marshall and Tanner guidelines. Serum levels of osteocalcin was measured by RIA using a commercial kit assay. Skinfold measurements were taken with a skinfold caliper(Lange Caliper, USA). Data were analyzed using the regression and GLM procedure of the statistical package SAS. The results indicated that the observed means for lumbar spine BMD and femoral BMD correspond to approximately 91% and 96% of the means for young adult females, respec tively. All subjects were menarchal, with the majority being in the middle to end stages of pubertal development. Total body BMD was positively related to fat mass(P<0.001), lean body mass and time since menarche, and negatively related to urine pyridinoline, serum alkaline phosphatase and osteocalcin. The data indicate that girls who reported lower age for menarche had significantly higher bone densities than girls who reported higher age for menarche. Attaining peak skeletal bone mass during puberty may reduce the incidence of osteoporosis in later life. this finding suggests that early menarche may augment peak bone mass, influencing the extent of bone loss later in adulthood. The results suggest that good nutrition in childhood appears to be needed not for growth and development, but possibly also to assure an optimal peak of bone mass and thus greater latitude for the maintenance or skeletal integrity in the face of bone losses. Troeps skinfold thickness was a better predictor of total BMD and total BMC than was any other skinfold thickness. The study did not find a relationship between total BMD and body fat %, but total fat was significantly positively related to total BMD(r=0.49) and total BMC(r=0.60). It supports earlier report that there was a significant correlation between TBMD and body weight. Conclusively, total fat, lean body mass and pubertal development could influence BMD in pubescent girls. Clearly, longitudinal studies are required to assess the effect of puberty on peak bone mass, and to define further the potential determinants of peak bone mass.

Comparison of Bone Ages in Early Puberty: Computerized Greulich-Pyle Based Bone Age vs. Sauvegrain Method (초기 사춘기의 골연령 비교: 전산화된 Greulich-Pyle 기반 골연령 대비 Sauvegrain 방법)

  • Purpose To compare the computerized Greulich-Pyle based bone age with elbow bone age. Materials and Methods A total of 2126 patients (1525 girls; 601 boys) whose elbow bone age was within the evaluable range by the Sauvegrain method, and who simultaneously underwent hand radiography, were enrolled in the study. The 1st-bone age and VUNO score of the hand were evaluated using VUNOMed-BoneAge software. The correlation between the hand and elbow bone age was analyzed according to the child's gender and the probability of 1st-bone age. Results The correlation between VUNO score and elbow bone age (r = 0.898) was higher than the correlation between 1st-bone age and elbow bone age (r = 0.879). Moreover, the VUNO score showed a better correlation with the elbow bone age in patients with a 1st-bone age probability of less than 70%, or in girls. Elbow bone age was more advanced compared to hand bone age, and this difference increased until the middle of puberty and gradually decreased in the latter half. Conclusion The computerized Greulich-Pyle based hand bone age showed a significant correlation with the elbow bone age at puberty. However, since the elbow bone age tends to advance faster than the hand bone age, caution is required while judging the bone age during puberty.

  • This study was conducted with 20 female gymnasts to examine the relationship between eating patterns, diet menstrual function and hematological status. According to the baseline data a nutrition counseling and education program was developed and evaluated improved the nutritional status and health of female gymnasts. Mean body weight at the onset of the study was 42.1$\pm$7.0kg and was reduced to 41.8$\pm$6.1kg after the nutrition counseling and education program. The percent of body fat was significantly reduced from 13.9$\pm$3.7% to 13.1$\pm$3.1%(p<0.01) skinfold thickness of subscapular and thighs was reduced significantly(p<0.01, p<0.05) Mean daily intake levels of energy, protein calcium iron thiamin riboflavin and niacin were significantly elevated after the nutrition counseling and education program but were lower than the Recommenced Dietary Allowances. For the nutrition knowledge and food habits, the posttest mean scores showed a significant increase. The hematological status(hematocrit, serum ferritin) and the early follicle level of estradiol were elevated to a mild degree although it was not significant,. The follicular stimulating hormone level was elevated significantly(p<0.01) Gymnastica has been one of the sports implicated by the medical profession as having probable detrimental effects. The implications of such training to childs growth and maturation have yet to be determined . Most female athletes, however, experience poor nutritional status and delayed puberty The priorities were to prepared a more effective nutrition program and education material status and delayed puberty. The priorities were prepared a more effective nutrition program and educational material for athletes coaches and adminstrators to prepare guidelines for the team physicians and coaches to follow for the physical and physiological examinations of female athletes.

Endocrine Profiles and Blood Chemistry Patterns of Cloned Miniature Pigs in the Post-Puberty Period

  • Although the majority of surviving pigs cloned by somatic cell nuclear transfer (SCNT) appear to be physiologically normal, there is a general lack of detailed hemato-physiologic studies for the period of early adulthood to substantiate this claim. In the present study, we investigated variation in blood chemistry and endocrinological parameters between mesenchymal stem cells (MSCs) derived from cloned and normal age-matched female and male miniature pigs. Cloned females and males showed normal ranges for complete blood count assessments. Biochemical assessments showed that ${\gamma}$-GGT, ALT and cholesterol levels of male and female clones were significantly (P<0.05 or P<0.01, respectively) higher than that of age-matched control miniature pigs. Variations in insulin and IGF-1 were higher in female clones than in male clones and controls. Thus, although female and male cloned miniature pigs may be physiologically similar to normal animals, or at least within normal ranges, a greater degree of physiological and endocrinological variation was found in cloned pigs. The above variation must be taken into account before considering cloned female or male miniature pigs for various biomedical applications.

  • Cherubism is a rare familial disease of childhood, characterized by proliferative lesion, which is within the maxilla and mandible. In a typical case, painless symmetric expansile lesions develop in the jaws. It shows substitution of the bone by proliferating fibrous tissue exhibiting mature fibroblasts and a number of multinucleated giant cells within an intercellular matrix. Usually, the disease manifests in early childhood, and becomes more marked until puberty, at which time the bony lesions begin to regress. As such, conservative approaches to management are advisable. However, excision of tissue through enucleation or curettage appears to be necessary in more aggressive cases, to reduce the maxillofacial deformity after puberty and to ensure a successful outcome without the risk of progression, requiring additional resection. This report describes 2 cases of manifestation of cherubism of oral and maxillofacial region. We present diagnosis, radiological - histopathologic features, and treatment of cherubism.

  • Rabson-Mendenhall syndrome(RMS) is first characterized in 1955 by Rabson and Mendenhall. RMS is a rare autosomal recessive variant with insulin resistance. This is due to insulin receptor mutations or other target-cell defects in insulin action. General findings include acanthosis nigricans, hypertrichosis, onychauxis, growth retardation, precocious puberty, genital enlargement, protuberant abdomen and xerotic skin. Characteristic oral and maxillofacial findings include dental dysplasia, coarse facial skin, prognathic jaw and fissured tongue. In this case report, dental characteristics of a 4-year old boy with Rabson-Mendenhall syndrome are described.

Current issues of pediatric inflammatory bowel disease in Korea

  • Inflammatory bowel disease (IBD) is a chronic relapsing disorder of unknown etiology, which is believed to be multifactorial. Recently, the incidence of pediatric IBD has steeply increased in Korea since 2000. Poorly controlled disease activity can result in complications such as intestinal fistulae, abscess, and stricture, as well as growth retardation and delayed puberty in children. Because of a lack of confirmative tests, various diagnostic modalities must be used to diagnose IBD. Onset age, location, behavior, and activity are important in selecting treatments. Monogenic IBD must be excluded among infantile and refractory very-early-onset IBD. Early aggressive therapy using biologics has recently been proposed for peripubertal children to prevent growth failure and malnutrition.

Relationship Between Body Mass Index and Early Menarche of Adolescent Girls in Seoul

  • Objectives: The object of this study was to determine the relationship between body mass index (BMI) and early menarche in adolescent girls in Seoul. Methods: A retrospective study was conducted with 144 middle school students in Seoul who provided informed consent. We measured their body composition, and used the questionnaire survey method for data collection from November to December 2008. Past elemental body composition data were collected from elementary school health records of first year of middle school. Results: The early menarcheal group was taller and heavier than the late menarcheal group (p<0.05 from 8-12 years old). The body fat percentage (%), BMI were higher in the early menarcheal girls than the late-menarcheal girls (p<0.05, age at 13). In the result of multiple logistic regression, the BMI at the age of 8 and 9 was associated with early menarche after adjusting for birth weight, breast feeding and age at menarche of the mother (BMI at the age of 8: p for trend=0.01, BMI at the age of 9: p for trend=0.04). An increase in BMI from 7 to 8 year was associated with early menarche after adjusting for birth weight, breast feeding, age at menarche of the mother (p for trend=0.048). Conclusions: The BMI at the age of 8 and 9 was associated with the early menarche of girls and increase in BMI from 7 to 8 year was associated with the early menarche of girls. These results suggest that BMI and increase in BMI before menarche cause early menarche. Although this study does not represent all Korean adolescent girls, it is one of the few studies that have investigated the temporal relationship between BMI and early menarche.

Mature and Bi-Sexual Phase Gonad Occurrence in Cultured Red Spotted Grouper, Epinephelus akaara

  • This study reports the presence of mature and bi-sexual phase gonads in red spotted grouper, Epinephelus akaara after less than a year of cultivation in a commercial indoor tank and a net cage. In December 2018, juveniles were placed in an indoor tank and cultured for five months. In June 2019, the fish were transferred to a net cage and cultured until September. The rearing temperatures ranged from 19.86℃-24.65℃ in the indoor tank and 21.86℃-27.65℃ in the net cage. During the net cage culture period, specimens were randomly selected for histological gonad examination. The highest gonadosomatic index (GSI) value was measured in July (3.38±2.53), and dramatically decreased in August (0.44±0.21) and September (0.42±0.30). In July, some mature fish showed signs of vitellogenic stage oocyte development (vitellogenic and oil droplet stage oocytes), but immature fish were in an early developmental stage containing peri-nucleolus stage (PNS) oocytes. Bi-sexual phase gonads containing spermatocytes and spermatids were observed in the lumen and several PNS oocytes. By August and September, most specimens showed early-stage ovary development. However, mature testis (in August) and bi-sexual phase gonads (in September) were also observed. These results provide evidence for early puberty and hermaphroditism in the red spotted grouper.

Comparisons of Dietary Behaviors and Growth Development with Different Sexual Maturation Velocities in Children of Pubertal Age (성 성숙 속도가 다른 사춘기 아동의 성장 발달과 식행동 비교)

  • The purpose of this study was to make a comparative analysis of dietary habits, snacks, eating out and fast-food intake between students with a relatively early sexual maturity and others with a relatively late sexual maturity in a bid to grasp the impact of dietary behaviors on sexual maturity. The subjects in this study were 565 boys and girls who resided in Seoul and Gyeonggi province. They included fifth- and sixth-grade school children and seventh-grade middle school students. To classify the selected students according to the level of sexual maturity, the students of the same month age were divided into two groups in consideration of the stage of their sexual maturity and that of the other age groups. The early mature group was significantly taller and weighed more than the late mature group irrespective of gender. According to Tanner stages, the boys of the early mature group reached a 3.03 stage of sexual maturity when the sexual organs were taken into account, and those of the late mature group reached a 1.50 stage of sexual maturity. The girls of the early mature group reached a 3.43 stage of sexual maturity on the basis of breast, and those of the late mature group reached a 2.73 stage of sexual maturity. Concerning eating out, the early mature group ate out more frequently than the late mature group. In regard to snacks, the early mature group had a snack more often. The findings of the study indicated that there were differences in physical development and some dietary behaviors according to sexual maturity. The early mature group had a snack and ate out relatively more often than the late mature group, which showed that some dietary behaviors had an impact on the sexual maturity of the teens who arrived at the age of puberty.
