  • 최근들어 극초단 광학펄스를 생성하고 증폭하는 기술의 발달로 인하여 시분해 레이저 분광학에 많은 영향을 끼치게 되었다. 그 중에서도 응용성이 넓은 순간흡수분광학장치를 본 연구실에서 개발하였다. 이 기술은 고출력 레이저 펄스를 이용하여 white light continuum pulse를 생성시켜 이를 조사빔으로 이용하고, pump pulse는 원래 상태의 레이저를 이용하는 것이다. 순간흡수율 변화를 여러 파장에서 측정가능하도록 photodiode array를 이용하였으며, pump와 probe pulse의 시간차이에 의한 순간흡수율 변화를 색소의 일종인 DDI를 이용하여 측정하였다.

Update on infantile hemangioma

  • The International Society for the Study of Vascular Anomalies classifies vascular anomalies into vascular tumors and vascular malformations. Vascular tumors are neoplasms of endothelial cells, among which infantile hemangiomas (IHs) are the most common, occurring in 5%-10% of infants. Glucose transporter-1 protein expression in IHs differs from that of other vascular tumors or vascular malformations. IHs are not present at birth but are usually diagnosed at 1 week to 1 month of age, rapidly proliferate between 1 and 3 months of age, mostly complete proliferation by 5 months of age, and then slowly involute to the adipose or fibrous tissue. Approximately 10% of IH cases require early treatment. The 2019 American Academy of Pediatrics clinical practice guideline for the management of IHs recommends that primary care clinicians frequently monitor infants with IHs, educate the parents about the clinical course, and refer infants with high-risk IH to IH specialists ideally at 1 month of age. High-risk IHs include those with life-threatening complications, functional impairment, ulceration, associated structural anomalies, or disfigurement. In Korea, IHs are usually treated by pediatric hematology-oncologists with the cooperation of pediatric cardiologists, radiologists, dermatologists, and plastic surgeons. Oral propranolol, a nonselective beta-adrenergic antagonist, is the first-line treatment for IHs at a dosage of 2-3 mg/kg/day divided into 2 daily doses maintained for at least 6 months and often continuing until 12 months of age. Topical timolol maleate solution, a topical nonselective beta-blocker, may be used for small superficial type IHs at a dosage of 1-2 drops of 0.5% gel-forming ophthalmic solution applied twice daily. Pulse-dye laser therapy or surgery is useful for the treatment of residual skin changes after IH involution.

A Multi-detection Fluorescence Dye with 5-ALA and ICG Using Modified Light Emitting Diodes

  • Extensive tumor resection accompanied by radiotherapy and chemotherapy is the standard of care for malignant gliomas. However, there is a significant obstacle to the complete resection of the tumor due to the difficulty of distinguishing tumor and normal brain tissue with a conventional surgical microscope. Recently, multiple studies have shown the possibility of fluorescence-guided surgery in malignant gliomas. The most used fluorescence dyes for brain tumor surgery are 5-aminolevulinic acid (5-ALA) and indocyanine green (ICG). In this paper, a new fluorescence guided operation system, which can detect both 5-ALA and ICG fluorescent images simultaneously, is presented. This operation system consists of light emitting diodes (LEDs) which emits 410 nm and 740 nm wavelengths. We have performed experiments on rats in order to verify the operation of the newly developed operation system. Oral administration and imaging were performed to observe the fluorescence of 5-ALA and ICG fluorescence in rats. When LEDs at wavelengths of 410 nm and 740 nm were irradiated on rats, 628 nm wavelength with a violet fluorescence color and 825 nm wavelength with a red fluorescence color were expressed in 5-ALA and ICG fluorescent material, respectively, thus we were able to distinguish the tumor tissues easily. Previously, due to the poor resolution of the conventional surgical microscope and the fact that the color of the vein is similar to that of the tumor, the tumor resection margin was not easy to observe, thus increasing the likelihood for cancer recurrence. However, when the tumor is observed through the fluorescence guided operation system, it is possible to easily distinguish the color with the naked eye and it can be completely removed. Therefore, it is expected that surgical removal of cancerous tumors will be possible and surgical applications and surgical microscopes for cancer tumor removal surgery will be promising in the future.

Azimuthal Angle Scan Distribution, Third Order Response, and Optical Limiting Threshold of the Bismarck Brown Y:PMMA Film

  • This paper studies various roughness parameters, besides waviness, texture, and nonlinear parameters of Bismarck brown Y (BBY)-doped Poly(methyl methacrylate) (PMMA) films based on the computed values of optical limiting (OL) threshold power and nonlinear refractive index. The films' morphology, grain size, and absorption spectra were investigated using atomic force microscopy in conjunction with ultraviolet-visible (UV-Vis) spectrophotometer. The particle size of the films ranged between 4.11-4.51 mm and polymer films showed good homogeneity and medium roughness, ranging from 1.11-4.58 mm. A polymer film's third-order nonlinear optical features were carried out using the Z-scan methodology. The measurements were obtained by a continuous wave produced from a solid-state laser with a 532 nm wavelength. According to the results, BBY has a nonlinear refractive index of 10-6 cm2/W that is significantly negative and nonlinear. The optical limiting thresholds are roughly 10.29, 13.52, and 18.71 mW, respectively. The shift of nonlinear optical features with the film's concentration was found throughout the experiment Additionally, we found that the polymer samples have outstanding capabilities for restricting the amount of optical power that may be transmitted through them. We propose that these films have the potential to be used in a wide variety of optoelectronic applications, including optical photodetectors and optical switching.

교합면 우식병소의 다양한 진단법에 관한 비교연구 (IN VITRO COMPARISON OF VARIOUS DIAGNOSTIC METHODS OF OCCLUSAL CAR10US LESIONS)

  • 교합면의 열구는 우식이 발생하기 쉬운 위치이며, 특히 초기우식병소는 건전해 보이는 법랑질 하방에서 진행되는 경우가 많고, 주위 건전 법랑질과 잘 구별되지 않아서 주기적인 검사에도 불구하고 우식의 치료 시기를 놓치는 경우가 종종 있다. 초기 우식을 조기에 진단할 경우 불소도포, 식이조절, 치태조절 능력개선 그리고 타액분비 촉진 등의 방법으로 그 병리적 과정을 차단하고, 조기에 치료할 수 있다는 관점에서 조기 진단은 중요시되고 있다. 본 연구는 발거된 치아에서 교합면 우식을 검출하기 위해 두가지 전통적인 진단법인 시진, 방사선 검사법에 대하여 레이저 형광측정법을 이용한 새로운 우식진단기구인 Diagnodent (Biberach, Germany)의 민감도, 특이도 그리고 정확도를 비교함으로서 교합면 초기 우식 병소에 대한 가장 신뢰성 있는 진단 방법에 대하여 평가하고자 하였으며 다음과 같은 결론을 얻었다. 1. Diagnodent는 건전치에서 7.8, 초기우식에서 25.4, 법랑질 우식에서는 30.5 그리고 상아질 우식에서는 53.8의 평균값을 나타냈다. 2. 염료를 이용한 치아조직표본검사와 스피어만과 피어슨 상관계수에서 Diagnodent(0.736, 0.619)와 시진(0.664, 0.666)에서 높은 상관성을 나타냈고 방사선 촬영검사(0.333, 0.335)에서 가장 낮은 상관성을 나타냈다(P<0.01 전체). 3. 교합면 우식진단의 정확도는 Diagnodent가 65%로 가장 높았고 방사선 검사가 34%로 가장 낮았다. 4. 초기 우식의 경우, Diagnodent의 민감도와 특이도가 가장 높았고 법랑질 우식의 경우, 시진의 민감도가 가장 높았으며 Diagnodent의 특이도가 가장 높았다. 상아질 우식의 경우, Diagnodent의 민감도와 특이도가 가장 높았으며 시진의 민감도가 가장 낮았다.ed sealant는 기계적 삭제 방법을 사용한 경우와 산부식 방법만을 사용한 경우 모두 열구 침투도나 미세누출에 있어서 유의한 차이가 없었다. 이상의 결과로 보아 치면열구전색제 도포 시 열구 침투도가 우수하고 미세누출이 적은 기계적 삭제 방법과 물리적 성질이 우수한 filled sealant의 사용이 바람직하다고 생각된다. 그러나 미세누출에 있어 교합력을 고려한 실험이나 장기간의 임상실험이 필요할 것이다.5).vac/PE필름은 장기저장시 사용이 좋을 것으로 사료된다. 2) 질소가스 및 탄산가스투과도 각각 $61.6{\mu}PVDC/PE/Al-vac/CPS\;12.5,\;59.8>96.9{\mu}PE/PVDC/PE\;7.1,\;42.0>79.3{\mu}ET/PVDC/L-LDPE\;6.4,\;34.2>72.2{\mu}PET/PVDC/PE/Al-vac/PE\;0.74,\;4.2cc/m^2.24hr{\cdot}atm$으로 가장 우수한 PET/PVDC/PE/Al-vac/PE 복합필름이 장기저장용으로 이용이 좋을 것으로 사료된다.태발생을 연구하는데 기초적인 자료로 활용될 수 있을 것으로 사료된다. 조사한 결과 유의확률이 0.05 이하인 수치들은 Hb과 Albumin, K, Na간, Neutrophil과 Leukocyte간이었고 상관계 수치는 $0.370{\sim}0.442$ 사이로 나타났다. 유의확률 0.01이하로 조사된 수치는 Cholesterol과 ALT간, LDH와 Platelet, Creatinine간, Platelet와 BUN간, Na와 K수치간 이었으며 상관계수는 $0.531{\sim}0.866$사이로 나타났다. 5. 농양환자에서의 검사치간 상관관계를 조사한 결과 유의확률이 0.05이하인 수치들은 CRP와 Neutrophil간 이었고 상관계수는 0.

생쥐 자궁에서의 Id-1 유전자의 발현 (Expression of Id-1 Gene in Mouse Uterus)

  • 연구 목적: Microarray data에 의해 밝혀진 생쥐자궁에서의 Id 유전자의 hormonal effect와 implantation process 동안의 관계를 조사하고자 실험을 수행하였다. 연구재료 및 방법: 난소절제한 생쥐에 estrogen을 주사하고 6시간, 12시간이 지난 후 자궁을 적출하여 두 가지 방법으로 sample을 준비하였다. 먼저 자궁전체의 RNA를 추출하여 실험하거나 laser capture microdissection (LCM) 방법으로 자궁내막상피세포, 자궁기질세포, 자궁근육층으로 분리하여 RNA를 추출하고 semi-quantative RT-PCR을 수행하여 Id 유전자의 발현을 조사하였다. 임신 4.5일째 생쥐에 Chicago blue dye를 주사하여 착상부위와 비착상부위를 분리하고 RNA를 추출하여 Id 유전자의 발현을 semiquantitative RT-PCR 방법으로 실험하였다. 결 과: Estrogen을 처리한 난소절제된 생쥐 자궁에서의 cDNA microarray 자료에서 Id-1 mRNA는 점진적으로 두 배 이상 증가하였고 Id-2 mNRA는 반대로 시간이 지날수록 두 배 이상 감소하였다. Microarray 자료를 재확인하기 위해 semi-quantitative RT-PCR을 이용하여 실험하였고, 그 결과 Id-1 유전자는 estrogen 처리 6시간까지는 큰 변화가 없었으나 12시간에서는 4배 이상의 높은 발현을 보였으며, Id-2 mRNA의 발현은 estrogen 처리 6시간과 12시간 모두에서 대조군에 비해 4배 가량 감소하였다. 이 실험군을 LCM을 이용하여 자궁내막상피세포, 자궁기질세포, 자궁근육층을 각각 분리하여 실험한 결과 estrogen 처리군에서 Id-1의 발현은 자궁내막상피세포에서만 높은 발현을 보였으며, estrogen 처리 6시간과 12시간에는 큰 차이를 보이지 않았다. 그러나, Id-2 mRNA는 자궁내막상피세포에서 estrogen 처리 6, 12시간 모두에서 높은 발현을 보였고, 근육세포층에서는 estrogen 처리 6시간에서는 변화가 거의 없었으나 12시간에는 현저하게 증가하였다. 단, 자궁기질세포에서는 대조군에 비해 estrogen 6, 12시간에서 Id-2 mRNA의 발현이 감소하였다. 임신한 생쥐 자궁의 착상부위에서는 Id-1 mNRA의 발현은 비착상부 위보다 월등하게 높은 증가를 보였다. 결 론: 난소절제 생쥐를 이용한 실험에서 Id-1, -3는 estrogen에 의해 발현이 증가하고, Id-2는 발현이 감소하였다. LCM을 이용한 실험에서는 Id-2는 이와는 달리 부위별 발현양상은 다르게 나타났지만 이는 넓은 부위를 차지하는 자궁기질에서의 발현감소가 전체적인 Id-2의 발현양상으로 나타난 것으로 추측된다. 착상부위에서의 Id의 발현은 Id-1만이 유일하게 월등한 증가를 보였다. 위의 결과를 종합해 볼 때 생쥐 자궁에서 Id 유전자는 estrogen에 의해 조절되며 직, 간접적으로 착상시기에 다양한 작용을 할 것으로 사료된다.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. For higher concentration than 300mg/41 about glucose, a saturation state of abforbance was observed at 500, 550 and 570nm. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The model demonstrated the influence of wavelength on the thermal process and proved to be a valuable tool to investigate internal temperature rise. 30536 T00401030536 ^x We investigated the structural geometry of thirty-eight Korean femurs. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. Our department has developed an information system for routine hematology using down-sized computer system. We have used an IBM 486 compatible PC with 16MB main memory, 210 MB hard disk drive, 9 RS-232C port and 24 pin dot printer. The operating system and database management system were SCO UNIX and SCO foxbase, respectively. For program development, we used Xbase language provided by SCO foxbase. The C language was used for interface purpose. To make the system use friendly, pull-down menu was used. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer. The authors suggests that this system based on down-sized computer could provide a progressive approach to total LIS based on local area network, and the implemented system could serve as a model for other hospital's LIS for routine hematology. 30609 T00401030609 ^x To develop an artificial bone substitute that is gradually degraded and replaced by the regenerated natural bone, the authors designed a composite that is consisted of calcium phosphate and collagen. To use as the structural matrix of the composite, collagen was purified from human umbilical cord.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer. Muscle types of the lower extremity were classified according to their mechanical property of contraction, which are slow muscle (msoleus m.) and fast muscle (medial gastrocneminus m., rectus femoris m., vastus lateralis m.). Optimal parameters of electrical stimulation for slow muscles were 20 Hz, 0.2 ms square pulse. For fast muscle, 40 Hz, 0.3 ms square pulse was optimal to produce repeated contraction.
