Practical Vibration Analysis for the Floor of Dwelling Building (공동주택 바닥판에 대한 실용적인 진동해석)

  • In these days the floor vibration is beginning to make its appearance of the environmental dispute in dwelling building. Standard floor system are suggested for the settlement of this issue by government. The sound of floor impact sound is needed to secure comfortable quality in housing. Also, it is required an accurate analysis and a proper evaluation for floor vibration. Refine model is necessary for the floor system of housing to analyze accurately the floor vibration. But this refine model is not efficient because it is required so much running time for vibration analysis and it is difficult of modeling of standard floor slab. In this paper, new modeling methods of standard floor slab are proposed for the accurate rigidity evaluation. By using the new modeling method, the accurate vibration response can be obtained and can accurately evaluate the rigidity of standard floor system with resilient materials. Therefore the proposed modeling method is of practical use for vibration analysis of floor system of housing.

A Prospective Study on an Association between Apolipoprotein E ${\varepsilon}4$ and Cognitive Change in Community-Dwelling Elders with Alzheimer's Disease (일 지역 알츠하이머병 노인에서 Apolipoprotein E ${\varepsilon}4$와 인지변화의 연관에 대한 전향적 연구)

  • Objectives : The aim of this study was to examine the prospective impact of the apolipoprotein E (APOE) ${\varepsilon}4$ on cognitive performance in the community-dwelling elderly individuals with Alzheimer's disease (AD). Methods : The total number of subjects was 30 (12 men and 18 women) who were diagnosed with AD from a Korean project of "Early Detection of Dementia". People aged 65-85 years were included in the analysis. The eight neuropsychological domains from the Korean version of Consortium to Establish a Registry of Alzheimer's Disease (CERAD-K) were conducted to test subjects. They have been followed at 24-month intervals with the same assessments at each interval. Their cognitive performance at 2 year intervals was compared by the occurrence of the APOE ${\varepsilon}4$. Results : The impact of ${\varepsilon}4$ allele was significant in the Word List Memory Test (WLMT, F = 4.345, df = 1, p = 0.021) and Word List Recall Test (WLRT, F = 5.569, df = 1, p = 0.033). Conclusions : The APOE ${\varepsilon}4$ allele was significantly correlated especially with verbal episodic memory domain in community-dwelling elders diagnosed with AD.

The Development and Effect of a Dementia Care Support Program among Community Dwelling Older Adult Men (지역사회 거주 남성노인을 위한 치매 돌봄 프로그램 효과)

  • Purpose: We developed and tested the effects of a care program for dementia patients among community- dwelling older adult men. Methods: This study used a non-equivalent control group pre-and post-test design. To verify the effectiveness of the care program for dementia patients, 26 community dwelling older adult men participated in this study. We used multiple intervention strategies including improving understanding of dementia through education, activities for dementia prevention, and promoting psychological change. In particular, based on the current understanding of the care of the men older adults, we used strategies to promote motivation and reinforce strengths. Results: After completing the 6-week intervention program, when compared with the control group, older adult men in the care program intervention group showed significant differences in scores for the following: cognitive functions (p=.035), attitude toward dementia (p=.026), preventive behavior (p=.007), geriatric depression (p=.013), caring confidence (p=.018), and self-esteem (p=.013). Conclusion: These results indicate that the care program for dementia patients has positive effects on increasing their cognitive function, attitude toward dementia, preventive behavior, caring confidence, self-efficacy, and on decreasing depression rates in this population. Based on this, we can recommend this program to men caregivers for the improved care of dementia in community centers.

Effects of Elastic Band Resistance Training on Muscle Strength among Community-Dwelling Older Adults: A Systematic Review and Meta-Analysis

  • The purpose of this study was to investigate the effectiveness of elastic band resistance training for muscle strength among community-dwelling older adults. The systematic review and meta-analysis was conducted by following the Preferred Reporting Items for Systematic Reviews and Meta-analyses (PRISMA). Data were pooled using fixed effect models. Sit to stand, arm curl, and grip strength were analyzed for main effects. Heterogeneity between studies was assessed using the I2 statistics and publication bias was evaluated by funnel plots. Twelves studies were included representing 611 participants. Elastic band resistance training was effective for lower (d=3.89, 95% CI: 3.03, 4.75) and upper extremity muscle strength (d=4.08, 95% CI: 2.94, 5.23). Heterogeneity was moderate and no significant publication bias was detected. Based on these findings, there is clear evidence that elastic band resistance training has significant positive effects on muscle strength among community-dwelling older adults. Further study will be needed to perform subgroup analysis using number of sessions and exercise intensity as predictors.

Contamination of Toxocara canis in soil of playground in a detached dwelling area of Daejeon city (대전광역시 관내 단독주택지역내 어린이공원에서 채취한 토양중 개회충란 오염실태)

  • In order to survey the prevalence of Toxocara cams in soil of playground in a detached dwelling area, 150 samples were taken from playground 50 unit. The survey was carried out during 3 months from July to September in 2006. The Egg of Toxocara canis were detected in 6 samples (4.0%) from 150 cases of playground. The prevalence of T canis by month was 6.0% in July, 4.0% in August, 2.0% in September, respectively. This study results suggested that the prevalence of T canis were decreased in 2006 compared to 18.5% (30 positive/162 samples) in 2000.

Prevalence and Factors Related to Sarcopenic Obesity among Community-dwelling Elderly Women (지역사회 거주 여성노인의 근감소성 비만 유병률과 관련요인)

  • Purpose: The aims of this study were to identify prevalence and identify factors related to sarcopenic obesity among community-dwelling elderly women. Methods: This is a secondary analysis of the prospective cohort study. Our analysis included 338 elderly women (${\geq}65$ years old) in South Korea as a part of the Community-dwelling Older Adult Health Cohort (COHC) Study (2014-2015). Sarcopenic obesity was defined as the Asian Working Group of Sarcopenia recommendations and upper two quintiles for percentage body fat. Logistic regression analysis was used to determine the factors related to sarcopenic obesity including chronic diseases, medications, stress, fatigue, depression, exercise, level of proteins on body compositions, smoking, and alcohol use. Results: The prevalence of sarcopenic obesity was 6.2%. A lower protein on body compositions (OR 0.017, 95% CI 0.003-0.081, p< .001), a larger number of medications (OR 2.104, 95% CI 1.404-3.152, p< .001), and a higher level of fatigue (OR 1.255, 95% CI 1.023-1.541, p= .030) were related factors of sarcopenic obesity. Conclusion: The findings suggest that nutritional interventions focusing on protein intakes should be needed to prevent sarcopenic obesity among the elderly women. Polypharmacy issue for preventing adverse outcomes and level of fatigue as indicator for early identification are also considered to develop community prevention programs.

  • The development of rapid science technology and data communication technology in 1990 was infiltrated into our real lives deeply and drew forth various changes in various sides. According to that with the new definition of the character of space the users' patterns in the space were changed. We come to have a turning point of change with the introduction of personal computer and digital revolution late in the 20th century. Accordingly, this study is to analyze the connected structure of the space on a basis of the apartment model house since 2000 in order to investigate the aspect of changing the space according to the era of informationization. This analysis of space structure makes the characteristics in the era of informationization objectified. And it will understand the distinct change of space by grasping the relations with the connection of space, and predict the direction that the dwelling space heads for the future.

Correlations between Intention to use Advance Directives and Death Anxiety in the Community-dwelling Elderly (지역사회 노인들의 사전의료의향서 작성의향과 불안과의 상관관계)

  • Purpose: The purpose of this study was to identify the relationship between intention to use advance directives and level of death anxiety in community-dwelling elders. Method: The participants were 200 older adults who were aged 60 or over and attended the community welfare centers. The level intention to use advance directive was measured by a questionnaire that was developed by the authors for the study. The measure by Kraus and Ellisond was administered to evaluate the level of death anxiety. Result: The average score for intention to use advance directives was $2.05{\pm}0.88$ and that of death anxiety was $6.2{\pm}2.28$. There were significant differences in the intention to use advance directives according to education levels and individual properties. Although there was no significant relationship between the intention to use advance directives and the level of death anxiety, they were negatively associated. Therefore, older adults who had lower death anxiety would tend to use advance directives. Conclusion: It would be necessary to screening the level of death anxiety to promote use of the advance directives. In addition, education programs for advance directives would be essential to consider about advance directives for their end-of-life especially for the community-dwelling elderly in Korea.

A Structural Equation Model of Fall Prevention Behavior among Community-dwelling Older Adults with Osteoarthritis (재가 관절염 노인의 낙상 예방행위 예측모형 구축)

  • Purpose: The purpose of this study was to explain fall prevention behaviors of community-dwelling elderly with osteoarthritis based on the Health Belief Model. Methods: A total of 200 older adults with osteoarthritis residing in community was recruited from July 10 to August 30, 2013. The direct and indirect effects of perceived fall risk, perceived benefits and barriers, and self efficacy of fall prevention were examined on fall prevention behaviors. Data were collected with structured questionnaires and analyzed using SPSS/WIN 20.0 and AMOS 20.0 program. Results: The hypothetical model was a good fit for the data based on the model fit indices. Among socio-demographic variables, age and fall knowledge showed significant direct effects on fall prevention behaviors. The constructed model explained 34.2% of the variance of fall prevention behaviors, including perceived fall risk and efficacy of fall prevention behaviors as significant predictors. Conclusion: The findings revealed the need to develop an effective nursing intervention to promote fall prevention behaviors of community-dwelling elderly with osteoarthritis by focusing on perceived fall risk and efficacy of fall prevention behaviors. Knowledge about fall can also be increased by an age-based education program.

Management Strategy by Evaluation on Comprehensive Health Problem in the Community-dwelling Elderly of Korea (재가노인의 포괄적 건강문제 평가와 관리방안)

  • Purpose: The aim of this study was to define the health problem in the community-dwelling elderly of Korea and to compare differences of CAPs(Client Assessment Protocols) by characteristics. Method: Data was collected by visiting nurse from 556 elderly over 65 years in selected metropolitan areas. To evaluate the functional state of elderly in the community, I used "RAI-MDS HC; Residental Assessment Instrument Minimum Data Set-Home Care(2.0 version)" and established information exchange system among resources, by developing the data into a computer program. Results: The health problem of 'preventive health measures' was the largest(99.6%), and then 'health promotion(85.3%)', 'visual function(75.5%)', 'psychologic drug(68.9%)', 'pain(68.5%)', ‘social function(59.2%)', 'communication disorders(56.2%)', 'environmental assessment(53.2%)', 'depression & anxiety(46.9%)', 'oral health(43.4%)' followed. The number of health problems was average 10.16 in the community-dwelling elderly. Conclusion: The results suggest the need to emphasize the importance of assessment of the health problem of the elderly. We can apply it in the distribution of community resources and the development of service providing programs by figure out the health problem and resource in need for the elderly in the community.
