Effectiveness of en-masse retraction using midpalatal miniscrews and a modified transpalatal arch: Treatment duration and dentoskeletal changes

  • Objective: The purpose of this study was to compare the treatment duration and dentoskeletal changes between two different anchorage systems used to treat maxillary dentoalveolar protrusion and to examine the effectiveness of en-masse retraction using two miniscrews placed in the midpalatal suture. Methods: Fifty-seven patients (9 men, 48 women), who had undergone level anchorage system treatment at Aichi-Gakuin University Dental Hospital (Nagoya, Japan) were divided into two groups according to the method of maxillary posterior anchorage reinforcement: midpalatal miniscrews (25 patients, mean age 22 years) and conventional anchorage (32 patients, mean age 19 years). The en-masse retraction period, overall treatment duration, pre-treatment effective ANB angle, and change in the effective ANB angle were compared with an independent-samples t -test. Results: Compared to the headgear group, the duration of en-masse retraction was longer by approximately 4 months in the miniscrew group (p < 0.001). However, we found no significant difference in the total treatment duration between the groups. Moreover, a greater change in the effective ANB angle was observed in patients treated with miniscrews than in those treated with the conventional method (p < 0.05). Conclusions: The level anchorage system treatment using miniscrews placed in the midpalatal area will allow orthodontists more time to control the anterior teeth during en-masse retraction, without increasing the total treatment duration. Furthermore, it achieves better dentoskeletal control than does the conventional anchorage method, thereby improving the quality of the treatment results.

  • Purpose: We analyzed to find out prognostic factors in the conservative treatment for the plantar fasciitis. Materials and Methods: The data were collected from 145 patients, 159 feet (M:F = 51:108) with conservative treatment and analyzed for possible prognostic factors : sex, age of onset, the duration of symptom before treatment, pain score before treatment, the duration for symptom remission, medication period, calcaneal pitch angle, and presence of calcaneal spur. Results: The duration of symptom before conservative treatment is affected to the prognosis, and the borderline of the effective period was about 6 months. Conclusion: With the conservative treatment of the plantar fasciitis, we found that (1) the duration of symptom before the conservative treatment was a prognostic factor, and (2) if the period before the conservative treatment was more than 6 months, the other treatment option such as surgery should be considered for this chronic group.

  • It is well known fact that many tuberculosis patients have often experienced interuption of treatment while their treatment. Because most of cases were inproved subjective symptome by adequate treatment for 3-6 months and patients misunderstood to be heal completely by their own dicision, It become the cause of emergency of drug resistance and chronic incurable diseases. The author intended to clarify the duration of treatment and its outcome by some characteristics of tuberculosis patients. The results obtained were as follows: 1. The rate of treatment for tuberculosis patients by each duration with 1-6, 7-12, 13-13, 19-24 and 25 or more months were 16, 23, 20, 14 and 25 percents respectively. The duration of treatment increased in patients with advancing age, better educated and registered at health center between 7-12 months after diagnosis for tuberculosis. 2. The rate of complete treatment, defaulters, death and migration were 43, 13, 8, and 10 percents respectively. The rate of complete treatment were high in older, better educated and registered at health center between 7-12 months after diagnosis. The rate of defaulters were high in younger, no educated and registered within 1 month after diagnosis. The rate of death was high in older and migration was high in younger.

A Single-Center Retrospective Study on the Effects of Korean Medicine in 342 Traffic Accident Cases

  • Objectives: In South Korea, traffic accident victims can be treated under automobile insurance coverage. Korean medicine (KM) clinics have reported the largest number of automobile insurance fee claims among medical institutions. This study investigated the status of the KM automobile insurance system in a single KM clinic. Methods: We retrospectively surveyed the medical charts of 342 traffic accident patients treated at the Jisung KM clinic between January 2009 and June 2017. Results: Most of the patients were men and in their 30s. The most common method of locating the clinic was an internet search. The most common traffic accident type was collision between vehicles (83.63%), with 70.76% of patients visiting during the most acute phase. The major disease codes included S434, M4836, F072, S0600, and S3350. The most frequent treatment period was within 1 month of the accident, and most patients received 10 or fewer treatments. The mean treatment duration and number of treatments were 37.68 ± 45.11 days and 11.68 ± 10.63 treatments, respectively. The initial pain numerical rating scale (NRS), 7.32 ± 0.96, decreased to 3.57 ± 1.40 at the end of treatment, with a symptom improvement score of 1.87 ± 0.60. Regarding sex, age, disease duration, location at the time of the accident, presence of additional and psychological symptoms, and chuna, there were statistically significant differences in treatment duration and number of treatments. A higher number of treatments and the longer treatment duration was associated with a higher initial NRS, lower post-treatment NRS, and better improvement score. Since the introduction of traffic accident (TA) pharmacopuncture, the rate of use of a single type of pharmacopuncture increased; however, no significant differences in treatment duration and number, NRS before and after treatment, and improvement score were observed between treatment groups before and after TA pharmacopuncture. No adverse reactions were observed for any treatment. Conclusion: This study confirmed the previous findings of a high treatment effect of KM under automobile insurance. We also observed significant correlations based on a detailed medical status, which may explain the increasing use of KM in the automobile insurance system. Additional multi-center studies in different regions are needed.

A Retrospective Clinical Investigation of Delayed Eruption of Premolars in the Mandible

  • This study aimed to investigate the treatment options for the delayed eruption of mandibular premolars and identify the predictors of spontaneous eruption using panoramic radiography. The prevalence of delayed mandibular premolar eruption in this retrospective analysis, comprising 254 patients (aged 9 - 15 years), was 5.19%, with no significant difference based on gender. The mandibular second premolars were most affected (4.39%) compared to the first premolars (0.76%). No significant difference in prevalence was observed between the left and right sides. Among the treated mandibular premolars, primary molar-related lesions were identified as the leading cause (7.85%) of delayed tooth eruption. The treatment duration varied based on the Nolla stage, eruption stage, and treatment method. Teeth with Nolla stage 7 or lower had a treatment duration of 22.89 ± 11.96 months, whereas those with stage 8 or higher had a 15.02 ± 6.34 month duration. The deeper the tooth was located in the bone, the longer the treatment period became. The treatment duration varied depending on the treatment method, and statistically, there was no significant difference. The treatment durations for affected mandibular premolars increased with the depth of impaction angle of inclination. In this study, the treatment duration for delayed eruptions varied depending on the Nolla stage, eruption stage, and treatment method. Variations in the impaction depth and inclination angle across various treatment approaches, as explored in this study, might offer valuable insights into the selection of the most suitable management options for delayed tooth eruptions.

Factors Associated with Treatment Duration Time of Forced Eruption in Impacted Maxillary Canines Using 3-Dimensional Computed Tomography (의도적 맹출술이 시행된 상악 매복 견치의 치료기간과 관련된 용인에 대한 3차원 단층촬영을 이용한 분석)

  • Early identification of maxillary canine impaction is very important, because of treatment duration time. Analysis used by 3-dimensional computed tomography is more important than before in treatment planning. The aim of this study was to analysis of factors associated with the treatment during time of forced eruption of the impacted maxillary canine using 3-dimensional computed tomography. 17 patients were analysed in Daejeon dental hospital of Wonkwang university(16 patients with unilateral impactions and 1 patient with bilateral impaction). In total, 18 maxillary impacted cannines were analysed by 3-dimensional computed tomography. The results obtained were as follow; 1. There was positive correlation between the age of patient and treatment duration time. 2. There was positive correlation between the distance of occlusal plane to cusp of maxillary impacted canine and treatment duration time, and negative correlation between the length of maxillary impacted canine and treatment duration time. 3. There was positive correlation between the angulation of midsagittal plane to axis of maxillary impacted canine and treatment duration time, and negative correlation between occlusal plane to axis of maxillary impacted canine and treatment duration time. In conclusion, age, tooth length, distance from the canine cusp tip to the occlusal plane, angulation of tooth axis to occlusal plane and midsagittal plane allow that we can predict treatment duration time.

  • Tuberculosis (TB) remains a major global health problem, and the incidence of TB cases has not significantly decreased over the past decade in Korea. The standard short course regimen is highly effective against TB, but requires multiple TB-specific drugs and a long treatment duration. Recent studies using late-generation fluoroquinolones and/or high-dose rifapentine-containing regimens to shorten the duration of TB treatment showed negative results. Extending the treatment duration may be considered in patients with cavitation on the initial chest radiograph and positivity in sputum culture at 2 months of treatment for preventing TB relapse. Current evidence does not support the use of fixed-dose combinations compared to separate drugs for the purpose of improving treatment outcomes. All patients receiving TB treatment should be monitored regularly for response to therapy, facilitation of treatment completion, and management of adverse drug reactions. Mild adverse effects can be managed with symptomatic therapy and changing the timing of the drug administration, but severe adverse effects require a discontinuation of the offending drugs.

Retrospective Study on 469 Halitosis Patients (구취환자 469례에 대한 후향적 연구)

  • There is a growing interest in halitosis and diverse Korean medical studies are being conducted about it. But there are few study about teatment effect of halitosis after Korean medical care, treatment duration, and factor affecting recovery rate. Thus, the purpose of this study is to research clinical characteristic of halitosis patients, factor affecting halitosis, recovery rate of halitosis, treatment duration, factor affecting recovery rate by analyzing halitosis patients retrospectively. People who were over 19 and visited Korean medical clinic in Seoul to want to be treated halitosis in 2014 were analyzed retrospectively. We analyzed general and living characteristics of halitosis patients, halitosis-related symptoms and diseases, level of halitosis and halitosis-related symptoms both before and after treatment, treatment duration of halitosis patients, prescription of halitosis patients, recovery rate of level of halitosis and halitosis-related symptoms by general characteristic and duration of symptoms. Female, thirties, and Nonsmoker had the highest proportion in general characteristic. The average duration of halitosis is 41.6months and treatment duration is 2.55months. The average of sensory evaluation score is 3.40±2.53, subjective evaluation score is 5.02±1.53, lump sensation on throat is 3.52±2.50, postnasal drip is 1.11±1.88, dry mouth is 4.13±2.17, quality of life is 6.07±1.13. Gamichiuitang is used the most among prescriptions. Sensory evaluation, subjective evaluation, lump sensation on throat, postnasal drip, dry mouth, and quality of life had better score compared with pre-treatment. Duration of symptoms is in weak inverse proportion to sensory evaluation, subjective evaluation, dry mouth, quality of life. This study is meaningful in the way to find recovery rate of halitosis after korean medical care, treatment duration, and factor affecting recovery rate not progressed until now. But this study also has limits such as not analyzing objective data using halitosis measuring instrument and lack of methodical scale about quality of life. Therefore, succeeding study such as clinical trials is needed to verify reliability of Korean medical treatment.

Ratio of fat to energy intake independently associated with the duration of diabetes and total cholesterol levels in type 2 diabetes

  • The importance of dietary intake in the treatment of type 2 diabetes was emphasized. This study was performed to investigate the dietary intakes of Korean type 2 diabetes patients according to the treatment and duration of diabetes and to examine the relationships between their diet and serum lipid profiles. The subjects were 111 type 2 diabetic patients who were treated by medical nutrition therapy only, oral hypoglycemic agents (OHA), or insulin with medical nutrition therapy. Dietary intake was assessed by a registered dietitian using semi-quantitative food frequency questionnaires Comparisons according to treatment type were made using covariance analyses. General linear models identified the independent effects of the different treatments after covarying for age, duration of diabetes, and 2-way interactions. There were no significant differences in age and BMI but was in duration of diabetes according to treatment type in these subjects. Carbohydrate to energy ratio was higher in the OHA group (P < 0.05), whereas the fat to energy ratio was higher in the insulin group for males (P < 0.05). Carbohydrate ($R^2$ = 0.24, P = 0.005) and fat ($R^2$ = 0.26, P = 0.02) to energy ratios were independently associated with the duration of diabetes after covarying for age, sex, treatment, and 2-way interactions. The levels of triglyceride (TG; $R^2$ = 0.32, P = 0.02) and total cholesterol (TC) were associated independently with energy intake and the carbohydrate ($R^2$ = 0.15, P = 0.02) and fat ($R^2$ = 0.15, P = 0.01) to energy ratios, respectively. The concern that the independent association of dietary intake with either duration of diabetes or dietary factors affects blood lipid levels could suggest that specific dietary recommendations may work better for identifiable groups of diabetes patients.

A Retrospective Statistical Analysis of Miniscalpel Needle Therapy for Herniated Intervertebral Disc or Spinal Stenosis

  • Background: This study examined the characteristics and prognosis of patients admitted to the Dunsan Korean medicine hospital for treatment of herniated intervertebral disc (HIVD) or spinal stenosis with Miniscalpel needle therapy (MSN). Methods: Patients were admitted to the Dunsan Korean medicine hospital from January 01, 2016 to September 30, 2017 for the treatment of HIVD or spinal stenosis with MSN. Crossover analysis, Independent sample t test, one-way ANOVA, multiple linear regression analysis, and binary logistic regression analysis were performed. Results: Crossover analysis showed statistically significant differences in treatment methods according to gender, current pain according to the disease duration, satisfaction of MSN according to disease duration, treatment methods, and intention of re-treatment with MSN according to treatment methods. Independent t test and one-way ANOVA showed that there was a difference in current Numeric Rating Scale (NRS) according to disease duration, and difference between discharge and current NRS, and number of MSN according to disease. Multiple linear regression analysis showed that age, disease duration, and number of MSN affect discharge NRS, disease duration, and number of MSN affect current NRS, and Western medical treatment after MSN, discharge NRS, and current NRS affect satisfaction of MSN. Binary logistic regression analysis showed that discharge NRS affects current pain, and gender, discharge NRS, and treatment methods affect intention of re-treatment with MSN. Conclusion: Characteristics, prognosis, satisfaction and variables affecting prognosis of MSN were statistically significant, indicating that more systematic studies are required to further examine the effects of MSN on HIVD or spinal stenosis.
