Objective: To compare the effects of bone suppression imaging using deep learning (BSp-DL) based on a generative adversarial network (GAN) and bone subtraction imaging using a dual energy technique (BSt-DE) on radiologists' performance for pulmonary nodule detection on chest radiographs (CXRs). Materials and Methods: A total of 111 adults, including 49 patients with 83 pulmonary nodules, who underwent both CXR using the dual energy technique and chest CT, were enrolled. Using CT as a reference, two independent radiologists evaluated CXR images for the presence or absence of pulmonary nodules in three reading sessions (standard CXR, BSt-DE CXR, and BSp-DL CXR). Person-wise and nodule-wise performances were assessed using receiver-operating characteristic (ROC) and alternative free-response ROC (AFROC) curve analyses, respectively. Subgroup analyses based on nodule size, location, and the presence of overlapping bones were performed. Results: BSt-DE with an area under the AFROC curve (AUAFROC) of 0.996 and 0.976 for readers 1 and 2, respectively, and BSp-DL with AUAFROC of 0.981 and 0.958, respectively, showed better nodule-wise performance than standard CXR (AUAFROC of 0.907 and 0.808, respectively; p ≤ 0.005). In the person-wise analysis, BSp-DL with an area under the ROC curve (AUROC) of 0.984 and 0.931 for readers 1 and 2, respectively, showed better performance than standard CXR (AUROC of 0.915 and 0.798, respectively; p ≤ 0.011) and comparable performance to BSt-DE (AUROC of 0.988 and 0.974; p ≥ 0.064). BSt-DE and BSp-DL were superior to standard CXR for detecting nodules overlapping with bones (p < 0.017) or in the upper/middle lung zone (p < 0.017). BSt-DE was superior (p < 0.017) to BSp-DL in detecting peripheral and sub-centimeter nodules. Conclusion: BSp-DL (GAN-based bone suppression) showed comparable performance to BSt-DE and can improve radiologists' performance in detecting pulmonary nodules on CXRs. Nevertheless, for better delineation of small and peripheral nodules, further technical improvements are required.

Purpose: This study aims to compare patients in whom fixation failure occurred via cut-out (CO) or cut-through (CT) in order to determine patient factors and radiographic parameters that may be predictive of each mechanism. Materials and Methods: This retrospective cohort study includes 18 patients with intertrochanteric (IT) hip fractures (AO/OTA classification 31A1.3) who underwent treatment using a single lag screw design intramedullary nail in whom fixation failure occurred within one year. All patients were reviewed for demographics and radiographic parameters including tip-to-apex distance (TAD), posteromedial calcar continuity, neck-shaft angle, lateral wall thickness, and others. Patients were grouped into cohorts based on the mechanism of failure, either lag screw CO or CT, and a comparison was performed. Results: No differences in demographics, injury details, fracture classifications, or radiographic parameters were observed between CO/CT cohorts. Of note, a similar rate of post-reduction TAD>25 mm (P=0.936) was observed between groups. A higher rate of DEXA (dual energy X-ray absorptiometry) confirmed osteoporosis (25.0% vs. 60.0%) was observed in the CT group, but without significance. Conclusion: The mechanism of CT failure during intramedullary nail fixation of an IT fracture did not show an association with clinical data including patient demographics, reduction accuracy, or radiographic parameters. As reported in previous biomechanical studies, the main predictive factor for patients in whom early failure might occur via the CT effect mechanism may be related to bone quality; however, conduct of larger studies will be required in order to determine whether there is a difference in bone quality.
전산화단층촬영(computed tomography, CT) 영상은 양성자 브레그 피크 위치 추정 및 치료 계획 시뮬레이션의 기초로 사용된다. Hounsfield Unit(HU) 기반의 양성자 저지능비(stopping pwer ratio, SPR) 예측 과정에서 환자의 밀도와 원소 구성의 작은 차이로 양성자 빔의 경로를 따라 브레그 피크 위치의 불확실성이 발생한다. 본 연구에서는 브레그 피크 위치 예측 불확실성 감소를 위하여 이중에너지 전산화단층촬영 영상 기반의 양성자 저지능비 예측 정확도의 잠재력을 연구를 하였다. 양성자 빔의 저지능비를 추정하기 위해 전산화단층촬영 시스템(Somatom Definition AS, Siemens Health Care, Forchheim, Germany)을 이용하여 전자밀도팬텀(CIRS Model 062M electron density phantom, CIRS Inc., Norfolk, VA, USA)의 단일에너지 및 이중에너지 영상을 획득하였다. 이를 검증하기 위해 미국 국립 표준기술 연구소(National Institute of Standards and Technology, NIST)에서 제공하는 표준 데이터를 통하여 추정한 실제 저지능비와 비교하였다. 그 결과 잡음이 제거된 이중에너지 영상 기반 방법을 통한 양성자 빔의 저지능비 예측에서 정확도 개선 가능성을 확인할 수 있었으며, 인체의 다양한 밀도와 원소 구성을 가진 대체물을 더욱 다양하게 제작하여 저지능비를 예측 할 경우 더욱 향상된 양성자의 브레그 피크 위치 예측이 가능할 것으로 사료된다.
목적 Twin beam dual-energy CT (이하 tbDECT)와 dual source DECT (이하 dsDECT)를 통해 얻은 true non-contrast image (이하 TNC)와 virtual non-contrast image (이하 VNC)의 attenuation values 차이를 평가해 보고자 한다. 대상과 방법 간 DECT를 촬영한 62명의 환자를 대상으로 하였다(tbDECT, 32명; dsDECT, 30명). TNC와 재구성한 arterial VNC (이하 AVNC), portal VNC (이하 PVNC), delayed VNC (이하 DVNC)에서 복부내 11개 장기에 대해 attenuation values를 측정하여 비교하였다. 또한 TNC와 VNC attenuation values의 절대오차가 10 Hounsfield units (이하 HU) 이하인 비율을 구하였다. 결과 TNC와 VNC의 평균 attenuation values 비교에서 각 DECT별 33개의 항목(3시기 VNC, 11개 장기) 중 tbDECT는 17개, dsDECT 19개 항목에서 유의한 차이를 보였다(Bonferroni correction p < 0.0167). 절대오차 10 HU이하인 비율은 tbDECT의 AVNC, PVNC, DVNC에서 각각 56.7%, 69.2%, 78.6%, dsDECT는 각각 70.5%, 78%, 78%이었고, 두 DECT모두 AVNC에서 가장 낮았다. 결론 두 DECT의 VNC는 적지 않은 attenuation values 차이로 TNC를 대체하기에는 충분하지 않다.
Two spaniel dogs were presented with a history of lethargy, weakness, refractory hypoglycemia, and episodic seizure. A pancreatic mass on the distal end of the left limb was identified through screening tests including dualphase CT angiography in each patient. Canine insulinoma was suspected with low blood glucose, low fructosamine, and normal to high serum insulin levels. Exploratory laparotomies were performed, and partial pancreatectomy using an ultrasonic-activated scalpel was achieved without enzyme leakage or fistula formation after surgery. The lesions were definitively diagnosed as insulinoma via histopathologic examination. One dog has been doing well until POD 870 with medication; in the other dog, diffuse hepatic nodules noted during surgery were confirmed as a liver metastasis. This dog died at POD 45. This case report addresses the utility of the ultrasonic scalpel in partial pancreatectomy for the treatment of canine insulinoma and differences in prognosis according to disease stage.
Phosphate absorption from a Na2H32PO4 solution by Oryza sativa L. was studied in order to elucidate kinetic mechanisms of ion transport. The rates of phosphate absorption from different concentraitons indicated the presence of dual mechanisms in root tips, one in the low (1$\times$10-6 to 8$\times$10-5M) and the other in the high (1$\times$10-4 to 8$\times$10-3M). A phosphate compensation point of phosphate transport was revealed with a 1$\times$10-6M solution of Na2H32PO4. The kinetic model that ion transport involves an exchange reaction of absorption and desorptin is prosposed as follows: where C represents an ionic-specific organic carrier in the membrane; M, Mo and Mi are the mineral ions, M-outside and M-inside; MC is a carrier-ion complex; and the K's represent rate constants. In approximately 10% of patients with acute myocardial infarction (MI), angiography does not reveal an obstructive coronary stenosis. This is known as myocardial infarction with non-obstructive coronary arteries (MINOCA), which has complex and multifactorial causes. However, this term can be confusing and open to dual interpretation, because MINOCA is also used to describe patients with acute myocardial injury caused by ischemia-related myocardial necrosis. Therefore, with regards to this specific context of MINOCA, the generic term for MINOCA should be replaced with troponin-positive with non-obstructive coronary arteries (TpNOCA). The causes of TpNOCA can be subcategorized into epicardial coronary (causes of MINOCA), myocardial, and extracardiac disorders. Cardiac magnetic resonance imaging can confirm MI and differentiate various myocardial causes, while cardiac computed tomography is useful to diagnose the extracardiac causes.
Although advances in multi-detector computed tomography (CT) technique make it possible to evaluate peripheral subsegmental pulmonary arteries, several studies have reported that small peripheral embolisms may still be missed. Recently, some reports demonstrated that dual-energy CT improved the capability to detect peripheral pulmonary embolism. We report a case of lymphoma presenting as disseminated microvascular pulmonary tumor embolism, detected by perfusion images using dual energy CT.
Purpose : This study evaluated the potential use of the computed tomography indices (CTI) on cone beam CT (CBCT) images for an assessment of the bone mineral density (BMD) in postmenopausal osteoporotic women. Materials and Methods : Twenty-one postmenopausal osteoporotic women and 21 postmenopausal healthy women were enrolled as the subjects. The BMD of the lumbar vertebrae and femur were calculated by dual energy X-ray absorptiometry (DXA) using a DXA scanner. The CBCT images were obtained from the unilateral mental foramen region using a PSR-$9000N^{TM}$ Dental CT system. The axial, sagittal, and coronal images were reconstructed from the block images using $OnDemend3D^{TM}$. The new term "CTI" on CBCT images was proposed. The relationship between the CT measurements and BMDs were assessed and the intra-observer agreement was determined. Results : There were significant differences between the normal and osteoporotic groups in the computed tomography mandibular index superior (CTI(S)), computed tomography mandibular index inferior (CTI(I)), and computed tomography cortical index (CTCI). On the other hand, there was no difference between the groups in the computed tomography mental index (CTMI: inferior cortical width). Conclusion : CTI(S), CTI(I), and CTCI on the CBCT images can be used to assess the osteoporotic women.
