The application of the knowledge from the Human Genome Project to clinical medicine will be through both industrial drug development and the application of pharmacogenomics (PG) to patient care. The slow uptake of clinical innovations into clinical practice can be frustrating, but understanding the history of acceptance and sustaining medical innovation is critically important to position PG to succeed. This primarily means that PG tests must have legitimacy; they must be thoroughly validated, must be cost-effective, must be widely accepted by medical practitioners, must be supported by public policy, and must have a way of being easily incorporated into current medical practice. They must also lead to actionalble decisions by health care providers for their patients. Innovative PG assays should be tested in the best US laboratories, and reimbursement for testing must be accepted at the federal and state level. The companies providing these PG tests should be capable of sup-porting the interpretation and use of the test throughout medical practice. Advances such as the addition of PG information to drug labeling and the routine use of validated biomarkers to determine choice of cancer chemotherapy have been made. The PG research community must pay attention to the principles that have been previously described for acceptance and sustaining medical innovations in order for PG to be widely accepted in clinical medical practice.
This study investigated social relief schemes for serious adverse drug reactions in foreign countries and deduced lessons and implications for Korea to implement the scheme. A social relief scheme for serious adverse drug reactions provides reliefs for diseases and such health effects as disabilities or deaths that were caused by adverse reactions to pharmaceuticals prescribed at hospitals and clinics as well as those purchased at pharmacies notwithstanding their proper use. The US and the UK do not have specific relief schemes for adverse drug reactions but apply rules of strict liability or negligence. New Zealand and Nordic countries provide no-fault compensation schemes for health effects or injuries caused by medical treatments or medicinal products. Japan and Taiwan have operated the schemes since 1980 and 2000, respectively. In designing the scheme in Korea, we suggested that cases eligible for relief be confined to serious adverse reactions such as death or disability and then extended to diseases. It is desirable to encourage the reporting system of adverse drug reactions and quality use of medicines for the relief scheme to work efficiently.

This study reviewed published studies on interventions used by hospitals, health insurance programs, or governments to improve use of medicines in foreign countries. Interventions to improve use of medicines are classified into two categories: 1) information strategies-dissemination of educational materials, group education, one-to-one educational outreach, drug utilization review, and feedback; 2) managerial strategies- formularies, prior authorization, and financial incentives. Dissemination of educational materials, which is a common intervention, was unsuccessful in changing physicians' prescribing behaviors. Problem-based small group education was more likely to change behaviors than didactic large group education. One-to-one educational outreach(academic detailing) was among the most effective strategies used to change prescribing behaviors. Prospective drug utilization review (DUR) program was more successful in improving use of medicines than retrospective DUR program. Feedback intervention has been reported to be ineffective to change behaviors. Formularies are frequently used to control medication use by most health insurance programs. Financial incentives provide physicians economic incentives according to appropriateness of prescribing. However, few published studies have assessed the efficacy of formularies or financial incentives. Prior authorization requires physicians to get authorization from health insurers before prescribing a certain group of drugs which is usually of high costs or risk. There is no magic bullet for quality use of medicines. Multifaceted interventions that help to predispose, enable, and reinforce desired behaviors are more likely to be successful.
Appropriate drug information is essential in pharmaceutical care practice. In recent years, educating patients with their prescription and drug therapy is becoming an increasingly important aspect of health care. Appropriate drug information has been shown to improve patients' adherence to their medication and the results of pharmacotherapy. The purpose of this study was to study patients' sources of drug information and their preferences in Korea. This study was a 11-questionnaire survey conducted from February 8, 2010 to February 25, 2010. Major sources of drug information used by respondents, proportions of respondents who received drug information on administration, dosage, indication, or warnings and precautions, sources of drug information which patients prefer, and the reasons of their preferences were studied. Of the 303 respondents, the most common sources of drug information were pharmacists (29.7%), doctors (23.8%), and the Internet (17.2%). There were significant differences according to sources of drug information in proportions of respondents who received drug information on administration, dosage, indication, or warnings and precautions. Patients preferred pharmacists as a drug information source the most (38.9%), and the reasons were reliability (72.0%) and easy to use (28.0%). Based on the results of this study, further studies should be conducted to establish the best way to provide appropriate drug information for patients and improve the results of pharmacotherapy.

The present study was undertaken to develop the in vitro sun protection factor(SPF) test method having good correlation with in vivo method using human. 8% homomentyl salicylate, P3 reference standard and commercially available sunscreen products were measured by the in vitro method using SPF 290S analyzer, and the SPFs were compared with the SPFs measured by in vivo test method. In vitro SPFs of 8% HMS and P3 reference standard were 4.59 $\pm$ 0.12 and 14.94 $\pm$ 0.83. There are good correspondence, correlation coefficients were 0.9506 and 0.9769 respectively, between the in vitro and in vivo SPFs for the sunscreen creams and lotions. Correlation coefficients of makeup base/liquid foundation, lotion labled with "shake before use" and compact powder were 0.8812, 0.8632 and 0.5984 respectively. The optimum mixture ratio of compact powder and cream base represents 1:0.8. These results suggest that the in vitro SPF test method will be able to be used as an alternative method for in vivo SPF in case of lotion and cream.

This study intended to investigate the characteristics of adolescent drug user in Korea. A questionnaire on adolescent drug use was administered to 1st or 2nd grade 898 students (465 males, 433 females) from 9 senior high schools in Seoul area. The extent of current using six representative drugs such as cigarettes, alcohol, chemical substance inducing hallucination(e,g., thinner, bond etc.), psychostimulant, hypnotic and minor tranqulizer was self-reported. Also their psychological distress, self-concept and individual-related variables such as academic grade, deviant behavior, drug-using peers, the perceived availability of drugs, the degree of family discipline, and the perceived parents' attitude for drinking and smoking were surveyed. Subjects were grouped into six(male) or five(female) subgroups (nonuser, simple experiencer, one kind of drug user, two kinds of drugs user, three kinds of drugs user(male only), minor tranqulizer user) to their extent of drug use. Then ANOVA procedure performed for group comparison of their psychological distress, self-concept and individual-related variables. The results showed that the most popular drugs among adolescents in Korea were cigarettes and alcohol. Minor tranqulizer was used by about 2.8% of male and 7.41% of female subjects. In general abusers showed lower academic grade, more deviant experiences, having more drug-using peers, higher degree of psychological distress, and lower postitive self-concept than nonuser group. According to the results of discriminant analysis, hostility and deviant behavior were reliable predictive variables that distinguished abuser group from nonuser group among the associated variables in this study. These results suggest that some common factors affect drug use of Korean high school students : high school students who have many common risk factors could be abusers more easily. These findings supported risk factor theory that the extent of drug use of adolescents might be associated with general maladjustment in everyday life.
Benzodiazepines are widely used for a variety of conditions in psychiatric field. In this article, the author reviewed the indications, the effects on anxiety and sleep disorders, the side effects, the drug-drug interactions, and the tapering strategies. Benzodiazepines were relatively safe and useful for the treatment of anxiety and sleep disorders. However, used clinically, benzodiazepines can induce many adverse effects (e. g. over-sedation, cognitive impairments, paradoxical effects, dependence and withdrawal symptoms, and so on). Currently available informations about their effects on the developing fetus is controversial. For this reason, pregnant women and nursing mother should be cautioned against the use of benzodiazepines. Drug-drug interactions have to be considered in combination treatments. For the tapering issues, gradual tapering was important to prevent the withdrawal symptoms. Especially, the tapering schedules have to be individualized for the each long-term benzodiapzepine users.
Background: The use of heated tobacco products (HTP) causes many adverse health effects. Although the use of HTP by adolescents is prevalent worldwide, research related to it is very limited. There is a considerable lack of research related to the current HTP use rather than ever-HTP use. In particular, research related to predictive factors for current HTP use in adolescents is scarce. Objectives: The purpose of this study was to analyze related characteristics according to the current use of HTP among South Korean adolescents. Methods: This was a cross-sectional study that used data from the Seventeenth Korean Youth Risk Behavior Web-based Survey (KYRBWS). A total of 54,848 students in 2021 were included in this study. Chi-square-test, multiple logistic regression analysis, and chi-square test for trend were used for analyzing related characteristics according to use of HTP. Results: Overall, 715 (1.3%) students responded as having used HTP during the last 30 days among the 54,848 students. It was found that residence type, subjective body type recognition, subjective health recognition, alcohol use, habitual drug experience, close friend current smoking, and conventional cigarette smoking were significantly associated characteristics with the current use of heated tobacco products. Comparing 'very thin recognition' with 'very fat recognition', the adjusted odds ratio (OR) was 1.93 (95% confidence interval [CI]: 1.29~2.87) for current use of HTP. Additionally, comparing 'very unhealthy recognition' with 'very healthy recognition', the aOR was 3.82 (95% CI: 2.40~6.07) for current use of HTP. Conclusions: Based on these results, residence type, subjective body type recognition, subjective health recognition, alcohol use, habitual drug experience, close friend current smoking, and conventional cigarette smoking were associated with significantly increased odds of current HTP use. Therefore, the results of this study can provide useful evidence about adolescent behaviors in predicting current HTP use.

A murine local lymph node assay (LLNA) has been developed as an alternative to guinea pig models for contact sensitization potential. However, a disadvantage of the LLNA is the need for the use of radioactive material. In this study, we aimed to investigate the development of non-radio isotopic endpoint for LLNA using immunohistochemistry.(omitted)
