Current research trends on starch nanoparticles (SNPs) (녹말 나노 입자의 연구 현황)

  • In recent years, starch nanoparticles (SNPs) have been received much attention due to their unique characteristics different from native starch. Also, SNPs have economic and environmental advantages because they are prepared from starch, a cheap and safe natural polymer. It can be used in various industrial applications such as food additives, drug carriers, etc. SNPs have been prepared using different methods and their physiochemical, functional properties and possible industrial applications have been reported. Based on these studies, SNPs are expected to be the promising food materials and expand their utilization in many industries in the future. This review covered the overall researches on SNPs, including preparation, physicochemical and functional properties, and discussed their current and future applications including resistant starch materials.

  • Animal welfare is often neglected by livestock productionists when considering the utilization of animal wastes for livestock feeds. The present review has been carried out to examine the nutritive value of poultry wastes for ruminants, the health risks involved with feeding it, the treatment and feeding methods and the production responses of animals fed on it. It was found that mineral, crude protein, crude fibre and metabolisable energy concentrations are influenced by the system of poultry production, the storage of the waste and the treatment method. Heating at $60^{\circ}C$ kills all pathogens apart from Clostidium botulinum whereas proper ensiling kills all. Apart from the kidney fat and the liver, animal tissues have not shown residues of drugs or heavy metals from poultry wastes. Palatability is affected when the moisture is more than 200 g/kg. Production responses are satisfactory when poultry wastes replace portions of concentrates. It was concluded that poultry litter generally has higher metabolisable energy contents than poultry manure, but research is needed to classify poultry litters on their energy values. The adverse effects of toxic minerals and drug residues are negligible in balanced poultry waste feeding systems.

Analysis of Drug Utilization after the Mandatory Application of the DRG Payment System in Korea (포괄수가제 당연적용 후 의약품 사용현황 분석)

  • Purposes: This study aims to investigate the policy effect of mandatory application of DRG for 7 disease groups in general and tertiary hospitals. Methodology: As DRG was fully implemented in July 2013, this study compares two periods before and after the change(from July 2012 to June 2013, and from July 2013 to June 2014). The benefit claim data of the National Health Insurance Service was used for the comparison. Target patients were those who visited general or tertiary hospitals between July 2012 to June 2014. For pharmaceutical consumption, Interrupted Time Series (ITS) analysis was used to see the effect of DRG mandatory application. Findings: The number of drugs prescribed per patient and pharmaceutical expenditure both showed significant reduction compared to before the DRG implementation. Practical Implications: This study used 2 sets of 1 year period data from before and after the full implementation of DRG to analyze pharmaceutical consumption. When the comparison data accumulates further, it would be possible to conduct more diverse analysis to assess policy effect and to provide way forward for the future.

Current Status and Future Direction of Artificial Intelligence in Healthcare and Medical Education (의료분야에서 인공지능 현황 및 의학교육의 방향)

  • The rapid development of artificial intelligence (AI), including deep learning, has led to the development of technologies that may assist in the diagnosis and treatment of diseases, prediction of disease risk and prognosis, health index monitoring, drug development, and healthcare management and administration. However, in order for AI technology to improve the quality of medical care, technical problems and the efficacy of algorithms should be evaluated in real clinical environments rather than the environment in which algorithms are developed. Further consideration should be given to whether these models can improve the quality of medical care and clinical outcomes of patients. In addition, the development of regulatory systems to secure the safety of AI medical technology, the ethical and legal issues related to the proliferation of AI technology, and the impacts on the relationship with patients also need to be addressed. Systematic training of healthcare personnel is needed to enable adaption to the rapid changes in the healthcare environment. An overall review and revision of undergraduate medical curriculum is required to enable extraction of significant information from rapidly expanding medical information, data science literacy, empathy/compassion for patients, and communication among various healthcare providers. Specialized postgraduate AI education programs for each medical specialty are needed to develop proper utilization of AI models in clinical practice.

A Critical Evaluation and International Comparison of Pharmaceutical Consumption and Sales Statistics (국내 2018년 의약품 소비량 및 판매액 통계 산출 및 국제 비교)

  • Background: Health statistics of pharmaceutical use and expenditure are essential to make and implement evidence-based pharmaceutical policy. This study aims to demonstrate the methods and results of pharmaceutical consumption and sales in 2018 according to the sources and methods given by the Organization for Economic Cooperation and Development (OECD). Methods: The medication list contains 39,346 medicines both reimbursed and non-reimbursed by the National Health Insurance in 2018. We used the therapeutic categories based on Anatomic Therapeutic Chemical Classification of World Health Organization. This study analyzed National Health Insurance claims data and supply data generated from wholesalers to health care facilities. The indicators are defined daily dose (DDD), per 1,000 inhabitants per day and US$ per capita. Results: In South Korea, the number of medications to which DDD were assigned was 18,055 and it was 45.9% of the total number of medications on the list. The consumption in anti-infective for systemic use (J) and musculo-skeletal system (M) was higher than the mean consumption among the OECD countries. The pharmaceutical sales per person in Korea was also higher than the mean sales per person across the OECD countries. Conclusion: We sought to explain the methods to produce pharmaceutical consumption and sales statistics which we had submitted annually to OECD. Considering the characteristics of pharmaceutical statistics, a direct comparison should be approached with caution. Since the growth in pharmaceutical spending has greatly increased over the past decade, we need to monitor pharmaceutical consumption and expenditure consistently.

Reviews on the Current Status and Appropriate Management of Polypharmacy in South Korea (우리나라의 다제약제 현황과 적정관리 방안에 대한 고찰)

  • Polypharmacy is increasing owing to an increase in the elderly population and multimorbidities associated with the increased risk of administration of potentially inappropriate medications (PIMs). The negative effects of polypharmacy on various health conditions and aspects, such as fall, fracture, mortality, cognitive function, and dementia, have been reported. The management of excess and inappropriate polypharmacy through proper interventions and local or national guidelines has been highlighted. The purpose of polypharmacy management is to appropriately prescribe medicines that are essential to treat diseases in patients and to avoid inappropriate polypharmacy, such as interactive or duplicate medicines under prescription and PIMs for specific diseases. Community pharmacists in Australia, the EU, USA, and Japan are collaborating with prescribers to review medications to ensure that the patients can be prescribed appropriate medications. The service cost is reimbursed by public or private insurers. A study in the United States has shown that even with medication review costs, the overall medication cost has reduced. In Korea, various projects such as Drug Utilization Review service and safe use of medicines have been conducted; however, no national guidelines or management measures have been established. It is necessary to implement a national long-term plan on polypharmacy management. Furthermore, a phased implementation plan is required. Shortly, active medication review services and education programs for healthcare professionals with the support of the government should be considered in Korea with reference to other countries in order to raise awareness of seriousness and risks of inappropriate polypharmacy.

Analysis of Research Trends in New Drug Development with Artificial Intelligence Using Text Mining (텍스트 마이닝을 이용한 인공지능 활용 신약 개발 연구 동향 분석)

  • This review analyzes research trends related to new drug development using artificial intelligence from 2010 to 2022. This analysis organized the abstracts of 2,421 studies into a corpus, and words with high frequency and high connection centrality were extracted through preprocessing. The analysis revealed a similar word frequency trend between 2010 and 2019 to that between 2020 and 2022. In terms of the research method, many studies using machine learning were conducted from 2010 to 2020, and since 2021, research using deep learning has been increasing. Through these studies, we investigated the trends in research on artificial intelligence utilization by field and the strengths, problems, and challenges of related research. We found that since 2021, the application of artificial intelligence has been expanding, such as research using artificial intelligence for drug rearrangement, using computers to develop anticancer drugs, and applying artificial intelligence to clinical trials. This article briefly presents the prospects of new drug development research using artificial intelligence. If the reliability and safety of bio and medical data are ensured, and the development of the above artificial intelligence technology continues, it is judged that the direction of new drug development using artificial intelligence will proceed to personalized medicine and precision medicine, so we encourage efforts in that field.

Analysis on the Determinants of Therapeutic Materials Expenditure in National Health Insurance (주요 치료재료 품목군의 건강보험청구액 결정요인분석)

  • Background: The use of therapeutic materials based on new health technology has increased in recent years in the field of medicine, raising concerns for medical practitioners regarding increased spending on the new therapeutic materials amid the rapid population ageing and increase of chronic diseases in Korea. While therapeutic materials have significant importance in the health care system, they have not been given appropriate attention in the academic world of Korea. The purpose of this study is to identify factors that affect the growth of expenditure on therapeutic materials and to derive implications for an effective management considering the diversity of therapeutic materials. Methods: Using the claims data of the National Health Insurance Services, specific utilization patterns of groups of therapeutic materials in the middle classification level of Health Insurance Review and Assessment Service from 2007 to 2014 were analyzed. Four categories (J5083: drug eluting coronary stent, D0302: nonmetallic anchor, K6014: gauze, K6023: gauze) that exhibit unique patterns with respect to price and volume were selected. Then, decomposition analysis was performed to identify the largest contributor to the spending growth by dividing the products into existing, new, and abandoned products for the period between 2010 and 2013. Results: The effect of new products had larger impact on spending growth than the effect of core items in drug eluting coronary stent (J5083) and nonmetallic anchor (D0302). In addition, existing products in general included items priced relatively lower when compared with another item manufactured by the same company. In the gauze category, however, existing products had the largest impact on expenditure and the effect of volume was greater than that in other categories. Conclusion: This study provides evidence that appropriate management measures classified by the characteristics of therapeutic materials are required for therapeutic materials pricing and reassessment in Korea.

Chemical Characterization and Utilization of $19{\alpha}-Hydroxyursane-type$ Triterpenoids in Rubus species (Rubus속 식물에 존재하는 $19{\alpha}-Hydroxyursane-type$ Triterpenoid의 특성과 이용)

  • The plant Rubus species (Rosaceae) mainly contains $19{\alpha}-hydroxyurane-type$ triterpenoids $(19{\alpha}-HUT)$ as bioactive components. Available functional food includes blackberry (the fruit of thornless Rubus sp.), red raspberry (R. idaeus) and black raspberry (R. occidentalis). However, the fruit of R. coreanus, which is used in Korea as a functional food, substitutes black raspberry. Rubi Fructus, which has been traditionally used as an oriental medicinal drug, designates only unripe fruit of R. coreanus but not its ripe fruit which indicates that it needs high content of $19{\alpha}-HUT$ as a crude drug. Throughout our experiment, we found that ripe fruits contain very little amount of $19{\alpha}-HUT$ when compared to unripe fruits. In addition, various and rich $19{\alpha}-HUT$ has been reported from Rubus species. The most common structure of $19{\alpha}-HUT$ of Rubus species, euscaphic acid or tormentic acid with $3{\alpha}-OH$ or $3{\beta}-OH$, respectively, can be glycosylated or oxidized to produce a number of $19{\alpha}-HUTs $as euscaphic acid and tormentic acid derivatives and even esterified to form dimeric triterpenoids. In this review, the bioactivity and biosynthetic pathway and chemical characterization of $19{\alpha}-HUTs$ found in Rubus species are discussed.

  • Options available for utilization of animal wastes include sources of plant nutrients, feed ingredients for farm animals, substrate for methane generation, and substrate for microbial and insect protein synthesis. The wastes have the most economic value for use as animal feed. Performance of animals fed diets containing animal wastes is similar to that of animals fed conventional diets. Processing of animal wastes to be used as animal feed is necessary for destruction of pathogens, improvement of handling and storage characteristics, and maintenance or enhancement of palatability. Feeding of animal waste has not adversely affected the quality and taste of animal products. In the USA copper toxicity has been reported in sheep fed high-copper poultry litter, but this is not a serious problem with cattle. Potential pathogenic microorganisms in animal wastes are destroyed by processing such as heat treatment, ensiling and deep stacking. Incidents of botulism, caused by Clostridium botulinum, have been reported in cattle in some countries, and this problem was caused by the presence of poultry carcasses in litter. This problem has not occurred in the USA. With appropriate withdrawal, heavy metal, pesticide or medicinal drug accumulation in edible tissues of animals fed animal wastes is not a problem. Feeding of animal wastes is regulated by individual states in the USA. The practice is regulated in Canada, also. With good management, animal wastes can be used safely as animal feed.
