Basic Principles of Drug Interaction (약물상호작용의 원리와 의의)

  • There is nothing that is harmless ; the dose alone decides that something is no poison(Paracelsus, 1493-1541). So, in a point of view to maximize the therapeutic efficacy of drug therapy in a way that minimize the drug toxicity, the knowledges of the drug-ineractions as well as the pharmacokinetic and pharmacodynamic principles of every therapeutic drug used in the medical clinic cannot be emphasized too much. Many drug interactions can be predicted if the pharmacokinetic properties, pharmacodynamic mechanisms of action of the interacting drugs are known, and most adverse interactions can be avoided. In this paper, the clinical importance, classification, and general principles of clinical drug-interactions are presentated with a few explanatory examples.

  • Fermented materials have been used for long time around the world and have been researched according to the excellent effect in the part of medical and food industry. However, when such materials are applied on skin, because of the skin barrier, the most effective ingredients are poorly absorbed. The absorption of the skin is exceedingly limited and the method of increasing skin absorption needs special procedures. The micro-needle therapy is a method used to improve the absorption of drug (solution) in the skin which is called "natural skin rejuvenation therapy". This therapy uses micro-needle which is equipped with very thin, delicate needles smaller than a 0.07 mm thick hair. During this therapy, the micro-needle makes small holes and helps absorb the solution into the skin. This is a very excellent therapy in skin absorption. It can be used in wide regions of the skin without any side effects and no recovery time. In 2007, the micro-needle is permitted to personal care. However, the solutions have not yet been developed professionally, and such skill is needed.

  • Vertigo is the sensation of self-motion of the head or body when no self-motion is occurring or the sensation of distorted self-motion during an otherwise normal head movement. Representative peripheral vertigo disorders include benign paroxysmal positional vertigo, Ménière disease, and vestibular neuritis. Vestibular neuritis, also known as vestibular neuronitis, is the third most common peripheral vestibular disorder after benign paroxysmal positional vertigo and Ménière disease. The cause of vestibular neuritis remains unclear. However, a viral infection of the vestibular nerve or ischemia of the anterior vestibular artery is known to cause vestibular neuritis. In addition, recent studies on immune-mediated mechanisms as the cause of vestibular neuritis have been reported. The characteristic clinical features of vestibular neuritis are abrupt true-whirling vertigo lasting for more than 24 hours, and no presence of cochlear symptoms and other neurological symptoms and signs. To accurately diagnose vestibular neuritis, various diagnostic tests such as the head impulse test, bithermal caloric test, and vestibular-evoked myogenic potential test are conducted. Various treatments for vestibular neuritis have been reported, which are largely divided into symptomatic therapy, specific drug therapy, and vestibular rehabilitation therapy. Symptomatic therapies include generalized supportive care and administration of vestibular suppressants and antiemetics. Specific drug therapies include steroid therapy, antiviral therapy, and vasodilator therapy. Vestibular rehabilitation therapies include generalized vestibular and customized vestibular exercises.

Development of PLGA Nanoparticles for Astrocyte-specific Delivery of Gene Therapy: A Review (별아교세포 선택적 유전자 치료전달을 위한 PLGA 나노입자 개발)

  • Recently, as nanotechnology has been introduced and used in various fields, the development of new drugs has been accelerating. Nanoparticles have maintained blood drug concentration for extended periods of time with a single administration of the drug. The drug can then be selectively released only at the pathological site, thereby reducing side effects to other non-pathological sites. In addition, nanoparticles can be modified for selective target sites delivery for other specific diseases, with polymers being widely used in the manufacture of these nanoparticles. Poly (D,L-lactic-co-glycolic acid ) (PLGA) is one of the most extensively developed biodegradable polymers. PLGA is widely used in drug delivery for a variety of applications. It has also been approved by the FDA as a drug delivery system and is widely applied in controlled release formulations, such as in gene therapy treatments. PLGA nanoparticles have been developed as delivery systems with high efficiency to specific cell types by using passive and active targeting methods. After the development of a drug delivery system using PLGA nanoparticles, the drug is selectively delivered to the target site, and the effective blood concentration for extended periods of time is optimized according to the disease. In this review paper, we focus on ways to improve cell-specific treatment outcomes by examining the development of astrocyte selective nanoparticles based on PLGA nanomaterials for gene therapy.

  • Objective: Case report of improvement in muscle pain, insomnia, and depression in a breast cancer patient using Ortho-Cellular Nutrition Therapy. Methods: A Korean woman in her 50s was diagnosed with breast cancer. After being diagnosed with stage 1 breast cancer, her quality of life has greatly deteriorated due to the side effects of the drugs administered. Results: Symptoms of insomnia and muscle pain improved after nutritional therapy. Conclusion: Applying nutritional therapy to patients with the aforementioned condition can help alleviate symptoms.

Case of Suspected Drug-Induced Liver Injury after Intravenous Wild Ginseng Pharmacopuncture (산삼약침 시술 후 발생한 것으로 의심되는 약인성 간손상 : 증례 보고)

  • Wild ginseng pharmacopuncture is a partly used Traditional Korean Medicine's therapy with no previously well documented hepatotoxicity. We report the the first case of suspected drug-induced liver injury (DILI) after intravenous Wild ginseng Pharmacopuncture therapy. Our clinical observation based on patient's laboratory studies, medical history, detailed drug history and ruling out other possible causes. Our patient's suspected diagnosis was wild ginseng pharmacopuncture-induced liver injury with correlation between detailed drug history and laboratory studies. This diagnosis was supported by the Council for International Organizations of Medical Sciences-Roussel Uclaf Causality Assessment Method(RUCAM). This report represents the first documented cases of suspected DILI after intravenous wild ginseng pharmacopuncture therapy, highlighting the need for future research regarding potential hepatotoxicity of Wild ginseng Pharmacopuncture.

  • Azathioprine is the most common drug used to maintain clinical remission in inflammatory bowel disease. This drug is also important as a steroid-sparing agent in steroid-dependent and chronically active inflammatory bowel disease. Nevertheless, many questions remain concerning the optimal treatment regimens of azathioprine. The dose of azathioprine has to be reduced or the therapy has to be discontinued frequently because of drug-induced toxicity. In this review, we discuss monitoring of thiopurines, adverse events, malignant complications and how to use azathioprine safely and usefully.

The mechanism of External therapy for prostatitis is some medicicne to come into prostate have anti-inframation and the other medicine to come into nerve system control pain. 2. For the method are uesed hip bath, enema and sticking the pacth to the umbilicus etc. 3. For the medicine are used safflower(紅花), patrinia(敗醫草), chrysanthemum flower(菊花) and Cortex Phellodendri(黃栢). nature and flavor of the medicine is almost cold, bitter in taste and hot in taste. 4. For the classification of medicine are used drug to induce diuresis to elominate stranguria(利尿通淋藥) and drug to induce diuresis to dispersing swelling(利水退腫藥). sometimes they are used to Heat-clearing and detoxifying drug(淸熱解毒藥) and blood-quickening aget(活血祛瘀藥).

Statin-Induced Autoimmune Necrotizing Myopathy Responsive to Immunosuppressive Therapy (면역억제치료에 반응하는 statin에 의한 자가면역성 괴사성 근병증)

  • Statin is commonly used for lowering cholesterols and can be myotoxic to cause drug-induced necrotizing myopathy. Statin-induced myopathy ranges from asymptomatic hyperCKemia to lethal rhabdomyolysis but is usually reversed by withdrawal of causative drugs. The patient in this study presented with statin-induced necrotizing myopathy, which was finally improved with immunosuppressive therapy, but not just with drug withdrawal. Since statin can induce myopathy through autoimmune processes, we should consider using immunomodulating agents in cases with statin-induced myopathy, which is refractory to drug withdrawal.
