Retreatment of failed regenerative endodontic of orthodontically treated immature permanent maxillary central incisor: a case report

  • A revascularization procedure was shown to be the best alternative therapy for immature teeth with necrotic pulp and apical infection. A 12 year old female with a history of trauma to her upper central incisor and a sinus tract was referred for endodontic treatment. She was an active orthodontic patient and had undergone regenerative endodontic treatment for the past 2 years. Clinical examination revealed no response to sensibility, percussion, and palpation tests. The preoperative radiograph showed an open apex and apical rarefaction. The case was diagnosed as previously treated tooth with asymptomatic apical periodontitis. Regenerative endodontic retreatment was performed, and the case was followed for 3 years. Clinical, radiographic, and cone-beam computed tomography follow-up examination revealed an asymptomatic tooth, with evidence of periapical healing and root maturation.

THE EFFECT OF LIGATION METHOD ON THE FRICTIONAL FORGE BETWEEN ORTHODONTIC BRACKET AND ARCHWIRE (결찰양식이 교정용 브라켓과 교정선 사이의 마찰력에 미치는 영향)

  • The frictional force has been considered as an harmful factor in an active unit where tooth movement occurs, but as an advantageous factor in anchor unit that resist tooth movement. That is, efficient tooth movement is planned by using ligation methods that have low levels of bracket-wire frictional force and the anchorage control can be achieved by using ligation methods that have high levels of bracket-wire frictional force that result in binding of the bracket accompanied by little or no tooth movement. The purpose of this study was to evaluate the frictional force generated between bracket and wire in accordance with the methods of ligation, the material of ligation and the passage of time under artificial saliva. Tested were 0.017x0.022 inch stainless steel wires in standard edgewise twin brackets for upper central incisors in a 0.018-inch slot. The wires were ligated into the brackets with elastomeric modules and stainless steel ligatures. Whole tie, half tie, twisting tie and double overlay tie were done with elastomeric modules. With 0.009-inch stainless steel ligature whole tie and half tie were done by needle holder and whole tie by ligature tying plier. With 0.012-inch stainless steel ligature whole ties were done by needle holder. Whole tie groups of elastomeric module were kept in artificial saliva bath at $37^{\circ}C$ for 28 days. The frictional force was recorded by means of an Instron universial testing instrument (4202 INSTRON, Instron Co., U.S.A.) at initial, 7, 14, 21, and 28 days. The results for ligated samples in a simulated oral environment revealed the fellowing : ${\cdot}$In elastomeric module whole tie, 28 days group was significantly greater mean static frictional force than any other group but there were no significant differences among any other group (p>0.05). ${\cdot}$Elastomeric module twisting ties were significantly greater mean static frictional forces than any other ligation method but there were no significant differences between twisting tie and double overlay tie (p>0.05). Twisting tie, double overlay tie, whole tie, half tie showed differences in decreasing order. ${\cdot}$Stainless steel half tie produced lower mean static frictional force than whole tie, ligation by ligature tying plier produced greater mean static frictional force than by needle holder and ligation with 0.012-inch stainless steel ligature produced greater mean static frictional force than with 0.009-inch stainless steel ligature (p<0.05). ${\cdot}$There were no significant differences between the mean static frictional forces of elastomeric whole tie and stainless steel whole tie (p>0.05).

A STUDY ON THE SEALING PROPERTIES OF TEMPORARY FILLING MATERIALS USED IN ENDODONTICS (근관치료시 사용되는 수종 임시충전재의 변연 폐쇄효과에 관한 연구)

  • The purpose of this study was to evaluate the sealing properties of the temporary filling materials used in endodontic treatment Access cavities were prepared in 135 extracted human molar teeth. Then, cotton pellets were placed in the pulp chamber until the depth of 5 mm for the temporary filling materials; Caviton, zine oxide-eugenol, double sealing (A) (stopping 15 mm + zinc oxide - eugenol, 3.5mm ) double sealing (B) (stopping 3.0 mm + zinc oxide - eugenol 2.0 mm) and gutta percha stopping. After filling the materials, the teeth were immersed in 1 % methylene blue solutions for 3 days, 1 week and 2 weeks. Then thermal cycling was performed at the temperature of $60^{\circ}C$ and $4^{\circ}C$, followed by longitudinal sections on the center of tooth. Finally, staining on the cotton pellet was evaluated. The following results were obtained. 1. Stopping showed lower marginal sealing quality than Caviton, zinc oxide - eugenol and double sealing. 2. In 1 week group, Caviton showed higher marginal sealing quality than zinc oxide-eugenol, double sealing and stopping. 3. Caviton and double sealing (B) showed a great decrease in marginal sealing quality with the increse of time. 4. Caviton had high marginal sealing quality in 3 day group and 1 week group, but in 2 week group, Caviton showed a great decrease. 5. Double sealing (B) showed fairly high marginal sealing quality in 3 day group, but decreased greatly after 1 week on.

  • The biomechanical prognosis of conventional removable partial denture is questionable in case of patient with few remaining teeth or periodontally compromised teeth. In this case, hybrid telescopic double crown RPD may be a successful treatment alternative. Hybrid telescopic double crown RPD has following advantages over conventional RPD: secondary splinting effect between abutments, more vertical stress direction and more convenient in repairing the denture after extraction of abutment tooth. In this clinical case, patient had deep overbite in anteriors and partially edentulous. The maxilla was restored with hybrid telescopic double crown RPD and the mandible was restored with implants and fixed prostheses. Long-term follow-up and supportive periodontal treatment were performed, and satisfactory results were achieved in terms of function and aesthetics.

  • Because of linear operation by direct drive and compact system, and the free accumulation of position error, LPM is suitable for proportional control valves actuator. In this study, to develop electromagnetic proportional valves actuator we designed tooth shape of double sides LPM and suggested the driving principle based upon the excitation method, It's the magnetic structure is so complex by the cubic effect configuration that finite element analysis is required. By the 3D finite element analysis, we got the magnetic distribution of LPM and analyzed static thrust force characteristics. By this analyzation, we obtained basic data required for the practical use of driving system for the proportional control valves. Thus, considering electrical and mechanical constants we could design and manufacture double sides LPM for the proportional control valves.

Study on Warm Precision Forging of Half Axle Gears

  • A typical die-set for enclosed-die forging of half axle gears in double action hydraulic press is presented, the important factors those influence on precision forming of half axle gears are analyzed, warm forming process of half axle gears is simulated by FEM software $DEFORM\_3D$. The results show, that proper die structure and dimension, suitable web thickness and position can improve material filling, ensure full filling of tooth cavity.

  • In the reduction gears for marine propulsion engine such as turbine or high speed diesel engine, the standard involute double helical gears are generally used. However the addendum modification gear can be used in the reduction gear as it has flexibility for gear design on the tooth strength, scoring and operating noise. In this case, the determination of gear shaft bearing load is difficult by the alternation of operating pressure angle. In this paper, the formulas of bearing load according to the arrangements of the reduction gears are derived and the diagrams of operating pressure angle according to the modification coefficient are presented.

  • As a part of an effort to minimize the polymerization shrinkage which is considered to be a major cause of failed bonds to tooth, newly designed 'Double LED system' was tested in the present study. Analyses were performed on the pattern of micro-leakage and the changes of strain which have occurred during the polymerization process. The results can be summarized as follows: 1. In the strain change, dramatic increase was observed with initiation of polymerization which was followed by subsequent gradual decrease with elapse of time in both the single LED system and double LED system. 2. The single LED system were shown to develop and maintain the maximum stress more than double LED system(p<0.05). 3. Less micro-leakage was found in the double LED system than in the single LED system(p<0.05). From the above-mentioned results, the double LED system can be a very useful tool in a sense of reducing polymerization shrinkage when compared to the single LED system. However, practical problems such as size of curing unit and its application method with its light intensity should be solved before its clinical application.

  • Nowadays, the development of dental scanner and CAD/CAM technology can facilitate the fabrication of hybrid prosthesis. Double scanning technique, scanning a trial prosthesis and master model, made it possible to realize virtual design and simplify the laboratory work. Instead of using the metal or zirconia framework with composite, ceramic or denture tooth, the new high performance polymer Polyetherketoneketone (Pekkton, Cendres+$M{\acute{e}}taux$, Biel, Switzerland) as a framework with Polymethyl methacrylate (PMMA) veneering teeth (Visio-lign, Bredent, Senden, Germany) was used in this case. This case report showed an acceptable treatment outcome and satisfaction of patient using Pekkton and Visio-lign. However, long term clinical evaluation is needed.

Full mouth rehabilitation of a severely worn dentition using intraoral scanner and the CAD/CAM double scanning technique (중증도의 치아 마모 환자를 구내 스캐너와 이중스캔을 이용해 수복한 완전구강회복 증례)

  • With the evolution of the computer-aided design/computer-aided manufacturing (CAD/CAM) technology, the intraoral scanners are playing an increasingly important role, as they are the first step towards a completely digital workflow. The CAD/CAM double scanning technique has been used to transfer the information from provisional restorations to definitive restorations. In this case, a 67-year-old male with esthetically compromised anterior teeth, generalized severe attrition of teeth, and reduced vertical dimension was treated with full mouth rehabilitation including a re-establishment of the lost vertical dimension of occlusion assisted by the crown lengthening procedure. The provisional restorations were fabricated using an intraoral scanner and the CAD/CAM double scanning technique. After the period of adaption, the definitive monolithic zirconia restorations were delivered. The CAD/CAM double scanning technique successfully transferred the occlusal and morphological characteristics, obtained from the provisional restorations, to the definitive restorations.
