Background: Previous research has suggested that single doses of a standardised Panax ginseng extract can decrease fasted blood-glucose levels and modulate cognitive performance in healthy young volunteers. The latter has generally been seen in terms of improved secondary memory performance. However, both the cognitive effects of chronic administration of ginseng and the potential modulation of working memory have received comparatively little research attention. Aims: The current double-blind, placebo-controlled, balanced cross-over study investigated the effects of 8-weeks administration of Korean ginseng extract (200 mg) on cognitive performance, gluco-regulatory parameters and ratings of subjective mood and 'quality of life'. Methods: 'Eighteen healthy young participants were assessed pre-dose and 3 hours post-dose on the mornings of Day 1, Day 29 and Day 57 of 8 week treatment regimens of both placebo and ginseng. A four-week placebo wash-out separated the treatment phases. Each assessment included the Cognitive Drug Research battery, computerised working memory tasks, and Bond-Lader mood scales. The WHO Quality of Life scale (WHOQOL-BREF) was completed once per visit. Gluco-regulatory parameters were assessed with assays of blood glucose, insulin and HbA1c. Results: Data from the 16 participants that completed the study showed that there were no significant, acute treatment related differences on Day 1 of treatment, or in gluco-regulatory parameters throughout the study. However, time related performance improvements were evident following chronic administration of ginseng on the '3-Back' and 'Corsi-block' computerised working memory tasks. Ginseng was also associated with an improved score on the 'social relations' subscale of the WHOQOL-100, and a significant shift on the 'calm' factor of the Bond-Lader mood scales (from calm/relaxed towards excited/tense). Conclusion: The results of the current study suggest that Korean ginseng extract can modulate working memory performance and subjective ratings of 'quality of life' and mood. Replication with a larger sample size may further elucidate the actions of this product.
Objective: Third-generation dual-source computed tomography (3rd-DSCT) allows dynamic myocardial CT perfusion imaging (dynamic CTP) with a 10.5-cm z-axis coverage. Although the increased radiation exposure associated with the 50% wider scan range compared to second-generation DSCT (2nd-DSCT) may be suppressed by using a tube voltage of 70 kV, it remains unclear whether image quality and the ability to quantify myocardial blood flow (MBF) can be maintained under these conditions. This study aimed to compare the image quality, estimated MBF, and radiation dose of dynamic CTP between 2ndDSCT and 3rd-DSCT and to evaluate whether a 10.5-cm coverage is suitable for dynamic CTP. Materials and Methods: We retrospectively analyzed 107 patients who underwent dynamic CTP using 2nd-DSCT at 80 kV (n = 54) or 3rd-DSCT at 70 kV (n = 53). Image quality, estimated MBF, radiation dose, and coverage of left ventricular (LV) myocardium were compared. Results: No significant differences were observed between 3rd-DSCT and 2nd-DSCT in contrast-to-noise ratio (37.4 ± 11.4 vs. 35.5 ± 11.2, p = 0.396). Effective radiation dose was lower with 3rd-DSCT (3.97 ± 0.92 mSv with a conversion factor of 0.017 mSv/mGy∙cm) compared to 2nd-DSCT (5.49 ± 1.36 mSv, p < 0.001). Incomplete coverage was more frequent with 2nd-DSCT than with 3rd-DSCT (1.9% [1/53] vs. 56% [30/54], p < 0.001). In propensity score-matched cohorts, MBF was comparable between 3rd-DSCT and 2nd-DSCT in non-ischemic (146.2 ± 26.5 vs. 157.5 ± 34.9 mL/min/100 g, p = 0.137) as well as ischemic myocardium (92.7 ± 21.1 vs. 90.9 ± 29.7 mL/min/100 g, p = 0.876). Conclusion: The radiation increase inherent to the widened z-axis coverage in 3rd-DSCT can be balanced by using a tube voltage of 70 kV without compromising image quality or MBF quantification. In dynamic CTP, a z-axis coverage of 10.5 cm is sufficient to achieve complete coverage of the LV myocardium in most patients.
Ionization chambers often exhibit a stem effect, caused by interactions of radiation with air near the chamber end, or with dielectric in the chamber stem or cable. In this study measured stem effect correction factor for length of ionization chamber from medical linear accelerator recommend to with the use of stem correction method. For a model of the Farmer-type chamber, were used to calculate the beam quality correction factor. These interactions contribute to the apparent measured exposure. Additionally, it needs to consider ionization chamber use of small volume and stem effect of cable by a large field. Linear accelerator generated photons energy and increased dose repeatedly measured by using stem correction method. Stem effect was dependence of the energy and increases with photon energy conditions improved of beam quality. In conclusion, stem effect correction factor was measured within 0.4% calculated according to the exposures stem length and also supposed to determined below 1% of another stem correction method.
수도권에 소재하는 의료기관 44개 곳에서 시행하는 흉부X선 간접촬영에서 사용한 간접촬영 필름의 크기에 따라 촬영조건, 화상의 물리적 평가 및 시각적 평가, 조사선량 등을 조사한 결과 아직도 대부분 저관전압 촬영을 하고 있어 적절한 촬영조건 개선이 필요하며, 화상평가 비교에서 물리적 평가에서는 비슷한 결과를 보였으나 시각적 평가에서는 100mm 필름을 사용한 경우가 70mm 필름을 사용한 경우보다 대체적으로 우수한 것으로 나타났고, 흉부표면에 조사되는 선량은 70mm 필름을 사용한 경우가 100mm 필름을 사용한 경우보다 3배 이상의 피폭이 더 되고 있어 환자의 피폭선량 경감과 화질개선에 있어서 필름 선택의 중요성을 재삼 인식시켜 주었다.
최근 방사선 피폭에 대한 관심이 높아지고 있다. 유방촬영용장치의 방사선 피폭은 일반 X선 촬영에 비해 비교적 흡수선량이 높기 때문에 화질은 개선하되 불필요한 피폭을 줄여야 노력을 할 필요가 있다. 최근 국제방사선방어위원회(ICRP)나 우리나라의 한국의료영상품질관리원(KIAMI)에서 권고하는 평균유선선량은 3 mGy이하이지만 이는 유방의 두께에 따라 다르므로 모든 경우에 권고수치가 넘지 않는다고 해서 적다고 말할 수는 없다. 일반적으로 방사선 촬영에 사용되는 선질은 영상의 화질과 피폭선량을 결정하는 중요한 인자로 촬영 관전압과 부가필터에 영향을 받는다. 일반적으로 유방촬영용장치에서 방출되는 X선 에너지는 연속 스펙트럼(spectrum)으로서 영상의 화질에 미치는 영향이 적은 저 에너지부터 이미지상의 대조도(contrast)를 저해하는 고 에너지 성분까지 포함하고 있다[1,3]. 현재 유방촬영용장치에 많이 사용되고 있는 부가필터는 몰리브덴(molybdenum, Mo), 로듐(rhodium, Rh)등이 있으며, 이는 사용되어지는 X선질의 에너지 영역에 따라 구분하여 사용되고 있다. 현재 유방촬영용장치에 가장 많이 사용되어지고 있는 부가필터의 물질인 몰리브덴(Molybdenum, Mo), 로듐(rhodium, Rh)외에 원자번호와 K-흡수단 영역이 비슷한 니오븀(niobium, Nb)이나 지르코늄(zirconium, Zr)재질의 부가여과판을 사용 하였을때 화질과 선량의 개선된 변화를 알아보고, X선의 저에너지를 흡수할 수 있는 알루미늄(aluminum, Al)재질을 복합으로 사용하였을 때 유방촬영용장치의 화질에 손실없이 알루미늄(aluminum, Al)두께에 따른 조사선량의 경감 정도를 확인하려 한다. 본 실험에서는 몰리브덴(molybdenum, Mo), 니오븀(niobium, Nb), 지르코늄(zirconium, Zr)등의 부가필터를 종류별로 단일필터로 사용할 경우와 이들 단일필터에 알루미늄(aluminum, Al)필터를 복합으로 사용한다. 이 경우 상기 부가필터의 종류에 따른 화질의 개선이 될 것으로 판단되고, 알루미늄(aluminum, Al)필터를 복합으로 사용함으로 인해 저 에너지의 불필요한 X선이 흡수되므로 동일한 농도가 되었을 때 화질에 큰 영향없이 선량이 감소될 것으로 기대된다.

The spinal cord dose is the one of the limiting factor for the radiation treatment of the head & neck (H&N) or the thorax region. Due to the fact that the cord is the elongated shaped structure, it is not an easy task to maintain the cord dose within the clinically acceptable dose range. To overcome this problem, the spinal cord partial block technique (PBT) with the dynamic Multi-Leaf Collimator (dMLC) has been developed. Three dimension (3D) conformal beam directions, which minimize the coverage of the normal organs such as the lung and the parotid gland, were chosen. The PBT field shape for each field was designed to shield the spinal cord with the dMLC. The transmission factors were determined by the forward calculation method. The plan comparisons between the conventional 3D conformal therapy plan and the PTB plan were performed to evaluate the validity of this technique. The conformity index (CI) and the dose volume histogram (DVH) were used as the plan comparison indices. A series of quality assurance (QA) was performed to guarantee the reliable treatment. The QA consisted of the film dosimetry for the verification of the dose distribution and the point measurements. The PBT plan always generated better results than the conventional 3D conformal plan. The PBT was proved to be useful for the H&N and thorax region.
The specific purpose of this study is to develop the numerical guide for the cost-benefit analysis of ORE ($/person-Sv reduction) to meet the criterion of ALARA in the design stage of the KNGR. In deriving the guide, the risk factor which is defined by the risk to unit collective radiation exposure dose (deaths/person-Sv) and the monetary value of human life ($/death) are required. The risk factor has been estimated from various clinical data accumulated for a number of years and continuously modified. And the monetary value of human life is usually quantified using the human capital approach. In this study, the risk to radiation exposure perceived by a group of people is investigated through an extensive poll survey conducted among university students in order to modify the existing risk factor for radiation exposure. And in evaluating the monetary value of human life, the QOL factor is introduced in order to incorporate the degree of public welfare or quality of life. As a result of study, a value within the range of 151, 000~172, 000 dollars per person-Sv reduction is recommended as the appropriate interim numerical guide for cost-benefit analysis of ORE to meet the criterion of ALARA in the design stage of the KNGR. A poll survey was also conducted in order to see whether the public acceptance cost of nuclear power should be incorporated in developing the guide, and the result of study shooed that such a cost does not need to be considered.
For the quality control procedure of diagnostic x ray units, a method for simultaneous measurement of air kerma, half value layer and tube potential was developed utilizing a computed radiography system for intraoral radiography and film badge case. The response of average pixel values under the windows were calibrated by x rays generated at tube potentials from 40 to 140 kV with filtration from 1.5 to 3.7 mmAl. The calibration curves for half value layer and tube potential were derived as functions of attenuation factors by the 1.4 mmAl filter and the 0.2 mmCu filter. The energy dependency of the open window response was corrected by the calibration factor as a function of the attenuation factor by the 1.4 mmAl filter. The uncertainty of the estimated half value layer, tube potential and air kerma were 0.2 mmAl, 3.6 % and 5 %, respectively. It was thus suggested that this system could be applied to quality control program to detect the variation of working condition of x ray units in clinical use.
Influences of dose and particle size of $Al_2O_3$ filler and sintering time on the dielectric properties of $Al_2O_3$ filler/CaO-$Al_2O_3-SiO_2$ (CAS) glass composites were investigated with a view to applying the composites to the substrate material in low temperature co-firing ceramic (LTCC) technology. The increased addition of $Al_2O_3$ filler with the particle size of 1 ${\mu}m$ monotonically decreased the density of the sintered specimen at a given temperature, while sintering of the 10 wt% $Al_2O_3$ added specimen at $925^{\circ}C$ for 2 h demonstrated 96.0 % of the relative density, dielectric constant of 6.34, and quality factor of 2,760 GHz. As for the influence of the particle size of the $Al_2O_3$ filler, there existed an optimum particle size (30 ${\mu}m$) to ensure successful densification (96.5 %) of the 10 wt% $Al_2O_3$/CAS composites at $925^{\circ}C$ for 2 h, at which condition the specimen demonstrated dielectric constant of 5.45 and quality factor of 3,740 GHz. When the influence of the sintering time of the 10 wt% $Al_2O_3$) (30 ${\mu}m$) added specimen was investigated at the sintering temperature of $925^{\circ}C$, an overly long sintering time degraded dielectric properties due to the over-sintering and the significant growth of the second phase such as anorthite, while the sintering for 4 h demonstrated 96.58 % of the relative density, dielectric constant of 5.4, and quality factor of 4,050 GHz. These results demonstrate the feasibility of the investigated material as the substrate material in LTCC technology.
세기조절방사선치료(IMRT)뿐만 아니라 3차원 입체조형치료(3D-CRT)와 같이 광자선을 이용한 방사선 치료 기술은 방사선을 받아야 하는 표적의 면적을 충분히 증가시키면서, 동시에 정상 조직은 방사선으로부터 보호하기 위하여 정확한 선량 계산을 필요로 한다. Jaw 콜리메이터와 다엽 콜리메이터가 그러한 목적을 위해서 사용되어 왔다. 우리 기관에서 사용하는 피나클 치료계획시스템은 모델기반의 광자선량 알고리듬을 사용하기 때문에 Jaw 콜리메이터 투과계수(JTF)와 다엽 콜리메이터 투과계수(MLCTF)와 같은 모델변수들의 집합이 측정된 데이터로부터 결정된다. 그러나, 이러한 자동모델화과정에 의해서 얻어진 모델변수들이 직접 측정하여 얻은 것들과 다를 수 있는데, 이는 선량분포에 영향을 줄 수 있다. 그래서, 이 연구에서 우리는 피나클 치료계획시스템에서 자동모델화과정에 의해 얻은 JTF와 MLCTF를 평가하였다. 먼저 우리는 이 연구에서 Jaw 콜리메이터 투과계수(JTF)와 다엽 콜리메이터 투과계수(MLCTF)를 직접 측정하여 얻었는데, 이것은 물팬톰 내 기준깊이에서 조사면이 $0{\times}0\;cm^2$일 때의 선량과 $10{\times}10\;cm^2$일 때의 선량의 비로 얻었다. 또한, JTF와 MLCTF는 치료계획시스템내 자동모델화 과정에 의해서도 얻어서, 이 값들이 3차원 입체조형치료시에 선량에 어떠한 영향을 끼치는지 팬톰 연구와 환자 연구를 통해서 평가하였다. 직접 측정한 경우 JTF는 6 MV의 경우에 0.001966, 10 MV의 경우에는 0.002971이었고, MLCTF는 6 MV의 경우에 0.01657, 10 MV의 경우에 0.01925이었다. 한편, 자동모델화 과정에 의해 얻은 경우, JTF는 6 MV의 경우에 0.001983, 10 MV의 경우에는 0.010431이었고, MLCTF는 6 MV의 경우에 0.00188, 10 MV의 경우에 0.00453이었다. JTF와 MLCTF의 경우에 직접 측정한 것은 자동모델화 과정에 의해 얻은 값과 큰 차이를 보였으나, 6 MV와 10 MV의 선질을 고려하면, 보다 합리적이었고, 이러한 값의 차이는 낮은 선량의 영역에서 선량에 영향을 미쳤다. JTF와 MLCTF의 잘못된 값은 선량의 오차를 다소 발생시킬 수도 있기 때문에, JTF와 MLCTF를 자동모델화과정에 의해서 얻은 값과 직접 측정하여 얻은 값을 비교하는 것은 빔커미셔닝 단계에서 도움이 될 것이다.
