Organ dose conversion coefficients in CT scans for Korean adult males and females

  • Dose monitoring in CT patients requires accurate dose estimation but most of the CT dose calculation tools are based on Caucasian computational phantoms. We established a library of organ dose conversion coefficients for Korean adults by using four Korean adult male and two female voxel phantoms combined with Monte Carlo simulation techniques. We calculated organ dose conversion coefficients for head, chest, abdomen and pelvis, and chest-abdomen-pelvis scans, and compared the results with the existing data calculated from Caucasian phantoms. We derived representative organ doses for Korean adults using Korean CT dose surveys combined with the dose conversion coefficients. The organ dose conversion coefficients from the Korean adult phantoms were slightly greater than those of the ICRP reference phantoms: up to 13% for the brain doses in head scans and up to 10% for the dose to the small intestine wall in abdominal scans. We derived Korean representative doses to major organs in head, chest, and AP scans using mean CTDIvol values extracted from the Korean nationwide surveys conducted in 2008 and 2017. The Korean-specific organ dose conversion coefficients should be useful to readily estimate organ absorbed doses for Korean adult male and female patients undergoing CT scans.

The role of natural rock filler in optimizing the radiation protection capacity of the intermediate-level radioactive waste containers

  • The present work aims to optimize the radiation protection efficiency for ion-selective containers used in the liquid treatment for the nuclear power plant (NPP) cooling cycle. Some naturally occurring rocks were examined as filler materials to reduce absorbed dose and equivalent dos received from the radioactive waste container. Thus, the absorbed dose and equivalent dose were simulated at a distance of 1 m from the surface of the radioactive waste container using the Monte Carlo simulation. Both absorbed dose and equivalent dose rate are reduced by raising the filler thickness. The total absorbed dose is reduced from 7.66E-20 to 1.03E-20 Gy, and the equivalent dose is rate reduced from 183.81 to 24.63 µSv/h, raising the filler thickness between 0 and 17 cm, respectively. Also, the filler type significantly affects the equivalent dose rate, where the redorded equivalent dose rates are 24.63, 24.08, 27.63, 33.80, and 36.08 µSv/h for natural rocks basalt-1, basalt-2, basalt-sill, limestone, and rhyolite, respectively. The mentioned results show that the natural rocks, especially a thicker thickness (i.e., 17 cm thickness) of natural rocks basalt-1 and basalt-2, significantly reduce the gamma emissions from the radioactive wastes inside the modified container. Moreover, using an outer cementation concrete wall of 15 cm causes an additional decrease in the equivalent dose rate received from the container where the equivalent dose rate dropped to 6.63 µSv/h.

≪상한론≫ 탕제(湯劑)에서 반하(半夏)의 일일 복용량 수정 (Revisoin of the Daily Dose of Pinelliae Tuber in Treatise on Cold Damage Diseases)

  • Objects : The daily dose of Pinelliae Tuber in ≪Treatise on Cold Damage Diseases≫ is half seung in volume, two and half ryang in weight, and fifteen in total number. But the daily dose should be a whole number. So I found out the background of this setting and correct solution. Methods : I searched Classics of Traditional Medicine, found out the background of the daily dose setting, solution. Results : The daily dose of Pinelliae tuber in ≪Hangdi's Internal Classic Miraculous Pivot≫, ≪Bohenggyuljangbuyongyakbeobyo≫ is half seung. ≪Treatise on Cold Dameage Diseases≫ followed the same daily dose of that because it referred to these books. In ≪Synopsis of Prescription of the Golden Chamber≫, the daily dose of that is half seung, one or two seung. The half seung of the Pinelliae Tuber is thirty three mL, but the diameter is 1~1.5 cm that accurate measurement by volume is difficult. The daily dose by weight is correct considering the unity of marking of the daily dose, accuracy of measurement, the fact that Pinelliae Tuber is currently distributed by cutting. So, two ryang is correct which is thirteen gram. Conclusions : Considering the traditionality, the convenience of measurement, the daily dose of Pinelliae Tuber in the ≪Treatise on Cold Damage Diseases≫ is half seung, but considering the unity, accuracy, current state of distribution, it is correct that the daily dose of it is two ryang. It corresponds to thirteen gram.

Field-in-Field Technique을 이용한 두경부암의 접합부위 선량개선에 관한 고찰 (Field-in-Field Technique to Improve Dose Distribution in the Junction of the Field with Head & Neck Cancer)

  • 목 적: 두경부암의 치료에 있어 상부 두경부의 양측면조사면과 하경부의 전방조사면의 접합부위에 균등한 선량을 조사하는 것은 매우 중요하다. 접합부위의 선량분포개선을 위하여 하경부 전방조사면의 치료시 Field-in-Field technique을 이용하여 부족선량(under dose)과 초과선량(over dose)으로 인한 선량불균등을 개선하고 일반치료와의 비교를 통하여 두경부암치료에 적용하고자 한다. 대상 및 방법: 상부 두경부의 양측면 조사시 빔의 확산으로 일어나는 입사점과 출사점의 선량차이를 알아보기 위하여 인체모형팬톰을 이용하였다. 인체모형팬톰을 전산화단층촬영하고 전산화치료계획에서 관심점의 선량비교를 시행하였고, 하경부 접합부위의 선량비율을 계산하여 이를 보정하였다. 조사면 접합부위의 선량분포를 알아보기 위하여 하경부의 접합부위에 저감도 필름을 놓고 일반적인 치료인 상부 두경부의 양측면조사와 하경부의 전방조사시 선량분포를 측정하였다. 또한, 상부 두경부 양측면 조사에 따른 빔의 확산을 고려한 Field-in-Field technique을 이용하여 하경부 전방조사를 할 때의 접합부위의 선량분포 차이를 측정하여 비교하였다. 접합부위의 관심점 선량을 알아보기 위하여 열형광선량계를 이용하여 인체모형팬톰내의 관심점에서의 선량변화를 비교, 분석하였다. 결 과: 전산화치료계획에서 하경부의 접합부위에 Field-in-Field technique을 적용하여 치료계획시 상부 두경부 양측면 조사와 선량합성을 한 경우 부족선량 영역의 선량이 4.7~8.65% 이상 증가하였다. 초과선량 영역의 선량도 2.75~10.45% 감소하였다. 또한, 저감도 필름을 이용한 측정에서는 부족선량영역에서 11.3% 증가, 초과선량영역에서 5.3% 감소한 것으로 나타났다. 열형광선량계를 이용한 관심점선량측정에서도 Field-in-Field technique 적용시 부족선량을 최소 7.5%에서 최대 17.6%까지 보정해주는 것으로 나타나 불균등한 선량분포를 개선할 수 있었다. 결 론: 전산화치료계획시 빔의 확산을 고려한 Field-in-Field technique을 적용하면 접합부위의 선량보정을 통해 냉점(cold spot)과 온점(hot spot)을 줄일 수 있었으며 특히, 빔의 확산에 따른 입사점의 부족선량을 보정할 수 있었다. 본 실험을 통해 Field-in-Field technique의 임상적용시 경부임파절의 저선량으로 인한 임파절전이에 대한 위험도를 감소시킬 수 있을 것으로 사료된다.

방사선피폭관리시스템를 위한 D-Shuttle 선량계의 방사선 선량측정 (Radiation Dose Measurement of D-Shuttle Dosimeter for Radiation Exposure Management System)

  • D-Shuttle (Chiyoda Technol Corporation, Tokyo, Japan) 선량계를 이용하여 개인피폭관리 및 자연방사선량의 모니터링을 위한 기초자료를 제공하는데 연구의 목적이 있다. D-Shuttle을 이용하여 선량을 산출하였다. 선량보고서에서 400 일 노출되었을 때에 1.346 mSv 이었고, 연간선량 (annual dose per year)은 1.228 mSv/year, 평균시간선량 (average dose per hour)은 $0.014{\mu}Sv/hr$ 이었다. 국내의 개인 외부피폭선량 (1.295 mSv/year =Korea average natural individual external dose), 국내의 연간부가선량 (additional dose per year)은 -0.0663 mSv/year 이다. D-Shuttle은 방사선모니터링을 위한 개인선량계로 방사선의 검출성능 우수한 기능, 실시간 방사선 피폭관리, 방사선 작업의 경보 기능, 효율적이고 사용이 편리한 개인 방사선선량의 피폭관리로 ALARA에 매우 유용한 선량계로 사용할 수 있다. 방사선작업종사자와 지역주민의 방사선모니터링 측정기기로 병원, 산업, 의료현장, 원전사고 지역과 비파괴 분야의 위험한 지역에서 방사선모니터링으로 활용될 수 있다.

ACM의 비글견을 이용한 단회 경구투여 용량증가 독성 시험 및 4주 반복 경구투여 용량 결정 시험 (Single Oral Dose-increasing Toxicity Test and Four Weeks Repeated Oral Dose Determinating Test of ACM (Added Chongmyung-tang) in Beagle Dogs)

  • Objectives : To provide information on the safety of ACM, we carried out a single oral dose-increasing toxicity and 4-weeks repeated oral dose determining test of ACM in beagle dogs. Methods : In a single oral dose-increasing toxicity test, beagles were treated with ACM orally increasing dose level (1,000, 2,000, 5,000 mg/㎏) at interval of 3 days. After administration, signs of toxicity were observed for two weeks. In 4-weeks repeated oral dose determinating test, beagles were treated with ACM with oral dose 500, 1,000, 2,000 mg/kg for 4 weeks. Mortality, clinical signs, body weight changes, food consumption, urinalysis, hematological and biochemical parameters, organ weights, necropsy findings, and histological findings were monitored during the study period. Results : In a single oral dose-increasing toxicity test, we found no mortality, abnormalities in clinical signs, body weight, and necropsy findings during the study period. In 4-weeks repeated oral dose determinating test, we found no mortality, abnormalities in clinical signs, body weight, food consumption, urinalysis, hematological and biological parameters, gross findings, organ weights, necropsy findings, and histopathological findings in any of the beagles tested. Conclusions : The results obtained in these studies suggest that maximum tolerated dose (MTD) of ACM in male and female beagle dogs was supposed to be over 5,000 mg/kg. For the future studies of toxicity, it is advisable that high dose and low dose are set at 2000 mg/kg and 500 mg/kg, respectively.

3차원 안모분석을 위한 저선량 Multi-detector CT 영상의 유효선량 및 화질 평가 (Evaluation of the effective dose and image quality of low-dose multi-detector CT for orthodontic treatment planning)

  • Purpose : This study was designed to compare the effective doses from low-dose and standard-dose multi-detector CT (MDCT) scanning protocols and evaluate the image quality and the spatial resolution of the low-dose MDCT protocols for clinical use. Materials and Methods : 6-channel MDCT scanner (Siemens Medical System, Forschheim, Germany), was used for this study. Protocol of the standard-dose MDCT for the orthodontic analysis was 130 kV, 35 mAs, 1.25 mm slice width, 0.8 pitch. Those of the low-dose MDCT for orthodontic analysis and orthodontic surgery were 110 kV, 30 mAs, 1.25 mm slice width, 0.85 pitch and 110 kV, 45 mAs, 2.5 mm slice width, 0.85 pitch. Thermoluminescent dosimeters (TLDs) were placed at 31 sites throughout the levels of adult female ART head and neck phantom. Effective doses were calculated according to ICRP 1990 and 2007 recommendations. A formalin-fixed cadaver and AAPM CT performance phantom were scanned for the evaluation of subjective image quality and spatial resolution. Results : Effective doses in ${\mu}Sv$ ($E_{2007}$) were 699.1, 429.4 and 603.1 for standard-dose CT of orthodontic treatment, low-dose CT of orthodontic analysis, and low-dose CT of orthodontic surgery, respectively. The image quality from the low-dose protocol were not worse than those from the standard-dose protocol. The spatial resolutions of both standard-dose and low-dose CT images were acceptable. Conclusion : From the above results, it can be concluded that the low-dose MDCT protocol is preferable in obtaining CT images for orthodontic analysis and orthodontic surgery.

Interpretation of Animal Dose and Human Equivalent Dose for Drug Development

  • Objectives: To introduce to TKM scientific dose conversion methods of human to animal or animal to human for new drug investigations. Methods: We searched guidelines of the FDA and KFDA, and compared them with references for drug-dose conversion from various databases such as PubMed and Google. Then, we analyzed the potential issues and problems related to dose conversion in safety documentation of new herbal drugs based on our experiences during Investigational New Drug (IND) applications of TKM. Results: Dose conversion from human to animal or animal to human must be appropriately translated during new drug development. From time to time, investigators have some difficulty in determining the appropriate dose, because of misunderstandings of dose conversion, especially when they estimate starting dose in clinical or animal studies to investigate efficacy, toxicology and mechanisms. Therefore, education of appropriate dose calculation is crucial for investigators. The animal dose should not be extrapolated to humans by a simple conversion method based only on body weight, because many studies suggest the normalization method is based mainly on body surface area (BSA). In general, the body surface area seems to have good correlation among species with several parameters including oxygen utilization, caloric expenditure, basal metabolism, blood volume and circulating plasma protein. Likewise, a safety factor should be taken into consideration when deciding high dose in animal toxicology study. Conclusion: Herein, we explain the significance of dose conversion based on body surface area and starting dose estimation for clinical trials with safety factor.

심장동맥 조영 검사 시 검사 조건에 따른 환자 선량 평가 (Evaluation of Radiation Dose to Patients according to the Examination Conditions in Coronary Angiography)

  • This study analyzed imaging conditions and exposure index through clinical information collection and dose calculation programs in coronary angiography examinations. Through this, we aim to analyze the effective dose according to examination conditions and provide basic data for dose optimization. In this study, ALARA(As Low As Reasonably Achievable)-F(Fluoroscopy), a program for evaluating the radiation dose of patients and the collected clinical data, was used. First, analysis of imaging conditions and exposure index was performed based on the data of the dose report generated after coronary angiography. Second, after evaluating organ dose according to 9 imaging directions during coronary angiography, with the LAO fixed at 30°, dose evaluation was performed according to tube voltage, tube current, number of frames, focus-skin distance, and field size. Third, the effective dose for each organ was calculated according to the tissue weighting factors presented in ICRP(International Commission on Radiological Protection) recommendations. As a result, the average sum of air kerma during coronary angiography was evaluated as 234.0±112.1 mGy, the dose-area product was 25.9±13.0 Gy·cm2, and the total fluoroscopy time was 2.5±2.0 min. Also, the organ dose tended to increase as the tube voltage, milliampere-second, number of frames, and irradiation range increased, whereas the organ dose decreased as the FSD increased. Therefore, medical radiation exposure to patients can be reduced by selecting the optimal tube voltage and field size during coronary angiography, maximizing the focal-skin distance, using the lowest tube current possible, and reducing the number of frames.

Dosimetric Validation of the Acuros XB Advanced Dose Calculation Algorithm for Volumetric Modulated Arc Therapy Plans

  • Acuros XB advanced dose calculation algorithm (AXB, Varian Medical Systems, Palo Alto, CA) has been released recently and provided the advantages of speed and accuracy for dose calculation. For clinical use, it is important to investigate the dosimetric performance of AXB compared to the calculation algorithm of the previous version, Anisotropic Analytical Algorithm (AAA, Varian Medical Systems, Palo Alto, CA). Ten volumetric modulated arc therapy (VMAT) plans for each of the following cases were included: head and neck (H&N), prostate, spine, and lung. The spine and lung cases were treated with stereotactic body radiation therapy (SBRT) technique. For all cases, the dose distributions were calculated using AAA and two dose reporting modes in AXB (dose-to-water, $AXB_w$, and dose-to-medium, $AXB_m$) with same plan parameters. For dosimetric evaluation, the dose-volumetric parameters were calculated for each planning target volume (PTV) and interested normal organs. The differences between AAA and AXB were statistically calculated with paired t-test. As a general trend, $AXB_w$ and $AXB_m$ showed dose underestimation as compared with AAA, which did not exceed within -3.5% and -4.5%, respectively. The maximum dose of PTV calculated by $AXB_w$ and $AXB_m$ was tended to be overestimated with the relative dose difference ranged from 1.6% to 4.6% for all cases. The absolute mean values of the relative dose differences were $1.1{\pm}1.2%$ and $2.0{\pm}1.2%$ when comparing between AAA and $AXB_w$, and AAA and $AXB_m$, respectively. For almost dose-volumetric parameters of PTV, the relative dose differences are statistically significant while there are no statistical significance for normal tissues. Both $AXB_w$ and $AXB_m$ was tended to underestimate dose for PTV and normal tissues compared to AAA. For analyzing two dose reporting modes in AXB, the dose distribution calculated by $AXB_w$ was similar to those of AAA when comparing the dose distributions between AAA and $AXB_m$.
