  • The author has studied on 910 roentgenograms of lower second molars, which were taken by intraoral technic, and obtained the following results. 1. The development of crown of lower second molars was completed 8.25 years. 2. The formation of mesial and distal roots in full length on lower second molars was completed as follows: a. mesial roots 15. 07 years b. distal roots 15. 53 years 3. The formation of apical foramina of mesial and distal roots of lower second molars was closured as follows: a. apical foramen of mesial root 18.00 years b. apical foramen of distal root 18.79 years 4. As a general rule, the mesial roots were developed more earlier than distal roots.

  • A 11 month-old Shihtzu was referred to the Veterinary Medical Teaching Hospital, Seoul National University. Clinical signs of this patient were lameness, shortening limb, angular deformity, rotation of foot, subluxaion of elbow joint and restricted range of movement of left forelimb. For the evaluation of the abnormalities of left forelimb, radiographic examination was carried out. Radiographic findings were characteristics of premature closure of distal ulna such as closure of distal ulna growth plate and cranial bowing of radius. With radiographic signs and physical examination, it was diagnosed as premature closure of growth plate of the left forelimb. After osteotomy of the radius and ostectomy of the ulna, radiographic evaluation of limb about angulation of elbow joint was performed every 2-3 weeks for 3 months. In case of premature closure of distal growth plate of ulna, radiography was very useful for diagnositic method of premature closure of distal ulna and monitoring of healing process.

  • We presented two cases of unstable fracture of the distal clavicle in children associating superior displacement of the ipsilateral distal clavicle and widening of the coracoclavicular (CC) interval. They obtained good osteosynthesis and reduction of the CC interval by conservative treatment. The primary conservative treatment should be considered for the treatment of the unstable distal clavicular fracture with widening of the CC interval in children.

Osteochondroma on distal tibiofibular joint in adult - One Case Report - (성인의 원위 경비골 관절에 발생한 골연골종 - 1예 보고 -)

  • Although osteochondromas are common lesions, osteochondroma accompanying diastasis of the ankle joint in a child or adult is an uncommon condition. No previous cases of distal tibiofibular diastasis caused by an osteochondroma in adult have been reported except in child. The authurs experienced a case of mild distal tibiofibular diastasis secondary to an osteochondroma in a 37-year-old female presented with right ankle pain. Osteochondroma of the distal tibiofibular joint were detected on magnetic resonance imaging (MRI). She was pain-free 4 weeks after excision of the osteochondroma. The patient has remained asymptomatic for 1 years postoperatively. Early excision obviates the need for complex reconstructive surgery to correct ankle deformity later.

Increased Biceps Translation: A Clinical Sign of Complete Distal Biceps Tendon Rupture

  • Various tests to help in the clinical diagnosis of distal biceps tendon ruptures have been described. In our experience these tests are painful in the acute setting. We suggest a simple alternative test wherein the biceps muscle belly is held by the examiner and translated medially and laterally. This is done with the forearm flexed to 90 degrees. It is first performed with the biceps relaxed and subsequently performed with the forearm flexed against resistance. In the relaxed forearm the biceps easily translates over 50% of its width. When placed under tension (by flexing against resistance) this translation is significantly reduced. In cases of complete distal biceps tendon rupture, the biceps still translates, even under resisted flexion of the forearm. This simple test is less painful than other described tests, is easy to perform, and aids in clinical diagnosis of distal biceps tendon ruptures.

Occupational Therapy Intervention for Malunion of Distal Radial Fracture: A Case Report

  • Objective: Malunion of distal radial fracture can result in pain, stiffness, and decreased function of the affected limb. Occupational therapy intervention can improve outcomes in these patients, but there is limited research on the effectiveness of such interventions. Case Description: This case report describes the occupational therapy intervention and outcomes for a 44-year-old female patient with malunion of left distal radial fracture. The patient received 8 weeks of occupational therapy intervention consisting of therapeutic exercises, joint mobilization, and fine motor activities. The patient demonstrated significant improvements in range of motion, grip strength, and activities of daily living (ADL) in her left hand. The patient also reported less pain and greater ease in performing activities of daily living. Conclusion: Occupational therapy intervention can be effective in improving outcomes for patients with malunion of distal radial fracture. This case report highlights the importance of early intervention and interdisciplinary collaboration among healthcare professionals to optimize patient outcomes. Further research is needed to explore the effectiveness of occupational therapy interventions for patients with malunion of distal radial fracture.

Laparoscopy-Assisted Distal Gastrectomy for Early Gastric Cancer in the Elderly

  • Purpose: With an increase in life expectancy, more elderly patients are presenting with gastric cancer. As a result it is yet be resolved whether laparoscopy assisted distal gastrectomy is a suitable treatment for elderly patients with early gastric cancer. This study retrospectively compared surgical outcomes of laparoscopy assisted distal gastrectomy between elderly and nonelderly patients with gastric cancer. Materials and Methods: The study group was comprised of 316 patients who underwent laparoscopy assisted distal gastrectomy between April 2005 and December 2010. Of these patients, 93 patients whose ages were 65 years or more were compared with 223 patients who were younger. Results: There were no differences in the short term outcome or minor complication rate between the elderly patients and the nonelderly patients. The hospital stay was significantly longer and the major complication rate was significantly higher for the elderly patients compared with nonelderly patients. Conclusions: Laparoscopy assisted distal gastrectomy for early gastric cancer in the elderly patients had comparable operation time and blood loss with the nonelderly group. However, the safety and advantage of laparoscopy assisted distal gastrectomy in the elderly patients need to be further studied in higher volume trials.

Heterotopic Ossification of Distal Tibiofibular Syndesmosis after Ankle Fractures (족관절 골절 후 발생한 원위 경비 인대의 이소성 골형성)

  • Purpose: To evaluate the effect on clinical course of heterotopic ossification of distal tibiofibular syndesmosis after ankle fractures. Materials and Methods: From June 2001 to May 2004, we found nine cases of heterotopic ossification of distal tibiofibular syndesmosis after ankle fractures. There were 8 male patients and 1 female patient; their mean age was 42 years old. There were 6 Weber type B and 2 Weber type C fractures, and there is 1 case with posterior malleolus fracture only. Among them, 8 ankle fractures were operated. Follow up period was averaged for 14 months. We were able to review radiographs at initial injury and to review clinical menifestation and radiographs at last follow up. We used an ankle-hindfoot scoring system of AOFAS which combined symptom, function and alignment with maximum score of 100 point. Results: In all cases ankle dorsiflexion and plantarflexion were not significantly different from that of the contralateral side. The patients who had developed heterotopic ossification in distal tibiofibular syndesmosis had a similar functional score. The mean ankle-hind foot score was 94 points. Conclusion: We concluded that the heterotopic ossification of distal tibiafibular syndesmosis after ankle fractures had little effect on clinical course and range of motion of ankle joint.

Treatment for Distal Humeral Fractures by Modified Anatomical Y-Plate (변형된 Y형 해부학적 금속판을 이용한 상완골 원위부 골절의 치료)

  • Comminuted fracture of the distal end of the humerus in adults is very rare and difficult to treat. In operative treatment, an implant which can achieve accurate anatoraical reduction and rigid fixation is needed. But the pre­existing Y -shaped plate had wide and thick limbs and some problems in fixation for most distal humeral fractures. So we devised a modified plate which is more narrow and th1n and has a different hole distance and wing length (long lateral wing). The aim of the study was to evaluate the clinical result of this modified anatomical Y -plate. From 1991 to 1997, we treated 23 cases of distal humeral fractures using modified anatomical plate and the results were as follows. 1. Fracture type in 18 patients(78%) was C-type(C1,C2,C3) which were intraarticular and mostly displaced or comminuted. 2. Bony union was obtained in 22 patients(96%) through rigid fixation and observed radiologically at 3.5 months(2-6months) on the average. 3. In 23 patients, 19 patients(82%) showed satisfactory results after 34 months(6-73months) follow-up. So if the modified anatomical Y-plate is used in the treatment of distal humeral fracture, a satisfactory result can be obtained through a more accurate, easy, and rigid fixation than preexisting plate.

Limb Salvage with Tumor prosthesis for Osteosarcoma of Distal Tibia and Fibula (경골 및 비골 원위부 골육종의 종양삽입물을 이용한 사지구제술)

  • Recently limb-salvage operation is widely used for the treatment of malignant bone tumor. But distal leg is out of range of limb-salvage operation due to its technical problem. We report satisfactory limb-salvage operation with tumor prosthesis in 3 cases of osteosarcoma of the distal tibia and fibula. Two cases involved in the tibia and 1 case in the fibula. Average age at operation was 23 years. Neoadjuvant and adjuvant chemotherapy were performed in all 3 cases. We used custom made tumor prosthesis which is designed by Seoul National University Orthopaedic Department. Overall Functional Evaluations by Enneking rating system were good in all 3 cases. All cases are CDF(continuosly disease free since the surgical procedure) state at mean follow-up 2 year and 9 months. In conclusion limb-salvage with tumor prosthesis is useful treatment modality for malignant bone tumor of distal tibia and fibula. Good functional results and few complications suggests limb salvage of distal lower leg may be replaceable with B-K amputation.
