  • 1147 preschool children aged from 3 to 6 living in some farm villages in Kyungki-do were surveyed on dmf and distribution of dental caries in primary dentition.   • The purpose of this study was to evaluate the patterns of skeletal changes of proximal and distal segments after one jaw surgery and two jaw surgery with posterior impaction using SSRO on mandible in order to determine the skeletal origin of relapse and compare the stability of surgical methods in anterior open bite. The points and lines from lateral cephalometrics were measured before, after surgery, and at least 6-month follow up period. And then, the positional change of the proximal and distal segment were evaluated respectively. The results obtained were as follows; In cases of two jaw surgery, the results were stabler because they had less relapse factors. In cases of one jaw surgery, the value of APD were increased but it didn't relapse to the original value. Both of proximal and distal segments were responsible for the relapse tendency. But in one jaw surgery, the rotation of proximal segment was more responsible, and in two jaw surgery, the rotation of distal segment was.

Repair of Achilles tendon tear with distal wire pulling (강선 견인을 이용한 아킬레스 건 파열의 수술적 치료)

  • Purpose: This study was designed to evaluate whether the method "Distal pulling with wire" after primary repair of Achilles tendon, can result in early restoration of ankle motion. Materials and Methods: In 14 cases of tendocalcaneus repair, the proximal stump was pulled down distally by wire and fixed the ends of wire to the transcalcaneal K - wire. We evaluated the range of motion of ankle joint at 4th, 6th, 12th week in 10 patients whom we followed up for more than 1 year. We evaluated the results of Achilles tendon repair by Hooker's criteria. Results: Range of motion of ankle joint revealed as follows. Degree of mean dorsiflexion improved - $5.1^{\circ},\;15.0^{\circ},\;22.4^{\circ}$ at 4th, 6th and 12th week respectively, while plantarflexion improved $21.5^{\circ},\;32.7^{\circ}$ and $42.3^{\circ}$ respectively. At one year follow up, seven of them had no problems in active daily life and sports activities. According to Hooker's criteria, the result rated excellent in nine, satisfactory in one. Conclusion: Early gain of ankle joint motion was possible by "Distal pulling with wire" after primary repair of Achilles tendon.

Percutaneous Fixation with Cannulated Screws and Ilizarov External Fixator in Triplane Fracture of the Distal Tibial Epiphysis in Children (소아 경골 원위 골단부 삼면 골절의 경피적 나사못 고정술과 Ilizarov 외고정 장치를 이용한 치료)

  • Purpose: To evaluate the result of percutaneous fixation with cannulated screws and Ilizarov external fixator in triplane fracture of the distal tibial epiphysis in children. Materials and Methods: Between May 2004 and December 2007, 14 cases with triplane fractures were treated by percutaneous fixation with cannulated screws and Ilizarov external fixator after underwent CT imaging to assess the fracture pattern, articular disruption and to plan further management. Mean age and follow-up period were 14.1 years old and 15 months respectively. Results: There were satisfactory results in all 14 cases that had excellent reduction and stable fixation. All cases regained full range of movement within 6 weeks. Conclusion: We obtained satisfactory result after percutaneous fixation with cannulated screws and Ilizarov external fixator in triplane fractures of the distal tibial epiphysis in children.

Juvenile Hemangioma Occurred in Distal Femoral Epiphysis (소아의 대퇴골 원위부 골단에 발생한 혈관종)

  • A hemangioma occurred in the bony epiphysis is extremly rare. A 5-year-old boy visited to our hospital with pain and flexion contracture on the right knee. MRI showed some lesions scattered in the epiphysis of the distal femur and the proximal tibia. Biopsy specimen from the distal femoral epiphysis revealed pathologic findings compatible with hemangioma. On 8 years follow-up, the lesion in the distal femoral epiphysis had been cured, and those in the proximal tibial epiphysis were spontaneously disappeared without surgery. The scanogram shows no leg length discrepancy and angular deformity. We reports a rare case of hemangioma occurred in the bony epiphysis with the results of 8 year follow-up with the review of literatures.

  • Vascularized free fibula head transfer is an established method for reconstruction of long bone defects of the upper limb involving the distal radius or the proximal humerus. For the wrist following tumor resection, in cases of resection of the radial articular surface, three reconstructive options are possible: 1. fibular head transfer to replace the radial joint surface, 2. fixation of the fibula to the scaphoid and lunate, 3. complete wrist fusion. The decision on the type of the operation depends on the amount of the resection and the remained normal anatomical structures, and also the necessity of function of the wrist in the future. The authors believe that the vascularized free fibula head graft is a safe and reliable method for reconstructing the upper limb, especially for patients with a defect of the distal radius, and report the operative methods, donor vascular consideration, complications, and functional result after this operation.

  • Purpose: Delayed rupture of flexor pollicis longus as a sequelae of the plate inserted for distal radius fracture is a very rare. This is the first case reported and published in Korea. Methods: A 48 years old female patient visited hospital, complaining flexor disturbance of interphalangeal joint of left thumb, which suddenly occurred without any external wound. We found that she had received operation of fixing plate for fracture of left distal radius 10 years ago. As operational opinion, we have checked that flexor pollicis longus tendon has been ruptured with oblique ways being stimulated by extended plate to palmar side over long period. Results: Authors performed tenorrhaphy of flexor pollicis longus without tendon graft and presented a successful active flexion of the left thumb interphalangeal joint 1 year after the operation. Conclusion: If the extruded part of the end plate is observed during the operation or follow-up, it is considered to be necessary to get rid of the plate as early as possible after the fracture healing.

Fracture-Separation of the Distal Humeral Epiphysis in Children (소아 상완골 원위부 골단의 골절 및 분리)

  • We have reviewed seven cases of fracture-separation of the distal humeral epiphysis, one of which was initially misdiagnosed as a fracture of the lateral condyle. There were difficulties in making the diagnosis. The injury must be distinguished from an elbow dislocation and a fracture of the lateral humeral condyle. All seven patients revealed posteromedial displacenemt of the distal humeral epiphysis on initial x-rays. Three patients were treated by closed reduction and cast immmobilization, and four patients by open reduction and internal fixation. All three patients with conservative treatment had slight cubitus varus(under 5 degrees). Two patients with operative treatment had significant deformities of the elbow, one 25 degrees of valgus and one 20 degrees of varus. In treatment of these injuries, accurate evaluation of the state of reduction is most important. We got acceptable results with a conservative treatment.

Clinical Study on Immediate Response after Acupuncture on Local Acupoints and Distal Acupoints for Neck Pain Patients (경항통 환자에 대한 원위취혈 및 근위취혈의 침치료 직후 효과에 대한 비교 연구)

  • Objectives : The purpose of this study was to compare immediate response after acupuncture on Local Acupoints group and Distal Acupoints group for neck pain patients. Methods : From January 2nd 2008 to August 31th 2008, 10 neck pain patients were divided into 2 groups. One group(test I group) was taken Local Acupoints, and the other group(test II group) was taken Distal Acupoints. To check immediate response and satisfaction of needling, visual analog scale(VAS), range of motion(ROM), clinical evaluation grade and five-point likert scale were used before and after treatment. Results : Both acupuncture therapy showed immediate response and good effect on neck pain. And test I group showed more immediate response than test II group on all categories. Conclusions : Local Acupoints group can be recommended as an immediate and useful therapy to treat neck pain.

A Radiographic Study on the Development of Roots of Mandibular Second Molars in Female (여성 하악 제2대구치 치근발육에 관한 방사선학적 연구)

  • In order to evaluate the relationship of age with the developmental stage on mandibular second molars in female, the author examined the radiographs in standard films taken by intraoral paralleling technic and analysed the development of 620 roots of 310 females ranged from 10 to 18 years. The development was divided into 7 stages : Crown complete (Cr.C.), Root lengthed 1/4(R.1/4), Root length 1/2 (R.1/2), Rooth length 3/4 (R.3/4), Rooth length complete (R.C), Apex 1.2 closed (A. 1/2), Apical closure complete (A.C) The obtained results were as follows : 1. The formation of roots in fulllength of mandibular second molars was completed at 14.21-year-age in mesial root and at 14.96-year-age in distal root. 2. The formation of apical foramen of mandibular second molars was completed at 17.11-year-age in mesial root and at 17.53-year-age in distal root. 3. The mesial root of mandibular second molar was generally developed earlier than distal root (p<0.01) 4. Coefficients of correlation and regression equations between for age(Y) and the developmental stage of mandibular second molar(X) were "Y=1.53X+6.54, r=0.91 (p<0.01)" in mesial root and "Y=1.52X+7.11, r=0.92(p<0.01)" in distal root respectively.l root respectively.
