비동일면 치료 시 자체 제작한 Kw-infrared Reflective Marker의 유용성 평가 (Evaluation of the Usefulness of the Self-developed Kw-infrared Reflective Marker in Non-coplanar Treatment)

  • 목 적: 종양의 움직임을 고려하여 치료할 수 있는 RPM (Real time Position Management, Varian, USA) system을 사용하여 호흡을 고려한 방사선치료 시행 시, 제조사에서 제공하는 적외선 반사체는 일정각도 이상 couch가 회전할 경우 호흡신호 획득이 불가능하다. 이런 단점을 개선하고자 본 저자는 어느 각도에서나 호흡신호를 얻을 수 있는 3차원 적외선 반사체(3D infrared reflective marker)를 자체 개발하여 Kw-반사체라 명하고 그 유용성을 평가 하고자 한다. 대상 및 방법: 호흡신호의 안정성을 측정하기 위해 호흡운동을 재현 할 수 있는 3차원 구동팬텀(3D moving phantom) 위에 3가지 조건(A: couch 0도, 제조사의 반사체 B: couch 0도, Kw-반사체 C: couch 90도, Kw-반사체)의 적외선 반사체를 올려놓고 각각 3분 동안 호흡신호를 획득 하였다. 획득한 호흡신호는 호흡분석 프로그램(Labview ver 7.0)을 통해 최고값(peak value), 최저값(valley value), 표준편차(standard deviation), 변동값(variation value), 진폭값(amplitude value)을 획득하였다. 타겟의 회전 오차와 타겟의 이동범위를 알아보기 위해 3차원 구동팬텀 위에 B.B팬텀을 올려놓고 온-보드 영상장치(OBI)로 couch 0도와 90도 이미지를 획득한 후 B.B팬텀 중심에 위치한 ball bearing의 X, Y, Z값(mm)을 획득하였다. 결 과: 호흡신호를 분석한 결과 최고점에서 표준편차가 A: 0.002 B: 0.002 C: 0.003이고 진폭에 대한 호흡의 안정성은 A: 0.15% B: 0.14% C: 0.13%로 Kw반사체는 안정적으로 호흡신호를 얻을 수 있었다. couch 90도 회전 시 타겟인 ball bearing의 평균 회전오차는 X: -1.25 mm Y: -0.45 mm Z: +0.1 mm로 모든 방향에서 평균 1.3 mm 이내이고 타겟의 이동범위 차이도 평균 0.3 mm 이내였다. 결 론: couch가 회전하는 비동일면(Non-coplanar) 치료 시 Kw-반사체로 호흡신호를 얻은 결과 Kw-반사체는 안정적인 호흡신호를 획득할 수 있었고 제조사의 적외선반사체를 대체하기에 충분했다. 타겟의 회전오차와 이동범위 차이가 거의 없어 couch 회전 시 반사체 움직임의 변위가 호흡신호의 scale 및 진폭을 변화시킨 원인임을 알 수 있었다. 향후 couch angle값에 따른 진폭 높이의 변환 값을 연구해 적용한다면 비동일면에 대한 위상기반 gating 치료도 가능 할 것으로 사료된다.

호흡으로 인한 움직임이 큰 종양의 방사선치료 시 Abdomen and Chest Motion Control Device (ABCHES)의 유용성 평가 (Evaluation on Usefulness of Abdomen and Chest Motion Control Device (ABCHES) for the Tumor with a Large Respiratory Motion in Radiotherapy)

  • 목 적: 호흡에 의한 장기의 움직임이 큰 흉부와 상복부에 위치한 종양의 토모테라피 치료 시 움직임을 최소화하기 위해 노력으로 호흡 조절 시스템을 적용하지만 여러 제약이 따라 이용이 제한적이었다. 이에 본 연구에서는 토모테라피 치료 시 효율적인 호흡 조절을 위한 ABCHES system의 적용 가능성 및 유용성을 평가해 보고자 하였다. 대상 및 방법: 본원에서 흉부와 상복부에 위치한 폐와 간, 쓸개, 췌장에 토모테라피 치료를 받은 5명의 환자를 대상으로 하였다. 모든 환자에게 ABCHES를 사용하여 자유 호흡법과 얕은 호흡법을 각각 교육한 후 치료 계획용 4D-CT를 실시하여 총 10개의 4차원 단층촬영영상을 획득 하였다. 한명의 전공의는 각 영상의 최대 흡기, 최대 호기, 호흡의 중간 위상, Average CT이미지에서 육안적으로 보이는 종양과 주변 정상장기를 그렸으며 MIM에서 선량체적 히스토그램과 종양의 움직임에 대한 정량적 분석을 실시하였다. 자유 호흡과 얕은 호흡 상태에서 장기의 움직임은 총 여섯 방향에서 평가하였고 주변 장기에 조사된 방사선량을 비교하였다. 결 과: 5명의 환자 중 호흡 조절 장치인 ABCHES를 이용하여 자유 호흡과 얕은 호흡 상태에서 장기의 움직임이 5 mm 이상 움직인 호흡은 자유 호흡이 12번인 반면 얕은 호흡에서는 2번으로 감소되었다. 선량 체적 히스토그램을 통한 비교 분석 결과 두 호흡간 치료 체적과 종양조직 주변 정상 장기 2개의 평균 선량 값과 정상조직에 방사선이 조사되는 용적에서 ABCHES를 이용한 얕은 호흡이 자유 호흡에 비해 모두 낮은 치료 결과 값을 확인 할 수 있었다. 결 론: ABCHES를 사용하여 규칙적이고 정확한 얕은 호흡을 함으로써 토모테리피 치료 시 유용성과 호흡에 따른 종양의 움직임을 최소화 할 수 있음을 확인 하였고, 주변 정상조직에 불필요하게 조사되는 방사선을 감소시킬 수 있었다.

말뚝의 횡방향 이격거리를 고려한 터널굴착이 인접 단독말뚝 및 군말뚝에 미치는 영향에 대한 연구 (A study on the effect of tunnelling to adjacent single piles and pile groups considering the transverse distance of pile tips from the tunnel)

  • 본 연구에서는 3차원 유한요소해석을 실시하여 말뚝에 인접한 터널시공으로 인한 말뚝의 거동을 터널로부터 말뚝선단의 횡방향 이격거리를 고려하여 분석하였다. 단독말뚝 및 간격 2.5d인 $5{\times}5$ 군말뚝을 고려하였다. 여기서 d는 말뚝의 직경을 의미한다. 수치해석에서는 순수하게 터널굴착(tunnelling-induced) 으로 인해 유발된 말뚝침하, 전단응력, 상대변위, 축력분포, 겉보기안전율 및 터널굴착 영향권을 고찰하였다. 말뚝이 터널굴착으로 인한 지반침하 영향권 내부에 존재할 경우 말뚝두부의 침하는 Greenfield 조건의 지표면 침하보다 최대 대략 111% 크게 산정되었고, 군말뚝의 경우 단독말뚝과 비교하여 말뚝침하가 크고 축력이 작게 나타났는데 이는 군말뚝내의 말뚝이 인접지반과 함께 블록(block)의 형태로 거동하는 것으로 분석되었다. 또한 말뚝의 상부에서는 상향의 마찰 저항력이 발생하고 말뚝의 하부에서는 하향의 마찰 저항력이 발생하여 순수하게 터널굴착(tunnelling-induced)으로 인해 말뚝에는 인장력을 발생시켰다. 한편 말뚝이 영향권 외부에 존재할 경우 말뚝에는 tunnelling-induced 압축력이 발생하였다. 수치해석을 통해 분석된 하중-침하 관계로부터 말뚝의 겉보기안전율을 계산한 결과 터널굴착 이전에 비해 대략 45% 감소된 것으로 나타났다. 따라서 이는 말뚝의 사용성에 심각한 문제를 유발시킬 수 있는 것으로 나타났다. 본 연구를 통해 지반침하 영향권에 따른 단독말뚝 및 군말뚝의 거동을 심도 있게 고찰하였다.

젖소의 분만 후 생식기의 회복 및 질병 발생 조사 (Postpartum Reproductive Tract Recovery and Prevalence of Health Problems in Dairy Cows)

  • 본 연구는 젖소에서 산후 자궁염증 및 난소주기 회복 정도를 평가하였으며, 또한 분만 사고 및 산후 기간 질병 발생에 대한 조사를 위하여 실시하였다. 충청 지역 4개 젖소 목장의 사육 젖소 224두를 이용하였다. 자궁의 염증상태를 평가하기 위하여, 분만 후 8주까지 매주 메트리체크 기구를 이용하여 자궁분비물 중 농의 비율에 따른 성상을 점수화(메트리체크 점수; 0-5점) 하였으며, 분만 후 4, 6, 8주에는 백혈구 비율 산정을 위한 자궁세포 검사를 실시 하였다. 분만 후 난소주기 재개 여부 확인을 위하여 4, 6, 8주에 프로게스테론 농도(${\geq}1ng/mL$)를 분석하였다. 분만 후 메트리체크 점수는 1주($3.4{\pm}0.1$)부터 4주($2.0{\pm}0.1$)까지 연속적으로 감소하였으며(p < 0.0001), 이 후 8주까지 비슷한 점수를 유지하였다(p > 0.05). 자궁세포 검사를 이용한 호중구 비율은 분만 후 4주($22.3{\pm}1.8%$)에서 6주($12.1{\pm}1.4%$)까지 감소하였으며(p < 0.0001), 이 후 8주($9.2{\pm}1.4%$)까지 비슷한 비율을 유지하였다(p > 0.05). 난소주기 재개는 분만 후 2주(12.1%)에서 8주(74.3%)까지 연속적으로 증가하였다(p < 0.0001). 난산과 후산정체 발생 비율은 각각 20.5%, 30.4%였다. 분만 후 케토시스, 유열, 4위전위증, 패혈성 자궁염, 자궁내막염, 준임상형 자궁내막염, 자궁축농증 및 난소낭종 발생율은 각각 18.8%, 3.6%, 4.0%, 36.2%, 29.5%, 17.0%, 4.5%, 14.7%였다. 소화기 질병 및 유방염 발생율은 각각 5.4%, 9.6%였다. 도태 및 폐사율은 각각 4.0%, 3.6%였다. 난산, 후산정체, 패혈성 자궁염, 자궁내막염, 난소낭종, 소화기 질병, 유방염 및 도태율은 목장간 차이가 있었다(p < 0.05, p < 0.01). 결론적으로, 분만 후 4주에서 8주까지 자궁염증 상태 및 난소주기 재개 확인은 축군의 번식위생 유지에 도움을 줄 수 있는 임상적인 수단으로 사용될 수 있으며, 조사 지역에서의 젖소에서 난산, 후산정체, 패혈성 자궁염 및 자궁내막염의 발생율이 높았음을 보여 주었다.

UAS 영상 및 지상 LiDAR 조합한 3D 수치모형 기반 비탈면 앵커의 손상인자 분석에 관한 연구 (A Study on Damage factor Analysis of Slope Anchor based on 3D Numerical Model Combining UAS Image and Terrestrial LiDAR)

  • 현행 비탈면 앵커공법의 보강성능평가는 앵커 두부와 지반밀착도, 앵커 두부의 균열 및 파손에 대해서 정성적으로 성능을 평가하고 있다. 이로 인해 성능저하 상태 점검을 위한 정량적 데이터베이스화와 이를 이용한 시간이력 관리는 어려운 실정이다. 이에 본 연구에서는 비탈면에 설치된 앵커공법의 정량적 유지관리에 활용하기 위하여 UAS 영상과 지상 LiDAR의 사각지대를 보완하기 위한 SfM기반의 조합 3차원 수치모형을 구현하여 손상인자의 수치데이터를 검출하였다. 비탈면과 같은 수직구조물에서 상대적으로 높은 z 좌표 오차를 갖는 UAS 3차원 수치모형에서 사각지대 데이터 공백을 상호 보완하기 위하여 지상 LiDAR 스캔 데이터를 조합하였고 z 좌표 정확도의 향상을 확인하였다. 비탈면에 설치된 10공의 앵커에 임의로 손상을 발생시킨 후에 3차원 수치모형을 구축하였고 정사투영을 통해 균열, 파손, 회전변위와 지반 밀착도에 대한 수치 값을 검출하였다. 8K 해상도로 균열 실측값과 비교시 ±0.05mm의 오차범위에서 0.3mm 미만의 균열 검출이 가능하였다. 앵커 두부의 최대 파손 면적은 설계대비 3% 이내로 발생된 것을 확인하였고, 파손부의 체적 또한 검출하였다. 특히 z 좌표 데이터가 중요한 지반밀착도의 경우 UAS 3차원 수치모형에서는 사각지대로 인한 데이터 공백으로 측정이 불가능하였지만 지상 LiDAR를 조합할 경우 앵커 저면과 지반의 불규칙한 표면에서 표고차 확인이 가능하여 임의의 20개 지점의 평균 표고차를 지반밀착도로 도출하였다. 또한, 앵커 두부의 1° 미만의 회전각과 이동 변위 값도 검출하였다. 이에 본 연구에서 구축한 3차원 수치모형에서 앵커 손상인자의 정량적 데이터 추출이 가능하였고, 이를 데이터베이스화 한다면 정량적 평가지표의 기초자료로써 활용이 가능할 것으로 판단된다.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. For higher concentration than 300mg/41 about glucose, a saturation state of abforbance was observed at 500, 550 and 570nm. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The model demonstrated the influence of wavelength on the thermal process and proved to be a valuable tool to investigate internal temperature rise. 30536 T00401030536 ^x We investigated the structural geometry of thirty-eight Korean femurs. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. Our department has developed an information system for routine hematology using down-sized computer system. We have used an IBM 486 compatible PC with 16MB main memory, 210 MB hard disk drive, 9 RS-232C port and 24 pin dot printer. The operating system and database management system were SCO UNIX and SCO foxbase, respectively. For program development, we used Xbase language provided by SCO foxbase. The C language was used for interface purpose. To make the system use friendly, pull-down menu was used. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer. To use as the structural matrix of the composite, collagen was purified from human umbilical cord. We propose a simple adaptive algorithm to increase capability of noise cancelation in QRS complex detection with two stage adaptive filter. At the first stage, background noise is removed and at the next stage, only spectrum of QRS complex components is passed.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer.
