ICD-10의 진단 기준에 새로이 포함된 과잉운동 품행장애(hyperkinetic conduct disorder)의 내용 타당도를 검증하기 위한 연구를 시행하였다. 이를 위한 구체적 방법으로, 주의력결핍 과잉 운동장애 환자군에서 품행장애가 공존한다는 기존의 연구 결과가 있으므로, 반대로 품행장애 환자군에서 주의력결핍 과잉운동장애가 공존하는 정도를 측정하였다. 대상군은 품행장애로 진단된 29명을 환자군으로 삼았고, 정상 대조군으로 41명을 비교하였으며, 각각에게는 Conners의 부모평가 척도의 DSM-III-R에 의한 부모 평가 척도를 시행하였고 결과는 다음과 같다. 1) 품행장애 환자군에서 대조군에 비하여 Conners의 부모 평가 척도와 DSM-III-R에 의한 부모 평가 척도에서 주의력 결핍 과잉운동장애의 정도가 의미 있게 높은 것으로 나타났다. 2) 품행장애 환자군에서 대조군에 비하여 주의력결핍 과잉운동장애의 공존율이 의미 있게 높았다. 이상의 결과로 미루어 품행장애 환자군에서 주의력결핍 과잉운동장애의 공존이 의미있게 높다는 것을 알 수 있었고, 따라서 과잉운동 품행장애의 내용 타당도가 인정되었다.
A prevalence study was carried out on 847 CMD patients who had visited the Department of Oral Medicine in Pusan National University from 1990 to 1993. To obtain the same type of information, all subjects were interviewed and examined clinically using a standardized examination form, The ratio of women to men was about 3:1 and all subjects were divided into acute and chronic groups on the basis of 6 months of duration. Diagnostic groups consisted of muscle disorder, joint disorder and muscle-joint disorder. As related to gender, duration and diagnosis subjective and objective symptoms in CMD were studied. The obtained results were as follows : 1. Muscle-joint disorder had the highest percent, followed by muscle disorder and joint disorder. 2. The most common reasons for CMD treatment were pain, joint noise and limited opening, while headache and neckache were relatively often reported as associated symptoms and dizziness, ringing in the ears also reported as secondary CNS excitatory effects. 3. Pain was more ofter seen in women, acute group and muscle-related disorder groups (p<0.05, p<0.01). Noise was significantly frequent in chronic group and joint-related groups (p<0.01). 4. Analysis of contributing factors presented that macrotrauma was found frequently in men (p<0.05), and that muscle-related groups were more related to stress than joint disorder grop (p<0.05). 5. Hard end feel was seen significantly often in joint-related disorder group (p<0.05). On the other hand, soft end feel was frequent in muscle disorder. 6. Reciprocal clicks and crepitation increased with chronicity. Subjects with joint-related disorder groups significantly often reported all kinds of noises (p<0.01). 7. Tender muscles and joints were more often reported in women and chronic group. Whereas muscle-related disorder groups revealed significantly more tender muscles (p<0.01). joint-related disorder groups presented significantly more tender joints (p<0.01).
Objectives : To investigate the validity of an abbreviated self-rated Korean version of MINI (Mini International Neuropsychiatric Interview) patient health survey which screening social anxiety disorder, panic disorder, generalized anxiety disorder, and major depressive disorder. Methods : 115 subjects completed MINI and MINI patient health survey. The validity of MINI patient health survey was assessed by whether the results from MINI patient health survey were compatible with the results from MINI or not. The Cohen's kappa value, specificity, sensitivity, positive predictive value, and negative predictive value was calculated for this purpose. Results : The Kappa values of social anxiety disorder (0.60), panic disorder (0.49), generalized anxiety disorder (0.60) and major depressive disorder without other co-morbid disorder (0.59) were at least moderate in strength of agreement. Conclusion : The abbreviated self-rated Korean version of MINI patient health survey has the moderate to good validity in social anxiety disorder, panic disorder, generalized anxiety disorder, and major depressive disorder without other co-morbid disorders. Our result suggests that this instrument might be useful for screening above 4 disorders if it is used under careful supervision of experienced clinicians.
Objectives:This study was performed to investigate the differences of the clinical feature between panic disorder with agoraphobic patients and panic disorder without agoraphobic patients. Methods:Two hundred nine patients meeting the criteria of DSM-IV panic disorder were recruited. One group was panic disorder with agoraphobia(n=78, 42 male(53.8%), mean age $37.6{\pm}9.9$ years), another was panic disorder without agoraphobia(n=131, 81 male(61.8%), mean age $40.5{\pm}10.3$ years). The numbers and frequency of panic symptoms were compared between two groups with t-test, and the logistic regression analysis were used for predicting panic disorder with agoraphobia. Results:The number of panic symptoms during panic attack was significantly higher in the group of panic disorder with agoraphobia than the group of panic disorder without agoraphobia(p<0.05).'Sweating','nausea or abdominal distress','fear of losing control of going crazy','chills or hot flushes'were more frequent in the group of panic disorder with agoraphobia(p<0.05). Among panic symptoms on logistic regression analysis,'sweating',' nausea or abdominal distress','fear of losing control or going crazy'turned out to correlate significantly with risk of development of agoraphobia in panic disorder. Conclusion:These results suggest that the frequency of some symptoms during panic attack may be a predictor of agoraphobia in patients with panic disorder.
Objectives Our study aimed to present the distinctive correlates of formal thought disorder in patients with schizophrenia, using the Clinical Language Disorder Rating Scale (CLANG). Methods We compared clinical characteristics between schizophrenia patients with (n = 84) and without (n = 82) formal thought disorder. Psychometric scales including the CLANG, the Brief Psychiatric Rating Scale (BPRS), the Young Mania Rating Scale (YMRS), the Calgery Depression Scale for Schizophrenia (CDSS) and the Word Fluency Test (WFT) were used. Results After adjusting the effects of age, sex and total scores on the BPRS, YMRS and WFT, the subjects with disorganized speech presented significantly higher score on the abnormal syntax (p = 0.009), lack of semantic association (p = 0.005), discourse failure (p < 0.0001), pragmatics disorder (p = 0.001), dysarthria (p < 0.0001), and paraphasic error (p = 0.005) items than those without formal thought disorder. With defining the mentioned item scores as covariates, binary logistic regression model predicted that discourse failure (adjusted odds ratio [aOR] = 5.88, p < 0.0001) and pragmatics disorder (aOR = 2.17, p = 0.04) were distinctive correlates of formal thought disorder in patients with schizophrenia. Conclusions This study conducted Clinician Rated Dimensions of Psychosis Symptom Severity (CRDPSS) and CLANG scales on 166 hospitalized schizophrenia patients to explore the sub-items of the CLANG scale independently related to formal thought disorders in schizophrenia patients. Discourse failure and pragmatics disorder might be used as the distinctive indexes for formal thought disorder in patients with schizophrenia.
상황 인식 환경에서 공황장애의 증상을 추출하고자 일상생활을 통해 자연스런 음직임에서 발생하는 신체 상황정보를 추출하여 공황장애의 구성요소를 찾아내고 온톨로지 이론을 적용하여 의미를 추론 과정을 거쳐 공황장애에 증상의 정도에 대해 정보를 제공할 수 있다. 온톨로지 기반의 정보 처리를 위해 공황장애의 구성요소는 클래스로 정의하고 공황장애의 상태를 이해할 수 있도록 온톨로지 모델링을 통해 공황장애지수를 도출하였다. 공황장애 구성요소와 공황장애 지수의 도출은 공황장애에 대한 상황 인식 기반의 정보 서비스를 가능할 것이며, 상황인식 기반 디바이스와 결합되어 상황정보는 주기적으로 동기화된다. 공황장애 지수의 결과를 관찰할 수 있으며 공황장애의 요인이 되는 생활습관을 개선하는데 적용할 수 있을 것이다.

Tic disorder is a childhood neuropsychological disorder characterized by abrupt, involuntary, and repetitive stereotyped muscle movement or vocal sound. Tourette's disorder shows a chronic prognosis, and can last for life if no treatment is applied. Although tic disorder has been known for ages, the underlying cause is still not well known. Non-pharmacological treatments have long been used for the tic disorder, but few clinical studies were conducted. However, the European Society for the Study of Tourette's Syndrome recently issued non-pharmacologic guidelines for treatment of tic disorders based on the research findings obtained so far. In addition, guidelines for non-pharmacologic evidence-based treatment were reported in Canada, North America. By synthesizing the newly reported foreign guidelines for treatment and review articles, the aim of this study is to investigate the non-pharmacologic therapies used for treatment of tic disorder or Tourette's disorder.
Objectives : By comparing the prevalence rates of suicide-related behaviors (suicide ideation, plan and attempt) between depressive disorder and anxiety disorder patients, we tried to find the characteristics of suicide-related behaviors in these patients. Methods : Four hundred-three patients participated in the study and the prevalence rates of suicide-related behaviors were investigated using Korean version of Mini International Neuropsychiatric Interview plus. Pearson Chi-Square test was used to find the association between depressive or anxiety disorders and suiciderelated behaviors. Results : Statistically significant differences were found between depressive disorder group and anxiety disorder group in terms of suicide ideation ($X^2$=6.173, df=1, p=0.013) and suicide attempt ($X^2$=8.008, df=1, p=0.005). We also found that patients in depressive disorder group were more likely to have suicide ideation (Odds Ratio=2.049, 95% Confidence Interval=1.155-3.635), and attempt suicide (Odds Ratio=4.970, 95% Confidence Interval=1.466-16.845) than patients in anxiety disorder group. Conclusion : These findings suggest that suicide ideation and suicide attempt rates are higher in depressive disorders than in anxiety disorders.
This study attempted to reveal the physiologic etiology or related factors associated with speech processing by comparing the compensation ability in speech motor control in children with and without articulation disorders. Subjects were 35 children with articulation disorder and 35 children without articulation disorder whose age ranged from 5 to 6 years. They were asked to rapidly repeat /$p^ha$/, /$t^ha$/, /$k^ha$/, /$p^hat^hak^ha$/ diadochokinetic movement while mandible was free and mandible was stabilized with bite block. The results showed that children with articulation disorder revealed significantly greater difference in elapsed time for diadochokinetic movement between mandible free and stabilized state compared to the without articulation disorder group. But the correlation between the percentage of consonants correct and the compensation ability in speech motor control in the articulation disorder group was irrelevant. These results point out to the fact that children with articulation disorder have poor compensation ability in speech motor control compared to the children without articulation disorder. On the other hand, the poor ability does not have any relation with the severity of articulation disorder. These results suggest either general or individual characteristics of children with articulation disorder.
Objectives: The purpose of this study was to investigate the prevalence of eating disorder high risk group and eating disorder among whom visit bariatric clinic for diet and its relationship with sex, ages and body measurement. Methods: 742 subjects who visit our clinic both at Seocho and Bundang from January to October in 2004 were surveyed by using EAT-26 to assess their eating attitude. Results: The prevalence of eating disorder high risk group was 17.9% and that of eating disorder was 3.2%. Mean score of EAT-26 was higher in females$(13.22{\pm}8.52)$ than males$(8.95{\pm}5.44)$ and was significantly higher in twenties than the thirties in females(p<0.05). Mean weight, BMI and % body fat were significantly lower in eating disorder high risk group than normal group(p<0.05). Conclusions: Eating disorder symptomatology was highly prevalent among whom visit bariatric clinic for diet.
