Extensive Tension Pneumocephalus Caused by Spinal Tapping in a Patient with Basal Skull Fracture and Pneumothorax

  • Tension pneumocephalus may follow a cerebrospinal fluid(CSF) leak communicating with extensive extradural air. However, it rarely occurs after diagnostic lumbar puncture, and its treatment and pathophysiology are uncertain. Tension pneumocephalus can develop even after diagnostic lumbar puncture in a special condition. This extremely rare condition and underlying pathophysiology will be presented and discussed. The authors report the case of a 44-year-old man with a basal skull fracture accompanied by pneumothorax necessitating chest tube suction drainage, who underwent an uneventful lumbar tapping that was complicated by postprocedural tension pneumocephalus resulting in an altered mental status. The patient was managed by burr hole trephination and saline infusion following chest tube disengagement. He recovered well with no neurologic deficits after the operation, and a follow-up computed tomography (CT) scan demonstrated that the pneumocephalus had completely resolved. Tension pneumocephalus is a rare but serious complication of lumbar puncture in patients with basal skull fractures accompanied by pneumothorax, which requires continuous chest tube drainage. Thus, when there is a need for lumbar tapping in these patients, it should be performed after the negative pressure is disengaged.

Guidelines for the Establishment and Vitalization Scheme on Facility of Nature Environmental Conservation and its Utilization (자연환경보전·이용시설 가이드라인 및 활성화방안)

  • Although the demand and necessity for the facilities of nature environmental conservation and its utilization have been increased recently, the local governments has been experiencing difficulties to launch the facilities since there are no confirmed principles or guidelines for the direction of the facility and distinctions from the ordinary parks. In this regard, the aim of this paper is to provide the overall framework for establishing the facility by equipping checklists from planning to operational stage. The stages are disengaged into 5 steps of siting feasibility, planning, preliminary environmental assessment and environmental impact assessment, design, construction, and operation. This paper also include the recommendations to vitalize the existing facilities, of which expansion of educational program development; utilization of various manpower; provision of appropriate settings; strengthened monitoring; and increased public relations. Furthermore, it is suggested to consider the strengthened local characteristics, reviewing management plan at the stage of applying a new facility establishment to the concerned authority, fostering experts, and expansion of the facilities through restoration of environmentally damaged areas for a new facility.

  • The safe operation and reliable maintenance of nuclear power plants is one of the most fundamental and important tasks. It is known that a loose part such as a disengaged and drifting metal inside of reactor coolant systems might lead to a serious damage because of their impact on the components of the coolant system. In order to estimate the impact position of a loose par, three accelerometers attached to the wall of the coolant system have been used. These accelerometers measure the vibration of the coolant system induced by loose part impact. In the conventional analysis system, the low pass filtered version of the vibration data was used for the estimation of the position of a loose part. It is often difficult to identify the initial point of the impact signal by using just a low passed time signal because the impact wave is dispersed during propagation into the sensor. In this paper, the impact signal is analysed by use of various time frequency methods including the short time Fourier transform(STFT), the wavelet transform, and the Wigner-Vill distribution for finding a convenient way to identify the starting point of a impact signal and their advantages and limits are discussed.

A Study of Conceptual Representations of the Self in Sohak ("소학"에 나타난 자아의 개념적 표상에 관한 연구)

  • The purpose of this study was to investigate conceptual representations of the self in Korean culture by analyzing normative conception of the self present in Sohak. The method of this study was the historical method and the material for analysis is 'Reverencing for the Self of Sohak, which was the textbook for childhood education and contained normative tasks that the society of the Chosen Dynasty period held for what people should do in their lives. According to the analysis, the Chosen Dynasty had interdependent conception of the self, which had strong emphasis on connected relationships, especially between self and parents, and regarded respect, rightness, and politeness as important attributes of the self, and expected reciprocal exchange of mutual support in social roles. Also, there were many instrumental and terminal goals in normative expectations of the self, which were toward harmonious interdependent relationships. Finally, the attributes of the self that were thought of as positive characteristics served collective interests rather than individualistic ones. In addition, the society of the Chosen dynasty regarded socially disengaged emotion such as pride, anger, and pleasure as negative ones.

A Study on the Conversion Time to Minimize of Transient Response during Inter-Conversion between Control Laws (제어법칙 간 상호 전환 시 과도응답 최소화를 위한 전환시간에 관한 연구)

  • The inter-conversion between different control laws in flight has a lot of risk. The SWM(Switching Mechanism) including logic and stand-by mode is designed to analyze the transient response of aircraft during inter-conversion between different control laws, based on the in-house software for non-real-time and real-time simulation. The SWM applies the fader logic of TFS(Transient Free Switch) to minimize the transient response of an aircraft during the inter-conversion, and applies the reset '0' type of the stand-by mode to prevent surface saturation due to integrator effect in the disengaged flight control law. The transition time is also important to minimize the objectionable transient response in the inter-conversion, as well as the transition control law design. This paper addresses the results of non-real-time simulation for the characteristics of transient response to different transition time to select the adequate transient time, and the real-time pilot evaluation, using SSWM(Software Switching Mechanism) and HSWM(Hardware Switching Mechanism), which is met for Level 1 flying qualities and assures safety of flight.

Discourse On the Male Body Represented In Fashion Advertisement (패션 광고에 표상된 남성 몸에 관한 담론)

  • In this study, the image of the male body represented in fashion advertisement is analyzed based on discourses on the male body. Fashion brand advertisements, which emphasized the images of the male body, were selected from two magazines: GQ, a men's magazine, and VOGUE, a representative women's magazine. The published dates of the selected images were from Feb. 2010 to Oct. 2012, and these images were used for the analysis. The study results of the discourse on the male body appearing in fashion advertisement based on the discussion of changing masculinity suggests the following 4 features: i) macho, powerful and muscular male representing the hegemonic manhood; ii) refined and decorated male representing the wealthy and disengaged figure of a successful businessman; iii) androgynous male represented by the deconstruction of masculinity and femininity embedded in gender; iv) as an aesthetic object, the male with sex role of changed from a subject to an ornament, whose body becomes the object of voyeuristic view. This study tried to grasp the ideal and modern masculinity, and in particular, attempted to offer suggestions in different approaches to the male body image depending on the consumer type in order to enhance the brand image. This new masculinity is thought to be a foundation on which the advertisement and products suitable for the demands of future customers can be produced.

  • The objective of the current research study is to examine those elements/factors that can reduce silence so that organizations can become more efficient, innovative, and adaptable by sharing knowledge and work-related problems. The purpose behind conducting this research was to explore the effects of emotional intelligence on silent behavior. The study also focuses on other individual negative aspects that can increase counterproductive behavior like silence and examines the effects of emotional intelligence on silence through moral disengagement. Data was collected from 400 employees using stratified sampling to ensure adequate representation of males and females. Data was collected from nurses and young doctors using the adopted measurement scale through a self-administered questionnaire. Since the sample included nursing staff and they usually are not well versed in English, the instrument was translated into Urdu. Data were analyzed using SEM to assess the direct and indirect effects of EI on employee silence. The result indicates that emotional intelligence has a positive impact on employee silence and moral disengagement. In contradiction to theory, the findings suggest that people with high emotional intelligence tend to get morally disengaged. As a result, they will remain silent and withhold information regarding work-related issues and problems.

Irreducible Elbow Dislocation Associated with Hill-Sachs-like Lesion over the Capitellum

  • Irreducible dislocation of the elbow is an uncommon event. We present the case of a posterolateral elbow dislocation after a fall injury in a 67-year-old woman. A closed reduction performed in the emergency department was unsuccessful since the limited passive range of motion resulted in difficulty to perform longitudinal traction and flexion. Computed tomography images showed that the posterolateral aspect of the capitellum was impacted by the tip of the coronoid process, thus appearing similar to the Hill-Sachs lesion in the humeral head. Subsequent open reduction of the elbow revealed the dislocation to be irreducible since the tip of the coronoid process had wedged into a triangular Hill-Sachs-like lesion in the capitellum. The joint was reduced by providing distal traction on the forearm, and main fragments were disengaged using digital pressure. At the 3-month follow-up, the patient reported no dislocations, and had an acceptable range of motion. Thus, we propose that to avoid iatrogenic injury to the joint or other nearby structures, irreducible dislocations should not be subjected to repeated manipulation.

An Upshift Improvement in the Quality of Forklift's Automatic Transmission by Learning Control (학습제어를 이용한 지게차 자동변속기 상향 변속품질 개선)

  • Recently, automatic transmissions caused a good improvement in the shift quality of a forklift. An advanced shift control algorithm, which was based on TCU firmware, was applied with embedded control technology and microcontrollers. In the clutch-to-clutch shifting, one friction element is released and the other friction element is activated. During this process, if the release and application timings are not synchronized, an overrun or tie-up occurs and ultimately leads to a shift shock. The TCU, which measures only the speed of the forklift, inevitably applies the open-loop shift control. In this situation, the speed ratio does not change during the clutch fill. The torque phase occurs until the clutch is disengaged. In this study, an offline shift logic of the learning control was proposed. It induced a synchronous shift when the learning control progressed. During this process, the reference current trajectory of the release clutch was corrected and applied to the next upshift. We considered the results of the overrun/tie-up characteristics of the upshift performed immediately before. The vehicle test proved that the deviation in shift quality, which was caused by the difference in the mechanical characteristics of the clutch, could be improved by the learning control.

Study on the Lubricant Flow Behaviors in the Wet Clutch Pack System of Dual Clutch Transmission (습식 DCT(Dual Clutch Transmission) 클러치 팩 내부에서의 체결 동작에 따른 변속기유 거동 연구)

  • This work studies the flow behaviors in the gap between the friction pad and separator in wet-clutch systems. The fluid volume of the lubricant is modeled using the entire system of wet-clutch pack of a dual clutch transmission that has larger outer radius of odd gear shifts and smaller inner radius of even gear shifts. Flow behaviors in the gap of the clutch pad are computed using the gear shift modes that consider the real relative velocities between the friction pad and separator. Flow behaviors in the gap of the disengaged clutch pad are mainly investigated for the wet-clutch system, whereas the engaged clutch pad is modeled with no fluid rate through the contacting surfaces. The developed hydrodynamic fluid pressures and velocity fields in the clutch pad gap are computed to obtain the relevant information for managing flow rates in wet-clutch packs under dual operating conditions during gear shifts. These hydrodynamic pressures and velocity fields are compared on the basis of each gear level and gap location, which is necessary to determine the effects of groove patterns on the friction pad. Shear stresses in the gap locations are also computed on the basis of the gear level for the inner and outer clutch pads. The computed results are compared and used for the design of cooling capacity against frictional heat generation in wet-clutch pack systems.
