중풍 변증 모델에 의한 진단 정확률과 예측률 비교 (Comparison of Diagnostic Accuracy and Prediction Rate for between two Syndrome Differentiation Diagnosis Models)

  • In spite of abundant clinical resources of stroke patients, the objective and logical data analyses or diagnostic systems were not established in oriental medicine. In the present study we tried to develop the statistical diagnostic tool discriminating the subtypes of oriental medicine diagnostic system, syndrome differentiation (SD). Discriminant analysis was carried out using clinical data collected from 1,478 stroke patients with the same subtypes diagnosed identically by two clinical experts with more than 3 year experiences. Numerical discriminant models were constructed using important 61 symptom and syndrome indices. Diagnostic accuracy and prediction rate of 5 SD subtypes: The overall diagnostic accuracy of 5 SD subtypes using 61 indices was 74.22%. According to subtypes, the diagnostic accuracy of "phlegm-dampness" was highest (82.84%), and followed by "qi-deficiency", "fire/heat", "static blood", and "yin-deficiency". On the other hand, the overall prediction rate was 67.12% and that of qi-deficiency was highest (73.75%). Diagnostic accuracy and prediction rate of 4 SD subtypes: The overall diagnostic accuracy and prediction rate of 4 SD subtypes except "static blood" were 75.06% and 71.63%, respectively. According to subtypes, the diagnostic accuracy and prediction rate was highest in the "phlegm-dampness" (82.84%) and qi-deficiency (81.69%), respectively. The statistical discriminant model of constructed using 4 SD subtypes, and 61 indices can be used in the field of oriental medicine contributing to the objectification of SD.

  • The diagnostic requirements were suggested and explained regarding the systems of differentiation of symptoms and signs in the third year study of standardization and unification of the terms and conditions used for diagnosis in oriental medicine. 탈북여성들에 대한 남한 사회의 '종족화된 낙인(ethnicized stigma)'과 탈북여성들의 공동체 형성 및 활동 (The Ethnicized Stigma against Women Escaped from North Korea and Their Community Building and Coping Strategies toward it in Contemporary South Korea)

  • 본 연구는 남한 사회에서 탈북여성들이 경험하는 차별과 낙인을 역사적 배경과 연관된 '종족화된 낙인(ethnicized stigma)'의 관점에서 살펴보고 북한이탈주민의 남한사회 적응을 위한 사회복지실천 방안을 모색하는데 목적이 있다. 이를 위해 탈북여성 8명, 전문가 4명(2명은 탈북여성)을 심층 면접하였다. 연구결과, 참여자들은 언어와 의사표현 등 소통의 어려움과 내국인으로의 불인정을 경험하였으며, 이로 인해 종족 정체성(ethnic identity)의 혼란을 겪는 것으로 나타났다. 주요 원인으로 북한이탈주민에 대한 선(先)경험과 잘못된 정보의 일반화, 오랜 분단의 역사와 공간적 거리감에서 초래된 종족성(ethnicity)의 부인 등이 작용하는 것으로 나타났다. 탈북여성들은 부정적 인식과 차별로 인한 부정적 정서와 외로움을 해소하고 적응하기 위해 그들만의 공동체를 구축하였으며, 이것은 정서적 도구적 지지 기능을 하는 것으로 나타났다. 그러나 공동체 내 구성원간의 갈등, 후원금과 자원배분의 문제, 공동체 경험의 부재, 북한체제에서 겪은 불신과 호상비판의 일상, 신변노출의 문제, 자유와 인권에 대한 몰이해 등으로 갈등과 해체의 위기를 경험하였다. 결론에서는 이와 같은 연구결과를 바탕으로 북한이탈주민의 공동체를 지속적으로 유지하면서 남북한 교류를 위한 모임과 프로그램을 확대할 필요성을 제언하였다.

갈륨 스캔을 이용한 폐암의 치료 후 섬유성 변성과 재발의 감별 (Discrimination of Postradiotherapy Lung Fibrosis from Recurrence by Gallium-67 Scan in Lung Cancer)

  • 폐암의 방사선 치료후 발생하는 치료부위의 섬유성 변성과 암종의 재발의 구별이 어려울때가 많으나 아직 이렇다 할 감별진단 방법이 없다. 경북대학교 병원 치료방사선과에서 1989년 1월부터 1990년 6월까지 방사선 치료를 받은 폐암 환자 중 섬유성 변성이나 재발의 가능성이 높은 22명을 대상으로 Gallium-67 스캔의 유용성을 연구하였다. 대상환자는 편평 세포암 17명, 소세포암이 3명, 선암 2명이었고, 편평 세포암은 약물치료를 하지 않았고, 소세포암은 CAV와 Vp-16및 cis-platin으로 병용치료 하였으며, 선암은 폐엽절제술을 시행하였다. 방사선 치료는 조직형과 병용치료의 양상에 따라 5~6주에 걸쳐 45~60 Gy를 투여하였으며, 치료후 정기적으로추적 검진하였다. 재발과 상관 없이 방사선 폐염이 발생한군에서는 모두 Gallium 축적을 보였고, 12명의 재발 혹은 잔여 종양이 있는 군에서는 11명 이 Gallium 축적을 보였다(92%), 무병상태의 10명 중 방사선 폐염을 가진 5명에서는 Gallium축적이 있었으나, 섬유성 변성만 있는 5명에서는 4명에서 Gallium이 축적되지 않았다(80\%). 섬유성 변성은 모두 치료 종료 후 8개월 이후에 임상적으로 출현되었다. 이 시기에 재발과 섬유성 변성의 구별이 용이하지 않고 치료방침에도 결정적 영향을 미치므로, 치료 후 1년이 지난 환자에서 Gallium-67 scan이 임상적으로 유용할 것으로 사료된다.

  • 세계 브랜드 축제는 지역의 정체성과 문화를 핵심요소로 하는 차별성과 독창성 그리고 지역민과 방문객이 함께 체험하고 공유할 수 있는 꺼리를 통하여 브랜드화에 성공하였다. 경쟁적이고 모방적인 축제보다는 지역의 차별화된 문화를 함유한 지역축제는 독창적인 브랜드를 창출하고, 이를 통한 문화적 경제적 경쟁력 확보 뿐 아니라, 지역의 문화와 정체성을 보존 발전시키는 최고의 지역 자산이 될 것이다. 축제 브랜드를 구축하기 위해서는 무엇보다도 차별화 전략이 최우선시 되어야 하며, 변별력 있는 지역문화의 발굴과 전개 그리고 감성의 시대에 가장 강력한 마케팅 수단으로 제시되고 있는 스토리텔링을 통한 브랜드 자산을 방문객에게 전달하여야 한다. 지역축제 스토리텔링은 창조 감성의 시대에 맞는 축제의 방향성과 패러다임의 핵심동인으로 지역 이미지 개선과 경제 활성화 등 중장기적인 지역발전 기반 구축과 차별화된 지역 문화 창출로 삶의 질을 향상시킬 수 있을 것이다. 지역의 문화적 경제적으로 가장 중요한 자원인 지역축제의 성공을 위해서는 일률적이고 단기적인 기획과 프로그램 구성, 관 중심의 일방적인 운영 등이 아닌 참여 공유 소통을 중시하는 스토리텔링 기법의 적용이 필수불가결하다고 판단된다.

Plasma Phosphoproteome and Differential Plasma Phosphoproteins with Opisthorchis Viverrini-Related Cholangiocarcinoma

  • This study was conducted to investigate the plasma phosphoproteome and differential plasma phosphoproteins in cases of of Opisthorchis viverrini (OV)-related cholangiocarcinoma (CCA). Plasma phosphoproteomes from CCA patients (10) and non-CCA subjects (5 each for healthy subjects and OV infection) were investigated using gel-based and solution-based LC-MS/MS. Phosphoproteins in plasma samples were enriched and analyzed by LC-MS/MS. STRAP, PANTHER, iPath, and MeV programs were applied for the identification of their functions, signaling and metabolic pathways; and for the discrimination of potential biomarkers in CCA patients and non-CCA subjects, respectively. A total of 90 and 60 plasma phosphoproteins were identified by gel-based and solution-based LC-MS/MS, respectively. Most of the phosphoproteins were cytosol proteins which play roles in several cellular processes, signaling pathways, and metabolic pathways (STRAP, PANTHER, and iPath analysis). The absence of serine/arginine repetitive matrix protein 3 (A6NNA2), tubulin tyrosine ligase-like family, member 6, and biorientation of chromosomes in cell division protein 1-like (Q8NFC6) in plasma phosphoprotein were identified as potential biomarkers for the differentiation of healthy subjects from patients with CCA and OV infection. To differentiate CCA from OV infection, the absence of both serine/threonine-protein phosphatase 2A 56 kDa regulatory subunit beta isoform and coiled-coil domain-containing protein 126 precursor (Q96EE4) were then applied. A combination of 5 phosphoproteins may new alternative choices for CCA diagnosis.

No Genetic Differentiation of Elaphe schrenckii Subspecies in Korea Based on 9 Microsatellite Loci

  • The Russian ratsnake, Elaphe schrenckii, is found in Russia, China, and Korea, and is considered to be an endangered species by the Ministry of Environment in South Korea. Due to habitat loss and use in oriental medicine, their population has been severely decimated. In South Korea, two subspecies of E. schrenckii has been defined according to body color: E. s. schrenckii (blackish) and E. s. anomala (yellow-brownish). Molecular genetic studies on Elaphe schrenckii are very scarce and the taxonomy of Elaphe schrenckii subspecies is uncertain. From the present study, we attempted to identify the genetic differences of these two subspecies using species-specific microsatellites developed from the genomic library of E. schrenckii. Nine polymorphic loci were tested on 19 individuals from E. s. schrenckii (n=10) and E. s. anomala (n=9) in South Korea. The mean number of alleles was 3.78 in E. s. schrenckii and 4.11 in E. s. anomala. The average expected heterozygosity was 0.542 and 0.511 in E. s. schrenckii and E. s. anomala, respectively. We found a lack of genetic structure between two subspecies ($F_{ST}=0.016$) and no genetic discrimination between two subspecies was found. Based on the present findings by microsatellites, two subspecies can be considered as one species, E. schrenckii. However, further investigations on taxonomical status using mitochondrial and nuclear DNA sequences need to be performed and morphological & ecological data should be revised. The genetic markers should benefit future studies of the endangered species of other Elaphe species for the study of genetic diversity and potential conservation management.

얼굴의 진단적인 가치에 대한 기초적 연구 (A basic study on the diagnostic values of facial color and shape)

  • For the purpose of the basic educated-establishment on the diagnostic methods of "facial color and shape which reflect human's spiritual essence and personality", we study on the diagnostic value and application of the human face. The study's domain is divided the form and color of human face. And the form and color of human face is respectively observed the diagnostic value and contents. The form of human face reflect plenty the information of the mankind, and the observation of the face is applied to the "Physiognomie" refering to the external features of humans. Therefore the diagnosis on the form of human face is the primary factor in the grouping of five-element human, the discrimination of the Sasang constitution, and the classification of Hyunsang type. The color of human face reflect the physical information of internal organs and the pathological change of disease, therefore we examine the region, character and grade of disease by the inspection of complexion including the changes of color and luster of the facial skin. The inspection on the color is also the primary factor in the grouping of five-element human, the classification of Hyunsang and the differentiation of syndromes. The value of the inspection of complexion including the changes of color and form of the face is widely known. In the future, we think, we need to study more about the theory of the diagnostic value and application of the human face.

Molecular differentiation of Russian wild ginseng using mitochondrial nad7 intron 3 region

  • Background: Cultivated ginseng is often introduced as a substitute and adulterant of Russian wild ginseng due to its lower cost or misidentification caused by similarity in appearance with wild ginseng. The aim of this study is to develop a simple and reliable method to differentiate Russian wild ginseng from cultivated ginseng. Methods: The mitochondrial NADH dehydrogenase subunit 7 (nad7) intron 3 regions of Russian wild ginseng and Chinese cultivated ginseng were analyzed. Based on the multiple sequence alignment result, a specific primer for Russian wild ginseng was designed by introducing additional mismatch and allele-specific polymerase chain reaction (PCR) was performed for identification of wild ginseng. Real-time allele-specific PCR with endpoint analysis was used for validation of the developed Russian wild ginseng single nucleotide polymorphism (SNP) marker. Results: An SNP site specific to Russian wild ginseng was exploited by multiple alignments of mitochondrial nad7 intron 3 regions of different ginseng samples. With the SNP-based specific primer, Russian wild ginseng was successfully discriminated from Chinese and Korean cultivated ginseng samples by allele-specific PCR. The reliability and specificity of the SNP marker was validated by checking 20 individuals of Russian wild ginseng samples with real-time allele-specific PCR assay. Conclusion: An effective DNA method for molecular discrimination of Russian wild ginseng from Chinese and Korean cultivated ginseng was developed. The established real-time allele-specific PCR was simple and reliable, and the present method should be a crucial complement of chemical analysis for authentication of Russian wild ginseng.

Insulin-like Growth Factor-1, IGF-binding Protein-3, C-peptide and Colorectal Cancer: a Case-control Study

  • Context: Insulin-like growth factor peptides play important roles in regulating cell growth, cell differentiation, and apoptosis, and have been demonstrated to promote the development of colorectal cancer (CRC). Objective: To examine the association of insulin-related biomarkers including insulin-like growth factor-1 (IGF-1), insulin-like growth factor binding protein-3 (IGFBP-3) and C-peptide with CRC risk and assess their relevance in predictive models. Materials and Methods: The odds ratios of colorectal cancer for serum levels of IGF-1, IGFBP-3 and C-peptide were estimated using unconditional logistic regression models in 100 colorectal cancer cases and 100 control subjects. Areas under the receiving curve (AUC) and integrated discrimination improvement (IDI) statistics were used to assess the discriminatory potential of the models. Results: Serum levels of IGF-1 and IGFBP-3 were negatively associated with colorectal cancer risk (OR=0.07, 95%CI: 0.03-0.16, P for trend <.01, OR=0.06, 95%CI: 0.03-0.15, P for trend <.01 respectively) and serum C-peptide was positively associated with risk of colorectal cancer (OR=4.38, 95%CI: 2.13-9.06, P for trend <.01). Compared to the risk model, prediction for the risk of colorectal cancer had substantially improved when all selected biomarkers IGF-1, IGFBP-3 and inverse value of C-peptide were simultaneously included inthe reference model [P for AUC improvement was 0.02 and the combined IDI reached 0.166% (95 % CI; 0.114-0.219)]. Conclusions: The results provide evidence for an association of insulin-related biomarkers with colorectal cancer risk and point to consideration as candidate predictor markers.
