SECONDARY REPAIR OF LATE ENOPHTHALMOS WITH CALVARIAL BONE GRAFTS AROUND INTRAORBITAL CONTENT -REPORT OF 2 CASES- (안와주위에 두개골이식을 통한 안구함몰의 이차적 성형재건 치험례)

  • In the midfacial fracture, the orbital region presents many additional complication unique to the orbit. Among them are ectropion, entropion, lid ptosis, injury to the lacrimal apparatus, diplopia or the late development of enophthalmos. The residual problem confronting the surgen is usually enophthalmos or diplopia. Enophthalmos becomes cosmetically obvious at 3mm and if more severe it can interfere with vision from obstruction by the orbital rim. In this clinical situation, many patients prefer the simpler intraorbital volume expansion to the more complex orbital osteotomy. In general, except in mild cases of enophthalmos, the procedure of choice is osteotomy and repositioning for zygoma fracture and volume augmentation for blow-out fracture. Late treatment is performed by volume augmentation based on the CT findings behind the axis of the globe. Inferiorly placed grafts elevate the globe, posterior superior grafts move the globe anterior and medially positioned grafts push the globe laterally. In this two cases, the patients who has stable orbitozygomatic rim, the use of calvarial bone grafts more than 3 areas around intraorbital content, we corrected late enophthalmos combined with diplopia. As result, the first patient had 2mm advance in exophthalmometric check with improvement of the diplopia gradually. The second patient had 1.5mm advance with correction of vertical ocular dystopia and cosmetically good results respectively.

A Case Report of a Patient with Diplopia and Ataxia Diagnosed as Claude's Syndrome Treated with Korean Medicine (클로드 증후군으로 진단된 복시와 운동실조 증상을 가진 환자의 한방 치료 증례 1례 보고)

  • Objective: The aim of this study was to report the effect of Korean medicine treatment on Claude's syndrome with ataxia and diplopia. Methods: We applied electroacupuncture, scalp acupuncture, and physical therapy to the patient. Diplopia was assessed with the distance between two images of one object, and ataxia was assessed with the Korean Version of the Scale for the Assessment and Rating of Ataxia (K-SARA). Results: After 21 days of treatment, diplopia, abnormal extraocular movement, and ataxia were improved on the basis of the K-SARA score. Conclusion: This case demonstrates the effectiveness of Korean medicine treatment in improving the symptoms of Claude's syndrome.

  • Objectives : The aim of this study is to report the effect of acupuncture and moxibustion treatment for patient with oculomotor nerve palsy. Methods : We treated a patient who had suffered from right. ptosis, diplopia, ophthalmoplegia and diagnosed with oculomotor nerve palsy with acupuncture(including pharmacopuncture, electroacupuncture), moxibustion, cupping therapy. Photographs of eye movement were used to evaluate the changes in limitation of eye movement. We also evaluated symptoms such as diploma, ptosis. Results : After treatment, patient's symptoms such as limitation of eye movement, diplopia, ptosis were improved. There are no adverse effects and no relapse after treatments. Conclusions : This case report suggests that acupuncture and moxibustion treatment can be effective for patient with limitation of eye movement, diplopia, ptosis in oculomotor nerve palsy.

  • The aim of this report is to describe improved symptom changes in eye movement disorders and dizziness of a patient with Miller Fisher syndrome after receiving combined Korean medicine treatment (CKMT). A 24-year-old male was diagnosed with MFS accompanied by eye movement disorder, diplopia, and dizziness. For 6 weeks, the patient received acupuncture, electro-acupuncture, pharmacopuncture, herbal medicine, and physical therapy treatment. Visual analogue scale were checked and other outcomes (the range of eye movement, the distance that diplopia appears, diplopia questionnaire) were measured. Following CKMT, the movement of both eyes improved in both the horizontal and vertical direction. Also for diplopia and dizziness, there was a decrease in the visual analogue scale and the distance that diplopia appeared. The present case report suggests that CKMT may have a role in treating eye movement disorders and dizziness in patient diagnosed with MFS.

Clinical Significance of Orbital Inferiomedial Blow Out Fracture (안와하내벽 파열골절의 임상적 의의)

  • Background: The incidence of blow out fractures is increasing and the techniques of diagnosis and treatment have been recently evolving. Despite its clinical significance, there has been no study on orbital inferiomedial blow out fractures. Therefore, this study was designed to investigate the clinical significance of treatment of orbital inferiomedial blow out fractures. Methods: A retrospective review of fifty-seven patients who could be followed up for at least 1 year after surgical reconstruction of pure inferiomedial blow out fracture was undertaken. The transconjunctival approach was performed in all cases. The onlay technique was used in 32 patients and the inlay/sheet method was used in 25 patients. We evaluated the clinical outcomes using the chi-square test. Results: In the group using the onlay technique, postoperative diplopia and enophthalmos were observed in 14 cases and 3 cases, respectively. Of these, 5 cases and 3 cases lasted for more than 6 months, respectively. In the group using the inlay/sheet method, postoperative diplopia was observed in 9 cases, but there were no cases of enophthalmos. Among the 9 diplopia cases, 4 lasted for more than 6 months. Conclusion: Postoperative diplopia and enophthalmos were increased after treatment of inferiomedial blow out fractures compared to isolated medial (0.6%, 0.3%) or inferior (1.8%, 0.6%) blow out fractures. Therefore, careful dissection is necessary not to injure the inferior oblique muscle to decrease the incidence of postoperative diplopia. Moreover, the inlay/sheet method is an effective option for reconstruction of inferiomedial blow out fractures.

  • This study aimed to report the improvement in a vascular oculomotor nerve palsy patient treated with Korean medicine. A 68-year-old man with vascular oculomotor nerve palsy was treated with acupuncture, pharmacopuncture, and an herbal medicine (Gangwhalyufung-tang). This case was measured and assessed by the numerical rating scale (NRS), EuroQol-5 Dimension Index (EQ-5D), score of diplopia, and ratio of MRD1. The NRS decreased, and the patient showed improvement in EQ-5D, score of diplopia, and ratio of MRD1. Korean medicine treatments are considered helpful in treating diplopia and ptosis due to vascular oculomotor nerve palsy.

  • This is a case report of correction of posttraumatic diplopia and enophthalmos using autogenous composite conchal cartilage and temporal fascia. The results are as follows. 1. Diplopia and enophthalmos may be persistent complications in zygomatic complex fracture. 2. The concha is a reservior of autogenous cartilage which leaves no cosmetic and functional deformity in reconstruction of orbital floor. 3. Warpping the cartilage in temporal fascia may serve to increase implant stability and get graft with smooth edge. 4. Donor incision lines are well hidden within the scalp and in auriculocephalic fold.

  • Although the inferior oblique (IO) muscle is positioned considerably deep in the orbit, transconjunctival lower lid blepharoplasty may affect it and transient or permanent IO muscle palsy might result. Therefore diplopia should be explained before cosmetic blepharoplasty performed with transconjunctival approach.

  • Objective: Improvement of myasthenia gravis through Ortho-Cellular Nutrition Therapy (OCNT). Methods: A 52-year-old patient with myasthenia gravis was prescribed 21 different OCNT treatments, including Cyaplex. Results: Following the administration of OCNT, symptoms such as eyelid ptosis, masticatory dysfunction, diplopia, and fatigue showed improvement. Conclusion: OCNT can assist in alleviating and mitigating symptoms for patients suffering from thymoma and myasthenia gravis.
