공공데이터를 활용한 3차원 공간정보 객체의 수직위치 정확도 분석 (An Analysis of Vertical Position Accuracy for the Three-Dimensional Spatial Data Object Utilizing the Public Information)

  • 최근 정부는 정부3.0이라는 새로운 정부운영의 패러다임으로 공공데이터를 적극적으로 개방하고 공유하는 정책을 펼치고 있다. 이와 함께 국토교통부에서는 국가공간정보 및 3차원건물, 영상지도를 포함한 다양한 콘텐츠를 일반인에게 제공하는 공간정보 오픈플랫폼 통합지도서비스(이하 브이월드)를 운영하고 있다. W3C재단의 오픈데이터 현황 보고서(2013)의 평가결과 우리나라는 정부의 정책적 지원 및 계획 부분에서는 긍정적인 결과를 나타낸 반면 데이터 관리 분야에서 취약하여 이에 대한 품질개선이 필요한 실정이다. 또한, 3차원 공간정보 객체 데이터(이하 3차원 모델)의 경우 데이터 구축에 활용한 디지털항공사진영상의 최신성이 부족하여 데이터의 현행화가 필요하다. 이에 본 논문에서는 3차원 모델의 품질요소인 수직위치 정확도에 대해 공공데이터이며, 실측 데이터인 건축물 대장의 높이 데이터를 기준으로 국내표준 품질평가 기법을 적용하여 데이터 품질을 측정하고, 측정오차에 대한 원인을 분석하여 공공데이터를 활용한 3차원 모델의 수직위치 정확도 향상 및 최신성의 유지 가능성을 제시하고자 한다. 연구 수행 결과 건축물대장의 높이 값을 품질평가 기준으로 적용하였을 경우 수직위치 정확도가 향상되는 것을 확인하였으며, 건축물 대장의 재건축, 증축 정보를 활용할 경우 3차원 모델의 최신성의 유지를 할 수 있음을 확인하였다.

Head motion during cone-beam computed tomography: Analysis of frequency and influence on image quality

  • Purpose: Image artifacts caused by patient motion cause problems in cone-beam computed tomography (CBCT) because they lead to distortion of the 3-dimensional reconstruction. This prospective study was performed to quantify patient movement during CBCT acquisition and its influence on image quality. Materials and Methods: In total, 412 patients receiving CBCT imaging were equipped with a wireless head sensor system that detected inertial, gyroscopic, and magnetometric movements with 6 dimensions of freedom. The type and amplitude of movements during CBCT acquisition were evaluated and image quality was rated in 7 different anatomical regions of interest. For continuous variables, significance was calculated using the Student t-test. A linear regression model was applied to identify associations of the type and extent of motion with image quality scores. Kappa statistics were used to assess intra- and inter-rater agreement. Chi-square testing was used to analyze the impact of age and sex on head movement. Results: All CBCT images were acquired in a 10-month period. In 24% of the investigations, movement was recorded (acceleration: >0.10 [m/s2]; angular velocity: >0.018 [°/s]). In all examined regions of interest, head motion during CBCT acquisition resulted in significant impairment of image quality (P<0.001). Movement in the horizontal and vertical axes was most relevant for image quality (R2>0.7). Conclusion: Relevant head motions during CBCT imaging were frequently detected, leading to image quality loss and potentially impairing diagnosis and therapy planning. The presented data illustrate the need for digital correction algorithms and hardware to minimize motion artefacts in CBCT imaging.

Cone-beam computed tomography를 이용한 미맹출 영구치의 계측 (ACCURACY OF CONE-BEAM COMPUTED TOMOGRAPHY IN PREDICTING THE DIAMETER OF UNERUPTED TEETH)

  • 본 연구는 CBCT(Cone-beam computed tomography)영상에서 미맹출 치아 크기 측정의 재현성과 정확성을 평가하기 위해 시행되었다. 매복치를 주소로 부산대학교 치과병원 소아치과에 내원한 혼합치열기 환자 중 진단 목적으로 CBCT 채득에 동의한 환자의 미맹출 견치 및 소구치 69개를 대상으로 하였다. CBCT 영상에서 측정한 미맹출 치아의 최대 근원심 폭경 계측치와 동일한 치아가 구강 내로 완전히 맹출한 후 채득한 석고 모형에서 대상 치아를 digital caliper로 측정한 최대 근원심 폭경 계측치를 비교하여 다음과 같은 결론을 얻었다. 1. CBCT 영상에서 미맹출 치아를 계측하는 방법은 재현성이 높다(ICC=0.91). 2. CBCT 영상에서 측정한 미맹출 치아의 최대 근원심 폭경 계측치와 석고 모형에서 측정한 대상 치아의 최대 근원심 폭경 계측치는 높은 상관 관계가 있었다(r=0.91). 3. CBCT 영상에서 측정한 미맹출 치아 최대 근원심 폭경 계측치와 석고 모형에서 측정한 대상 치아의 최대 근원심 폭경 계측치는 통계적으로 유의한 차이가 있었다($p$ <0.05). CBCT 영상에서 측정한 미맹출 치아 최대 근원심 폭경의 계측치가 석고 모형에서 측정한 대상 치아의 최대 근원심 폭경 계측치보다 평균 0.2 mm 작게 측정되었다. 그러나 이러한 차이는 임상적으로 수용 가능한 수준이라 생각된다.

완전무치악 환자에서 디지털 가이드 수술 방식을 이용한 무피판절개 임플란트 식립증례: 증례보고 및 5년 추적관찰 (Flapless implant placement with digital 3D imaging and planning system in fully edentulous patient: A case report and 5-year follow-up)

  • 디지털 방식을 이용한 치과 임플란트 진료에서 가장 주목받는 분야 중 하나가 디지털 소프트웨어와 컴퓨터 단층촬영을 이용하여 수술용 스텐트를 제작, 임플란트를 식립하는 분야이다. 컴퓨터를 이용한 디지털 임플란트 치료계획 시스템과 이를 이용한 수술용 가이드는 예지성과 심미성, 기능성을 갖춘 임플란트의 식립과 수복을 가능하게 한다. 특히 이러한 가이드 수술은 임플란트 수복물의 형태와 기능을 고려한 임플란트의 식립 시 더욱 유리하다. 최근 여러 제조사에서 보철물 제작을 위해 치과용 스캔부터 제작방법까지 다양한 디지털 방식의 제작과정을 제시하고 있다. 이러한 방법들은 정확하고 예지성 있는 예후를 위해 여러 연구자들에 의해 연구되어지고 있으며, 점차 발전하고 있다. 이번 증례는 완전무치악의 여성 환자에서 임플란트 고정성 보철물을 이용한 수복증례이다. 하악은 all-on-four 방식을 적용하여 4개의 임플란트를 식립하였으며, 상악은 이에 기초하여 6개의 임플란트를 식립하였다. 최종보철물에 기초한 임플란트의 식립을 위해 임플란트 수술용 스텐트를 이용하였으며, 환자는 치료 결과에 만족하였으며, 5년 추적관찰 결과, 임플란트와 보철물 모두 안정적으로 유지되고 있다.

적절한 전방 유도 재현을 위해 수정된 Dahl 원리 및 CAD/CAM 복제 기법을 이용하여 전치부의 기능 및 심미성을 개선한 보철 수복 증례 (Functional and esthetic improvement through reconstruction of anterior guidance using the modified Dahl principle and copy-milled technique of CAD/CAM system: A case report)

  • 상악 전치부와 같은 기능 및 심미성의 개선이 조화롭게 요구되는 부위의 치료 시에는 다른 부위보다 더 많은 지식과 기술을 필요로 한다. 특히 전방 유도(anterior guidance)를 결정하는 상악 전치부 설면 외형을 제대로 형성하지 못하면, 기능적인 불편감과 함께 전체 치열의 불안정성을 야기한다. 적절한 원리를 이용하여 전방유도를 설정한 후 임시 수복물 제작 및 조정을 통해 조화로운 전방 유도를 확보했다면 임시 수복물의 설면 외형을 최종 보철물로 정확하게 재현하는 방법에 대해 주의 깊게 고려해야 할 필요가 있다. 본 증례에서는 체계적인 진단 및 치료를 위하여 수정된 Dahl 원리(modified Dahl principle) 및 computer-aided design/computer-aided manufacturing (CAD/CAM) 시스템의 복제 기법(copy-milled)을 이용하여 적절한 전방 유도를 설정하고, 지대치의 디지털 이미지와 임시 수복물의 디지털 이미지를 중첩시켜 보다 정확하게 보철물 형태를 재현하고자 하였다. 이에 기능적, 심미적 개선을 도모하여 환자와 술자 모두에게 만족스러운 치료결과 및 예후를 얻었기에 보고하는 바이다.

자체 제작한 QA Phantom을 이용한 양성자 PPS (Patient Positioning System)의 정확성 평가 (Evaluation on the Accuracy of the PPS in the Proton Therapy System, Which Uses the Self Made QA Phantom)

  • 목 적: 양성자 치료는 DIPS (Digital Imaging Positioning System)를 이용하여 기준이 되는 DRR (Digital Reconstruction Radiography) 영상과 당일 치료 과정에서 얻은 X-ray 영상의 해부학적 구조를 비교하여 교정 값을 얻어 PPS (Patient Positioning System)를 교정 후 치료한다. PPS는 환자 셋업의 정확성을 위해 6방향(X축 Lateral, Y축 Longitudinal, Z축 Vertical, Rotation, Pitch, Roll)의 이동을 기반으로 움직인다. 따라서 DIPS의 교정 값과 PPS 움직임 구현성의 정확성 평가가 필요하다. 이에 자체 제작한 PPS QA 팬텀을 이용하여 PPS의 정확성을 평가하고자 한다. 대상 및 방법: 환자의 해부학적 구조를 대신 할 납 볼이 부착된 PPS QA 팬텀을 기준점에 셋업 한다. 임의의 수치 값으로 PPS 6 방향의 움직임을 만들어 PPS QA 팬텀을 이동시킨다. DIPS로 얻은 교정 값을 적용 후 기준점과 기준점으로 돌아온 PPS QA 팬텀과의 교정 값이 허용 범위 내에 들어오는지 평가한다. 결 과: 기준점에 위치한 PPS QA 팬텀의 X축, Y축, Z축의 3개의 좌표를 함께 1 cm에서 5 cm까지 1 cm씩 이동한 후 기준점과의 평균 교정 값은 PPS 6축에서 각각 0.04 cm, 0.026 cm, 0.022 cm, $0.22^{\circ}$, $0.24^{\circ}$, $0^{\circ}$이다. PPS QA 팬텀의 6방향의 이동 좌표계를 모두 1과 2만큼 이동 시켰을 때 평균 교정 값은 1 이동 시 0.06 cm, 0.01 cm, 0.02 cm, $0.1^{\circ}$, $0.3^{\circ}$, $0^{\circ}$이었고, 2만큼 이동 시에 0.02 cm, 0.04 cm, 0.01 cm, $0.3^{\circ}$, $0.5^{\circ}$, $0^{\circ}$이었다. 결 론: 교정 값을 평가한 결과 Lateral, Longitudinal, Vertical의 경우 허용 범위인 0.5 cm Rotation, Pitch, Roll의 경우 허용 범위 $1^{\circ}$ 안에 모두 들어오는 것을 알 수 있었다. 하지만 더욱 정확한 양성자 치료를 위해 DIPS의 이미지 질 개선과 DIPS 매칭 시스템의 발전 노력, 정기적인 장비 관리를 함으로써 현재 발생하는 기계적인 오차를 더욱 줄여나가야 하겠다.

갠트리 회전에 의한 온-보드 영상장치 회전중심점의 정도관리 프로그램 개발 (Development of Quality Assurance Program for the On-board Imager Isocenter Accuracy with Gantry Rotation)

  • 본 연구에서는 방사선치료용 선형가속기의 갠트리 회전에 따른 온-보드 영상장치(on-board imager, OBI)의 회전중심점의 위치 정확도 확인을 위해 제조사에서 제공된 고객인수시험절차서(customer accetpance procedure, CAP)상에서 명시된 방법을 비롯하여 OBI 선원 위치 $0^{\circ},\;90^{\circ},\;180^{\circ},\;270^{\circ}$에서 획득된 영상, 갠트리 각도 $10^{\circ}$ 간격으로 촬영된 영상, 콘빔 CT 재구성을 위한 미처리 투사영상 등에 디지털 영상처리 기법을 적용하여 자동으로 오차를 계산하는 새로 제안된 세 가지의 방법들을 각기 적용하여 그 오차를 평가하고 각 방법의 효용성에 대하여 검증하였다. 갠트리 회전에 따른 OBI 회전중심점의 오차 변화 양상 확인을 위해서는 $10^{\circ}$ 간격으로 영상 촬영 후 5차 다항식을 이용하여 조정함수(fitted function)를 구하는 방법이 적절하지만 정도관리 목적으로 최대 오차만을 구하고자 할 경우에는 $0^{\circ},\;90^{\circ},\;180^{\circ},\;270^{\circ}$ 등 네 방향에서 촬영된 영상을 이용하여 계산하는 것으로도 충분하였다. 각 방법을 적용하여 오차를 구한 결과 OBI 선원의 위치가 $90^{\circ}$부터 $180^{\circ}$ 사이일 경우 가장 크게 나타났으며 최대값은 0.44 mm였다. 또한 기간에 따른 OBI 회전중심점의 변화 양상은 최대 0.6 mm 이내로 안정적으로 유지되고 있음을 확인하였다. 본 연구에서 제안된 방법이 주기적인 정도관리에 적용된다면 간단하면서도 비교적 정확하게 평가를 수행할 수 있을 것으로 기대된다.

방사광 전파위상대조 동결미세단층촬영법을 활용한 3차원 조직학 (3D Histology Using the Synchrotron Radiation Propagation Phase Contrast Cryo-microCT)

  • 조직표본의 실제적인 3차원 구조에 대한 정보를 3차원 조직학이라고 하였다. 무른 성분들이 섞여 있고, 물을 포함 하고 있는 조직 내부의 미세구조의 3차원적 분석을 위해 방사광의 X선을 광원으로 하는 위상대조 미세단층 촬영이 활용되고 있다. 하지만, X선 위상대조영상 분석에서 물을 포함하고 있는 조직에서는 위상대조가 제대로 구현되지 않다는 것을 알게 되었다. 이러한 현상을 해결하기 위해 다양한 방법들을 적용하였으며, 표본을 얼렸을 때 위상대조가 강화된다는 사실을 확인하였다. 방사광 전파위상대조 동결미세단층촬영은 포항가속기연구소 X선 영상빔라인에서 수행하였다. 표본을 동결상태로 유지하면서 $0.18^{\circ}$ 간격으로 $180^{\circ}$ 회전하였으며, 표본을 통과한 X선에 의해 섬광기에 맺힌 영상을 광학렌즈로 확대하여 CCD카메라로 모았다. 각 표본 전체 투사영상을 OCTOPUS 소프트웨어로 재구성하여 2차원 단면영상으로 만들고, Amira 소프트웨어를 이용하여 3차원 영상으로 재구성하였으며, 단면영상에서 각 구조에 대한 구역화와 랜더링 작업을 수행하였다. 물에 의한 위상대조 방해 영향을 줄이기 위해 표본을 얼렸을 때 위상대조는 강화되었으나 동결팽창에 의한 조직변형이 관찰되었다. 표본을 막힌 공간에 넣고 주위를 포매제로 채워 급속냉동 동안 표본이 압박되도록 하였을 때 위상대조의 강화와 동결팽창에 의한 조직변형을 줄일 수 있었다. 결론적으로, 생체조직 내부 미세구조의 비파괴, 고해상도 3차원 영상분석에 있어 조직표본을 동결포매제로 포매 후 급속냉동하고, 방사광에서 방출되는 X선을 광원으로 하는 전파위상대조 동결미세단층촬영법은 효과적인 방법이 될 수 있을 것으로 기대한다.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. For higher concentration than 300mg/41 about glucose, a saturation state of abforbance was observed at 500, 550 and 570nm. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms.
