Comparative Effects of Novel Modified Sleeper and Cross-body Stretching on Scapular Anterior Tilting and Shoulder Internal Rotation in Subjects With Anterior Tilted Scapular and Shoulder Internal Rotation Deficits

  • Background: Posterior capsule tightness (PCT), commonly seen in overhead athletes, is a soft tissue adaptation that is also noted in non-throwers. PCT is associated with scapular and humeral kinematic alterations, significant restriction of shoulder internal rotation (IR) range of motion (ROM), and significant scapular anterior tilting. Sleeper and cross-body stretches (CBS) are suggested for PCT and IR deficits, and have been modified since introduction. A novel modified sleeper stretch (NMSS) was designed in this study to prevent the risk of anterior translation of the humeral head. Though the effects of posterior shoulder stretching exercise have been widely studies, to the best of our knowledge, no previous studies have investigated the effectiveness of posterior shoulder exercises in decreasing scapular anterior tilting. Objects: To compare the immediate effects of two posterior shoulder stretching exercises (NMSS and CBS) on scapular anterior tilting and shoulder IR ROM. Methods: Thirty-two subjects with anteriorly tilted scapula and IR deficits [mean age: 24.3 ± 2.5 years; 15 males and 17 females] participated in this study. Subjects were randomly assigned to either the NMSS or CBS groups. Scapular anterior tilting (at rest and at shoulder 60° active IR) and shoulder IR ROM were measured before and immediately after intervention. Results: Scapular anterior tilting significantly decreased, while the shoulder IR ROM significantly increased in both groups. However, there was no significant group-by-time interaction effect or significant difference between the groups. Conclusion: Both stretching exercises were effective in restoring shoulder IR ROM and decreasing scapular anterior tilting.

Effect of culture-promoting ingredients (CPI-107) on the culture of Mycobacterium tuberculosis (결핵균 배양에 대한 배양촉진물질(CPI-107)의 효과)

  • Mycobacterium tuberculosis complex (M. tuberculosis complex) is a causative agent of contagious chronic disease in a wide range of mammalian hosts, mainly cattle, goat, pigs, wildlife, and humans. The definite diagnosis of tuberculosis is made based on culture of M. tuberculosis, but it takes a long time. In the present study, we analyzed whether the detection time of M. tuberculosis could be reduced when cultured in the medium containing the culture-promoting ingredients-107 (CPI-107) using the BacT/Alert 3D system, an automatic culture system. The time to detection (TTD) tended to decrease as the added concentration of CPI-107 increase. In the case of low numbers of M. tuberculosis, it decreased by 21.0% at 1.2 mg/mL of CPI-107 and by 15.9% in the case of high numbers of M. tuberculosis. In the culture using clinically isolated M. tuberculosis strains, the shortening of the culture time by CPI was more evident. In conclusion, the detection time of M. tuberculosis was shortened in the medium added with CPI-107, and this could be used for isolation, culture and drug susceptibility test of M. tuberculosis.

Determinants of Intention to Take Korean Medicine: Focusing on the recognition of korean medicine treatment effects by disease (한약 복용 의향 결정요인 연구 : 질환별 한방 치료효과 인식을 중심으로)

  • The purpose of this study was to analyze the impact on people's willingness to take korean medicine based on the perception of korean medicine treatment effects by disease, and provide basic data when the herbal medicine (pharmacology) health insurance pilot project is carried out. The survey 'Usage and Consumption of Korean Medicine Report in 2017' was used, and frequency analysis, chi-square test, and logistic regression analysis were conducted on 3,032 people. Logistic regression results showed that the perception of herbal treatment effects on digestive diseases and arthritis was a significant factor in the future intention to take korean medicine. As the herbal medicine health insurance pilot project aims to popularize herbal medicine treatment through the management of the quality of korean medicine by incorporating it within the system, it is necessary to consider highly acceptable digestive diseases and arthritis when expanding target diseases.

Age-related Differences in Ankle-joint Proprioception and Postural Balance in Women: Proprioception of Force Versus Position

  • Background: During postural control, older adults are more dependent on proprioception than are young adults. Ankle proprioception, which plays an important role in maintaining postural balance, decreases with age. Published studies are insufficient to establish a significant age difference in postural sway resulting from the known age-related decrease in ankle proprioception and do not examine various detailed test conditions. Objects: The present study aimed to compare ankle proprioception between older and younger groups along dimensions of position vs. force proprioception and dorsiflexion vs. plantarflexion. The present study also aimed to compare postural sway between young and older women during quiet standing under two sensory conditions. Methods: We recruited seven young women aged 21-24 and seven older women aged 60-63. Ankle proprioception was assessed as the accuracy of the joint position sense (JPS) and the force sense (FS). Postural sway was assessed using center-of-pressure measurements recorded during quiet standing under two sensory positions: eyes open and eyes closed with head tilted back. Results: Older women had lower JPS in dorsiflexion and lower FS in plantarflexion than did younger women. We found no significant age differences in JPS in plantarflexion or in FS in dorsiflexion. We observed a main effect of group on postural sway in two sway parameters out of three. We observed significant differences in JPS with dorsiflexion, and in FS with plantarflexion. Conclusion: Proprioception for ankle plantar flexor decreased more significantly with aging than did that for ankle dorsiflexor, accounting for the impaired postural balance observed in older women.

Current Status of Work Performance and Support Plan for Public Health Doctors in the COVID-19 Quarantine (코로나19(COVID-19) 방역상황에서 공중보건의사의 업무 수행 현황과 지원방안)

  • The purpose of this study is to investigate the current status of work performance of public health doctors(PHDs) involved in quarantine of COVID-19, and to suggest a plan to support PHDs for effective national epidemic prevention and control in the future. As a result of the study, it was found that PHDs mainly performed sample collection, interview, and treatment. 39% of PHDs worked in places without negative pressure facilities, and personal protective equipment and welfare support were poor. In addition, it was investigated that they experienced high-risk infectious diseases, mental distress, exclusion from the decision-making process, conflicts with officials, problems with work guidelines, and lack of prior education. For effective infectious disease management, it is necessary to assign appropriate ranks and to participate in the decision-making process for quarantine, to specify appropriate compensation and regulations, to education, and to support mental health.

Development of an Automatic PCR System Combined with Magnetic Bead-based Viral RNA Concentration and Extraction

  • Human respiratory viral infections such as COVID-19 are highly contagious, so continuous management of airborne viruses is essential. In particular, indoor air monitoring is necessary because the risk of infection increases in poorly ventilated indoors. However, the current method of detecting airborne viruses requires a lot of time from sample collection to confirmation of results. In this study, we proposed a system that can monitor airborne viruses in real time to solve the deficiency of the present method. Air samples were collected in liquid form through a bio sampler, in which case the virus is present in low concentrations. To detect viruses from low-concentration samples, viral RNA was concentrated and extracted using silica-magnetic beads. RNA binds to silica under certain conditions, and by repeating this binding reaction, bulk samples collected from the air can be concentrated. After concentration and extraction, viral RNA is specifically detected through real-time qPCR (quantitative polymerase chain reaction). In addition, based on liquid handling technology, we have developed an automatic machine that automatically performs the entire testing process and can be easily used even by non-experts. To evaluate the system, we performed air sample collection and automated testing using bacteriophage MS2 as a model virus. As a result, the air-collected samples concentrated by 45 times then initial volume, and the detection sensitivity of PCR also confirmed a corresponding improvement.

Study on OCR Enhancement of Homomorphic Filtering with Adaptive Gamma Value

  • AI-OCR (Artificial Intelligence Optical Character Recognition) combines OCR technology with Artificial Intelligence to overcome limitations that required human intervention. To enhance the performance of AI-OCR, training on diverse data sets is essential. However, the recognition rate declines when image colors have similar brightness levels. To solve this issue, this study employs Homomorphic filtering as a preprocessing step to clearly differentiate color levels, thereby increasing text recognition rates. While Homomorphic filtering is ideal for text extraction because of its ability to adjust the high and low frequency components of an image separately using a gamma value, it has the downside of requiring manual adjustments to the gamma value. This research proposes a range for gamma threshold values based on tests involving image contrast, brightness, and entropy. Experimental results using the proposed range of gamma values in Homomorphic filtering suggest a high likelihood for effective AI-OCR performance.

Comparison of Serratus Anterior and Abdominal Muscle Activity During Push-up Plus Exercise With Hip Adduction and the Abdominal Drawing-in Maneuver

  • Background: The serratus anterior (SA) is a muscle that performs protraction of the scapulothoracic joint and plays a role in stabilizing the scapula. Imbalances or weaknesses in SA activation are associated with a variety of shoulder dysfunctions, making selective SA strengthening important for rehabilitation. Objects: We aimed to compare the muscle activation of the pectoralis major (PM), SA, external oblique (EO), and internal oblique (IO) during the push-up plus (PUP) exercise with isometric hip adduction (HA) and abdominal drawing-in maneuver (ADIM). Methods: Nineteen healthy male participants performed three PUP exercises: standard PUP, PUP with ADIM, and PUP with HA. Surface electromyography was used to measure and analyze the muscle activity for PM, SA, EO, and IO. Results: PUP with HA showed the lowest PM activity and highest SA activity, and no significant difference was observed between PUP and PUP with ADIM. PUP with ADIM showed significantly the highest EO and IO activity, followed by PUP with HA and PUP. Additionally, PUP with HA showed the lowest PM/SA ratio, and no significant difference was noted between PUP and PUP with ADIM. Conclusion: PUP with HA was able to show high SA muscle activity while reducing PM muscle activity. In addition, PUP with HA can lead to higher EO and IO muscle activity than standard PUP. This exercise could be used as a practical exercise method to selectively strengthen SA and improve scapular muscle stability during early shoulder rehabilitation.

Customer cultural trends are as follow as that 1) personalization of taste and experience, 2) spreading and sharing about personal use and experience, 3) consumption centered the value, 4) retreat and healthcare, 5) counter trend comes into the reaction about the forceful major trend. Contemporary customer changes essence of the life on the ground of emotion, symbol, image, and value not a commodity, function, and logos, and demands into change about direction of business to mostly enterprises. Customer's need and desire are on the increase as various emotion and value. Therefore, change of customer cultural trend is barometers for the growth and development of new fashion industry in $21^{st}$ century.

The Legal Protection of Digital Medical Imaging in U-healthcare (U-헬스케어에 있어서 디지털 의료영상정보의 법률적 보호)

  • 원격진료 홈네트워크 아파트 진료용 키오스크 모바일주치의 등으로 대표되는 U-헬스케어에 있어서 기초가 되는 것은 의료정보를 디지털화해서 전자적 자료의 형태로 저장 보관하고 이를 송 수신할 수 있는 기술이라고 할 수 있다. 우리나라의 경우, U-Korea 전략의 하나로 보건복지부가 주축이 되어 2005년 10월 현재 국가보건의료정보화계획(ISP)을 수립하기 위한 작업을 추진중에 있다. 여기서, 예컨대 임상병리검사소견이나 방사선촬영소견 등의 의료정보가 전자적 장치에 의해 디지털화 할 경우 디지털 의료정보가 되는 것이며, 이 가운데 특히 방사선촬영소견 등 방사선분야의 모든 촬영기록이 PACS시스템을 통해 기재되거나 저장 전송될 경우 이를 디지털 의료영상정보라고 할 수 있다. 그런데 오늘날 정보통신기술의 발달로 말미암아 디지털 의료영상정보를 포함한 디지털의료정보는 대량적으로 수집 저장되고 유통 내지 공동활용이 보편화되어 감에 따라 그 의료정보의 보호에 관한 문제가 중요한 이슈로 대두되고 있다. 결론적으로 말하자면, 이러한 디지털 의료영상정보가 전자의무기록(EMR) 형태로 저장 보관되는 경우 이는 전자의무기록에 관한 법률규정이 적용되어 법률적 보호를 받게 되며, 그 보호의 강도는 종래 오프라인 상의 의료정보 보호보다 한층 강화된 규정을 두고 있다. 이와 같은 흐름에 있어서 최근 정부가 국가보건의료정보화계획 수립과 함께 제정작업을 추진하고 있는 가칭 의료정보화촉진 및 개인정보보호에 관한 법률(안)은 시사점이 크다고 보기 때문에 소개하고자 한다.
