헬스케어 패러다임 변화에 따른 형사법적 쟁점과 과제 (Criminal Law Issues and Challenges Due to Changes in the Healthcare Paradigm)

  • 헬스케어 산업은 제4차 산업혁명 기반 기술과 접목된 디지털 헬스케어로 개인의 건강과 의료에 관한 정보 등을 다루는 분야로 건강관리 서비스와 의료 과학기술이 융합된 형태이다. 패러다임 변화에 따른 디지털 헬스케어는 기존 「의료법」상의 의료행위 개념에 포섭되어 논의가 가능한 것인지 의문이 생긴다. 「의료법」상 의료행위의 개념에 관한 명확한 정의 규정은 없지만, 판례를 통해서 그 개념을 정립하고 있다. 그리고 「의료법」상 의료행위의 주체는 의료인으로 한정하고 있다. 그러나 디지털 헬스케어는 의료인에 의 한 디지털 기술을 이용한 질병 진단과 치료행위를 하는 경우가 있다. 이와는 달리 비의료인에 의해서 가능한 것이 디지털 헬스케어이다. 왜냐하면 디지털 헬스케어는 운동, 식습관 그리고 체중조절 등과 같은 건강관리를 포함하는 개념으로 이해되기 때문이다. 이로 인하여 디지털 헬스케어에 대한 「의료법」상 의료행위 개념에 포섭하는 경우 「의료법」 제27조에 규정된 '무면허의료행위'로 형사처벌의 대상이 된다. 보건의료산업은 디지털 전환과 정보통신기술과의 융복합이 빠르게 진행되고 있다. 이로 인하여 기존의 의료행위와 구분하여 '디지털화된 의료행위' 또는 '정보통신기술(ICT) 기반 의료행위'로 새롭게 규정할 필요성이 대두되고 있다. 의료행위 개념은 고정불변한 개념이 아닌 가변성을 가진다. 그렇다고 이러한 요구에 따라 의료행위 개념의 무한 확장이 아닌 그 범위의 재설정을 요청하는 것이다. 따라서 의료서비스체계에 대한 수요자의 요구를 반영하여 의료행위 개념을 법제화하여야 할 것이다.

디지털 헬스케어 생태계 활성화를 위한 라이프로그 공통데이터 참조모델 (A Lifelog Common Data Reference Model for the Healthcare Ecosystem)

  • Healthcare lifelog, a personal record relating to disease treatment and healthcare, plays an important role in healthcare paradigm shifts in which medical and information technology converge. Healthcare services based on various healthcare lifelogs are being launched domestically by both large corporations and small and medium enterprises, however, they are being built on an individual platform that is dependent on each company. Therefore, the terms of lifelog data are different as well as the measurement specifications are not uniform. This study proposes a reference model for minimum common data required for sharing and utilization of healthcare lifelog. Literature study and expert survey derived 3 domain, 17 essential items, and 51 sub-items. The model provides definition, measurement data format, measurement method, and precautions for each detailed measurement item, and provides necessary guidelines for data and service design and construction for healthcare service. This study has its significance as a basic research supporting the activation of ecosystem by ensuring interoperability of data between heterogeneous healthcare devices linked to digital healthcare platform.

디지털 치료기기의 현황 및 개발 동향과 시사점 (Current Status, Development Trends and Implications of Digital Therapeutics (DTx))

  • As the demand for a healthy life increases and the use of information technology expands, interest in digital healthcare has increased. Among the digital healthcare technologies, digital therapeutics (DTx), which are capable of disease prevention, management, and treatment rather than simple healthcare, are expected to play a key role in future healthcare services. As interest in untact remote treatment that can minimize the risk of viral infection has rapidly increased since the spread of COVID-19, the application of DTx has received much attention because it can partially replace face-to-face treatment for mental illnesses, chronic diseases, and other diseases, reducing concerns about infection. In addition, because of the nature of software, DTx have lower toxicity and fewer side effects than existing treatments and do not require manufacturing, transportation, and storage like general medicines. Hence, they can be supplied in large quantities at low cost and have the advantage of lowering medical costs. However, despite these advantages, it has been pointed out that there are difficulties in investment and universal use because of the complexity of pricing and malpractice compensation. In other words, if it is difficult to prove and measure the improvements in disease management and treatment using DTx and it takes a considerable amount of time and money to do so, it will be difficult to attract investment from stakeholders such as medical providers and pharmaceutical companies. In this paper, we examine the domestic and global application status and development trends of DTx and determine the relevant implications.

인공 치아우식 발생 모델에서 디지털 방사선 공제술을 이용한 인접면 치아우식증의 진단 (Proximal caries detection using digital subtraction radiography in the artificial caries activity model)

  • Purpose: The purpose of the experiment was to evaluating the diagnostic ability of dental caries detection using digital subtraction in the artificial caries activity model. Materials and Methods: Digital radiographies of five teeth with 8 proximal surfaces were obtained by CCD sensor (Kodak RVG 6100 using a size #2). The digital radiographic images and subtraction images from artificial proximal caries were examined and interpreted. In this study, we proposed novel caries detection method which could diagnose the dental proximal caries from single digital radiographic image. Results: In artificial caries activity model, the range of lesional depth was $572-1,374{\mu}m$ and the range of lesional area was $36.95-138.52mm^2$. The lesional depth and the area were significantly increased with demineralization time (p<0.001). Furthermore, the proximal caries detection using digital subtraction radiography showed high detection rate compared to the proximal caries examination using simple digital radiograph. Conclusion: The results demonstrated that the digital subtraction radiography from single radiographic image of artificial caries was highly efficient in the detection of dental caries compared to the data from simple digital radiograph.

  • In the last decades patient's suffering with Parkinson's disease is increasing at a rapid rate and as per prediction it will grow more rapidly as old age population is increasing at a rapid rate through out the world. As the performance of wearable sensor based approach reached to a new height as well as powerful machine learning technique provides more accurate result these combination has been widely used for assessment of various neurological diseases. ON state is the state where the effect of medicine is present and OFF state the effect of medicine is reduced or not present at all. Classification of ON/OFF state for the Parkinson's disease is important because the patients could injure them self due to freezing of gait and gait related problems in the OFF state. in this paper wearable sensor based approach has been used to collect the data in ON and OFF state and machine learning techniques are used to automate the classification based on the gait pattern. Supervised machine learning techniques able to provide 97.6% accuracy while classifying the ON/OFF state.

A grid-line suppression technique based on the nonsubsampled contourlet transform in digital radiography

  • In radiography, an antiscatter grid is a well-known device for eliminating unexpected x-ray scatter. We investigate a new stationary grid artifact suppression method based on a nonsubsampled contourlet transform (NSCT) incorporated with Gaussian band-pass filtering. The proposed method has an advantage that extracts the Moiré components while minimizing the loss of image information and apply the prior information of Moiré component positions in multi-decomposition sub-band images. We implemented the proposed algorithm and performed a simulation and an experiment to demonstrate its viability. We did this experiment using an x-ray tube (M-113T, Varian, focal spot size: 0.1 mm), a flat-panel detector (ROSE-M Sensor, Aspenstate, pixel dimension: 3032 × 3800 pixels, pixel size: 0.076 mm), and carbon graphite-interspaced grids (JPI Healthcare, 18 cm × 24 cm, line density: 103 LP/inch and 150 LP/inch, ratio: 5:1, focal distance: 65 cm). Our results indicate that the proposed method successfully suppressed grid artifacts by reducing them without either reducing the spatial resolution or causing negative side effects. Consequently, we anticipate that the proposed method can improve image acquisition in a stationary grid x-ray system as well as in extended x-ray imaging.

디지털 덴탈 헬스케어 분야에서의 빅데이터 활용 전망에 대한 연구 (A study on the applications and prospects of big data in the field of digital dental healthcare)

  • Purpose: The purpose of this study is to investigate the applications and prospects of big data in digital dental healthcare. Methods: The study included 30 participants in the dental field (dentists, technicians, professors, and graduate students). From June 25 to 30, 2023, the contents of the study were thoroughly explained, consent was obtained from the research subjects, and a questionnaire was administered via an internet service. The questionnaires of 28 participants who responded completely were used for analysis. The collected data were statistically processed using IBM SPSS Statistics ver. 22.0 (IBM). Results: The use of big data in digital dental healthcare, digital dental health system, mobile dental health, dental health analysis, and telehealthcare were all heavily surveyed, with an average score of 3.97 or higher on a 5-point Likert scale. The areas where big data can be utilized in digital dental healthcare are as follows. The utilization rate for three-dimensional digital product development via linkage with big data systems and industrial field manufacturing technology was found to be 4.11±0.67, and the analysis of trends by age in the occurrence of various oral diseases was found to be 4.00±0.98. Conclusion: In the future, research into the viability of big data's success in the medical data field, which is directly related to human life, is needed. Additionally, social policies and regulations regarding big data-related information and standards in dental healthcare are necessary.

원격의료서비스에서 생체정보를 이용한 암호화키 생성방법 연구 (Process of the Encryption key using a Physical Information in the U-Healthcare Service)

  • 최근 세계가 고령화 사회로 진입함에 따라 U-Healthcare 서비스가 새롭게 각광받고 있다. 이러한 U-Healthcare 서비스가 발전하기 위해서는 U-Healthcare 환경에 최적화된 보안 솔루션이 요구된다. 그러나 U-Healthcare 환경은 기존 보안 솔루션의 적용이 어렵고 표준이 부재한 상황에 있다. 이러한 시점에서 안전한 U-Healthcare 환경을 구축하기 위해 데이터의 기밀성을 보장하는 목적으로 신체정보를 이용한 암호화키 생성방법을 제안하고자 한다.

  • 본 연구의 목적은 4차 산업혁명 시대에 요구되는 신기술의 이해와 복합적이고 실무적인 능력을 갖춘 창의융합형 인재 양성을 위하여, 디지털 헬스케어 분야에 디자인씽킹 방법론을 적용하여 캡스톤디자인 프로그램을 개발하는데 있다. 이를 위하여 디지털 헬스케어 분야, 캡스톤디자인 등의 문헌 및 선행 연구를 분석하고 디지털 헬스케어 분야의 캡스톤디자인 모델 및 적용 방안을 제안하였다. 전문가 검토를 통해 실효성을 확인하였으며, 캡스톤디자인 프로그램을 시범 적용하여 효과성을 분석하였다. 이는 디지털 헬스케어 분야에서 캡스톤디자인 적용 시 단계별로 활용할 수 있는 운영가이드로서 활용될 수 있다는데 의의가 있다.

Pulsed electromagnetic field potentiates etoposide-induced MCF-7 cell death

  • Etoposide is a chemotherapeutic medication used to treat various types of cancer, including breast cancer. It is established that pulsed electromagnetic field (PEMF) therapy can enhance the effects of anti-cancer chemotherapeutic agents. In this study, we investigated whether PEMFs influence the anti-cancer effects of etoposide in MCF-7 cells and determined the signal pathways affected by PEMFs. We observed that co-treatment with etoposide and PEMFs led to a decrease in viable cells compared with cells solely treated with etoposide. PEMFs elevated the etoposide-induced PARP cleavage and caspase-7/9 activation and enhanced the etoposide-induced down-regulation of survivin and up-regulation of Bax. PEMF also increased the etoposide-induced activation of DNA damage-related molecules. In addition, the reactive oxygen species (ROS) level was slightly elevated during etoposide treatment and significantly increased during co-treatment with etoposide and PEMF. Moreover, treatment with ROS scavenger restored the PEMF-induced decrease in cell viability in etoposide-treated MCF-7 cells. These results combined indicate that PEMFs enhance etoposide-induced cell death by increasing ROS induction-DNA damage-caspase-dependent apoptosis.
