  • This research is studied for investigative purposes of preparation status for healthcare telematics service enforcement via making an analysis of understanding & expectation effect about healthcare telematics introduction. The study is investigated with two groups, professional medical persons (doctors, nurses, pharmacists) and medical demanders (customers), to analyze the recognition difference between two groups. Questions are carried in face to face interviews by using structured questionnaire & Delphi technique. The survey result shows medical demander's expectation level is higher than the other's at all items such as social changes, medical service provider, medical service users, national and government agencies, medical system suppliers.

A Study on the Validity Test of Patient Classification System for Optimal Nursing Manpower of Hospital in China (중국 일 종합병원에서 적정 간호인력 추정을 위한 환자분류체계의 타당성 검증)

  • Purpose: This study was to setup the basis on hospital and national nursing manpower estimation accurately according to apply patient classification system of Song's study to China hospital system. Method: This study was surveyed to 964 patients at surgical and medical ward on Yanbian University Hospital in China from 17th to 31th January, 2005. Results: There was study results to test hypotheses for estimating optimal nursing manpower as follows. First, a trimodel classification scheme was developed which suggested three categories of patients as minimal care(category 1), moderate care(category 2), intensive care(category 3). Second, there was not significant difference with nursing time by sex. Third, there was not significant difference with nursing time by medical wards. Fourth, there was not significant difference with average nursing care time for each category of patients. Category 1 was estimated to spend average 19.59minutes for patients, Category 2 was about 35.68 minutes, Category 3 was 72.07minutes respectively. Total nursing hours was 62,610 minutes. Conclusion: Patient classification system of Song's study is validity for optimal nursing manpower of hospital in China.

  • The low dose radiation is done for a long period, thus researchers have to know the exact dose distribution for the irradiated mouse. This research has been conducted in order to find out methods in transmitting an exact dose to mouse in a mouse irradiation experiment carried out using $^{137}Cs$ irradiation equipment installed in the DIRAMS (Dongnam Institution of Radiological & Medical Sciences) research center. We developed a single mouse housing cage and shelf with adjustable geometric factors such as distance and angle from collimator. The measurement of irradiated dose showed a maximal 42% difference of absorbed dose from the desired dose in the conventional irradiation system, whereas only 6% difference of the absorbed dose was measured in the self-developed mouse apartment system. In addition, multi mice housing showed much difference of the absorbed dose in between head and body, compared to single mouse housing in the conventional irradiation system. This research may allow further research about biological effect assessment for the low dose irradiation using the self-developed mouse apartment to provide more exact doses which it tries to transmit, and to have more reliability for the biological analysis results.

  • The purpose of this study is to help the hearing impaired's speech training with a visible speech processing system. In brief, this system converts the features of speech signals into graphics on monitor, and adjusts the features of hearing impaired to normal ones. There are form ant and pitch in the features used for this system. They are extracted using the digital signal processing such as linear prediotive method or AMDF(Average Magnitude Difference Function). In order to effectively train for the hearing impaired's abnormal speech, easilly visible feature has been being studied.

Differences of Satisfaction between Patients and Hospital Employees with Medical Care Services in the General Hospital (환자와 병원종사자간의 의료서비스 만족도 차이분석 -종합병원을 중심으로-)

  • The aim of this study is to suggest theoretical foundations on which hospital employees could estimate medical care services not in the position of providers but consumers. For this purpose this study compares satisfaction of providers but consumers. For this purpose this study compares satisfaction of patients with medical care service with that of hospital employees. 287 patients and 261 employees in three university hospitals in Daegu were selected as samples of investigation and asked questions. Statistical analysis was taken using SAS package. The result of this study are as follows. 1) Among the five fields of medical services patients and hospital employees have different views in doctor hospital staff(except doctor) treatment system availability service but parking service 2) Hospital employees overestimate the level of medical care services compared with patients 3) Demographical variables make a difference in medical satisfaction 4) In satisfaction-evaluation after r ceiving treatment medical system service is influential variables in both patients and hospital employees. And in patient group doctor service is following variables while availability and parking service is influential variables in employees.

  • Purpose: To investigate the difference of fractional anisotropy (FA) values between 6- and 25-direction diffusion tensor images (DTI) in normal adult brain. Materials and Methods: DTI was peformed in 28 normal subjects (15 subjects with 6-direction, 13 subjects with 25-direction) in a 1.5 T MR system. DTI was done with SE-EPI sequence with TR/TE/NEX 10000/84/1, 5mm slice thickness and b=1000 s/mm2. FA values were measured from 8 different anatomical locations which included both cerebral peduncles, both posterior limbs of the internal capsules, both corona radiata, genu and splenium of the corpus callosum. Statistical difference of FA was tested between 6-and 25-direction DTI.

A Comparative Study on Medical Care Services in Two Community Health Sub-centers(Seo San-Gun, Choong Chung Nam-Do) (일선 2개 보건지소의 진료실적 비교 분석 (충남 서산군))

  • Several detailed aspects of medical care services during one year (Aug 1, $1982{\sim}$July 31, 1983) were compared between two community health sub-centers; one (A) was served by a local clinician, the other (B) by a trainee of preventive medicine. The results were analysed as follows: 1. Total spells of medical services during the same period in 'A' sub-center was 4,087 and that in 'B' 2,347. Medicaid visitors in 'A' was 1,051 (25.7% of the total), that in 'B' was 250 (10.7%). Significant difference was observed. 2. Average number of visits for the same case in 'A' was 2.0, that in 'B' was 1.4. Average duration of service by day for the same case in 'A' was 3.9, that in 'B' 3.0. 3. Average cost of service for a case in 'A' was 6,770 won, that in 'B' 4,230 won. 4. Difference in age distribution of visitors in the two sub-centers was insignificant. $0{\sim}9$ year group was 38.5% of the total. Difference in distribution by sex (between two sub-centers) was minimal. Male utilisers more than female by ca. 10%. 5. Concerning distribution of visitors by month, utilisation in July, August and September was the highest; the sum of them was 32.7% of the total in 'A' and 32.9% in 'B'. 6. Distribution by diagnoses in 'A' in decreasing order of frequency was Respiratory system diseases (26.1%), Digestive system diseases (23.0%), Diseases of skin and subcutaneous tissue (17.4%), Accidents and poisoning (11.5%) and Diseases of musculoskeletal system and connective tissue (5.4%). That in 'B' was Respiratory system diseases (39.0%), Digestive system diseases (23.0%), Diseases of skin and subcutaneous tissue (14.2%), Accidents and poisoning (10.9%) and Infection and parasitic diseases (9.1%). 7. The 10 most frequent diagnoses comprised 68.5% of the total in 'A' and 65.5% in 'B' 8. Utilisation by accessibility to each sub-center was studied. It is shown that the nearer the distance or the easier the accessibility, the more visits to services. The distinct pattern is manifested in the utilisation in 'A'-Myun.

  • The purpose of this study was to investigate user satisfaction at Defense Medical Information System as it has been utilized as a part of National Defense Information Strategies since Aug. 2000. The subjects of this study were nurse officers and army surgeons of a hospital with 1,000 beds. The results of this study are as follows; First, according to the characteristics of the subjects, there was statistically significant difference in system satisfaction, job satisfaction and information satisfaction at Military Hospital Information System. Second, according to the intention of participation, there was a statistically significant difference in user satisfaction at Military Hospital Information System. Third, according to a level of dexterity at Military Hospital Information System, there was statistically significant difference only in job satisfaction. Fourth, factors to influence system satisfaction were one's duty, using period of Military Hospital Information System, and a level of dexterity at Military Hospital Information System. Factors to influence job satisfaction were a level of dexterity at Military Hospital Information System, one's duty and intention of participation. Factors to influence information satisfaction were one's duty, a level of dexterity at Military Hospital Information System, and intention of participation. As the first study on user satisfaction at Military Hospital Information System, this study suggests the followings. First, a plan to inspire users with intention of participating in information system should be prepared. Second, it is necessary to provide policy activities for job standardization of Military Hospital Information System and legal system improvement.

An Analysis of Actual States of the Nursing Grade of Medical Institutions

  • Purpose. This study pursued the way for the effective application of the differentiated charge (Nursing grading system) by the nursing manpower which is performed for the nursing service quality improvement to the in-patients in Korea and the minimum employment problem solution of nurses. Methods. For this matter, the status of the nursing grade for 1,452 hospitals (44 high class general hospitals, 259 general hospitals, 265 hospitals, 59 oriental medicine hospitals and 825 recuperation hospitals) was identified which were registered in the Health Insurance Review and Assessment Service in March 2011 status quo. Results. In the most nursing grade by the kind of medical institutions, 70.5% of the nurses were third-graded in upper general hospitals, 38.1% were sixth graded in general hospitals, 62.7% were seventh-graded in oriental medicine hospitals and 40.4% were first-graded in recuperation hospitals. In the nursing grade by the scale of hospitals (in terms of the number of beds), there was a significant difference in general hospitals, but there was no significant difference between oriental medicine hospitals and recuperation hospitals. In the nursing grade by the location of hospitals and the foundation type of hospitals, there was a significant difference between general hospitals and recuperation hospitals. Conclusion. For the effectiveness of applying differentiated nursing fees by the number of nurses, it seems necessary to consider adjusting the present differentiated inpatient-charge system for the better so that small and medium-sized hospitals may induce more nurses.

Violence experiences, coping, and response of paramedics in the emergency room (응급실 내 1급 응급구조사의 폭력경험과 대처 및 반응)

  • Purpose: The purpose of this study was to investigate experiences of violence with patients or family members by paramedics working at emergency rooms. Methods: A questionnaire was administered from June 1 to 31, 2017 to 225 paramedics working at 27 emergency medical centers. The collected data were analyzed with SPSS statistics ver 24.0 program. Results: Within the past year, 208(92.9%) of 224 participants experienced violence among whom 202(90.2%) experienced verbal abuse, 193(86.2%) experienced physical threat, 89(39.7%) experienced physical violence, and 52(23.2%) experienced sexual violence. The level of violence response depending on the overlapping experience of violence type showed significant difference from emotional response (p= .001), social response (p= .001), physical response (p= .004), and overall violence response (p= .001). Conclusion: In conclusion, paramedics are frequently exposed to violence in the emergency rooms, of which they mostly experience verbal abuse. In addition, because the reporting system in the event of violence and the coping process are not well-informed, paramedics are unable to sufficiently utilize the reporting system and programs established within the institution. Therefore, the support of the legal system is needed to create a safe working environment for the medical staff who work in the emergency medical centers.
