Association between vitamin D intake and bone mineral density in Koreans aged ≥ 50 years: analysis of the 2009 Korea National Health and Nutrition Examination Survey using a newly established vitamin D database

  • BACKGROUND/OBJECTIVES: Vitamin D plays an important role in skeletal growth and maintenance and in the prevention of various diseases. We investigated the relationship between vitamin D intake and bone mineral density (BMD) in Korean adults aged ${\geq}50$ years using the 2009 Korea National Health and Nutrition Examination Survey data. SUBJECTS/METHODS: This study was conducted in 1,808 subjects aged ${\geq}50$ years with BMD data in Korea. Dietary vitamin D levels were assessed by the 24-hour recall method. BMD was measured using dual-energy X-ray absorptiometry. We investigated general characteristics and the association between these characteristics, vitamin D status, and BMD. RESULTS: Vitamin D intake was significantly lower in the osteoporosis group among women (P < 0.05). Among all subjects, the higher the serum 25(OH)D concentration, the higher the whole-body total BMD (WBT-BMD), femoral total hip BMD, and femoral neck BMD (P < 0.01). In the serum vitamin D-deficient group of both the total population and women, serum 25(OH)D concentration was associated with WBT-BMD (P < 0.05). Among women with a calcium intake < 537.74 mg/day, BMD of those with a vitamin D intake > $2.51{\mu}g/day$ (average intake of women) was higher than that of women with a vitamin D intake ${\leq}2.51{\mu}g/day$ (P < 0.001). CONCLUSIONS: Korean adults should increase their BMD by increasing serum 25(OH)D concentration. Furthermore, increasing vitamin D intake could improve BMD, especially in Korean women who consume less calcium than the estimated average requirement.

Comparison of nutrient intakes by nutritional anemia and the association between nutritional anemia and chronic diseases in Korean elderly: Based on the 2013-2015 Korea National Health and Nutrition Examination Survey Data

  • BACKGROUND/OBJECTIVES: The elderly are reported to have a high prevalence of nutritional anemia when they have lower intakes of nutrients or chronic diseases. This study was conducted to compare nutritional status according to nutritional anemia and to determine associations between nutritional anemia and chronic diseases in Korean elderly. SUBJECTS/METHODS: This study utilized data on 3,258 elderly aged ${\geq}65$ years gathered during the $6^{th}$ Korea National Health and Nutrition Examination Survey 2013-2015. Subjects were divided into nutritional anemia (NA) group (n = 415) and non-NA group (n = 2,843) by hemoglobin concentration. Nutrient intakes were assessed using dietary intake data obtained using the 24-hour recall method. The odds ratios (ORs) for nutritional anemia by chronic diseases were determined. Statistical analysis was performed using SPSS Ver. 23.0. RESULTS: Of 3,258 subjects, 12.7% had nutritional anemia. Intakes of potatoes, pulses, and mushrooms by males and potatoes, fruits, meats, eggs, and seafood by females were significantly lower in NA group than in non-NA group. The proportion of the subjects whose intakes of protein, vitamin A, vitamin $B_1$, vitamin $B_2$, niacin, and iron less than estimated average requirement (EAR) were significantly higher in NA group compared to non-NA group. After adjusting for age, the number of family members, energy intake, and alcohol drinking, ORs for nutritional anemia in the subjects with diabetes and myocardial infarction or angina pectoris were significantly higher by 1.74 times and 1.59 times as compared to the subjects without those diseases, respectively. However, ORs for nutritional anemia in the subjects with obesity, abdominal obesity, and hypertriglyceridemia were significantly lower by 0.64 times, 0.60 times, and 0.59 times as compared to the subjects without those diseases, respectively. CONCLUSIONS: The results of this study suggested that nutritional management should be done to enable the Korean elderly to consume foods with high hematopoietic nutrients density to prevent nutritional anemia. Korean elderly need to make regular efforts to check for nutritional anemia.

The strategies for the supplementation of vitamins and trace minerals in pig production: surveying major producers in China

  • Objective: Adequate vitamin and trace mineral intake for pigs are important to achieve satisfactory growth performance. There are no data available on the vitamin and trace mineral intake across pig producers in China. The purpose of this study was to investigate and describe the amount of vitamin and trace minerals used in Chinese pig diets. Methods: A 1-year survey of supplemented vitamin and trace minerals in pig diets was organized in China. A total of 69 producers were invited for the survey, which represents approximately 90% of the pig herd in China. Data were compiled by bodyweight stages to determine descriptive statistics. Nutrients were evaluated for vitamin A, vitamin D, vitamin E, vitamin K, thiamine, riboflavin, vitamin B6, vitamin B12, pantothenic acid, niacin, folic acid, biotin, choline, copper, iron, manganese, zinc, selenium, and iodine. Data were statistically analyzed by functions in Excel. Results: The results indicated variation for supplemented vitamin (vitamin A, vitamin D, vitamin E, vitamin K, vitamin B12, pantothenic acid, niacin, and choline) and trace minerals (copper, manganese, zinc, and iodine) in pig diets, but most vitamins and trace minerals were included at concentrations far above the total dietary requirement estimates reported by the National Research Council and the China's Feeding Standard of Swine. Conclusion: The levels of vitamin and trace mineral used in China's pig industry vary widely. Adding a high concentration for vitamin and trace mineral appears to be common practice in pig diets. This investigation provides a reference for supplementation rates of the vitamins and trace minerals in the China's pig industry.

한국인을 위한 신체활동분류표 개발: 미국의 신체활동목록 (Compendium of physical activities)을 이용하여 (Development of physical activity classification table for Koreans: using the Compendium of physical activities in the United States)

  • To set the estimated energy requirement (EER) in Dietary Reference Intakes for Koreans (KDRI), we need the coefficient by physical activity stage, as determined by the physical activity level(PAL). Thus, there has been demand for a tool to calculate PAL based on the physical activity diary. This study was undertaken to develop a physical activity (PA) classification table for Koreans, using the 2011 Compendium of physical activities in the United States. The PA classification table for Koreans contains 262 codes, and values of the metabolic equivalent of task (MET) for specific activities. Of these, 243 PAs which do not have Korean specific data or information, were selected from the 2011 Compendium of PAs that originated in the United States; another 19 PAs were selected from the previous research data of Koreans. The PA classification table is codified to facilitate the selection of energy values corresponding to each PA. The code for each PA consists of a single letter alphabet (activity category) and four numeric codes that display the activity type (2 digit number), activity intensity (1 digit number), and specific activities (1 digit number). In addition, the intensity (sedentary behavior, low, middle and high) of specific PA and its rate of energy expenditure in MET are presented together. The activity categories are divided into 4 areas: Daily Activity (A), Movement (B), Occupation (C), and Exercise and Sports (D). The developed PA classification table can be applied to quantify the energy cost of PA for adults in research or practice, and to assess energy expenditure and physical activity levels based on self-reported PA.

우리나라 노인의 체질량지수에 따른 영양소 섭취 수준과 건강 상태 비교: 저체중 노인을 중심으로 (Comparison of the Nutrient Intake and Health Status of Elderly Koreans According to their BMI Status: Focus on the Underweight Elderly Population)

  • Objectives: With an increase in the population of the elderly in Korea, their nutritional status has become a cause for concern. This study was designed to compare the nutritional intake and health status of the Korean elderly according to their body mass index. Methods: The subjects were 3,274 elderly people aged 65 and above who had participated in the 2016-2018 Korea National Health and Nutrition Examination Survey. The subjects were divided into four groups: underweight, normal, overweight, and obese, based on their BMI. The general characteristics, daily energy, and nutrient intakes, nutrient intakes compared to the recommended nutrient intake, percentage of participants whose nutrient intake was lower than the estimated average requirement (EAR), index of nutrient quality, the mean adequacy ratio (MAR), intakes by food group, and health status of the four groups were compared. Results: Underweight elderly people showed lower energy, lipids, dietary fiber, vitamin C, riboflavin, niacin, phosphorus, sodium, and potassium intake and MAR score (P < 0.001) compared to the normal or obese elderly. The mean protein, riboflavin, niacin, vitamin C, phosphorus, and iron intake of the underweight elderly was lower than the EAR (P < 0.05). Underweight elderly people also had a lower intake of vegetables and fats, oil and sweets food groups than the other groups (P < 0.001). The prevalence of diabetes and dyslipidemia was higher in the obese group, but the percentage of anemia was higher in the underweight group. Conclusions: Underweight elderly people were vulnerable to undernutrition and were at a higher risk of anemia.

한국 소아청소년을 위한 신체활동분류표: 미국의 청소년 신체활동목록 (Youth Compendium of Physical Activities)을 이용하여 (Physical activity classification table for Korean youth: using the Youth Compendium of Physical Activities in the United States)

  • The total energy expenditure (TEE) consists of the basal energy expenditure (BEE), physical activity energy expenditure (PAEE) and the thermic effect of food. The PAEE accounts for a significant portion of the TEE and can be changed according to individual efforts, and the difference between individuals of PAEE is large. Even for the same physical activity, there is a difference in energy expenditure between adults and children. Therefore, a physical activity classification table for youth is needed to classify the physical activity recorded in the physical activity diary prepared to evaluate children's energy expenditure. It is also necessary to calculate the physical activity level required to set the estimated energy requirement in the Dietary Reference Intakes for children and adolescents in Korea. This paper reports a physical activity classification table for Korean youth using the 2017 Youth Compendium of Physical Activities in the United States. This physical activity classification table includes 110 specific activities classified into 14 major categories by four age groups (6-9, 10-12, 13-15, and 16-18 years old) and their metabolic equivalent values. Of these, 87 physical activities were selected from the 2017 Youth Compendium reported in the United States. Nine physical activities such as washing and going to the bathroom, which are daily activities of children and adolescents not included among them, were selected from the another list (2008) of physical activities in America. The remaining 15 physical activities were selected from the research results, which measured the energy expenditure of Korean children and adolescents. Activity categories were divided into 4 areas: daily activity (A), movement (B), school work (C), exercise and sports (D). This physical activity classification table will help standardize the interpretation and scoring process of physical activity of youth in related studies and community health surveys.

Nutrient Utilization, Body Composition and Lactation Performance of First Lactation Bali Cows (Bos sondaicus) on Grass-Legume Based Diets

  • A study on energy and protein utilization, and milk production of Bali cows on grass-legume diets was carried out using 12 first lactation cows (initial BW $263.79{\pm}21.66kg$) during a period of 16 weeks starting immediately post calving. The animals were randomly allotted into 4 dietary treatment groups R1, R2, R3 and R4, receiving from the last 2 months of pregnancy onwards, graded improved rations based on a mixture of locally available grass and legume feed ad libitum. R1 contained on a DM basis 70% elephant grass (PP, Penisetum purpureum) plus 30% Gliricidia sepia leaves (GS), R2 was 30% PP plus 55% GS supplemented with 15% Hibiscus tilliactus leaves (HT, defaunating effect), R3 and R4 were 22.5% PP+41.25% GS+11.25% HT+25% concentrate, where R3 was not and R4 supplemented with zinc di-acetate. TDN, CP and zinc contents of the diets were 58.2%, 12.05% and 18.3 mg/kg respectively for R1, 65.05%, 16.9% and 25.6 mg/kg respectively for R2, 66.03%, 16.71% and 29.02 mg/kg respectively for R3 and 66.03%, 16.71% and 60.47 mg/kg respectively for R4. Milk production and body weight were monitored throughout the experimental period. In vivo body composition by the urea space technique validated by the body density method and supported by carcass data was estimated at the start and termination of the experiment. Nutrient balance and rumen performance characteristics were measured during a balance trial of 7 days during the 3rd and 4th week of the lactation period. Results indicated that quality of ration caused improvement of ruminal total VFA concentration, increments being 52 to 65% for R2, R3 and R4 above R1, with increments of acetate being less (31 to 48%) and propionate being proportionally more in comparison to total VFA increments. Similarly, ammonia concentrations increased to 5.24 to 7.07 mM, equivalent to 7.34 to 9.90 mg $NH_3-N/100ml$ rumen fluid. Results also indicated that feed quality did not affect DE and ME intakes, and heat production (HP), but increased GE, UE, energy in milk and total retained energy (RE total) in body tissues and milk. Intake-, digestible- and catabolized-protein, and retained-protein in body tissues and milk (Rprot) were all elevated increasing the quality of ration. Similar results were obtained for milk yield and components with mean values reaching 2.085 kg/d (R4) versus 0.92 kg/d (R1) for milk yield, and 170.22 g/d (R4) vs 71.69 g/d (R1), 105.74 g/d (R4) vs 45.35 g/d (R1), 101.34 g/d (R4) vs 46.36 g/d (R1) for milk-fat, -protein, and -lactose, respectively. Relatively high yields of milk production was maintained longer for R4 as compared to the other treatment groups. There were no significant effects on body mass and components due to lactation. From the relationship $RE_{total}$ (MJ/d)=12.79-0.373 ME (MJ/d); (r=0.73), it was found that $ME_{m}=0.53MJ/kgW^{0.75}.d$. Requirement of energy to support the production of milk, ranging from 0.5 to 3.0 kg/d, follows the equation: Milk Prod. ($Q_{mp}$, kg/d)=[-2.48+4.31 ME($MJ/kg^{0.75}.d$)]; (r=0.6) or $Q_{mp}$=-3.4+[0.08($ME-RE_{body\;tissue}$)]MJ/d]; (r=0.94). The requirement for protein intake for maintenance ($IP_m$) equals $6.19 g/kg^{0.75}.d$ derived from the relationship RP=-47.4+0.12 IP; (r=0.74, n=9). Equation for protein requirement for lactation is $Q_{nl}$=[($Q_{mp}$)(% protein in milk)($I_{mp}$)]/100, where $Q_{nl}$ is g protein required for lactation, $Q_{mp}$ is daily milk yield, Bali cow's milk-protein content av. 5.04%, and $I_{mp}$ is metabolic increment for milk production ($ME_{lakt}/ME_{m}=1.46$).

심각한 열스트레스에 의한 육성기 거세한우의 사료 소화율 변화 탐색 및 유지를 위한 에너지 요구량 설정 연구 (A Study on Changes in Feed Digestibility and Establishment of Energy Requirement for Maintenance of Growing Hanwoo Steers under Severe Heat Stress)

  • 우리는 심각한 열스트레스 및 서로 다른 사료 에너지 급여가 사료 소화율 변화에 미치는 영향과 심각한 열스트레스 조건에서 유지를 위한 에너지 요구량을 설정하기 위해 한우 육성기 거세우 4두(평균체중 200±11.7kg)를 이용하였다. 사료 처리는 서로 다른 에너지 공급을 위해 4 시험구(TDN 기준, 쾌적한 상태 하에서 한국사양표준 한우 육성우 에너지 요구량의 100%(대조구), 심각한 열스트레스 하에서 에너지 100%(E100), 115%(E115), 130%(E130)로 나누어 4×4 라틴 방각법으로 수행하였다. 온도 및 습도에 노출한 열스트레스 조건을 위해 대조구는 쾌적한 온습도지수(THI)=71.5, 나머지 3개 처리구는 THI=81.4로 심각한 스트레스 수준으로 설정하였다. 심각한 열스트레스는 진정 건물소화율을 유의적으로(p<0.05) 감소시켰으며(대조구 81.5 vs E100 79.1, E115 77.0 및 E130 76.0%), 사료에너지 공급 수준의 증가는 건물소화율의 감소를 가져오는 것으로 나타났다. 심각한 열스트레스 및 서로 다른 사료 에너지 급여 수준은 혈중 생리대사물질 함량 및 체온 변화에 유의적인 영향을 미치진 않았다(p>0.05). 본 연구를 통해 심각한 열스트레스 하에서 육성기 한우거세우의 유지를 위한 에너지 요구량은 Y=0.235X+115.03과 같은 산정식으로 계산될 수 있으며, 이러한 결과는 육성기 한우거세우에게 열스트레스 하에서 체중변화없는 유지 에너지 요구량을 만족시키기 위해서는 현재 한국사양표준 대비, 사료에너지 15.03%를 더 공급해야 할 것으로 판단된다.

한국인 상용 식품의 요오드 데이터베이스 업데이트와 이를 활용한 한국 성인의 요오드 섭취량 및 배설량 평가: 2013-2015 국민건강영양조사자료를 이용하여 (Revision of an iodine database for Korean foods and evaluation of dietary iodine and urinary iodine in Korean adults using 2013-2015 Korea National Health and Nutrition Examination Survey)

  • 본 연구는 한국인의 상용 식품에 대한 요오드 데이터베이스를 개정하고, 버전을 달리하여 한국인의 요오드 섭취수준을 평가하였으며, 배설량을 이용하여 섭취량과의 상관성을 살펴보았다. 요오드 데이터베이스는 기존의 Han 등 [2]의 데이터베이스를 기초로 최신의 참고문헌들을 활용하여 개정하였다. 요오드 데이터베이스 구축은 상용 식품 목록에 분석값, 문헌값, 대체값, 결측값 등의 우선순위 순서로 요오드값을 적용하였고, 분석값은 총 166개 (19.4%), 문헌값 318개 (37.2%), 대체값 총 247개 (28.9%), 결측값 124개 (14.5%)이었다. 대체값은 레시피나 수분함량을 고려하여 계산하거나 유사식품들의 일반값을 생성하여 사용하였고, 일반값 생성 시 평균값과 최솟값을 선정하여 버전 1과 2로 구분하였다. 한국 성인의 요오드 섭취 수준 평가는 제6기 국건영을 활용하였고, 요오드 섭취량의 중앙값은 전체의 경우 352.1 ㎍/일이었고 남자는 400.8 ㎍/일이며, 여자는 389.0 ㎍/일이었다. 이는 버전 2과 비교하여 조금 높았지만 큰 차이는 보이지 않았다. 전체 요오드 섭취량에 대한 식품군별 기여도의 경우 해조류 55.7%, 절임 채소류 16.0%, 우유 및 유제품류 6.9%이었고 버전 2에서도 순서는 동일하였다. 해조류 섭취 여부에 따라 해조류 섭취군과 미섭취군으로 나누어 평가한 결과, 해조류 섭취군의 요오드 섭취량의 중앙값은 495.7 ㎍으로 미섭취군의 241.2 ㎍보다 약 2배 이상 높았다. 해조류의 분석값 차이에 따른 요오드 섭취량의 차이를 살펴보기 위해 세부 버전을 사용하였고, 버전 1_1을 이용하였을 때의 요오드 섭취량의 중앙값은 478.1 ㎍으로 버전 1의 요오드 섭취량의 중앙값은 495.7 ㎍에 비해 낮았다. 요오드 섭취수준 평가는 해조류 미섭취군에서는 평균 필요량 (95 ㎍/일) 미만으로 섭취하는 비율이 남자 8.8%, 여자 8.1%로 높게 나왔고, 해조류 섭취군에서는 상한섭취량 (2,400 ㎍/일) 초과하여 섭취하는 비율이 남자 8.0%, 여자 6.3%로 높게 나타났다. 요오드 섭취량과 배설량 간의 상관성을 소변 요오드 배설량과 소변 속 크레아티닌의 비율을 이용하였고 섭취량과의 회귀계수는 버전 1은 0.11718, 버전 2는 0.11512이었고 모두 유의적 상관성을 나타내었다 (p < 0.0001). 이상의 결과에서 요오드 데이터베이스를 개정하고 데이터베이스 버전에 따라 한국의 요오드 섭취수준을 파악하였고, 해조류 섭취가 한국인의 주요한 급원 식품이었으며, 해조류 섭취 시 요오드 섭취의 과잉이, 해조류 미섭취 시 요오드 섭취의 부족이 나타날 수 있음을 제시하였다. 본 연구는 향후 요오드 데이터베이스의 지속적인 개정과 관리에 기초자료가 될 것으로 사료되고, 해조류 섭취 유무에 따른 요오드 섭취의 교육자료나 영양 프로그램 자료에 활용될 수 있을 것이며, 요오드 섭취와 연관성이 높은 갑상선 관련 질환과의 연구에도 활용될 수 있을 것으로 생각된다.

생화학적(生化學的) 측정방법(測定方法)에 의한 우리나라 여대생(女大生)들의 리보플라빈 영양상태(營養狀態)에 관한 연구(硏究) (A Study on the Riboflavin Nutritional Status by Biochemical Tests in Healthy Female College Students in Korea)

  • 본 연구는 우리나라 여대생들의 riboflavin (RF) 영양상태를 평가하기 위하여 서울거주 여대생 48명 (정상식이를 하는 학생 31명과 lacto-ovo vegetarian 인 학생 17명)을 대상으로 식이섭취조사, 임상검사, EGRAC 검사 및 소변검사를 실시하여 다음과 같은 결과를 얻었다. 1. 본 조사 대상 학생들은 동연령층에 대한 한국인 영양권장량에 비하여 열량섭취량은 낮고, 단백질 섭취량은 비슷하나 리보플라빈 섭취량은 높았다. 채식군의 학생들은 정상식이군에 비하여 열량과 단백질 섭취량은 낮았으나 리보플라빈 섭취량은 정상식이군과 비슷하였다. 2. 전 대상자들의 평균 EGRAC 값은 $1.24{\pm}0.03$이었으며, 정상식이군은 $1.19{\pm}0.04$, 채식군은 $1.32{\pm}0.06$으로 두 군의 평균값은 0.05수준에서 유의적 차이가 없었다. ERGAC값이 1.2이상인 대상자는 전 대상자 중 65%이었는데 정상식이군은 55%, 채식군은 82%이었다. 3. EGRAC값이 1.2이상인 대상자 중 18명에게 1일 5mg의 리보플라빈을 1주일간 투여한 후 재검사한 결과 모두 1.2이하로 감소하였다. 4. 상관분석 결과, 대상자들의 EGRAC값과 리보플라빈식이 섭취량 사이에는 유의적인 상관관계가 없었다. 이와 같은 결과가 나온 가능한 이유들에 대하여 고찰하였다. 이러한 결과들로 부터 본 조사 대상자들은 리보플라빈 섭취량은 한국인 영양권장량보다 높으나 생화학적 기능의 손상이 비교적 높은 비율로 존재함을 알 수 있다. 따라서 리보플라빈 식이섭취량이 낮은것으로 보고된 다른 집단들에 대하여 좀 더 집중적인 연구가 요청된다. 아직 우리나라 사람들을 대상으로 리보플라빈 식이섭취와 생화학적 결핍에 대하여 연구된 바가 적으므로 리보플라빈의 생화학적 결핍판정과 한국인의 RDA수준에 대하여 더 많은 연구가 요청된다.
