Causal relationship among quality factors, emotional responses, and satisfaction of school food service in Henan province, China

  • BACKGROUND/OBJECTIVES: School food service has played an important role in promoting the health and physical condition of students by providing students with a balanced and nutritious diet. Therefore, boosting the quality of school food service and improving the students' satisfaction is critical. For this purpose, this study examined the structural causal relationship among the quality of school food service factors, emotional responses, and satisfaction in China. SUBJECTS/METHODS: This study was conducted with 4th-6th-grade students from 6 junior high schools in Henan province of China, with 590 questionnaire responses (87.3%) collected and statistically analyzed. RESULTS: The school food service quality factors (including menu management, dietary education, facilities management, price and food distribution management, and personal hygiene during meals) must be enhanced to boost the students' satisfaction. In addition, the study used questionnaire survey data to validate the full mediation of students' emotional responses between school food service quality factors and student satisfaction. CONCLUSIONS: Students' emotions also play an important role in influencing the quality of school food service, all of which affect the emotional responses of students. Therefore, students' positive emotions are an important indicator for improving the quality of school food service. A national support policy is necessary for the ongoing maintenance and development of various programs that drive students' satisfaction and promote the adoption of education guidelines for school food service in China.

양평군 노인에서 우울 지수, 영양불량과 식생활 실천 요인 (Dietary Habits and Factors Associated with Depression in Yangpyeong-gun Elderly)

  • This study was conducted to analyze relationships between depression indices, mini nutritional assessment scores, and nutritional quotients among 80 elderly in Yangpyeong-gun and to identify factors that help prevent depression and malnutrition. Nutrition assessment scores were low in the high-risk group (PHQ-9 score ≥10), and nutritional quotient scores were lower in the high-risk group than in the normal group (PHQ-9 score ≤4). Interestingly, the consumption frequencies of fruits, eggs, and nuts were low in the high-risk group, and subjective health awareness, dental condition, and sleep were poorer. The total PHQ-9 score was correlated with malnutrition, body mass index, calf circumference, weight change, independent daily living, reduced meal amount, water intake, and the need for help when eating. Analysis of correlations between items of the PHQ-9 and nutritional status evaluation indices showed that a self-perceived feeling of depression, low energy, difficulty controlling sleep or appetite, negative thoughts (e.g., failure, disappointment), and difficulty concentrating were negatively correlated with total nutritional status scores. These results show that attention is required when food or water intake decreases and that deviation from normal sleep and appetite cycles flags the need to prepare guidelines to prevent depression.

채소 및 과일의 섭취가 백내장 유병율에 미치는 영향 : 2012년 국민건강영양조사 자료를 이용하여 (Intake of fruits and vegetables may modify the risk of cataract in Korean males: data from Korean National Health and Nutrition Examination Survey 2012)

  • 본 연구는 국민건강영양조사 자료 중 안검진이 가장 처음 진행된 2012년 자료를 바탕으로 남성 1,332명 (환자 222명) 및 여성 2,012명 (환자 337명)에서 나타난 채소 및 과일류 섭취 종류와 수준이 백내장에 미치는 영향을 분석하였다. 남자 대상자의 낮은 TVF, TV, NSV, NSVF 섭취량은 백내장 위험을 약 1.4 ~ 1.7배 증가시키는 것으로 나타났으나, 여자 대상자에서는 식이섭취와 백내장 위험도의 사이에 의미있는 상관성이 관찰되지 않았다. 본 연구결과는 한국인 남성의 백내장 위험률 감소를 위해 충분한 채소와 과일을 섭취이 권장되며, 이를 위해 다양한 보건 영양 정책의 개발과 효과적 시행이 필요함을 시사한다.

초등학교 아동의 혈청 지질 상태 및 지방 섭취에 관한 연구 (A Study on Serum Lipid Levels and Dietary Fat and Fatty Acid Intakes in Primary School Children)

  • The purpose of this study was no investigate serum lipid levels of primary school children and to estimate their intakes of total fat and fatty acids. Subjects fasting blood samples were obtained and analyzed for serum triglycerides(TG), total cholesterol(Chol) and high density lipoprotein-cholesterol (HDL-Chol). Low density lipoprotein cholesterol (LDL-Chol), LDL-Chol/HDL-Chol ratio(LPH), and atherogenic index(AI) were calculated,. Dietary intake of nutrients was assessed by means of a 24-hour recall method using food models and other measuring tools . The serum levels of TG, Chol and LDL-Chol in girls were higher than those in boys, but the serum HDL-Chol level of girls was lower than that of boys. As the degree of obesity increased, the serum TG level of girls was lower than that of boys. As the degree of oesity increased , the serum TG level of girls increased. The serum LDL-Chol level was higher in obese boys than in normal ones. Percentage s of subjects at risk of cardiovascular disease based on corresponding criteria of TG, Chol,HDL-CHol and LDL-Chol were 25.9%, 7.6%, 20.7% and 10.1%, respectively. The serum TG level of children provided with the national school lunch program(NSLP) was lower than that of children without NLSP. The total fat intake of boys was higher than that of girls, but calorie-adjusted fat intake became similar between the two groups. Intakes of polyunsaturated fatty acids(PUFA), monounsaturated fatty acids(MUFA) and saturated fatty acids(SFA) were 13.3$\pm$9.5g, 16.1$\pm$9.3g and 21.4$\pm$14.2g in boys, respecitively , and 12.6$\pm$11.3g, 15.3$\pm$9.7g and 19.9$\pm$13.1g in girls, respectively . The ratios of polyunsaturated /monoun-saturated /saturated /saturated fatty acids(P/M/S) in boys and girls were 0.7/0.8/1.0 and 0.8/0.8/1.0 respectively. The ratios of $\omega$6/$\omega$3 fatty acids in boys and girls were found to be 12.1 /1.0 and 8.6/1.0 , respectively. These results indicated the urgent need of nutritional education in primary schools to prevent further increase in childhood obesity and hyperlipidemia . Therefore, this study will contribute to the establishment of dietary guidelines and health recommendation for school children.

Self-management levels of diet and metabolic risk factors according to disease duration in patients with type 2 diabetes

  • BACKGROUND/OBJECTIVES: Metabolic risk factors should be managed effectively in patients with type 2 diabetes mellitus (T2DM) to prevent or delay diabetic complications. This study aimed to compare the self-management levels of diet and metabolic risk factors in patients with T2DM, according to the duration of illness, and to examine the trends in self-management levels during the recent decades. SUBJECTS/METHODS: Data were collected from the Korea National Health and Nutrition Examination Surveys (KNHANES, 1998-2014). In our analysis, 4,148 patients with T2DM, aged ${\geq}30years$, were categorized according to the duration of their illness (< 5 years, 5-9 years, and ${\geq}10years$). Demographic and lifestyle information was assessed through self-administered questionnaires, and biomarker levels (e.g., fasting glucose level, blood pressure, or lipid level) were obtained from a health examination. Dietary intake was assessed by a 24-recall, and adherence level to dietary guidelines (meal patterns and intake levels of calories, carbohydrates, vegetable/seaweed, sodium, and alcohol) were assessed. Multivariable generalized linear regression and unconditional logistic regression models were used to compare the prevalence rates of hyperglycemia, dyslipidemia, and hypertension according to the duration of patients' illness, accounting for the complex survey design of the KNHANES. RESULTS: In the multivariable adjusted models, patients with a longer duration (${\geq}10years$) of T2DM had a higher prevalence of hyperglycemia than those with a shorter duration of T2DM (< 5 years) (odds ratio 2.20, 95% confidence interval 1.61-3.01, P for trend < 0.001). We did not observe any associations of disease duration with the prevalence of hypertension and dyslipidemia. In addition, the adherence levels to dietary recommendations did not significantly differ according to disease duration, except adherence to moderate alcohol consumption. There were significant decreasing trends in the prevalence of hyperglycemia in patients with a duration of illness ${\geq}10years$ (P for trend = 0.004). CONCLUSION: Although the proportion of patients with adequate control of glucose levels has improved in recent decades, poorer self-management has been found in those with a longer disease duration. These findings suggest the need for well-planned and individualized patient education programs to improve self-management levels and quality of life by preventing or delaying diabetic complications.

임신부의 건강식생활 가이드를 위한 애플리케이션 개발 소비자요구도 조사 (A study on Consumer's Needs for Development of Diet Guide Application for Pregnant Women)

  • This study was conducted to assess needs of educational mobile application (App) development for nutritional management and information on pregnant women. A total of 105 pregnant women were investigated on general characteristics, dietary habits, health behavior and needs for contents and composition of the application. The mean age of the subjects was 31.9 years and the mean gestation period was 25.4 weeks. The rate of skipping meal was 39.0% and the rate of irregular meal time was 46.6%. The consciousness of the meal as balanced nutrition and health was 19.9%. Eating out at least forth a week was 35.3%. Obtaining information about pregnancy and childbirth were internet (35.3%), hospital or health center (19.9%), books (17.1%), experience (15.2%), mobile (8.6%) and friends or acquaintances (4.8%). If the application is developed, subject replied 'frequently use' (51.4%), 'when needed' (47.6%) respectively. The favour topic in developing application were 'nutrition information of pregnant and fetal' (36.2%), 'weight management, feeding' (33.3%), 'food choice and cooking' (21.9%), 'shopping' (5.7%), 'example of menu' (1.9%), 'effect of smoking, drinking, exercising' (1.0%). The favorite content was 'include sufficient amount about information' (44.8%). Depending on the age and education level, the best age for pregnancy group have significantly higher ability for utilize and information gathering than old age pregnant group. Also the best age for pregnancy group have high demands of design, convenience and various contents in App development. Therefore, mobile application (App) for pregnant women could be widely used as an effective dietary guide.

돈육섭취에 의한 항생제 내성 황색포도상구균 및 독소의 식이노출평가 (Estimation of Dietary Exposure to Antimicrobial Resistant Staphylococcus aureus from Pork-based Food Dishes)

  • 식중독균의 항생제 내성은 주요 식품 안전 이슈로, 적절한 안전관리 옵션을 도출하기 위해서는 항생제 내성균의 노출평가가 필수적이다. 이에 식품섭취에 의한 항생제 내성균의 인체노출 정도를 평가하고자 확률노출모델을 이용하여 돈육식품 중 항생제 내성 S. aureus 및 생성 독소의 식이노출을 추정하였다. 시판 돈육 제품에서 S. aureus를 분리하고 주요 항생제에 대한 내성율을 분석한 결과 124개 시료에서 분리된 30주의 S. aureus는 penicillin과 ampicillin에 대해 각각 76.7%와 70.0%의 내성을 보였다. Oxacillin 내성은 6.7%였으며 vancomycin에 대해서는 내성을 나타내지 않았다. 가장 내성율이 높은 penicillin 내성 S. aureus를 대상으로 원료 중 오염도, 항생제 내성율, 작업지침 준수여부 등 변수를 이용한 집단급식 조리 돈육 불고기 섭취에 대한 확률노출평가 모델을 시뮬레이션한 결과, 조리 전 또는 서빙 전 단계에서 돈육 중 내성균의 추정 오염도는 평균 0.3 Log CFU/g이며, 보수적인 접근인 95% percentile값의 경우도 penicillin 내성 S. aureus의 알려진 독소 생성 수준보다 낮았다. 이 결과는 현재 주어진 조건에서 돈육조리식품 섭취에 의한 항생제 내성 S. aureus의 인체노출가능성이 낮으며 돈육조리식품 섭취에 의하여 penicillin 내성 S. aureus이 생성하는 독소에 노출될 가능성은 매우 낮음을 시사한다. 그러나 원료의 오염도, 조리 중 온도, 시간관리가 준수되지 않을 경우 식중독이 발생할 가능성은 증가할 수 있으므로 작업지침 등을 준수해야 할 것이다.

발톱 크롬 수준에 영향을 미치는 독립 요인과 크롬 수준과 이상지질혈증과의 연관성 분석 (Independent correlates of the toenail chromium level and the association between the chromium level and dyslipidemia)

  • 본 연구는 SELEN 코호트 연구의 기반조사 자료를 활용하였으며, 성인 남녀의 발톱 크롬 수준에 독립적으로 영향을 미치는 결정요인을 파악하고, 크롬 수준과 이상지질혈증과의 관련성을 규명하고자 하였다. 그 결과, 면류, 채소류, 과일류가 발톱 크롬 수준에 독립적으로 영향을 주었으며, 교란인자를 보정한 후 크롬 수준과 주요 이상지질혈증간의 유의적인 연관성을 보이지 않았다. 그러나 발톱 크롬 농도에 독립적으로 영향을 미치는 요인을 찾아내어 제시하게 된 바, 한국인을 대상으로 한 최초의 크롬 관련 영양 역학연구의 결과로서 그 의의가 크다. 또한 국내 연구가 부족한 상황에서 본 연구 결과는 크롬의 한국인 영양 섭취 기준 개정 시 기초자료로 활용될 수 있을 것으로 기대한다. 향후 크롬 섭취 및 체내 크롬 농도와 이상지질혈증과의 명확한 관련성을 규명하기 위하여 한국인 크롬 식이 데이터베이스를 구축하고, 대규모 코호트 연구와 임상시험연구가 진행되어야 할 것으로 사료된다.

Protein Intake Recommendation for Korean Older Adults to Prevent Sarcopenia: Expert Consensus by the Korean Geriatric Society and the Korean Nutrition Society

  • Sarcopenia, a common clinical syndrome in older adults, is defined as decreased muscle mass, strength, and physical performance. Since sarcopenia is associated with the incidence of functional decline, falls, and even mortality in older adults, researchers and health care providers have been keen to accumulate clinical evidence to advocate the screening and prevention of sarcopenia progression in older adults. The factors that may accelerate the loss of muscle mass and function include chronic diseases, inactivity, and deficiency in appropriate nutritional support. Among these, nutritional support is considered an initial step to delay the progression of muscle wasting and improve physical performance in community-dwelling older adults. However, a nationwide study suggested that most Korean older adults do not consume sufficient dietary protein to maintain their muscle mass. Furthermore, considering age-associated anabolic resistance to dietary protein, higher protein intake should be emphasized in older adults than in younger people. To develop a dietary protein recommendation for older adults in Korea, we reviewed the relevant literature, including interventional studies from Korea. From these, we recommend that older adults consume at least 1.2 g of protein per kg of body weight per day (g/kg/day) to delay the progression of muscle wasting. The amount we recommend (1.2 g/kg/day) is 31.4% higher than the previously suggested recommended daily allowance (i.e., 0.91 g/kg/day) for the general population of Korea. Also, evidence to date suggests that the combination of exercise and nutritional support may enhance the beneficial effects of protein intake in older adults in Korea. We found that the current studies are insufficient to build population-based guidelines for older adults, and we call for further researches in Korea.

청소년을 위한 영양지수 개발과 타당도 검증 (Development of NQ-A, Nutrition Quotient for Korean Adolescents, to assess dietary quality and food behavior)

  • 본 연구는 생애주기별 영양지수 중 청소년을 위한 영양지수 (Nutrition Quotient for Adolescents, NQ-A) 모형을 개발하고자 수행되었다. 영양지수 평가항목 선정을 위해 식품섭취영역에서 20개 문항, 식행동 식습관 영역에서 21개 문항의 총 41개 문항으로 구성된 파일럿 조사용 체크리스트를 개발하였다. 청소년 213명을 대상으로 1일의 식사섭취실태 조사를 포함한 파일럿 조사를 수행하고, 체크리스트 항목 점수와 식사섭취실태 관련 변수들 간의 상관관계가 높은 24개의 평가항목으로 구성된 전국단위 조사용 체크리스트를 개발하였다. 전국단위 조사는 시도별 집락별로 층화추출된 중고등학생 1,547명에 대해 구조화된 설문지로 면대면 조사를 수행하였고, 탐색적 요인분석을 수행하여 청소년 영양지수의 평가 항목을 19개 항목의 5개요인으로 분류하고, 구조방정식 모형을 이용해 평가 항목의 구성타당도를 검증하고 추정된 경로계수를 가중치로 적용하였다. 청소년 영양지수를 위한 5개의 요인과 가중치는 각각 균형 0.25, 다양 0.15, 절제 0.25, 환경 0.15, 실천 0.20이었다. 균형 영역에는 과일 섭취 빈도, 흰 우유 섭취 빈도, 콩류 섭취 빈도, 생선 섭취 빈도의 4개 항목이 포함되었고, 다양 영역은 채소 반찬 섭취 빈도, 반찬 골고루 먹는 정도, 편식 정도의 3개 항목, 절제 영역은 과자 또는 달거나 기름진 빵 섭취 빈도, 가공 음료 섭취 빈도, 라면 섭취 빈도, 카페인 음료 섭취 빈도, 야식 섭취 빈도, 길거리 음식 섭취 빈도의 6개 항목, 환경 영역은 아침식사 빈도, 식탁에 앉아서 식사하는 정도, TV 핸드폰 컴퓨터 사용 시간의 3개 항목, 그리고 실천 영역은 영양 표시 확인 정도, 음식 먹기 전 손 씻는 정도, 운동 빈도의 3개 항목이 포함되었다. 전국단위 조사대상자의 영양지수 (NQ-A) 평균 점수는 56.0점이었고, 세부 영역별로는 균형 58.6점, 다양 55.6점, 절제 48.1점, 환경 70.6점, 실천 52.1점이었다. 영양지수 점수와 각 영역별 등급은 전국 조사 결과의 백분위수 분포를 기준으로 75~100 백분위의 경우 '상' 등급, 25 ~ <75 백분위의 경우 '중' 등급, 0 ~ <25 백분위는 '하' 등급으로 분류하였다. 영양지수의 경우 63.1~100점은 '상' 등급, 49.0~63점은 '중' 등급, 0~48.9점은 '하' 등급으로 판정되었다. 본 연구에서 개발된 청소년을 위한 영양지수는 19개 항목으로 구성된 식행동 체크리스트를 통해 영양지수 점수와 균형, 다양, 절제, 환경, 실천 영역의 점수를 산출할 수 있으며, 상대적인 NQ 등급 부여를 통해 식사의 질과 식행동이 양호한 지를 판정할 수 있다. 본 연구에서 개발된 청소년용 영양지수 (NQ-A)가 청소년의 식사의 질과 식행동 평가 및 영양교육의 효과 평가 등에 많이 활용될 것으로 기대된다.
