소아, 청소년 비만 약물 치료의 올바른 이해 (Recommendations of Pharmacological Treatment in Childhood and Adolescents Obesity)

  • The incidence of childhood obesity has increased dramatically. Childhood obesity is an increasing health problem because of its strong associations with chronic health problems in children and adults. These health problems significantly contribute to the development of common chronic diseases in later life, including hypertension, type2 diabetes, hyperinsulinemia, coronary heart disease, and other psychological disorders. So it is an important issue to prevent and treat obesity during childhood and adolescent. Diet and exercise are the cornerstones of treatment for obesity and related complications. For obese children, some clinical trials have shown improvement with diet, exercise, and /or behavioral interventions. Promising interventions for high-risk individuals, such as bariatric surgery and novel pharmacological agents, also require rigorous assessment with attention to long-term patient important outcomes. There are various pharmacological approaches to the treatment of obesity in the adolescent population some of which have FDA approval. In the article we discuss pharmacological approaches to guide the treatment of obesity in the pediatric population, including risks of treatment, monitoring of potential side effects.

구미시에 거주하는 노인들의 건강실태 (A Study on the Health Condition of Senior's in Gumi)

  • This study examined people over 60 years old living in Gumi, Gyeongbuk Province and their health and disease were analyzed. The results are as followings; First, as for seniors' condition of health, females were worse than males: the older they were and the lower their schooling and income were, they recognized bad health condition. Second, as for seniors' health care, males cared for themselves better than females: the higher their schooling and income were, the better they cared for themselves. Using the methods of health care such as walking, hiking, diet care, We found out that the low income class did not get the health examination at all. Lastly, as the most seniors in Gumi city suffer a chronic disease, the city office is needed to promote various policies like prevention of age-related disease, early discovery and delay of retrogression at its own initiative.

폐경 후 여성의 식사의 질과 관련 요인 분석 (The Analysis of the Factors Related to Diet Quality in the Postmenopausal Women)

  • This study was performed to assess the relationship between diet quality and general characteristics, stress, exercise habits, and nutritional knowledge score in the postmenopausal women. The data of dietary intakes were obtained using food frequency questionnaires which were collected from 151 postmenopausal women in urban area. Diet quality was assessed by INQ(index of nutritional quality), MAR(mean adequacy ratio), DDS(dietary diversity score), DVS(dietrary variety score), DQI(diet quality index). The results are summarized as follows. The mean age of the subjects was 59.9 years old. The means of height, weight, and BMI were 154.7cm, 57.2kg and 23.9 respectively. The subjects who did not exercise regularly were 70.9% and those who excercised at least once a week were 29.1%. The subjects who had regular meal time were 69.5% and those who ate breakfast regularly were 72.6%. More than 2/3 of subjects had regular eating behavior. Overall dict quality was significantly(p < 0.05) associated with INQ, MAR, DDS, DQI. However, there was no significant association between income level and diet quality. In conclusion, it would be beneficial to provide nutritional education included dietary diversity, dietary variety, dietary guideline, and adequate flood amount, to prevent chronic degenerative disease and maintain healthful life in the postmenopausal women.

Flavonols, Flavones, Flavanoues and Human Health: Epidemiological Evidence

  • Polyphenolic flavonoids are among a wide variety of phytochemicals present in the human diet. Basic research, animal model, and human studies suggest flavonoid intake may reduce the risk of several age-related chronic diseases. The vast number of flavonoids and mixtures of their subclasses, including flavonols, flavones and flavanones, and the variety of agricultural practices that affect their concentration in foods have presented a challenge to the development of adequate food composition databases for these com-pounds. Nonetheless, dietary assessments have been applied to cohort and case-control epidemiological studies and several reveal an inverse association with risk of some forms of cancer, cardiovascular disease, and other chronic conditions. Those observational studies that have examined these relationships with regard to flavonols, flavones, and flavanones are reviewed. The requirement for caution in interpreting these studies is discussed with regard to the limited information available on the bioavailability and biotransformation of these flavonoids. As the totality of the available evidence on these flavonoids suggests a role in the prevention of cancer and cardiovascular disease, further research is warranted, particularly in controlled clinical trials.

Protective Effects of Ecklonia stolonifera Extract on Ethanol-Induced Fatty Liver in Rats

  • Chronic alcohol consumption causes alcoholic liver disease, which is associated with the initiation of dysregulated lipid metabolism. Recent evidences suggest that dysregulated cholesterol metabolism plays an important role in the pathogenesis of alcoholic fatty liver disease. Ecklonia stolonifera (ES), a perennial brown marine alga that belongs to the family Laminariaceae, is rich in phlorotannins. Many studies have indicated that ES has extensive pharmacological effects, such as antioxidative, hepatoprotective, and antiinflammatory effects. However, only a few studies have investigated the protective effect of ES in alcoholic fatty liver. Male Sprague-Dawley rats were randomly divided into normal diet (ND) (fed a normal diet for 10 weeks) and ethanol diet (ED) groups. Rats in the ED group were fed a Lieber-DeCarli liquid diet (containing 5% ethanol) for 10 weeks and administered ES extract (50, 100, or 200 mg/kg/day), silymarin (100 mg/kg/day), or no treatment for 4 weeks. Each treatment group comprised of eight rats. The supplementation with ES resulted in decreased serum levels of triglycerides (TGs), total cholesterol, alanine aminotransferase, and aspartate aminotransferase. In addition, there were decreases in hepatic lipid and malondialdehyde levels. Changes in liver histology, as analyzed by Oil Red O staining, showed that the ES treatment suppressed adipogenesis. In addition, the ES treatment increased the expression of fatty acid oxidation-related genes (e.g., PPAR-${\alpha}$ and CPT-1) but decreased the expression of SREBP 1, which is a TG synthesis-related gene. These results suggest that ES extract may be useful in preventing fatty acid oxidation and reducing lipogenesis in ethanol-induced fatty liver.

Association of Demographic and Socioeconomic Factors With Risk Factors for Chronic Kidney Disease

  • Objectives: The goal of this study was to examine the association of various demographic and socioeconomic factors with risk factors for chronic kidney disease (CKD). Methods: We used nationally representative pooled data from the Korea National Health and Nutrition Examination Survey (KNHANES), 2007-2013. We estimated the glomerular filtration rate (GFR) using the Modification of Diet in Renal Disease equation. We defined CKD as a GFR <60 mL/min/$1.73m^2$, and 1304 of the 45 208 individuals included in the KNHANES were found to have CKD by this definition. The outcome variable was whether individual subjects adhered to the CKD prevention and management guidelines recommended by the Korea Centers for Disease Control and Prevention. The guidelines suggest that individuals maintain a normal weight, abstain from alcohol consumption and smoking, manage diabetes and hypertension, and engage in regular exercise in order to prevent and manage CKD. Results: This study found that individuals with CKD were more likely to be obese and have hypertension or diabetes than individuals without CKD. In particular, male and less-educated CKD patients were less likely to adhere to the guidelines. Conclusions: Although the prevalence of CKD, as indicated by the KNHANES data, decreased from 2007 to 2013, the prevalence of most risk factors associated with CKD fluctuated over the same time period. Since a variety of demographic and socioeconomic factors are related to the successful implementation of guidelines for preventing and managing CKD, individually tailored prevention activities should be developed.

사상체질에 따른 식품 및 영양소 섭취상태 조사 (Intakes of Food and Nutrients According to Sa-Sang Constitution)

  • Sa-Sang, one of the Korean traditional medicines, classifies people's constitution into 4 types, which are referred to as Tae-Yang-In, Tae-Eum-In, So-Yang-In and So-Eum-In. The purpose of this study was to examine demographic and dietary factors and to test any significant differences between Sa-Sang constitution and related factors. The participants were 483 college students and the data of 428 students were finally analyzed. Constitution was determined identically through both the questionnaire on Sa-Sang constitution and an inquiry by a Korean traditional medical doctor. Tae-Eum-In showed statistically significant higher BMI and higher smoking frequency. Tae-Eum-In also had significantly higher intakes in iron, potassium, sodium, vitamin B$_1$, niacin, $\beta$-carotene and vitamin E. So-Eum-In had significantly lower intakes in protein, phosphorous, and folate. From this study, we could expect to get some scientific, objective and accurate diet information on the types of constitution. Considering the fact that most chronic degenerative diseases could be developed by any lifestyle factors, it is necessary to conduct educational programs about lifestyles including the dietary habit for maintaining good health. Therefore, it will be better to continue a long-term follow-up study on any chronic degenerative disease based on the types of Sa-Sang constitution in the future. Effect of Capsaicin on the Body fat and Adipocyte in the Diet induced-obese Mice

  • As the obesity has been known to be related with the hyperlipidemia, cardiovascular disease, cerebral apoplexy, fatty liver, and other chronic diseases, recent researches have focused on the functional food materials and their anti-obesity activities. This study was performed to study the effects of vanilloid family capsaicin, major pungent ingredient of hot chillies and peppers, on anti-obesity activities. ICR male mice were fed one of the pellet diet, basal diet, and high fat diet with capsaicin (45 $\mu\textrm{g}$/day) solution for 5 days. Mice in the corresponding control groups were given water for 5 days. In results, capsaicin reduced body weights in any diet groups. Percent weight and cell size of the abdominal white adipose tissue in mice on the high fat diet with capcaicin were significantly lower compared with those in mice on the high fat diet with water. However, percent brown adipose tissue weight per body weight in mice on the high fat diet was not affected by capsaicin. Capsaicin reduced the levels of s-triglyceride and s-total cholesterol in the pellet diet or high fat diet groups. There was no difference in the s-protein levels between the capsaicin group and the control water group. These data indicate that 1) orally administered capsaicin has a reducing effect on the blood triglyceride and total cholesterol levels, and 2) capsaicin has lowering effects on the body weight, percent weight and cell size of the abdominal white adipose tissue.

Fenugreek seeds reduce aluminum toxicity associated with renal failure in rats

  • Despite the reports on safety concerns regarding the relationship between aluminum salts and neurological and bone disease, many countries continue to use aluminum as phosphate binders among patients with renal failure. In search for a diet supplement that could reduce aluminum toxicity related to renal failure, we carried out this prospective animal study in which the fenugreek seeds were assessed for their effects on rats nephrotoxicity induced by aluminum chloride ($AlCl_3$). Oral $AlCl_3$ administration during 5 months (500 mg/kg bw i.g for one month then 1600 ppm via drinking water) led to plasma biochemical changes, an inhibition of alkaline phosphatase (ALP), a decrease of total antioxidant status (TAS), and an induction of lipid peroxidation (LPO) in the blood and brain, in addition to kidney atrophy and morphological alterations at the level of Bowman's capsule, the glomerulus and different sorts of tubules, reminiscent of some known kidney disease. The treatment with the whole fenugreek seed powder (FSP) (5% in the diet) during the last 2 months showed its effectiveness in restoring normal plasma values of urea, creatinine, ALP and glucose, as well as re-increasing the TAS, inhibiting LPO and alleviating histopathological changes in the injured kidneys. This study highlights the induced nephrotoxicicity, as well as the related toxicity in the brain and bone, by chronic oral ingestion of the aluminum salts. However, the maintenance of a diet supplemented with fenugreek seeds could offer protection for the kidney, bone and brain, at the same time.

사상체질에 따른 식습관과 주관적 건강상태와의 연관성 (Association between Dietary Habits and Self-rated Health According to Sasang Constitution)

  • Dietary habits are known to be closely related not only to chronic diseases such as cardiovascular disease and diabetes, but also to self-rated health (SRH). Consequently, there is a highly important lifestyle component in maintaining a healthy life. The aim of the present study was to determine the correlation between diverse dietary habits based on the Sasang constitutions (SC) and SRH. The data were collected from 2,984 participants who met the constitution pharmacology criteria and whose SC were clinically confirmed by SC professionals. Dietary habits were observed by assessing the regularity of diet, meal volume, and meal speed; single-answer questions were used for SRH. Fisher's exact/chi-square test and logistic regression were used to analyze the correlation between dietary habits depending on constitutional type and SRH. In association with constitutional SRH, the Taeum type is related to the amount of meal, the Soeum and Soyang types are related to the regularity of meal and meal volume. In the Taeum type, there was a significantly higher risk of lower SRH when meal volumes were irregular less than previous amounts. Irregular diet, as well as increased or decreased meal volume, negatively correlated with SRH in the Soeum type; in the Soyang type, irregular diet and decreased or irregular meal volume negatively affected SRH. This showed the presence of the different effects of dietary habits on health depending on constitution and also confirmed the association of the regularity of diet and meal volume based on constitution and the frequency of bad dietary habits with SRH. We suggest that personalized healthcare with adequate dietary improvement considering constitution is needed for health management or improvement.
