성인 여성에서 비만 여부에 따른 식사섭취상태 및 식사의 질 평가 : 2007-2009 국민건강영양조사 자료를 이용하여 (Evaluation of nutrient and food intake status, and dietary quality in Korean female adults according to obesity : Based on 2007-2009 Korean National Health and Nutrition Examination Survey)

  • 본 연구에서는 성인 여성을 대상으로 비만 여부에 따른 영양소 및 식품 섭취의 질을 평가하고, 식사의 질과 비만지표와의 관련성에 대하여 분석하였다. 총 3,565명의 대상자 (정상군 2,841명, 비만군 724명)가 분석에 포함되었고, 그 결과를 요약하면 다음과 같다. 1일 총 섭취 열량에서는 정상군이 1,707.81 kcal로 비만군의 1,634.94 kcal에 비해 유의적으로 높게 나타났으며 (p = 0.0199), 또한 섭취 열량 1,000 kcal당 영양소 섭취량을 분석한 결과 정상군은 비만군에 비해 탄수화물 (p = 0.0298) 및 나트륨 (p = 0.0169)의 섭취량은 유의적으로 낮았던 반면 칼슘 (p = 0.0261)의 섭취량은 유의적으로 높게 나타났다. 칼슘의 경우 정상군에서 권장섭취량 대비 섭취율이 비만군에 비해 유의적으로 높았으며 (p = 0.0178), 평균필요량에 미달되게 섭취하는 대상자의 비율은 유의적으로 낮게 나타났다 (p = 0.0152). 영양소별 NAR 및 MAR을 분석한 결과, 정상군에서 칼슘의 NAR이 0.63으로 비만군의 0.60에 비해 유의적으로 높게 나타났으며 (p = 0.0158), 영양소별 NAR의 평균치인 MAR의 경우 정상군이 0.80, 비만군이 0.79로 두군간 유의한 차이를 보이지 않았다. 또한 1일 총 식품 섭취량은 정상군 1,297.36 g, 비만군 1,271.28 g으로 두군간 유의한 차이를 보이지 않았으나, 우유 및 유제품의 섭취량에서 정상군이 비만군에 비해 유의적으로 높게 나타났다 (p = 0.0162). DDS는 전체 대상자에서 3.753점으로 나타났으며, 정상군이 3.769점으로 비만군의 3.686점 보다 유의적으로 높은 것으로 나타났다 (p = 0.0253). 또한 전체 연구대상자의 DVS는 30.07개로 나타났으며, 정상군과 비만군 각각 30.41개, 28.63개로 나타나 유의적인 차이를 보였다 (p = 0.0003). 식품섭취와 관련된 식사의 질 지수 (DDS, DVS)와 비만과 관련된 신체계측치 (체질량지수, 허리둘레)와의 관계를 분석한 결과, DVS가 많은 대상자들의 허리둘레 및 체질량지수가 더 낮은 경향을 보였으며 (p for trend < 0.01), 이 결과는 연령, 음주 상태, 신체활동 정도 및 열량 섭취량을 보정한 후에도 같은 양상으로 나타났다 (p for trend < 0.01). 이상의 결과를 종합하면, 비만한 성인 여성의 경우 정상 체중인 여성에 비해 칼슘 섭취 및 유제품 섭취의 질이 유의적으로 낮았으며, DDS 및 DVS가 유의적으로 낮아 다양하지 못한 식품 섭취 양상을 보이는 것으로 나타났고, DVS는 체질량지수 및 허리둘레와 유의적인 음의 관계를 보였다. 따라서 이와 같은 결과를 비만한 성인 여성을 대상으로 한 비만 관리에 적용할 때 고른 영양 섭취를 위해 식품군의 구성, 섭취하는 식품의 가짓수 등 영양균형에 중점을 두어 식사지도를 해야 할 것으로 생각되며, 칼슘 섭취의 질과 비만과의 인과관계를 명확히 분석하기 위하여 한국인을 대상으로 한 전향적인 코호트 연구 및 추가 분석이 필요하다고 사료된다.

아토피 피부염 환자에서 12 주간의 심층 영양관리에 따른 식사의 질과 혈중 지방산조성 및 면역지표에 관한연구 (Effects of Medical Nutrition Therapy on Dietary Quality, Plasma Fatty Acid Composition and Immune Parameters in Atopic Dermatitis Patients)

  • Atopic dennatitis (AD) is one of the major public health problem. It has been reported that the prevalence of AD in children and adults are 10-20% and 1-3%, respectively. Westernization of food habits, urbanization, and environmental pollution are contributing factors toward the recent rise in prevalence. Excessive dietary restriction leads to chronic malnutrition in atopic dermatitis patients. The purpose of this study was to investigate the effects of medical nutrition therapy (MNT) on quality of diet and blood immune parameters in atopic dermatitis patients. The 19 atopic dermatitis patients (7 men and 12 women) admitted to K University Medical Center were studied. During the 12 weeks of intervention, the subjects were given MNT by a dietitian for 30-45 minutes every other week. MNT was comprised with general dietary therapy, intake of balanced meals, emphasis on n-3 fatty acid contents in foods, and food allergies. Anthropometric and dietary assessment and blood analysis were taken at baseline and after 12 weeks of MNT. After 12 weeks of MNT, the subjects' dietary qualities, including dietary diversity score (DDS), meal balance score (MBS) and dietary variety score (DVS) were significantly increased (p < 0.05). According to significantly increased intake of EPA and DHA, dietary n-6/n-3 fatty acid ratio decreased to the recommended level for the atopic dermatitis patients (p < 0.05). These changes of dietary fatty acid consumption were reflected erythrocyte fatty acid composition. After 12 weeks of MNT, serum levels of IgE and IL-4 levels were significantly decreased, however, the levers of INF-$\{gamma}$, WBC, lymphocyte and TLC were not changed. As a conclusion, the individualized MNT improved the quality of diet in atopic dermatitis patients thereby influenced RBC fatty acid composition and IgE and IL-4 levels.

한국 여자 노인의 인지능력과 영양섭취패턴과의 관계 (Relationship between Cognitive Function and Dietary Patterns in Korean Elderly Women)

  • The relationship between cognitive function and dietary patterns in Korean elderly women was investigated in this study. Twenty-four-hour dietary recall and cognitive function tests were performed in 170 elderly women aged over 60 years. Average age of the subjects was 71 years and the average number of family members was 3.3. Most of the subjects had less than seven years of education and their monthly income was much lower than the Korean average. Most of the subjects engaged in regular exercise and did not indulge in drinking and smoking. The average cognitive function score of the subjects was 58.4. Most of the subjects appeared to consume insufficient amount of food ; average intakes of energy, Ca, iron, vitamin A, and riboflavin were lower than RDA for the elderly. Cognitive function score was positively related to education level and regularity of exercise. Also, cognitive function score had a strong relationship with food intake such as total amounts of foods, fish and shellfish, milk and dairy products, total animal food, fruit, bread and sugar. Energy, protein, Ca, p, riboflavin, and niacin were also shown to have positive relationships to cognitive function score, while carbohydrate caloric ratio had a negative relationship with cognitive function. According to the results of multiple regression analysis, factors that most influenced on cognitive function were education level and riboflavin intake. The subjects who had more than seven years of education had significantly higher cognitive function scores than those with no school education. And cognitive function scores f3r those who consumed more than 2/3 of the RDA for most nutrients were significantly higher than the ones fir the subjects who consumed less than 2/3 of the RDA. These results suggest that proper education and adequate nutrient intake in quantity and quality by achieving food diversity are essential in maintaining cognitive function in later life (Korean J Nutrition 31(9) : 1457-1467, 1998)

여대생의 아침 결식 여부에 따른 식행동 및 식이 섭취 상태와 DQI-I를 기준으로 한 식사의 질 평가 (Evaluation of Dietary Behavior, Nutrient and Food Intake Status, and Dietary Quality Based on Diet Quality Index-International (DQI-I) in Female University Students)

  • The purpose of this study was to evaluate the dietary behavior, nutrient and food intake status and dietary quality based on nutrient and food group intake and Diet Quality Index-International (DQI-I) according to breakfast eating status in female university students. The survey was conducted using questionnaires and 3-day dietary records for 181 female university students residing in Seoul, Gyunggi, and Gangwon. The subjects were divided into two groups by frequency of breakfast eating: skipping breakfast (frequency of eating breakfast under 4 times/week, n=69) and eating breakfast (frequency of eating breakfast over five times/week, n=112). The skipping-breakfast group was significantly lower in its frequency of having lunch than that of the eating-breakfast group. There was no significant difference of energy intake between the two groups; however, the intake of carbohydrate, fiber, folate, vitamin C, vitamin E, Ca and Fe in the skipping-breakfast group was significantly lower than those in the eating-breakfast group. There was no significant difference in Korean's Dietary Diversity Score (KDDS) between the skipping-breakfast group ($4.65{\pm}0.56$) and eating-breakfast group ($4.73{\pm}0.50$). The average DQI-I of the skippingbreakfast group ($56.96{\pm}9.04$) was significantly lower than that of the eating-breakfast group ($61.32{\pm}7.99$). In conclusion, female university students with a high frequency of skipping breakfast may have a lower diet quality and DQI-I score. Therefore, in support of proper dietary management, it is necessary to promote and support breakfast eating.

고등학생의 식품섭취 실태 및 식사의 질 평가 (Evaluation of Food Intake and Diet Quality in High School Students)

  • 본 연구는 춘천지역에 살고 있는 고등학생들의 식품 섭취와 식사의 질을 평가하기 위하여 318명의 조사대상자들에게 24시간 회상법을 이용하여 식이조사를 하였다. 총 식품섭취량 중에서 식물성 식품의 섭취비율은 $71.4\%$이고 나머지는 동물성 식품으로 섭취하였다. 식사의 질은 식품군 섭취유형과 주요식품군 점수(DDS) 및 총 식품점수(DVS)에 의해 평가하였다. 주요식품군 섭취 평가에서 전체 조사대상자 중 DDS가 3인 경우의 비율은 $37.1\%$이고 4일 때는 $48.4\%$로 나타났다. 5가지 주요식품군을 모두 섭취한 경우는 $14.5\%$에 불과하였다. 또한 하루에 섭취하는 평균 식품가짓수는 26.7가지인데 남학생은 27.3가지, 여학생은 25.8가지로 남학생의 비율이 유의하게 높았다. 영양소 적정섭취비와 총 식품점수와의 상관관계에서는 비타민 $B_2$의 0.39에서부터 인의 0.61까지 범위의 상관계수를 보였다. 또한 영양소 적정섭취비도 섭취 식품수나 주요식품군 섭취가 증가함에 따라 유의하게(p<0.001) 증가하였으며, NAR과 DDS의 관계에서보다 NAR과 DVS와의 관계에서 더 높은 양의 상관관계를 보였다. DDS와 DVS를 이용하여 대상자들의 식사의 질을 평가하였을 때 많은 학생들은 바람직한 식품 섭취를 하지 못했음을 알 수 있었다. 그러므로 영양적으로 균형있는 식사를 하기 위해서는 주요식품군을 모두 섭취하고 섭취식품 가짓수가 많은 식생활을 하도록 학생과 학부모를 대상으로 바람직한 식생활을 위한 영양교육 및 식사지침 제시가 절실히 필요할 것으로 생각된다..0종으로 감소되었다. 우점잡초의 변동은 무비구에서 명아주, 바랭이, 여뀌순위였고, 지렁이분 150kg\10a 시비구에서는 쇠비름, 바랭이, 여뀌 순위로, 450kg/10a 시비에서는 여뀌, 새포아풀, 바랭이 순위로, 600kg/10a 시비구에서는 여뀌, 바랭이, 쇠비름 순위였다. 이상의 시험결과를 보아 제주지역에서 Creeping bentgrass의 점유율과 밀도를 최대로 증대시킬 수 있는 지렁이분 적정시비량은 10a당 $450\~600kg$으로 추정할 수 있다.든 영역의 실생활 활용도가 낮게 나타났다.용이 가능할 것으로 생각된다.100kg/ha와 화학비료 100kg/ha 혼합하거나 톱밥발효돈분만 N 400kg/ha 까지가 적정 수준이다.EX>$b^{\ast}(\pm,\;yellowness/blueness)$ 값은 반응기질보다 유의적으로 가장 높은 55.1을 나타내었다(p<0.05).준에서 유의적인 차이가 있었다. 허리와 엉덩이 둘레는 남자는 $86.3\pm19.9cm$$90.0\pm90.8cm$, 여자는 $84.2\pm12.4cm$$96.8\pm6.72cm$로 남녀간에 유의한 차이는 없었으나 WHR이 여자의 경우 0.8이상이 되어서 심혈관계 질환의 위험 범위에 속하는 수준이었다. 삼두근의 두겹 두께는 남녀 각각 $20.2\pm8.58cm,\;22.2\pm4.40mm$으로 남녀간에 유의한 차이는 없었다. 조사대상자의 식습관 상태는 전체 대상자의 $84.4\%$가 대부분이 하루 세끼 식사를 규칙적으로 하고 있었으며 식사속도는

Dietary Diversity during Early Infancy Increases Microbial Diversity and Prevents Egg Allergy in High-Risk Infants

  • We aimed to investigate associations of dietary diversity (DD) with gut microbial diversity and the development of hen's egg allergy (HEA) in infants. We enrolled 68 infants in a high-risk group and 32 infants in a control group based on a family history of allergic diseases. All infants were followed from birth until 12 months of age. We collected infant feeding data, and DD was defined using 3 measures: the World Health Organization definition of minimum DD, food group diversity, and food allergen diversity. Gut microbiome profiles and expression of cytokines were evaluated by bacterial 16S rRNA sequencing and real-time reverse transcriptase-polymerase chain reaction. High DD scores at 3 and 4 months were associated with a lower risk of developing HEA in the high-risk group, but not in the control group. In the high-risk group, high DD scores at 3, 4, and 5 months of age were associated with an increase in Chao1 index at 6 months. We found that the gene expression of IL-4, IL-5, IL-6, and IL-8 were higher among infants who had lower DD scores compared to those who had higher DD scores in high-risk infants. Additionally, high-risk infants with a higher FAD score at 5 months of age showed a reduced gene expression of IL-13. Increasing DD within 6 months of life may increase gut microbial diversity, and thus reduce the development of HEA in infants with a family history of allergic diseases.

만성 폐쇄성 폐질환 환자에서 병기에 따른 영양상태 평가 (Nutritional Status of Chronic Obstructive Pulmonary Disease Patients according to the Severity of Disease)

  • The purpose of the study was to investigate nutritional status of chronic obstructive pulmonary disease (COPD) patients and to find out the differences according to the stages of disease. From March to October, 2006, 41 stable male patients of mild to severe COPD patients were recruited from Seoul National University hospital. The patients' of body weight and fat free mass were assessed by bioelectrical impedance analysis. The nutritional status of the patients was also assessed by 3-day recall, index of nutritional quality (INQ), dietary diversity score (DDS), dietary variety score (DVS), food group index pattern and dietary quality index (DQI). The total of 41 patients were classified into three groups, stage I, stage II and stage III groups according to the classification of Global Initiative for Chronic Obstructive Lung Disease (GOLD) standard. The mean age of the patients in each stage were 67.2-66.9 years showing no significant difference. The ratio of $FEV_1$/FVC were $57.5{\pm}7.3$, $46.9{\pm}7.6$ and $38.2{\pm}6.8%$, respectively showing significant differences according to the stages of disease. The fat free mass of the stage II ($48.2{\pm}4.7kg$) and III ($47.3{\pm}4.5kg$) was significantly lower than that of stage I ($53.1{\pm}6.9kg$) patients. There were significant correlation of fat free mass with $FEV_{1}$, and BMI (body mass index) with $FEV_{1}$/FVC ratio (p < 0.05). COPD patients showed the diet-related clinical symptoms of anorexia, dyspnea, dyspepsia, and chewing difficulty. Daily intakes of calorie, K, vitamin $B_2$ and folate of the patients were very low ($83.8{\pm}20.7%$, $58.9{\pm}14.4%$, $70.7{\pm}19.6%$ and $74.4{\pm}10.2%$, respectively) however, they did not significantly different according to the stages of disease. Daily intake of calcium was significantly lower in the stage III patients (p < 0.05). The mean scores of dietary variety score was significantly lower in the stage III patients (p < 0.001). Dietary quality index of the patients were not different among the stages of disease and the scores indicated poor quality of diet. As a summary, we found that body fat free mass, regularity of exercise, frequency of having snacks and dietary variety score were significantly associated with the severity of chronic obstructive pulmonary disease.

채식을 하는 중$\cdot$노년기 여성의 영양소 섭취 상태와 식사의 질 평가 (A Evaluation Study on Nutrient Intake Status and Diet Quality of Middle and Old Aged Vegetarian Women in Korea)

  • The purpose of this study was to evaluate on nutrient intake status and diet quality of middle and old aged vegetarian women in Korea, The research group was composed of vegetarian women (n=91), all of them were Seven Day Adventists, who had been on vegetarian diets, over 20 yrs. Their anthropometric measurements, dietary intakes, and diet quality indices were compared to omnivores (n=122). The average age of vegetarians and omnivores were 61.8 yrs and 60.3 yrs, respectively. The mean daily energy intakes of vegetarians and omnivores were 1428.8 kcal and 1424.5 kcal, respectively and there was no significant difference. The vegetarians consumed significantly lower intakes of protein (p < 0.05), zinc (p < 0.001) and vitamin $B_{2}$ (p < 0.05) compared to omnivores. In the diet quality, zinc and vitamin $B_{2}$ nutrient adequacy ratio (NAR) of vegetarians were significantly lower than those of omnivores. Mean adequacy ratio (MAR) of vegetarians and omnivores were 0.80 and 0.77, respectively and there was no significant difference. The KDDS (Korean's Dietary Diversity Score) of vegetarians and omnivores were 3.7 and 4.0, respectively, and there was significant difference (p < 0.01). The KDQI (Korean's Diet Quality Index) of vegetarians (1.5) was significantly lower than that (2.1) of omnivores (p < 0.001). In conclusion, vegetarian women have low intake status of protein, zinc and vitamin $B_{2}$, and partly low diet quality. Therefore it was needed that well planed diets to replace the nutrients supplied from excluded food groups in middle and old aged vegetarian women. 수도권에 거주하는 중국인 여성결혼이민자의 한국 식생활적응 관련 요인 (Factors related to Korean Dietary Adaptation in Chinese Female Marriage Immigrants living in the Seoul Metropolitan Area)

  • This study aimed to investigate factors related to Chinese female marriage immigrants' dietary adaptation. An in-person survey was conducted by a research institute on Chinese female marriage immigrants married to Korean men, having one child or more aged 1~6 years old, and having resided in Korea for at least 1 year before the survey. Data were collected from 309 respondents comprising 151 Han Chinese and 158 Korean-Chinese during the summer of 2013. About two-thirds of respondents were in their 30s and had resided in Korea for 5 to 10 years. Based on the overall mean score for dietary adaptation level (3.50 out of 5 points), the respondents were classified into two groups: low dietary adaptation group (mean score 3.11) and high dietary adaptation group (mean score 3.81). The results of comparative analysis between the two groups showed that the levels of acculturation (p<0.01) and healthy dietary behavior (p<0.01) for the high dietary adaptation group were significantly higher than those of low dietary adaptation group. The number of respondents of the high dietary adaptation group reporting increased food diversity (p<0.01) and decreased frequency of skipping meals (p<0.01) was significantly higher compare to the low dietary adaptation group. Multiple regression analysis was conducted to identify factors related to dietary adaptation. General characteristics, levels of acculturation, and healthy dietary behavior were included as independent variables. As a result, levels of acculturation and healthy dietary behavior as well as education level, monthly household income, and length of residence in Korea were associated with dietary adaptation. In conclusion, Chinese female marriage immigrants showing acculturated and healthy dietary behaviors adapted well to Korean dietary life. The results from this study suggest that diet-related adaptation support programs for Chinese female marriage immigrants may positively affect their acculturation and dietary behaviors.

  • 본 연구는 강원도 강릉시에 위치한 중학교 2학년 남녀 학생들을 대상으로 3일간 식사 기록법을 이용하여 이들의 식품섭취실태를 파악하고 식품섭취에 의한 식사의 질을 평가하여 중학생들의 식생활 향상에 기여하고자 수행되었다. 연구결과를 하면 다음과 같다. 하루에 섭취한 총 식품량은 1028.5g으로서 식물성식품이 773.7 g(75.2%), 동물성식품은 254.9 g(24.8%)이었다. 하루 섭취한 식사중에서 5가지 주요 식품군(유제품군, 육류군, 곡류군, 과일군, 채소군)중 유제품과 과일군을 섭취하지 않은 경우가 전체의 52.7%로 가장 많았고 다음으로 유제품군만이 제외된 식사(18.1%), 과일군만이 제외된 식사(16.4%)순으로 나타났으며 가장 이상적인 5가지 주요 식품군을 모두 섭취한 경우는 전체의 10.2%로 나타났다. 한가지 이상의 식품군을 섭취하지 않았을 경우 유제품군과 과일군의 비율이 비교적 높게 나타났다 하루에 섭취하는 주요식품군별 총 식품가지수는 남녀 모두 채소류가 11.1가지로 가장 많았고 다음으로는 육류, 곡류, 과일류, 유제품류순으로 나타났고, 식품섭취 가지수는 남녀별로 유의한 차이가 없었다. 또한 총 식품수를 나타내는 총식품 점수는 하루에 21~25가지의 식품을 섭취하는 사람이 전체의 26.5%로 가장많았고, 그 다음으로는 16~20가지 (19.0%), 11~15가지 (15.0%)로 높게 나타났다. 섭취하는 주요 식품군의 수(DDS)와 하루에 섭취하는 총 식품가지수(DVS) 간에는 상관성을 보여 섭취하는 식품군의 종류가 다양해지면 섭취하는 식품의 가지 수가 증가되었다. 식품섭취의 다양성을 나타내는 식품군점수(DDS), 총식품점수(DVS)와 영양소 섭취상태(MAR, NAR)와의 관계를 살펴보면 DDS가 3인 경우 MAR 값은 0.68이며 DDS가 증가함에 따라 MAR 값이 유의하게(p<0.001) 증가하여 DDS가 4, 5인 경우 MAR값은 각각 0.79, 0.85로 증가하였다. 대상자들의 식품군 점수(DDS)에 따른 각 영양소의 적정섭취비(NAR)는 9가지 영양소 모두에서 어느 정도 차이는 있으나 DDS가 증가함에 따라 NAR이 유의하게(p<0.001) 증가하는 경향을 보였으며 DDS, DVS는 NAR로 나타낸 모든 영양소의 섭취상태와 유의적인(p<0.001) 양의 상관관계를 보였다. 따라서 섭취하는 식품의 종류가 많을수록 영양소의 섭취수준은 향상됨을 알 수 있었다. 또한 DDS보다 DVS에서 높은 상관계수를 나타내어 식품군의 다양성보다는 전체적인 식품의 종류가 많을수록 식사의 질이 더 향상됨을 알 수 있었다. 이상의 결과들을 종합해 볼 때 조사된 대상자들의 적지 않은 학생들이 유제품군과 과일군을 섭취하지 않고 있었으므로 이로 인한 청소년들의 성장과 건강에 좋지 않은 영향을 기칠 것으로 우려된다. 또한 섭취하는 식품의 종류가 많을수록 영양소의 섭취수준은 향상되었으며 이는 주요식품군 점수보다 총 식품점수에서 높은 상관관계를 나타내며 식품군의 다양성보다는 전체적인 식품의 종류가 많을수록 식사의 질이 더 향상됨을 알 수 있었다. 따라서 옆 양적으로 균형 있는 식사를 하기 위해서는 주요식품군을 모두 섭취하고 섭취 식품 가지수가 많은 식생활을 하도록 학생과 학부모를 대상으로 영양교육 및 식사지침 제시가 절실히 필요할 것으로 생각된다.
