Pilot Study for the Assessment of Physical Fitness among Male Workers in a TV Component Manufacturing Factory (일부 TV부품 제조업체 남자 근로자들의 체력평가 기초연구)

  • In a TV component manufacturing factory, 102 male workers aged $20{\sim}39$ years old were participated in testing for physical fitness. At the same time, worker's periodic health examination was done. Test battery for physical fitness include grip strength, trunk flexion, standing long jump, side step, single leg balance with eye close, push ups and Harvard step test. As a result of testing for physical fitness, synthetically, there is no difference between manufacturing workers and officers. By bioelectrical impedance test, it means a declining tendency to all 7 factors in the obese workers, and so, it is important for obese workers not only to promote physical fitness but also to promote health. Excluding grip strength and single leg balance with eye close, 5 fitness factors are negatively associated with degree of diastolic Blood pressure, but it is statistically not significant. And levels of sGOT & sGPT have no association with physical fitness factors.

The Effects of a Low Intensity Excercise Program on Psychosocial Factors of Excercise and Physical Factors in Elderly Women (저강도 운동프로그램이 여성노인의 운동에 대한 사회.심리적 및 신체적 요인에 미치는 효과)

  • Purpose: An exercise program, which was composed of motions simple and easy to follow, was developed and applied to aged women as a way to improve their health conditions. Method: To ascertain the effects of the exercise, which was conducted twice a week and 16 times in total for 8 weeks from September 10 to November 5, 2005 in two experimental groups (31 Persons) and a control group (31 persons), this study conducted quasi-experiments of nonequivalent control group pre-post test design. Data were analyzed using SPSS/PC Win 10.0, the reliability of the measuring tools using Cronbach's alpha, the verification of equivalence for general properties and dependent variables using Chi-square test and t-test, and the verification of hypotheses using t-test and ANCOVA. Results: After the exercise program. the aged women showed profitable evaluation of decisional balance, high feeling of self-efficacy as well as improved subjective awareness of health. Their diastolic blood pressure and total cholesterol were also reduced significantly after the exercise. Conclusion: After the conduct of the exercise program for 8 weeks, aged women showed increased profitable decisional balance, self-efficacy and subjective awareness of health. This suggests that continuous and proper exercise is effective as an intervention for health improvement for the aged as well as the prevention of and recovery from geriatric diseases.

A Study on Hyperlipidemia in Koran -Specially Related to Hematological Characteristics and Risk Factors of Hypercholesterolemia- (한국인의 고지혈증에 관한 연구 -고콜레스테롤혈증 소견자의 혈액학적 특성과 위험인자를 중심으로-)

  • This study was conducted to investigate the hematological characteristics and risk factors of hypercholesterolemia(HC) in Korean. 344 adult men who took the annual health check ups at D or J hospitals were participated in this cross sectional study. The subjects were grouped by plasma total cholesterol level into three groups: normal cholesterolemic(n=139), borderline hypercholesterolemic (n=93) and hypercholesterolemic(n=112) groups. The data of systolic and diastolic blood pressure (SBP/DBP), hemoglobin, plasma glucose, albumin and cholesterol levels were collected from medical records. The levels of plasma triglyceride(TG), HDL C, phospholipid, insulin, apo A and apo B were measured. The levels of plasma LDL C, VLDL C and atherogenic index(AI) were calculated. The subjects with HC had significantly higher SBP and DBP, albumin, TG, phospholipid, HDL C, LDL C, VLDL C and apo B level than those of the normal subjects. The relative and attributable risks on HC were 1.59 and 0.18 for hypertension(SBP/DBP$\geq$160/95), 2.08 and 0.35 for hypertriglyceridemia (TG $\geq$400mg/dl). Plasma total cholesterol level was positively correlated with several hematologic parameters: age(r=0.1242, p<0.05), DBP(r=0.1194, p<0.05), albumin(r=0.2029, p<0.001), plasma TG(r= 0.3829, p<0.001), phospholipid(r=0.6036, p<0.001), LDL C(r=0.8572, p<0.001), HDL C(r=0.2399, p< 0.001), AI(r=0.3116, p<0.001), apo B(r=0.2602, p<0.05) and Lp(a)(r=0.1372, p<0.05). However, plasma total cholesterol level was negatively correlated with the levels of Apo A1(r= 0.2922, p<0.001), and ABR(r= 0.3598, p<0.001).

Awareness, Treatment and Control of Hypertension and Related Factors in the Jurisdictional Areas of Primary Health Care Posts in a Rural Community of Korea (보건진료소 관할 농촌 지역사회 성인의 고혈압 관리 실태와 관련 요인)

  • Objectives: This study aimed to identify and assess the factors related to the awareness, treatment, and control of hypertension based on jurisdictional areas of primary health care posts in a rural community of Korea. Methods: This study was performed on 4598 adults aged over 30 years in a rural community and we measured their blood pressure (BP) from October. 2007 to August. 2009. Hypertension is defined as a condition characterized by a systolic BP ${\geq}$ 140 mmHg, a diastolic BP ${\geq}$ 90 mmHg or reported treatment with antihypertensive medications. We analyzed the factors related with the prevalence, awareness, treatment and control of hypertension using chi-square test and multivariate logistic regression analysis. Results: The age-adjusted prevalence of hypertension was 34.7%. The age-adjusted rates of hypertension awareness, treatment and control were 50.6%, 93.9% and 64.1%, respectively. Awareness of hypertension was related with increasing age. Higher awareness was found among men who were felt more stress, were obese and had hypercholesterolemia, and among women who were regulary taking medicine for hypertension, were obese and had diabetes mellitus. In women, the hypertension treatment was related a Medical aid and education for hypertension management. Controlled hypertension was more common among men who were educated about the management of hypertension and among women who had hypercholesterolemia. Conclusions: The awareness of hypertension was low and the control of hypertension was high compared with the nationwide data (KNHANES 2005). The results suggest that understanding the characteristics of hypertension in a community is important to perform a community based hypertension control program.

Comparison of Risk Factors for Inducing Aging-Related Diseases according to Single and Multi-Person Households among Young Adults using the 2016-2019 Korea National Health and Nutrition Survey (젊은 성인층의 단독가구와 다인가구에서 고령호발질환유발 위험요인 비교 연구 - 2016~2019년 국민건강영양조사 활용 -)

  • In this study, data from the 7th (2016~2018) and 8th (2019) Korea National Health and Nutrition Examination Surveys were used, which included 5,325 subjects. Health behavior, dietary and nutrient intake status, physical measurement and biochemical characteristics, and risk factors for elderly related chronic diseases were classified and analyzed according to the changing composition of single households and other households in the current society. As a result, the ratio of current smokers and drinkers in young adult single households, walking less than 30 minutes per day, subjective health status was poor, breakfast rate less than three times per week, eating out frequency more than once a day, lipid intake ratio to total calories, saturation fatty acid intake were significantly higher. In addition, waist circumference, and diastolic blood pressure were significantly higher. However, dietary fiber intake level was significantly lower. The results for hypertension, which is the representative chronic disease that causes old age-related chronic diseases, were significantly higher in single households (ORs=1.400 (95% CI: 1.095, 1.791), p=0.007). Although young adults may not have showed particularly serious health problems yet, education is believed as important to recognize and prevent age-related disease risk factors.

Xuefuzhuyu Decoction for Essential Hypertension: Meta-Analysis and Systemic Review (혈부축어탕의 본태성 고혈압에 대한 치료 효과 관한 메타 분석 및 체계적 고찰)

  • Objective: The purpose of this study was to investigate the effect reducing blood pressure (BP) using Xuefuzhuyu Decoction (XFZYD) in adults with essential hypertension (EH). Method: Search engine, such as PubMed, EMBASE, Cochrane library, Web of Science, J-STAGE, CiNii, CNKI, OASIS, NDSL, KISS, RISS, and DBpia, were used. The search period was from the beginning of the search engine to June 30, 2018 and there were no limits regarding languages. The selection and extraction of literatures were performed independently by two authors. Meta-analysis was done on the total effective rate (TER), Systolic BP (SBP) and Diastolic BP (DBP). Cochrane's risk of bias (ROB) was used as the methodological quality assessment scale. Results: Twenty studies were finally selected. We observed that a combination treatment using XFZYD and an antihypertensive drug (AHD) was 5.1 times more effective in lowering BP than using AHD alone on TER. The mean differences in SBP and DBP were -10.65 mmHg (95% Confidential Interval (CI) -13.55 mmHg, -7.74 mmHg, P<0.00001), -5.92 mmHg (95% CI -7.14, -4.38, P<0.00001), respectively. Conclusion: A combination treatment using XFZYD and AHD may be more effective in reducing BP than using AHD alone. Because of the poor methodological quality of the studies conducted thus far, high-quality clinical trials will be required in the future.

Cardiac Dysfunction Is Not Associated with Increased Reintubation Rate in Patients Treated with Post-extubation High-Flow Nasal Cannula

  • Background: Cardiac dysfunction patients have long been considered at high risk of reintubation. However, it is based on past studies in which only conventional oxygen therapy was applied after extubation. We investigated association between cardiac dysfunction and reintubation rate in situation where high-flow nasal cannula (HFNC) was widely used during post-extubation period. Methods: We conducted a retrospective observational cohort study of patients treated with HFNC after planned extubation in medical intensive care unit of single tertiary center. Patients were divided into normal function group (ejection fraction [EF] ≥45%) and cardiac dysfunction group (EF <45%). The primary outcome was reintubation rate within 72 hours following extubation. Results: Of 270 patients, 35 (13%) had cardiac dysfunction. Baseline characteristics were similar in both groups. There were no differences in the changes in vital signs between the two groups during the first 12 hours after extubation except diastolic blood pressure. The reintubation rates were 20% and 17% for cardiac dysfunction group and normal function group, respectively (p=0.637). In a multivariate Cox regression analysis, cardiac dysfunction was not associated with an increased risk of reintubation within 72 hours following extubation (hazard ratio, 1.56; p=0.292). Conclusion: Cardiac dysfunction was not associated with increased reintubation rate within 72 hours when HFNC is immediately applied after planned extubation.

Influencing factors for Sleep Disturbance in the Intensive Care Unit Patients: A Systematic Review (중환자실 환자의 수면에 영향을 미치는 요인: 체계적 고찰)

  • Purpose : Sleep disturbances in patients in the intensive care unit (ICU) are related to health problems after discharge. Therefore, active prevention and management are required. Hence, identification of the factors that affect sleep in patients who are critically ill is necessary. Methods : The PubMed, Cochrane Library, CINAHL, EMBASE, and Web of Science databases were searched. Selection criteria were observational and experimental studies that assessed sleep as an outcome, included adult patients admitted to the ICU, and published between November 2015 and April 2022. Results : A total of 21,136 articles were identified through search engines and manual searches, and 42 articles were selected. From these, 22 influencing factors and 11 interventions were identified. Individual factors included disease severity, age, pain, delirium, comorbidities, alcohol consumption, sex, sleep disturbance before hospitalization, chronic obstructive pulmonary disease (COPD), cardiovascular disease, and high diastolic blood pressure (DBP), low hemoglobin (Hb), and low respiratory rate (RR). Environmental factors included light level, noise level, and temperature. Furthermore, treatment-related factors included use of sedatives, melatonin administration, sleep management guidelines, ventilator application, nursing treatment, and length of ICU stay. Regarding sleep interventions, massage, eye mask and earplugs, quiet time and multicomponent protocols, aromatherapy, acupressure, sounds of the sea, adaptive intervention, circulation lighting, and single occupation in a room were identified. Conclusion : Based on these results, we propose the development and application of various interventions to improve sleep quality in patients who are critically ill.

The Association of Waist to Height Ratio and Resting Heart Rate with Cardio-metabolic Risk Factors in Korean Postmenopausal Women (한국 폐경 여성의 허리둘레/신장비 및 안정시 심박수와 심혈관-대사 위험요인과의 상관성)

  • PURPOSE: The purpose of this study was to examine the association of waist to height ratio (WHtR) and resting heart rate (RHR) with cardio-metabolic risk factors among Korean postmenopausal women. METHODS: A cross-sectional analysis was performed using the 2015 Korea National Health and Nutrition Examination Survey. The analysis included a total of 1,540 postmenopausal women. RESULTS: Individuals with higher WHtR (>0.56) showed significantly higher glucose, triglyceride, insulin, Homeostatic Model Assessment for Insulin resistance (HOMA-IR), total cholesterol, systolic and diastolic blood pressure compared with ones with lower WHtR (≤0.51). Similar findings were found in those with higher RHR (≥90 bpm) compared with ones with lower RHR (<60 bpm) for glucose and HOMA-IR. When determining the combined effects of WHtR and RHR on the prevalence of metabolic syndrome, individual with WHtR above 0.5 and RHR above 80 bpm showed 10.39 times higher prevalence of metabolic syndrome compared with those with WHtR below 0.5 and RHR below 70 bpm. We further performed multiple linear regression analysis to understand how WHtR and RHR contribute to fasting glucose, and found that both WHtR and RHR contribute to fasting glucose levels independent of age, education level, marital status and income level. CONCLUSIONS: The current study showed that the WHtR and RHR are associated with cardio-metabolic risk factor and prevalence of metabolic syndrome in Korean postmenopausal women.

Distribution Dynamics and Proposed Determinants: Exploring Morphological, Clinical Laboratory, and Lifestyle Factors in the Coexistence of Age-Related Skeletal Muscle Mass Loss and Obesity among Young Men: A Nationwide Cross-Sectional Study

  • PURPOSE: This study examined the distribution dynamics and proposed determinants, including morphological measurements, clinical laboratory tests, and lifestyle factors among young Korean men aged 20 to 29 years with the coexistence of age-related loss of skeletal muscle mass and obesity (CALSMO). METHODS: Six hundred and sixty-six participants were divided into two groups based on their skeletal muscle mass index, with 12 individuals categorized in the CALSMO group and the remaining 654 in the normal group. The proposed determinants variables consisted of three main components: morphological measurements, clinical laboratory tests, and lifestyle factors. The morphological measurement variables were height, weight, body mass index, waist circumference, and skeletal muscle mass index. The clinical laboratory tests were fasting glucose, triglyceride, total cholesterol levels, and systolic and diastolic blood pressure. The lifestyle factors considered were alcohol consumption and tobacco use. Complex sampling analysis was performed for the evaluation. RESULTS: The distribution dynamics were determined to be 1.81(1.02-3.18) %. Morphological factors, such as height, weight, body mass index, waist circumference, and skeletal muscle mass index, showed significant differences (p < .05). The clinical laboratory test variables, specifically the fasting glucose, triglyceride, and total cholesterol levels, also exhibited significant differences (p < .05). The lifestyle factor, alcohol consumption, also showed a significance (p < .05). CONCLUSION: This study provides insights into the distribution dynamics. The proposed determinants in young Korean individuals with CALSMO are height, weight, body mass index, waist circumference, skeletal muscle mass index, fasting glucose, triglyceride, total cholesterol levels, and alcohol consumption.
