Antibiotics-Associated Hemorrhagic Colitis Caused by Klebsiella oxytoca: Two Case Reports

  • Nowadays, Klebsiella oxytoca is described as a causative organism for antibiotic-associated hemorrhagic colitis (AAHC). Here we report two cases of pediatric AAHC, from which K. oxytoca was cultured after starting amoxicillin-clavulanate or amoxicillin treatment. The patients developed severe abdominal pain and a large amount of bloody diarrhea. K. oxytoca was obtained in intestinal fluid culture of a boy through the colonoscopy. On the other hand, colonic tissue culture and intestinal fluid culture were negative of the other patient. K. oxytoca was detected in stool culture when he was admitted. These cases showed characteristic endoscopic findings of segmental hemorrhagic colitis, and both boys recovered spontaneously within 2-3 days after they stopped taking the antibiotics. Therefore, in children who develop relatively large amount of bloody diarrhea after antibiotic treatment, we should consider AAHC caused by K. oxytoca.

Recombinant S-Layer Proteins of Lactobacillus brevis Mediating Antibody Adhesion to Calf Intestine Alleviated Neonatal Diarrhea Syndrome

  • A chimeric gene encoding enhanced green fluorescent protein (EGFP) and a S-layer protein from Lactobacillus brevis KCTC3102, and/or two copies of the Fe-binding Z-domain, a synthetic analog of the B-domain of protein A, was constructed and expressed in Escherichia coli BL21(DE3). The S-layer fusion proteins produced in a 500-1 fermentor were likely to be stable in the range of pH 5 to 8 and $0^{\circ}C$ to $40^{\circ}C$. Their adhesive property enabled an easy and rapid immobilization of enzymes or antibodies on solid materials such as plastics, glass, sol-gel films, and intestinal epithelial cells. Owing to their affinity towards intestinal cells and immunoglobulin G, the S-layer fusion proteins enabled the adhesion of antibodies to human epithelial cells. In addition, feeding a mixture of the S-layer fusion proteins and antibodies against neonatal calf diarrhea (coronavirus, rotavirus, Escherichia coli, and Salmonella typhimurium) to Hanwoo calves resulted in 100% prevention of neonatal calf diarrhea syndrome (p<0.01), whereas feeding antibodies only resulted in 56% prevention.

  • Microorganisms causing bovine mastitis, swine diarrhea and swine pneumonia were isolated from farms in Chungbuk and Kyunggi Province and the isolates were identified using microbial identification system(MIS). The most common isolates from bovine mastitis were Staphylococcus sp.,Streptococcus sp., and Corynebacterium sp., those from swine diarrhea were Escherichia coli, Salmonella sp., and Clostridium perfringens, those from swine pneumonia were Bordetella bronchiseptica and Pasteurella multocida. Examination of 16 antibiotics against these pathogens revealed that the incidence of antibiotic-resistant microoganisms were very high and that many of these isolates had multiple resistance to various commercially available antibiotics such as penicillin, ampicillin, erythromycin, streptomycin, norfloxacin, and tetracycline. These results suggest that the use of antibiotics in a farm should be controlled in order to decrease the number of antibiotic-resistant microorganisms were very high and that many of these isolates had multiple resistance to various commercially available antibiotics such as penicillin, ampicillin, erythromycin, streptomycin, norfloxacin, and tetracycline.   • 1. Objectives We introduced a case of postoperation stomach cancer patient with metastasis to peritoneum and colon who suffered from repetitive diarrhea and abdominal pain. 2. Methods Stomach cancet is the most common cancer in Korea. Eventhough the 5-year survival rate of stomach cancer is increasing recently, metastasized stomach cancer does not show good prognosis, especially when the surgical operation is not applicable. After partial resection of stomach, most of patients experienced gastrointestinal disorders like dyspepsia, reduced meal size, abdominal discomfort, loose stool, and constipation. 3. Results and Conclusions Taeumjowi-tang, a Sasang Constitutional Medicine, improved the diarrhea and abdominal pain.

Epidemiology of astrovirus infection in children

  • Human astrovirus (HAstV) is a major cause of acute diarrhea among children, resulting in outbreaks of diarrhea and occasionally hospitalization. Improved surveillance and application of sensitive molecular diagnostics have further defined the impact of HAstV infections in children. These studies have shown that HAstV infections are clinically milder (diarrhea, vomiting, fever) than infections with other enteric agents. Among the 8 serotypes of HAstV identified, serotype 1 is the predominant strain worldwide. In addition to serotype 1, the detection rate of HAstV types 2 to 8 has increased by using newly developed assays. HAstV is less common compared with other major gastroenteritis viruses, including norovirus and rotavirus; however, it is a potentially important viral etiological agent with a significant role in acute gastroenteritis. A better understanding of the molecular epidemiology and characteristics of HAstV strains may be valuable to develop specific prevention strategies.

Breast-Feeding Protects Infantile Diarrhea Caused by Intestinal Protozoan Infections

  • This study investigated the effect of breast-feeding in protection against protozoan infection in infants with persistent diarrhea. Infants were classified into 2 groups; 161 breast-fed infants and the same number of non-breast-fed infants. Microscopic examinations of stool were done for detection of parasites and measuring the intensity of infection. Moreover, serum levels of IgE and TNF-${\alpha}$ were measured by ELISA. Cryptosporidium spp., Entamoeba histolytica/Entamoeba dispar, Giardia lamblia, and Blastocystis sp. were demonstrated in infants with persistent diarrhea. The percentage of protozoan infections was significantly lower in breast-fed infants than that in the non-breast-fed infants. The levels of IgE and TNF-${\alpha}$ were significantly lower in the breast-fed group than in the non-breast-fed group. There were significant positive associations between the serum levels of IgE and TNF-${\alpha}$ and the intensity of parasite infection in the breast-fed group. It is suggested that breast-feeding has an attenuating effect on the rate and intensity of parasite infection.

도적강기탕(導赤降氣湯)으로 호전된 크론병 환자 1례 (A Case Study of a Soyangin Patient with Crohn's Disease who Reported Symptomatic Improvement after Being Treated with Dojeokgangki-tang)

  • 1. Objectives : The aim of this study was to report significant improvement of chronic diarrhea and hematochezia after treatment with Dojeokgangki-tang in a Soyangin Patient with Crohn's Disease. 2. Methods : The patient's subjective and objective symptoms were observed daily throughout the hospitalization period, and the Crohn's disease activity index(CDAI) was calculated to gauge the progress or lack of progress. 3. Results : The symptoms of diarrhea and hematochezia disappeared by the end of the hospitalization period without recurrence, and the CDAI score dropped from 92.5 to 47. 4. Conclusions : A patient with Crohn's disease, who was in the state of repetitive relapse and remission after long term clinical courses was treated with Dojeokgangki-tang in a short period and the the symptoms of diarrhea and hematochezia disappeared without recurrence. Then the patient could stop taking corticosteriods and taper off immune-suppressing drugs.

  • Immunohistochemical study on the intestinal tissues obtained from the 21 pigs of the 14 terms in Korea in which the clinical and epidemiological features had indicated the possible outbreaks of porcine epidemic diarrhea(PED) was performed using the indirect immunofluorescence test and/or the immunoperoxidase method in order to detect PED viral antigens in the infected cells of the intestines, and histopathological features were described as well. By immunohistochemical analysis, PED viral antigens were detected in the epithelial cells covering the small intestinal villi and recognized slightly in the cells lining the colonic surface epithelium as well. Occasional fluorescence was also seen in a few intestinal crypt epithelium. On light microscopy, the piglets with PED showed marked villous atrophy and fusion, and severe enterocyte degeneration and desquamation. On the other hand, the older pigs more than 4 week old age was mild villous atrophy and fusion, severe villous epithelial cell proliferation, and moderate mononuclear cell infiltration.

Serological characterization of bovine viral diarrhea virus isolates

  • Bovine viral diarrhea viruses (BVDVs) were isolated from cattle with respiratory and diarrhea signs as well as persistently infected cattle. These isolates were analysed serologically to characterize serogroups and to compare serological relationship with reference viruses of type I and II. Most isolates from calf diarrheal cases and persistently infected individuals showed a significant difference in cross-neutralization test with the viruses isolated from nasal discharges showing severe respiratory signs. Serologically most of the commercial vaccine strains could be classified into classical BVDV (type I) such as NADL strain. This serological difference among BVDV isolates suggested the need for new vaccines to protect cattle from both respiratory and enteric BVDV infections in field. The immunogenicity of BVDVs which showed a good propagation capability in MDBK cells and high rates of neutralizing activity (isolate : KD26-1, PHG, B5 and 95002) against all viruses used in this study, was confirmed in guinea pig when treated as single or combined groups.

한우 송아지의 소바이러스성 설사바이러스 지속감염률 조사 (Prevalence for persistent infection with bovine viral diarrhea virus in Korean native calves)

  • Bovine viral diarrhea (BVD) is very important disease in cattle industry with a worldwide distribution. Detection and elimination of persistently infected calves with bovine viral diarrhea virus (BVDV) was valuable strategy for BVD eradication because those calves were main source for transmission. We surveyed persistent infection with BVDV by reverse transcription polymerase chain reaction (RT-PCR) and immunohistochemistry (IHC) using whole blood and skin. Five hundred thirty nine Korean native calves were tested. Four calves (0.7%) were positive for BVDV antigen for both tests. Those calves remained positive for follow-up by RT-PCR and IHC. Therefore they were identified as persistently infected with BVDV. We confirmed that immunohistochemistry using skin biopsy samples was very useful tool to detect persistently infected calves with BVDV. As far as we know, this would be first report on persistent infection with BVDV in Korea.
