개에서의 선천성 복막-심낭 횡경막허니아 1예 (Congenital Peritoneopericardial Diaphragmatic Hernia in a Dog)

  • A 5 kg, seven-month-old, female Pekingese dog was presented to Seoul National University Veterinary Medical Teaching Hospital with the history of anorexia and exercise intolerance. Muffled cardiac sound and mild abdominal pain were detected in the physical examination. In positive contrast peritoneography, contrast medium was observed in enlarged pericardial sac through the diaphragm. According to the history taking, physical examination and contrast radiographic study, the dog was diagnaosed congenital peritoneopericardial diaphragmatic hernia. Following midline celiotomy, herniated falciform ligament and greater omentum were repositioned to abdrminal cavity. The diaphragmatic defect was closed with absorbable suture. Clinical signs related to peritoneopericardial hernia disappeared immediately after surgical treatment. There had been no evidence of recurrence of the peritoneopericardial hernia for 1 year.

외상성 횡격막 손상 (Traumatic Diaphragmatic Injuries)

  • The records of 25 patients with traumatic diaphragmatic injuries seen at Chosun University Hospital from February 1977 to May 1991 were reviewed. We treated 20 male and 5 female patients ranging in age from 6 to 72 years. The diaphragmatic injuries were due to blunt trauma in 19 cases[traffic accident 13, fall down 4, compression injury 2] and penetrating trauma in 6 cases[stab wound 5, gun shot 1]. Most common symptoms were dyspnea[72%], chest pain[56%] and abdominal pain [40%], Chest X-ray were normal in 7 cases[28%] and 22 cases[88%] were diagnosed or suspected as diaphragmatic injuries preoperatively. The repair of 25 cases were performed with thoracic approach in 16 cases, thoracoabdominal approach in 6 cases and abdominal approach in 3 cases. Postoperative complications included atelectasis, wound infection and empyema. there was no postoperative death.

  • 양측성 횡격막 마비는 드문 질환으로 외상, 심흉부 수술, 신경근육질병, 경부척추굳음증, 감염이 원인이 될 수 있다. 우폐상엽, 심낭, 상대정맥과 무명정맥을 침범한 흉선암의 적출술을 시행한 후 60세 남자 환자에서 양측성 횡격막 마비가 발생하였다. 심한 호흡곤란이 발생하였고, 인공호흡기 이탈이 불가능하였다. 양측 횡격막 주름성형술을 시행하였고, 인공호흡기 이탈 및 누운 자세에서 수면이 가능하게 되어 만족할 만한 결과를 얻었기에 문헌 보고하는 바이다.

  • Congenital diaphragmatic hernia through of Bochdalek foramen, posterolateral diaphragmatic hernia, is the result of a congenital malformation of the posterolateral region of the diaphragm. Bochdalek hernia is not a rare anomaly and is the most common type of congenital diaphragmatic hernia. The defect is more frequently on left [about 5 times]. The purpose of this paper to present the Authors recent experience with two cases of the congenital posterolateral diaphragmatic hernia adult women which was treated surgically in the Dept, Thoracic Surgery, Chosun University Hospital. The postoperative course was uneventful and both of them were discharged without any other complications. Our study and surgical experience was discussed and the literature reviewed.

성인에서 발생한 Bochdalek 탈장;수술 치험 2례 (Surgical Repair of Bochdalek Hernia in Adult; A Report of Two Cases)

  • Congenital diaphragmatic hernia through Bochdalek foramen, posterolateral diaphragmatic hernia, is the result of a congenital malformation of the posterolateral region of the diaphragm. Bochdalek hernia is the most common type of congenital diaphragmatic hernia, but it is a extremely rare anomaly which is detected in adulthood because it has severe cardiopulmonary and gastrointestinal symptoms after birth immediately.We experienced two cases of Bochdalek hernia which were detected in adulthood. Among them, one was a 43 years old man who was treated with simple closure in left Bochdalek hernia and the other was 41 years old woman who was treated with patch closure using silastic sheet in right Bochdalek hernia. Their postoperative courses were uneventiful. So we report two cases of rare Bochdalek hernia which was detected in adulthood with review of literatures.

Diaphragmatic Hernia in a Two-month Old Cat

  • A 2 months old female Korean domestic shorthair cat weighing 1.2 kg was come to hospital because of respiratory discomfort and lethargy. Heart sounds was more intense and clear on the right side than the left. On radiographic views, loss of the normal diaphragm line, undistinguishable shadow of heart, shadow of gas-containing intestines could be observed in thoracic cavity. Diaphragmatic herniorrhaphy was performed by using propofol 8 mg/kg IV and isoflurane without any complication. On 7th day after the operation, almost all the clinical signs and radiographs including diaphragmatic line, cardiac silhouette, liver and small intestines were turned to normal.

  • Traumatic diaphragmatic rupture is quite uncommon and rarely lethal injury. However, delayed presentation between the injury and the diagnosis can cause a life-threatening condition with various complications such as intestinal hernia, obstruction, strangulation, respiratory distress. Here, we present a case of delayed presentation of traumatic diaphragmatic rupture in a 51-year-old man, and then discuss about the clinical implication of delayed presentation of diaphragmatic rupture with a review.

Diaphragmatic deformity in a cat mimicking a cardiac mass

  • A four-month-old intact femlae Abyssinian cat was present for routine health evaluation, because her littermate was recently died of ventricular septal defect. Diagnostic imaging studies showed a large caudal paracardiac mass in thoracic radiography and homogenous mass adjacent to heart in the echocardiography. Further echographic study revealed that the mass was liver and the diaphragmatic line was intact. The positive contrast celiogram revealed that no extravasation of the contrast media across the diaphragm and the prolapse of diaphragm into the pleural cavity. Based on our diagnostic imaging studies, the case was diagnosed as diaphragmatic deformity in a cat mimicking a cardiac mass.

두개강 공명의 영상해부학적 고찰 (The imaging Anatomical Consideration of the Resonance of the Cranial Cavity)

  • Because vocal technique is the basis of singing a song beautifully, so this study was undertaken to use the scientific and correct technique in order to get much better musical expressions. Shimadzu X - ray remote control TV system was used for checking the supporting state of diaphragmatic respiration after 5, 10, 15, 20, 25 and 30 sec during phonation in the state of full inhalation between professional singer and non - professional singer. Shimadzu Magnet $nex-{\alpha}$(SMT -50CX/H) was used for examining the scattering state from the resonance of nasal cavity to that of cranial cavity on the basis of diaphragmatic respiration. The results obtained were summerized as follows : 1. The resonance of cranial cavity must be scattered by the energy of diaphragmatic respiration after gathering the foci of the fundamental 5 vowels. 2. while raising the epiglottis, the resonance of nasal cavity must be clearly in order to maintain the resonance of cranial cavity beautifully. 3. We can get musical expressions by maintaining the elasticity of diaphragmatic respiration.

  • Traumatic diaphragmatic injury (TDI) occurs in 1% of patients of blunt abdominal trauma. Most TDIs involve the left diaphragm, however the authors experienced TDI accompanied by a liver laceration of the right diaphragm. When detected early, TDI can be easily treated, however serious complications can occur if not. When diaphragmatic injury is suspected due to clinical manifestation, comprehensive analysis of the patient data including radiologic findings is important.
