복부접근법을 이용한 Morgagni Hernia의 교정 - 1예 보고 - (Repair of Morgagni Hernia through the Abdominal Approach - A case report -)

  • Morgagni 탈장은 선천성 탈장의 약 3%를 차지하며 주로 증상이 없이 우연히 성인기에 발견된다. 단순 흉부촬영으로 쉽게 진단될 수 있으나 탈장이 없는 경우는 흉부 전산화 단층 촬영 등으로 진단된다. 장관의 감돈 위험이 있으므로 발견되면 수술적 치료가 바람직하다. 흉강이나 복강으로 접근이 가능하나 본 저자들은 복강경을 이용하여 교정이 가능하였기에 보고하는 바이다.

수축성 심막염에 대한 심막절제술 전후의 수술방법에 따른 혈역학적 비교 (Pre- and Postoperative Hemodynamic Studies in the Patients with Constrictive Pericarditis)

  • 15 patients with constrictive pericarditis who underwent interphrenic pericardiectomy from January, 1981 to April, 1983 and 11 patients who underwent radical pericardiectomy from May, 1983 to September, 1984 were compared to the clinical improvement and the results of pre- and postoperative cardiac catheterization. In the group of partial pericardiectomy the pericardium was removed anteriorly from the left phrenic nerve to the right phrenic nerve and in the group of radical pericardiectomy the pericardium was removed from almost entire surface of the heart including diaphragmatic surface and posterior wall of the left ventricle. The following results were obtained. 1. Both group of the patients showed marked symptomatic improvement early after operation. 2. The central venous pressure was decreased significantly after operation in both group of the patients. 3. The right atrial mean pressure and pulmonary arterial mean pressure decreased significantly after operation in both group of the patients and there was no significant difference in the amplitude of decrease between the two groups. 4. The right ventricular end-diastolic pressure and left ventricular end-diastolic pressure were decreased postoperatively in both group of the patients and the patients of the radical pericardiectomy showed more decrease than the patients of interphrenic pericardiectomy, and in the group of radical pericardiectomy the right and left ventricular end-diastolic pressure were normalized postoperatively but in the group of partial pericardiectomy they showed abnormally high pressure persistently. 5. The ejection fraction showed normal level pre- and postoperatively in both group of the patients.

The Laparoscopic Repair of a Morgagni Hernia in a Child

  • A 12-year-old female presented with the abnormal findings on the chest PA. The chest CT revealed a retrosternal defect of the diaphragm and a fatty opacity in the pleural cavity, resulting in a diagnosis of Morgagni hernia. It was decided to undergo a laparoscopic surgery. The retrosternal defect of the diaphragm measuring 3.5 cm in diameter was found, through which a portion of the greater omentum and the fatty tissue connected with the falciform ligament were herniated into the pleural cavity. The greater omentum was pushed back into the peritoneal cavity and the fatty tissue connected with falciform ligament was excised. The mediastinal pleura was plicated and the defect of the diaphragm was repaired primarily. Immediately after the operation, the patient developed a right pneumothorax for which a chest tube was inserted. She was discharged at the post-operative third day without any further complications.

  • The increased embryotoxicity of the antifolate drug pyrimethamine (PYM) with concomitant dietary dosing of folic acid (FA) was examined in mice. The preventive effects of folinic acid (FNA) on PYM embryotoxicity were also examined. Six groups were constructed: PYM I (pyrimethamine 80 ppm), PYM II (pyrimethamine 150 ppm), PYM II+FNA (pyrimethamine 150 ppm and folinic acid 12 mg/kg/day), PYM II+FA (pyrimethamine 150 ppm and folic acid 350 ppm), FA (folic acid 350 ppm) and a control group. The agents were administered for 7 days from day 6 throughout 12 of gestation. PYM and FA were administered with mashed feed and FNA was intraperitoneally injected. The high incidence of fetal realformations was observed in the PYM II group; these included kinky tail, open eyelids, club foot, cleft palate, absence of the pulmonary lobe, diaphragmatic hernia, fused sternebrae, fused cervical or thoracic vertebral arch, among others. All embryos of the PYM II+FA group were resorbed. No realformed fetuses were observed in the PYM II+FNA group. These results show that the concomitant dosing of FA augments PYM embryotoxicity. The preventive effects of FNA were also observed.

Chronic Gastric Volvulus as a Late Complication of Hepatectomy for Hepatoblastoma in a Child: A Case Report

  • Gastric volvulus (GV) is an uncommon pathology, with 10-20% of cases occurring in children, typically before one year of age. It often occurs in people with congenital diaphragmatic hernias, intestinal malrotation, eventration of the diaphragm, paraesophageal hernias, wandering spleens, asplenism, or intra-abdominal adhesions. We report a rare case of chronic GV after left hemihepatectomy for hepatoblastoma in a child. The patient was a 9-year-old boy who complained of upper abdominal pain and postprandial upper abdominal distension for one year. At the age of 4 months, he was diagnosed with hepatoblastoma and had undergone left hemihepatectomy. The upper gastrointestinal contrast study revealed chronic organoaxial gastric volvulus. After a surgical procedure involving adhesiolysis and an anterior wall gastropexy, the patient improved and the symptoms resolved. Although GV is a rare disease, it should be suspected in a patient with a previous abdominal surgical history who is complaining of abdominal distension and pain.

  • Purpose - Work stress environment is expanding alongside the speed of worldwide change and psychological well-being concerns are coming about because pressure over-burden can bring about lost workdays, lower representative usefulness, and high occupation turnover rates. For this reason, this study investigates to provide how practitioners can manage employee work stress using health psychology based on prior studies. Research design, Data, and methodology - Qualitative textual research involves collecting and analyzing various non-numeric data to establish different concepts or opinions in prior literature. That means that the present researcher carried out qualitative research in multiple ways, investigating many kinds of previous textual data, such as prior published peer-reviewed resources and case studies analysis. Result - The results revealed that directed symbolism has been displayed to improve prosperity and tranquility among employees with an assortment of persistent infections. Like diaphragmatic breathing, reflection practices consolidate zeroing in on a particular article or sensation (like breathing) while at the same time separating from other distracters and controlling inward encounters (for example, feelings, musings) through non-critical acknowledgment. Conclusion - This research proposes that intellectual rebuilding or figuring out how to perceive maladaptive musings and change them to reflect more versatile; judicious idea examples can fundamentally further develop temperament, diminish pressure, and reduction negative feelings for employees.

Role of Esophageal High-Resolution Manometry in Pediatric Patients

  • Purpose: Dysphagia, vomiting and feeding difficulties are common symptoms, with which children present. Esophageal function testing with high resolution manometry can help in diagnosing and treating these patients. We aim to access the clinical utility of high-resolution manometry of esophagus in symptomatic pediatric patients. Methods: A retrospective chart review was done on all symptomatic patients who underwent esophageal high-resolution manometry between 2010 and 2019 at Sydney Children's Hospital, Australia. Manometry results were categorized based on Chicago classification. Demographic data, indication of procedure, manometric findings, and details of treatment changes were obtained and analyzed. Results: There were 62 patients with median age of 10 years (9 months-18 years). The main indication for the procedure was dysphagia (56%). Thirty-two percent of patients had a co-morbid condition, with esophageal atresia accounting for 16%. The majority (77%) of patients had abnormal manometry which included, ineffective esophageal motility in 45.2%. In esophageal atresia cohort, esophageal pressurization was seen in 50%, aperistalsis in 40% and 10% with prior fundoplication had esophago-gastric junction obstruction. Patients with esophago-gastric junction obstruction or achalasia were treated by either pneumatic dilation or Heller's myotomy. Patients with ineffective esophageal motility and rumination were treated with a trial of prokinetics/dietary texture modification and diaphragmatic breathing. Conclusion: Esophageal high-resolution manometry has a role in the evaluation of symptomatic pediatric patients. The majority of our patients had abnormal results which led to change in treatments, with either medication, surgery and/or feeding modification with resultant improvement in symptoms.

Traumatic abdominal wall hernia with hemoperitoneum caused by blunt injury: laparoscopic exploration with mini-laparotomy repair. A case report

  • Traumatic abdominal wall hernia is a very rare clinical entity. Herein, we report the case of a patient who was transferred from a local clinic to the emergency department because of left lower abdominal pain. Initially, an intra-abdominal hematoma was observed on computed tomography and no extravasation was noted. Conservative treatment was initiated, and the patient's symptoms were slightly relieved. However, though abdominal pain was relieved during the hospital stay, bowel herniation was suspected in the left periumbilical area. Follow-up computed tomography showed traumatic abdominal wall hernia with hemoperitoneum in the abdomen. We performed a laparoscopic exploration of the injury site and hernia lesion. The anterior abdominal wall hernia was successfully closed.

Effect of Thoracic Joint Mobilization and Breathing Exercise on The Thickness of The Diaphragm, Expansion of The Chest, Respiratory Function, and Endurance in Chronic Stroke Patients

  • Objective: This study was performed to investigate the effects of thoracic joint mobilization and breathing exercises on diaphragmatic thickness, chest expansion, respiratory function, and endurance in patients with chronic stroke. Design: Randomized controlled trial Methods: The study included 24 chronic stroke patients who were randomly divided into two groups. The experimental group (12 people) performed 15 minutes of thoracic joint mobility exercises and 15 minutes of breathing exercises, three times a week for 6 weeks, 30 minutes each time. The control group (12 people) received 15 minutes of conservative physical therapy and 15 minutes of breathing exercises, 3 times a week for 6 weeks, 30 minutes per session, the same as the experimental group. The experimental and control groups performed the same breathing exercises. To assess training effectiveness, changes in diaphragm thickness, chest expansion, respiratory function, and endurance were measured. Results: As a result, the experimental group exhibited significant improvements in diaphragm thickness, chest expansion, and respiratory function. The endurance mode also displayed significant enhancement (p<0.05), a finding consistent with the control group. However, the experimental group displayed more substantial improvements in non-affected diaphragm thickness and thoracic expansion compared to the control group (p<0.05). Conclusions: Drawing from these findings, breathing exercise which combine thoracic mobilization, will be actively utilized in addition to physical therapy interventions in clinical trials as an effective intervention method.

  • 흉부외상에 의한 흉곽내 이물의 기관지 침식 또는 이동은 극히 드물게 발생하는 경우이다. 41세 남자가 만성 기침과 농성 객담을 주소로 내원하였다. 과거력상 30년전 어린시절 나무에서 떨어져 왼쪽가슴을 나무에 찔린 사고를 당해 약 6개월간 병원에서 치료받았으며 상처부위에서 나무토막이 나왔다고하였다. 단순 흉부사진에서 3x/cm 크기의 석회화된 음영이 좌측 폐후기저구역에서 보였으며 흉부 전산화 단층촬영 사진에서 좌측 폐 횡격막측흉막 근처에 기관지확장증이 동반된 석회화 소견을 보였다. 좌하엽 절제술후 합병증없이 경쾌 퇴원하였다.
