공황장애 호흡기 아형과 비호흡기 아형의 임상 특성 비교 (Comparison of Clinical Characteristics Between Respiratory and Non-Respiratory Subtypes of Panic Disorder)

  • Objectives : The goal of this study was to compare the clinical characteristics of panic disorder respiratory subtype(PD-R) and non-respiratory subtype(PD-NR). Methods : 84 patients with panic disorder were enrolled and divided into 2 groups, 29 PD-R and 55 PD-NR. Diagnosis of panic disorder was evaluated using Diagnostic and Statistical Manual of Mental Disorders $4^{th}$ edition and Mini International Neuropsychiatric Interview. They were also measured with Hamilton Rating Scale for Anxiety(HAM-A), Hamilton Rating Scale for Depression(HAM-D), and Panic Disorder Severity Scale (PDSS). Results : PD-R group showed significantly higher scores in PDSS than those of PD-NR group(p=.027). After controlling for the severity of panic disorder and gender, PD-R group showed higher HAM-D and somatic anxiety subscale of HAM-A than those of PD-NR group. Furthermore, results of logistic regression analysis suggested that the somatic anxiety was a possible risk factor of PD-R(OR=1.404,p=0.009). Conclusion : These results suggest that somatic anxiety and depressive symptom would be important clinical characteristics of PD-R.

Blink reflex changes and sensory perception in infraorbital nerve-innervated areas following zygomaticomaxillary complex fractures

  • Background Infraorbital nerve dysfunction is commonly reported after zygomaticomaxillary complex fractures. We evaluated sensory changes in four designated areas (eyelid, nose, zygoma, and lip) innervated by the infraorbital nerve. This evaluation was conducted using the static two-point discrimination test and the vibration threshold test. We assessed the diagnostic significance of the blink reflex in patients with infraorbital nerve dysfunction. Methods This study included 18 patients, all of whom complained of some degree of infraorbital nerve dysfunction preoperatively. A visual analog scale, the infraorbital blink reflex, static two-point discrimination, and the vibration threshold were assessed preoperatively, at 1 month postoperatively (T1), and at a final follow-up that took place at least 4 months postoperatively (T4). The results were analyzed using a multilevel generalized linear mixed model. Results Scores on the visual analog scale significantly improved at T1 and T4. The infraorbital blink reflex significantly improved at T4. Visual analog scale scores improved more rapidly than the infraorbital blink reflex. Two-point discrimination significantly improved in all areas at T4, and the vibration perception threshold significantly improved in the eyelid at T4. Conclusions Recovery of the infraorbital blink reflex reflected the recovery of infraorbital nerve dysfunction. We also determined that the lip tended to recover later than the other areas innervated by the infraorbital nerve.

한의 복합치료로 호전된 불응성 위식도역류질환 (PPI-refractory GERD) 환자 1례 - 복진소견 평가를 중심으로 - (Clinical Case Report of PPI-refractory Gastro-Esophageal Reflux Disease (GERD) with Korean Medicine Treatment - A Focus on Evaluation of the Abdominal Examination)

  • This case report describes a 60-year-old man who had reflux symptoms despite treatment with proton pump inhibitors (PPIs). Korean traditional medicine, including a herbal prescription, acupuncture, and moxibustion was administered to the patient during his 19 days of hospitalization. The visual analogue scale (VAS), gastrointestinal scale (GIS), and gastrointestinal symptom rating scale (GSRS) scores were assessed on the dates of admission and discharge. An abdominal examination, which is one of the Korean medicine diagnostic tools, was performed every day to evaluate the treatment progress. During the treatment, the improvement of symptoms was clinically correlated with abdominal examination outcomes. We suggest that administration of traditional Korean Medicine on PPI-refractory GERD patients could be effective and we recommend utilization of the abdominal examination as an assessment tool for Korean Medicine treatment.

Raman Spectroscopy Studies of Graphene Nanoribbons and Chemical Doping in Graphene

  • Atom-thick graphene membrane and nano-sized graphene objects (NGOs) hold substantial potential for applications in future molecular-scale integrated electronics, transparent conducting membranes, nanocomposites, etc. To realize this potential, chemical properties of graphene need to be understood and diagnostic methods for various NGOs are also required. To meet these needs, chemical properties of graphene and optical diagnostics of graphene nanoribbons (GNRs) have been explored by Raman spectroscopy, AFM and STM scanning probes. The first part of the talk will illustrate the role of underlying silicon dioxide substrates and ambient gases in the ubiquitous hole doping of graphene. An STM study reveals that thermal annealing generates out-of-plane deformation of nanometer-scale wavelength and distortion in $sp^2$ bonding on an atomic scale. Graphene deformed by annealing is found to be chemically active enough to bind molecular oxygen, which leads to a strong hole-doping. The talk will also introduce Raman spectroscopy studies of GNRs which are known to have nonzero electronic bandgap due to confinement effect. GNRs of width ranging from 15 nm to 100 nm have been prepared by e-beam lithographic patterning of mechanically exfoliated graphene followed by oxygen plasma etching. Raman spectra of narrow GNRs can be characterized by upshifted G band and strong disorder-related D band originating from scattering at ribbon edges. Detailed analysis of the G, D, and 2D bands of GNRs proves that Raman spectroscopy is still a reliable tool in characterizing GNRs despite their nanometer width.

CLUSTERS OF GALAXIES: SHOCK WAVES AND COSMIC RAYS

  • Recent observations of galaxy clusters in radio and X-ray indicate that cosmic rays and magnetic fields may be energetically important in the intracluster medium. According to the estimates based on theses observational studies, the combined pressure of these two components of the intracluster medium may range between $10\%{\~}100\%$ of gas pressure, although their total energy is probably time dependent. Hence, these non-thermal components may have influenced the formation and evolution of cosmic structures, and may provide unique and vital diagnostic information through various radiations emitted via their interactions with surrounding matter and cosmic background photons. We suggest that shock waves associated with cosmic structures, along with individual sources such as active galactic nuclei and radio galaxies, supply the cosmic rays and magnetic fields to the intracluster medium and to surrounding large scale structures. In order to study 1) the properties of cosmic shock waves emerging during the large scale structure formation of the universe, and 2) the dynamical influence of cosmic rays, which were ejected by AGN-like sources into the intracluster medium, on structure formation, we have performed two sets of N-body /hydrodynamic simulations of cosmic structure formation. In this contribution, we report the preliminary results of these simulations.

불안에 대한 태극권 치료의 해외 임상연구 동향 (A Review on Clinical Research Trends in the Treatment of Tai Chi on Anxiety)

  • Objectives: This study was performed to review the research trends in the treatment of Tai Chi on anxiety. Methods: We searched for articles in Pubmed and China National Knowledge Infrastructure (CNKI) from January 2009 to December 2018. The selected studies were evaluated by Jadad Scale. Results: One pre-post comparison study and seven controlled clinical trials were selected. HAMA (Hamilton Rating Scale for Anxiety) and STAI (State Anxiety Inventory) were the most frequently used diagnostic criteria. For outcome measurement, STAI was again the most commo. From Jadad Quality Assessment Scale of 8 articles, the overall quality of the studies was low. Conclusions: According to this study, it seemed that Tai Chi treatment on anxiety disorder is an effective and safe intervention. Therefore, based on this study, more clinical research on diverse forms of treatments of anxiety disorder should be performed in Korean medicine in the near future.

DEA 모형을 이용한 주업과 겸업 표고재배 임가의 경영효율성 비교 분석 (Management Efficiency of the Full-time and Part-time Oak Mushroom Farms using DEA models)

  • 본 연구는 비모수추정 방법인 자료포락 기법을 이용하여 우리나라 표고버섯 재배 임가를 대상으로 경영효율성을 평가하였다. 조사자료는 2013년 3월부터 10월까지 실시한 '표고버섯 경영 표준진단표' 적용 결과 중에서 20개 임가의 투입요소와 산출요소를 대상으로 하였다. 그리고 DEA 기법중 CCR 모형과 BCC 모형을 이용하여 기술효율성, 순수기술효율성, 규모효율성을 분석하였고 비효율성 임가의 개선방안도 제시하였다. 추가적으로 주업과 겸업재배임가의 경영효율성을 비교 분석하였다. 연구결과, 표고버섯 재배 임가의 전체기술효율성은 평균 0.655로 나타나 전체적으로 비효율성이 높은 것으로 나타났고 비효율성의 원인은 순수기술효율성(0.830)보다는 규모효율성(0.747)에 있는 것으로 분석되었다. 효율성 값이 1인 표고버섯 재배임가는 DMU2, DMU5, DMU6, DMU8, DMU10, DMU15, DMU20으로 7개 임가로 나타났다. 그리고 DMU7 재배임가의 경영효율성 진단 결과, 투입요소 중 신규접종비와 조성비에서 준거집단과 비교해 볼 때 과도하게 투입되고 있는 것으로 분석되었다. 주업과 겸업임가의 효율성 비교 결과 겸업임가가 상대적으로 비효율성이 높은 것으로 나타났고 그 원인으로 규모효율성이 문제가 있는 것으로 파악되었다.

자살시도로 응급실에 내원한 환자의 자살 위험성과 양극성 장애 가능성의 연관성 (The Relationship between Possibility of Bipolar Disorder and Suicidal Attempt in Emergency Room)

  • 연구목적 본 연구는 자살 시도자를 대상으로 자살 생각의 심각성과 관련된 요인들을 찾아보고자 하였다. 방 법 일 대학병원 응급실에 자살 시도로 내원한 환자 중 50명을 대상으로 임상가가 면담, Columbia Suicide Severity Rating Scale(C-SSRS), Beck Depression Inventory(BDI), Korean Version of Mood Disorder Questionnaire(K-MDQ), Bipolar Spectrum Diagnostic Scale(BSDS)를 시행하였다. 결 과 자살 경향이 높은 고위험군은 C-SSRS의 자살 생각의 심각성 점수와 BDI 점수가 높았고(p<0.001), MDQ에서 양극성 장애 가능성의 양성으로 판정된 자는 자살생각의 심각성 점수가 높았다(p=0.033). 결 론 자살 시도자의 자살 생각의 심각성과 양극성 장애의 가능성은 서로 연관성이 있을 것으로 생각된다.

신 서비스유통포맷으로서의 비즈니스센터의 서비스품질 평가척도 개발 (Developing the Service Quality Scale of Business Center: A New B2B Service Retailing Format)

  • Purpose - This study aims to verify the construct of the service quality suitable to the characteristics of the business center, to develop the scale that can measure the service quality objectively, compare the diverse business centers in Korea, and present desirable methods of business center management and operation. Research design, data, and methodology - In the first stage, literature research and focus group interviews with current users and actual business experts of business centers were conducted. In addition, 69 early stage scale items were derived. In the second stage, a Delphi survey was conducted on the experts of business and academic circles. The measurement items in the early stage were supplemented and revised. The content validity rate (CVR) of 69 measures was computed and then the experts' opinions were used. As a result, the business center service quality scale, made up of 9 organizing factors and 36 measurement items, was constructed. In the third stage, a survey was conducted on the occupants who were currently using the business center. It was to confirm if the developed business center service quality scale could be actually utilized. The data of 435 samples collected were used. To refine the scale items, the exploratory factor analysis (EFA) was conducted repeatedly. Results - The business center service quality scale, made up of 8 factors and 33 items, was developed again. The 8 factors are reliability, speed, kindness, specialty, responsibility, appearance, convenience, and public interest. The reliability coefficient (Cronbach's Alpha) of each factor is greater than .7. Thus, it was confirmed that there was reliability in the measurement items. Further, the model suitability was verified using the confirmatory factor analysis (CFA), and the convergent validity and the discriminant validity were verified. This proved the construct validity. Through this process, the Korea Business Center Service Quality Scale was developed. Based on factor analysis, the developed business center service quality (BCSQ) scale consists of 4 aspects, 8 factors, and 33 items, comprising the "process quality" aspect: reliability (3 items), speediness (4 items), kindness (3 items) factors, the outcome quality aspect: expertise (3 items), responsibility (5 items) factors, the environment quality aspect: appearance (5 items), convenience (3 items) factors, and the public interest aspect: public interest (7 items) factor. However, according to CFA, the BCSQ scale consists of a total of 4 aspects, 8 factors, and 33 items, comprising the "process quality" aspect: reliability (3 items), speediness (3 items), kindness (3 items) factors, the outcome quality aspect: expertise (2 items), responsibility (4 items) factors, the environment quality aspect: appearance (3 items), convenience (2 items) factors, and the public interest aspect: public interest (3 items) factor. Conclusions - The BCSQ scale could serve as a diagnostic tool that will allow business center managers to determine service areas that are weak and in need of attention, and to improve the service quality of the business center.

Fabrication and Simulation of Fluid Wing Structure for Microfluidic Blood Plasma Separation

  • Human blood consists of 55% of plasma and 45% of blood cells such as white blood cell (WBC) and red blood cell (RBC). In plasma, there are many kinds of promising biomarkers, which can be used for the diagnosis of various diseases and biological analysis. For diagnostic tools such as a lab-on-a-chip (LOC), blood plasma separation is a fundamental step for accomplishing a high performance in the detection of a disease. Highly efficient separators can increase the sensitivity and selectivity of biosensors and reduce diagnostic time. In order to achieve a higher yield in blood plasma separation, we propose a novel fluid wing structure that is optimized by COMSOL simulations by varying the fluidic channel width and the angle of the bifurcation. The fluid wing structure is inspired by the inertial particle separator system in helicopters where sand particles are prevented from following the air flow to an engine. The structure is ameliorated in order to satisfy biological and fluidic requirements at the micro scale to achieve high plasma yield and separation efficiency. In this study, we fabricated the fluid wing structure for the efficient microfluidic blood plasma separation. The high plasma yield of 67% is achieved with a channel width of $20{\mu}m$ in the fabricated fluidic chip and the result was not affected by the angle of the bifurcation.
