An Efficient Correction Process of CT-Simulator Couch with Current Diagnostic CT Scanners (진단용 CT-모의치료기 테이블의 효율적인 교정 방법)

  • This study suggested that the table of CT-simulator and the laser alignment system using diagnostic CT scanner have an efficient method for improvement in alignment between the planned target center of traverse image with CT scanner. It was conducted on the daily QA when presented in the AAPM TG66 with correcting the laser alignment system using geometric trigonometric functions and investigated the effectiveness of correction methods as compared with those before and after correction. Before correction error was 3.82mm between the planned target center of image, the table longitudinal axis was twisted with 0.436o. The laser alignment system using geometric trigonometric functions in after correction was satisfied with tolerance limits of ${\pm}2mm$ when occurred about 0.7mm in errors between the planned target center. The table correction to satisfy the geometric accuracy is very inefficient over against the time and economic loss as well as technical limits in the case of application as only radiation therapy associated with CT-simulator with diagnostic CT scanner in use. But, the method which corrects the laser alignment system is economic and relatively simple with possibility of getting well geometric accuracy and we suppose that it is efficient method for applying in the clinic.

Quality Improvement Activity in Hospitals and Its Acceptance among Hospital Personnels (의료 질 관리 활동에 대한 병원종사자들의 태도와 인식)

  • Background : It is a primary goal for hospital personnel to improve the quality of patient care. In Korea the concept of quality assurance has been spread over the last five years. Many hospitals have implemented quality assurance programs that fit their own philosophy and needs. As a result, they are supposed to have different experiences and attitudes toward quality assurance. To investigate their diversity will be helpful to have a direction to the future and to enhance the quality assurance activities in Korean hospitals. Objectives : The aim of this study is to obtain information about hospital personnel's attitude and opinion toward quality assurance in hospitals. Methods : A questionnaire was developed which consisted of five parts; the general characteristics of respondent, the concepts of quality assurance, need for quality assurance program, current status of quality improvement activities, and participation in programs. Using the registry of Korean hospitals, 102 hospitals with more than 400 beds were selected. Questionnaires were mailed to hospital staffs of each hospital; top managers, clinical department heads, registered nurses, medical recorders, and administrators. Results : Of 2038 questionnaires sent, 877 were returned, giving response rate of 44%. Most respondents(70%) regarded quality assurance as efforts to provide patient with care in highest quality and to improve effectiveness or resource utilization. Ninety-nine percent of respondents agreed to need for quality assurance in their hospitals. There were current quality improvement programs implemented in the department of 553 respondents(62%), and most of the(85%) have participated in at least one program. Lack of motivation was pointed out as a barrier to implement the programs. Conclusion : Although most respondents have known of the concept and need for quality assurance, this study suggests that education and motivation of hospital personnel be needed to activate the quality assurance programs in hospitals.

  • Purpose: To date, there is no region-specific guideline for pediatric endoscopy training. This study aimed to illustrate the current status of pediatric endoscopy training in Asia-Pacific region and identify opportunities for improvement. Methods: A cross-sectional survey, using a standardized electronic questionnaire, was conducted among medical schools in the Asia-Pacific region in January 2024. Results: A total of 57 medical centers in 12 countries offering formal Pediatric Gastroenterology training programs participated in this regional survey. More than 75% of the centers had an average case load of <10 cases per week for both diagnostic and therapeutic endoscopies. Only 36% of the study programs employed competency-based outcomes for program development, whereas nearly half (48%) used volume-based curricula. Foreign body retrieval, polypectomy, percutaneous endoscopic gastrostomy, and esophageal variceal hemostasis, that is, sclerotherapy or band ligation (endoscopic variceal sclerotherapy and endoscopic variceal ligation), comprised the top four priorities that the trainees should acquire in the autonomous stage (unconscious) of competence. Regarding the learning environment, only 31.5% provided formal hands-on workshops/simulation training. The direct observation of procedural skills was the most commonly used assessment method. The application of a quality assurance (QA) system in both educational and patient care (Pediatric Endoscopy Quality Improvement Network) aspects was present in only 28% and 17% of the centers, respectively. Conclusion: Compared with Western academic societies, the limited availability of cases remains a major concern. To close this gap, simulation and adult endoscopy training are essential. The implementation of reliable and valid assessment tools and QA systems can lead to significant development in future programs.

Comparison of Horizontal and Vertical Noise Power Spectrum in Measurements by Using Various Electronic Portal Imaging Devices in Radiation Therapy (방사선치료 시 전자포털 영상장치를 이용한 잡음전력스펙트럼 수평 및 수직 측정비교)

  • The quality assurance (QA) is very important for diagnostic field and radiation therapy field to evaluate the characteristic of devices. The purpose of this study was to compare different NPS methodologies results which are measuring NPS with regard to horizontal and vertical directions by using megavoltage X-ray energies. The NPS evaluation methods were applied to the International Electro-technical Commission standard (IEC 62220-1). The electronic portal imaging devices (EPID) devices such as Siemens BEAMVIEW$^{PLUS}$, Elekta iViewGT and Varian Clinac$^R$ iX aS1000 were used. NPS data were expressed by corresponding each frequency about average of noise value corresponding the each frequency, and NPS were evaluated quantitatively by totaling up the noise values of average frequency which are on horizontal and vertical directions. In NPS results for Elekta iViewGT, NPS of horizontal and vertical by using 4 methods were indicated the difference of 3~5% between horizontal and vertical direction. In the results of Siemens BEAMVIEW$^{PLUS}$ and Varian Clinac$^R$ iX aS1000, the NPS of horizontal and vertical direction were indicated the difference of 15% when averaging the whole values. This study were evaluated the NPS of each devices by totaling up the noise values of average frequency which are on horizontal and vertical directions suggesting the quantitative evaluation method using the data.

Experimental Study with Respect to Dose Characteristic of Glass Dosimeter for Low-Energy by Using Internal Detector of Piranha 657 (Piranha 657의 Internal Detector를 이용한 저에너지에서 유리선량계의 선량 특성에 관한 연구)

  • Recently, Glass Dosimeter (GD) with thermoluminescent Dosimeter (TLD) are comprehensively used to measure absorbed dose from diagnostic field to therapy field that means from low energy field to high energy field. However, such studies about dose characteristics of GD, such as reproducibility and energy dependency, are mostly results in high energy field. Because characteristic study for measurement devices of radiation dose and radiation detector is performed using 137Cs and 60Co which emit high energy radiations. Thus, this study was evaluated the linearity according to Piranha dose which measured by changing tube voltage (50kV, 80kV and 100kV which are low energy radiations), reproducibility and reproducibility according to delay time using GD. Measurement of radiation dose is performed using internal detector of Piranha 657 which is multi-function QA device (RTI Electronic, Sweden). Condition of measurement was 25mA, 0.02sec, 2.5mAs, SSD of 100 cm and exposure area with $10{\times}10cm^2$. As above method, GD was exposed to radiation. Sixty GDs were divided into three groups (50kV, 80kV, 100kV), then measured. In this study, GD was indicated the linearity in low energy field as high energy existing reported results. The reproducibility and reproducibility according to delay time were acceptable. In this study, we could know that GD can be used to not only measure the high energy field but also low energy field.
