Splenic tuberculosis is an uncommonly considered diagnosis in clinical practice. This is a case report of splenic tuberculosis in a 13-year-old boy who was seronegative to HIV. He was just well until 7 days prior to this admission when he started to feel epigastric and left subchondral pain. Chest X-ray was not pathological. Abdominal ultrasonography showed slight splenomegaly with multiple hypoechoic nodules and abdominal CT disclosed multiple irregular hypodense lesions in the spleen. Radiological interpretation suggested the possibility of lymphoma or metastatic malignancy. Splenectomy was done and the histopathological findings showed extensive chronic granulomatous inflammation compatible with tuberculosis. Splenic tuberculosis must be included in the differential diagnosis of hypoechoic and hypodense lesions by means of sonography and computed tomography, respectively.

Objective This study is aimed to present the effective classification of Soeumin symptomatology and the standardized signs for classification which can be applied for KCD, ICD and the insurance codification system. 2. Results and Conclusions 1) The diagnosis criteria for Soeumin exterior-interior disease is based upon signs & symptoms of cold/heat, condition of stool, state of digestive system(such as digestion and appetite)among others. 2) The diagnosis criteria for Soeumin favorable-unfavorable disease is generally based upon whether the vital force of the spleen is damaged or not. More specifically, for the exterior disease, whether or not sweating is present. For the interior disease, whether or not dry mouth, body ache(a main symptom of the exterior state), and anxiousness are present. 3) For the Soeumin Wool-gwang disease, the diagnosis criteria of mild-severe disease is whether or not chills is present and the degree of body fever. 4) For Soeumin Mang-yang disease, the diagnosis criteria of dangerous-urgent disease is whether or not chills is, the degree of sweating and urine condition. 5) For the Soeumin Greater-Yin disease, Abdominal-pain bowel irritability pattern and Epigastric discomfort pattern are early state signs, Jaundice pattern is mild-state sign, edema & Greater-Yang disease Yin-toxin pattern are terminal state signs. 6) For the Soeumin interior disease, Abdominal-pain bowel irritability pattern and Epigastric discomfort pattern are of the dangerous state pattern, Jang-gual and Exuberant-Yin-repelling-Yang pattern are of the urgent state patterns.
As medical treatment is developing with technology, the men's average life expectancy is extended. Therefore, primary medical care becomes emphasized in order to reduce the medical expenses in the long term by satisfying individual's life being healthy. The date for this thesis was collected from January 2011 to June 2011. 889 patients who visited the university hospital emergency room and hospitalized in internal medicine, were picked as the research subjects and they were targeted to be recorded the distribution of chief complaint and principal diagnosis of the patients. Also, this record was used to apply to the standard Classification of Diseases(as known as ICD) and the method of detailed classification of the primary medical care(as known as ICPC) to compare each other. In order to analysis, frequency analysis was used to see vital statistics and the cross tabulations were used to see the distribution of chief complaint according to ICD and ICPC. Results of the research were Abdominal pain(17.7%), Dyspnea(13.5%), Fever (12.5%), and Haematemesis (9.8%), and those symptoms represented the 54.5% of overall chief complaints that is treated in primary care. Therefore, it is acceptable to use the classification of the primary medical care at doc-in-a-box. Also, in case of diagnosis of abdominal pain, it is classified to R10 in ICD and 116 patients(18.7%) belonged to it, but according to ICPC, it is subdivided to Epigastric(11.5%) and General(5.8%). ICPC classification, which is focused to primary medical care is more detailed than ICD classification. Because the data that is collected for this thesis is from only one hospital, it is hard to represent to all the cases, but ICPC in emergency medical care, it has more classification available and it can subdivide the patients effectively, so it is meaningful.
Pneumatic rupture is a rare cause of esophageal injury, as evidenced by only 19 cases reported in the literature. We experienced one case of esophageal rupture due to bursting of a truck inner tube. The patient, who was a 45-year old male, had severe chest pain, respiratory distress, flushing in the face and neck, and subcutaneous emphysema after tire explosion. Three days after the incident, a diagnosis of rupture of the thoracic esophagus was established by esophagogram using water soluble contrast media, and then emergency operation was done. The operation involved mediastinal and thoracic drainage and resection of the esophagus combined with cervical esophagostomy and feeding gastrostomy. On the 105th day after the operation, cervical esophagogastrostomy via substernal route was performed. The patient was successfully treated with the staged operations. As in the other reported cases, the injury was located in the lower one third of the esophagus. Four main characteristics of the clinical signs of pneumatic rupture are 1] wounds or burns to the face or mouth, 2] chest pain or epigastric pain, 3] subcutaneous emphysema, and 4] respiratory distress. We emphasize that the high index of suspicion of esophageal rupture is very important in diagnosis and that diagnosis should be based on the same findings common to other forms of esophageal injury.

Laryngopharyngeal reflux disease (LPRD) is common in laryngologic practice. In Korea, up to 1 out of every 5 patients who visit otorhinolaryngology clinic is supposed to have LPRD with symptoms and physical findings. Major symptoms of LPRD include hoarseness, cough, reflux symptom and mild dysphagia. Even though LPRD is common, its diagnosis may be difficult, because its symptoms are nonspecific and the laryngeal findings are not always associated with symptom severity. In Recent study, 66.4% of Patient who has LPRD also associated with esophageal motility disorders. Esophageal achalasia is a disease of unknown etiology characterized by an absence of peristalsis in the body of esophagus and nonrelaxing hypertension of the lower esophageal sphincter. Common cause is loss of ganglion cells in Auerbachs plexus. The classic triad of symptoms in achalasia includes dysphagia, regurgitation and weight loss. LPRD and esophageal achalasia have similar symptoms but have different treatment of choice. The Differentiation diagnosis of theses disease is important and should be established by history, radiologic examination and endoscopic examination. We recently assessed a 59-year-old female patient who complained of an epigastric pain, dysphagia and chronic cough. LPRD was initially diagnosed on Laryngoscopic examination and Reflux Symptom Index, but patient was not relieved of any symptoms after treatment of Proton Pump Inhibitor for 3 months. After high resolution manometry, esophageal achalasia was finally diagnosed. We report this case regarding the diagnosis and treatment with review of literatures because we have to think about esophageal motility disorders as a differential diagnosis in laryngology.

Gastric metastasis from ovarian carcinoma is extremely rare and the prognosis for patients is poor. We report a case of multimodal treatment improving the survival time of a patient with gastric metastasis from ovarian cancer. A 73-year-old woman with known serous ovarian cancer was admitted to the hospital due to epigastric pain and dyspepsia. On esophagogastroduodenoscopy, a protruding mass was noted at the gastric antrum. She underwent distal gastrectomy with Billroth I anastomosis and lymph node dissection, including the para-aortic lymph nodes. The final pathology revealed gastric metastasis from ovarian serous adenocarcinoma. In this case, after cytoreductive surgery, chemotherapy was performed each time a recurrence was diagnosed, and remission was accomplished. She survived for 108 months after the first diagnosis of the metastatic tumor in the stomach. Multimodal treatment of metastatic lesions since the first diagnosis allowed the patient to survive longer than those in previous reports.

Primary small-cell carcinomas of the stomach are rare and aggressive malignancies with poor survival rates. Preoperative diagnosis is difficult and a standard treatment is not yet established. We have recently experienced two cases of a primary small-cell carcinoma of the stomach. The first case was a 65-year-old man with epigastric soreness. Endoscopic biopsy showed an adenocarcinoma. He underwent a radical subtotal gastrectomy with D2 lymph-node dissection. Pathology revealed a collision tumor of a smallcell carcinoma and an adenocarcinoma with submucosal invasion and with metastasis in 20 out of 48 lymph nodes (T1N3M0). The second case was a 64-year-old man with epigastric soreness. Endoscopic biopsy revealed a small-cell carcinoma. There was no evidence of a primary tumor in the lung. A radical subtotal gastrectomy with D2 lymph-node dissection was performed. Pathology showed a pure smallcell carcinoma with proper muscle invasion and with metastasis in 1 out of 36 lymph nodes (T2aN1M0).

Objectives: The purpose of this study was to investigate the opinions of experts on abdominal examinations in Korean Medicine included in the curriculum of the College of Korean Medicine. Methods: Among Korean doctors, 14 experts on abdominal examinations were interviewed; the experts included 9 professors of Korean internal medicine, 1 expert in diagnostics of Korean Medicine, 1 primary care Korean medicine doctor, and 3 executives of a (former) Korean association of the abdomen. The interview consisted of questions regarding recognition of the clinical importance of abdominal examinations, how to perform abdominal examinations, the most frequent abdominal examination findings encountered in clinical practice, and the definition of some of the abdominal examination findings. Results: Most interviewees recognized abdominal examinations as important and used them in clinical practice. Opinions on additions and corrections were collected regarding observation items, posture, method, and order during abdominal examinations. Abdominal examination findings that were common clinically were abdominal fullness (腹滿), epigastric stuffness (心下痞鞕), abdominal tenderness, epigastric fullness (心下滿), and rib distention (胸脇苦滿). The answers to the question related to the definitions of abdominal examination findings included consent and supplementary opinions regarding definitions of deficiency-excess, cold-heat, abdominal tenderness, tension of abdominal muscles, succession sounds, and borborygmus; these were mainly selected based on abdominal symptoms that are highly quantifiable. Conclusions: In the future, based on the results of this study, additional research related to the drafting of a standard abdominal examination in Korean medicine should be conducted to provide an opportunity to increase the reliability of Korean medicine diagnosis.
Objectives: This study investigated the clinical characteristics and symptoms of patients with reflux esophagitis (RE) visiting a Korean medicine hospital. Methods: We retrospectively reviewed the medical records of patients with an RE diagnosis who visited a Korean medicine hospital from June 15, 2020, to April 15, 2021. We analyzed the clinical characteristics and the results of a symptoms questionnaire. Results: Our sample included 1,151 patients (711 females). The median age was 58 years; the most common age bracket was 60-69 years. A total of 837 patients (72.7%) presented with symptoms lasting more than one year, and 1,035 (89.9%) had been prescribed medications for more than three months before visiting a Korean medicine hospital. Belching (77.6%) and acid regurgitation (70.9%) were the most frequent symptoms, followed by epigastric pain (61.6%) and throat globus (58.5%). Throat globus was the most bothersome symptom, and 72.4% of the patients experienced the symptom on more than five days in the preceding two weeks. Patients with a longer symptom duration were older (p<0.01) and more likely to be overweight (p=0.03). Patients experiencing symptoms for more than five years were more likely to report throat globus (p=0.02), hoarseness (p<0.01), and coughing (p<0.01). Conclusions: Most RE patients visiting a Korean medicine hospital were females in their sixties with chronic and refractory RE. Atypical symptoms (belching, epigastric pain, and throat globus) were common. Throat globus was highly prevalent in severe cases, suggesting that it warrants attention when treating RE patients.
A case of Pseudoterranova decipiens infection was found in a 43-year-old man by gastroendoscopic examination on August 20, 1996. On August 6, 1996, he visited a local clinic, complaining of epigastric pain two days after eating raw marine fishes. Although the symptoms were relieved soon, endoscopic examination was done for differential diagnosis. A white, live nematode larva was removed from the fundus of the stomach. The larva was $38.3{\times}1.0{\;}mm$ in size and had a cecum reaching to the mid-level of the ventriculus. A lot of transverse striations were regularly arranged on the cuticle of its body surface, but the boring tooth and mucron were not observed at both ends of the worm. The worm was identified as the 4th stage larva of P. decipiens.
