A clinical report of cerebral infarction patient with schizophrenia (전증(癲證)을 동반한 졸중풍(卒中風) 환자 치험1례)

  • Metabolic syndrome consists of several risk factors of metabolic origin like elevated blood pressure, abdominal obesity, elevated triglyceride concentrations, decreased HDL-cholesterol concentrations, and elevated fasting glucose. It appear to directly increase the development of atherosclerotic cardiovascular diseases which leading to cardiovascular diseases in later life. This 39 years female patient was diagnosed schizophrenia at the age of 17. but she didn't take reasonable treatments. during 20 years, she stayed in home and lived unsuited life with her family. As a result she gains heavy weight and centeral obesity, diabetic mellitus and hypertention, and developed cerevral vascular accident twice. This patient was treated with herb medication, acupuncture, western medication, psychotherapy primarily in focusing of pre-illness state(未病) as well as the stagnation of phelm. During 140 days of treatment from admission, before 2nd attack tbe patient showed improvement of Lt. hemiparesis but after second attack improved a little. And symptom of schizophrenia improved considerably. Also diet habit changed.

Comparison of peripheral vascular compliance between normal and diabetic groups using the second derivative of photoplethysmogram (PPG 2차 미분을 이용한 정상인과 당뇨병 환자의 말초혈관 탄성도 비교)

  • Recently the prevalence rate of diabetes in Korea has been increasing rapidly due to high growth of economy and changes in dietary lifestyle. Vascular complication is one of diabetic complications which have frequently occurred by obesity, hyperglycemia and impaired glucose metabolism. Photoplethysmogram(PPG) measured from finger and toe is very useful for evaluation of vascular aging and sclerosis level since the PPG signal represents characteristics of peripheral vascular Several researchers have reported that second derivative of the finger PPG waveform was useful to evaluate vascular compliance and developed various analysis methods for vascular compliance. However, peripheral vascular compliance study for diabetic patient was never evaluated by using second derivative of PPG. Therefore, we aimed to objectively compare and to assess normal(n=850) and diabetic(n=50) groups vascular compliance using the second derivative of PPG waveform in this study. The evaluated factors of the second derivative of PPG are 'a', 'b', 'c', 'd', 'e' and b/a rapresents vascular compliance. This study found out that when vascular compliance is decreased, the absolute value of b/a is decreased. The average vascular compliance of 50 diabetic patients with neuropathy, $-1.09{\pm}0.14$ is statistically lower than the normal group, $-0.81{\pm}0.09$ (p<0.05). In conclusion, we suggest an objective evaluation of peripheral vascular compliance for diabetic patients and prevention of vascular complication.

A new diagnostic method for diabetic neuropathy using $TcpO_2$ ($TcpO_2$ 이용한 당뇨병성 신경병증 환자의 조기진단)

  • PDiabetic neuropathy is one of the most common diabetes related complications including diabetic nephropathy and retinopahty. In clinical practices, nerve conduction velocity (NCV) has been used as a standard method for diagnosing diabetic neuropathy. However, it applies maximum current of 100mA to nerves causing stress and pain to patients. In this study, as a non-invasive method, $TcpO_2$ was utilized to investigate the difference and relationship between $TcpO_2$ and $SpO_2$ of normal and diabetic neuropathy subjects. In addition, a new method of diagnosing diabetic neuropathy using $TcpO_2$ is suggested. 50 normal subjects and 50 diabetic patients with neuropathy diagnosed by NCV participated in this study. Parameters used in this study were $TcpO_2$, $TcpCO_2$, and $SpO_2$. As a result of the $TcpO_2$ measurements, statistical significances were found from $TcpO_2$ of hands and feet from normal and patients group(p<0.01). $SpO_2$ measured from index finger of normal and patient groups showed no statistical significance(p>0.05). On the other hand, $SpO_2$ measured from great toes of normal and patient groups showed statistical significance(p<0.01). Correlation coefficient between $SpO_2$ of finger and $TcpO_2$ of hand was 0.400 (p<0.01) and $SpO_2$ of toe and $TcpO_2$ of foot was 0.471(p<0.01). Both correlation values were statistically significant. Sensitivities and specificities of the $TcpO_2$ method were found to be 66 % and 92 %, respectively. If the suggested $TcpO_2$ method is used periodically, prevention and early diagnosis of diabetic neuropathy would be possible.

  • Recently, The medical services make use mobile healthcare system to transmit patient information on wireless environment. But It is difficult to satisfied for specified patient to send message, and it's not sufficient healthcare system to help regular treatment for example a diabetic or a pregnant woman. Thus, this paper proposes an healthcare scheduling application which manages a pregnant woman pregnant-checking, fetal movement sensing, antenatal training and promotion of health information using networking, data transmission method and application framwork of WPAN concepts.

  • Calciphylaxis is a rare disease that appear in patients with secondary hyper-parathyroidism or chronic renal failure or that show defect in calcium phosphate metabolism which is characterized by fibrin deposit or calcification of medial wall of vessels causing gradual ischemic skin necrosis. Calciphylaxis is a disease with poor prognosis as skin necrosis can progress rapidly. If left untreated, calciphylaxis will progress to sepsis with high mortality. The treatment is controversial but kidney transplantation or parathyroidectomy is suggested to recover calcium-phosphate metabolism. The authors have experienced calciphylaxis in a patient with chronic renal failure caused by DM nephropathy with characteristic skin lesion and rapid skin necrosis. We describe this case with documentary reviews.

Oral cavity lipoma: a case report

  • Intraoral lipomas are a rare clinical entity, comprising only 0.1% to 5% of all benign tumors in the intraoral cavity. A 56-year-old woman suffering from diabetes presented with this relatively rare intraoral lipoma and was treated by surgical excision under general anesthesia. Because the mass was located adjacent to the mental foramen, a precise dissection was necessary to ensure minimal nerve damage. No abnormalities or recurrence was noted at 1-year follow-up and the patient did not complain of numbness. We studied the occurrence of oral lipoma in this diabetic patient and reviewed the relationship between oral lipoma and diabetes in the literature.

A Study on the Mediating Effect of Patient Activation between Trust in Healthcare Professionals and Shared Decision Making in Diabetic Patients (당뇨병 환자의 의료진 신뢰와 공유의사결정의 관계에서 환자활성화의 조절효과)

  • This study aims to identify the moderating effect of patient activation in relation between trust in healthcare professionals and shared decision making in patients with diabetes mellitus. The participants were 186 patients who received treatment for diabetes at the department of endocrinology in the tertiary hospital located in Daejeon and consented to participate in our survey. The results of this study were a significant interaction between trust in healthcare professionals(β=0.32, p=.045) and patient activation(β=0.32, p=.024) was associated with shared decision making. The patient activation had a moderating effect between trust in healthcare professionals and shared decision making in patients with diabetes mellitus(β=0.25, p=.019). Therefore, it is necessary to assess trust in healthcare professionals to improve negative awareness of healthcare professionals and strengthen trust in them. In addition, to promote participation in shared decision making, it is necessary to develop a convergent program to focus patient activation.

A Patient with Mitochondrial Myopathy who Experienced Diabetic Ketoacidosis with Auto-antibody (사립체 근병증 환자에서 발생한 자가항체 양성의 당뇨병성 케톤산증 1례)

  • Mitochondrial myopathy results from a primary dysfunction of the respiratory chain and is frequently accompanied with endocrine manifestations. Among the endocrine manifestations of mitochondrial disease, diabetes mellitus is relatively common. Diabetes mellitus in the mitochondrial myopathy is usually insulin dependent due to the defect in insulin secretion resulted from mitochondrial dysfunction. But it is seldom manifested as diabetes ketoacidosis and doesn't usually have an auto-antibody. We report a patient with mitochondrial myopathy who was diagnosed as having diabetes mellitus by presenting as diabetes ketoacidosis and had both of the auto-glutamic acid decarboxylase (GAD) antibody and anti-insulin auto-antibody.

  • Pulmonary mucormycosis is an uncommon, but important opportunistic fungal infection associated with diabetes mellitus, leukemia, lymphoma and other immunocompromised states. Mucor species grow best in acidic-high glucose medium. which explaining the particular susceptibility of diabetic patient who are ketoacidic. Early consideration of this diagnosis, along with aggressive diagnostic evaluation, is critical to effective therapy and patient survival. We have experienced a case of pulmonary murcomycosis mimicking bilateral pulmonary edema on chest Xray that associated with diabetic ketoacidosis. A brief review of the literature was given.

Acute cardiovascular complications in patients with diabetes and hypertension: management consideration for minor oral surgery

  • Objectives: Medically compromised patients often fear required dental surgical procedures that can increase the risk of medical emergency when combined with reduced tolerance for stress. A stress reduction protocol (SRP) helps doctors minimize treatment-related stress and improves patient management with minimum complications. Diabetes and co-morbid hypertension carry 4-fold risk of aggravation of cardiovascular emergencies and 7.2-fold risk of mortality. Diabetic neuropathy can result in difficult diagnosis of myocardial infarction and reduces chances of surviving a myocardial infarction compared with a non-diabetic person. The aim of the study was to assess the feasibility of a protocol for management of patients having both diabetes and hypertension who required minor oral surgery to minimize the rate of cardiovascular emergencies. Materials and Methods: A prospective study was conducted in 140 patients having both diabetes and hypertension who required minor oral surgical procedures. A systematic approachable protocol was designed for management of such patients. Results: Among 140 patients, 6 patients (4.3%) had cardiovascular complications, while 3 patients (1 with syncope and 2 with hypertension) did not require any intervention other than observation. Two patients were managed with aspirin and nitroglycerin, and 1 patient had possible myocardial infarction (overall incidence 0.7%) with chest pain, S-T segment elevation on electrocardiogram, and troponin level of 0.60 ng/mL. Conclusion: The proposed protocol helps to improve management of patients having both diabetes and hypertension. We recommend that patients with uncontrolled diabetes and uncontrolled hypertension and/or patients having history of cardiovascular complication should be treated in a medical facility with a readily available cardiology unit. This facilitates prompt response to emergency and instant implementation of treatment, helping to reduce morbidity and mortality.
